Over-exertion in wielding or throwing objects

Employee was injured on 1/7/23 and was treated via first aid measures until 1/25/23 when they saw a specialist who then placed restrictions on their work. Employee was using a 3/4"" ratchet to loosen a tail-pulley nut. The ratchet slipped from the nut and caused the employees arm to jolt resulting in an injury to the shoulder.

A contractor technician was removing a sprocket with a mallet from a skid steer when a metal fragment from the sprocket shot out and went into their eye.

Employee was turning valve in water line, valve came loose from fitting causing laceration to his left index finger.

While pulling work boat to tie off and secure the rope broke sending employee off balance backwards and fell on digger ladder platform hurting left rib cage.

EE & another employee were carrying a set of steps to load onto a trailer. The ground was covered in frost & icy. EE's feet came out from under him & he fell face first onto the steps he was carrying. It chipped & cracked one of his front teeth.

EMPLOYEE WAS WORKING ON THE EXHAUST ON DREDGE WHEN A PIECE OF METAL BLEW INTO HIS EYE. HE WAS NOT WEARING SAFETY GLASSES.

EE WAS CUTTING THE TAPE OFF OF THE 4160 LEADS GOING TO THE MOTOR ON THE DREDGE. THE TIGHT AREA BETWEEN THE WALL AND THE MOTOR WAS CAUSING HIM TO CUT BACK TOWARD HIM. HE SLIPPED THROUGH A SOFT SPOT AND CUT HIS WRIST, REQUIRING TWO STITCHES.

EE WAS GRINDING ON A LIFTING EYE WITH THE SHOP BENCH GRINDER. HE WAS WEARING A FACE SHIELD WITH HIS GLASSES ON. HE FELT A PIECE OF METAL BOUNCE INTO HIS EYE. HE FLUSHED IT BUT IT STILL BOTHERE D HIM. THE DR FOUND EYE IRRITATED BY NO DEBRIS STILL IN IT. EE RETURNED TO WORK THE SAME DAY.

EMPLOYEE WAS TAKING BOLTS LOOSE ON THE LINER OF THE PUMP WITH AN 1 5/8"" WRENCH, PUSHING DOWNWARD WITH BOTH HANDS. THE WRENCH SLIPPED OFF SENDING HIS HANDS DOWN AGAINST THE NEXT BOLT BELOW, SMA SHING HIS MIDDLE FINGER. HE WAS WORKING UNDER HURRIED CONDITIONS AND WITHOUT GLOVES ON.

EMPLOYEE WAS MOWING WITH A TRACTOR. WHEN CLIMBING UP AND OVER A TOPSOIL PILE, HE DID NOT SEE A DUG OUT AREA BECAUSE OF THE TALL WEEDS AND HE WAS LOOKING BEHIND AT THE MOWER. THE TRACTOR FRONT END DROPPED OFF IN THE HOLE AND TIPPED THE TRACTOR OVER DOWN INSIDE.

EE WAS ADDING PIPELINE TO THE END OF THE PIPELINE. HE STEPPED OFF THE PIPELINE ONTO A FLATER AND HANDED A COME-ALONG TO A CO WORKER. HE LOST HIS BALANCE AND FELL INTO THE LAKE, TWISTING HIS LE FT ANKLE.

AT THE END OF HIS SHIFT, EE WAS SITTING IN THE FOREMAN'S OFFICE. HE ADVISED A COWORKER THAT HE HAD BEEN FEELING ILL AND HAD VOMITED. THE COWORKER NOTIFIED THE FOREMAN. EE WAS TAKEN TO ER WHERE HE HAD A HEART ATTACK. WE WERE ADVISED THAT HIS ARTERIES WERE VERY BLOCKED AND HE HAD STOPPED TAKING HIS HIGH BLOOD PRESSURE MEDICATION.

EE WAS WALKING DOWN THE RAMP TO THE BOAT DOCK. IT HAS BEEN RAINING AND THE PALLET STEPS WERE WET. WHEN HE STEPPED ON IT, THE STEP SHIFTED, CAUSING HIM TO FALL BACKWARDS. HE TRIED TO CATCH HIMS ELF BY PUTTING HIS LEFT HAND DOWN. HE CRACKED A BONE IN HIS LEFT HAND DOING SO.

EE WAS GOING OUT TO START THE FRESH WATER PUMP FOR THE PLANT. HE WAS WALKING ACROSS WALKWAY GOING TO PUMP HOUSE & HAD HIS HEAD DOWN SQUINTING HIS EYES FROM SUN. HE WALKED INTO CORNER OF STEEL FRAME FOR A SWING OUT WINDOW THAT HAD VIBRATED LOOSE & OPENED. RESULTED IN CUT TO RIGHT SIDE OFHEAD REQURING 6 STITCHES
