Fall onto or against objects

Employee was climbing out of plant 2 tanks on a ladder with 3 points of contact. 1 point was plunger rod. The rod is bolted on the bottom. Employee slipped off plunger rod causing a rapid fall, cutting EE's leg, took 7 stitches

EE was helping move river cables and tried stepping over a 14"" plastic pipe when he slipped and his right leg folded under. He received a contusion of his right knee.

Employee was coming down the tipple steps and tripped and fell on his wrist.

EE and another ee were replacing a wiper on brick flat belt so they had to remove a door off of the drop shoot and they went to put it back and it slipped and hit ee on the right side of his face, right temple area.

Customer truck driver was cleaning off tailgate on his truck. The tailgate closed mashing his finger which cut his right pointer finger requiring stitches.

This accident was reported on 9/9/04. EE caught his boot in the ladder exiting the highloader, on 8/4/04, he fell and landed on his left knee. He thought his knee would get better and when it did not, he decided to see a dr. He is on full duty, however, he will going to therapy for two weeks. he had a left knee sprain.

EE WAS CHANGING ARCTURN ROLLER. HE WAS HOLDING THE ROLLER UP WITH HIS RIGHT SHOULDER & APPARENTLY STRAINED HIS LOWER BACK. DID NOT HAVE TROUBLE UNTIL 11-17 & WENT TO THE CLINIC. WE THOUGHT ONE TIME VISIT. ON THE 254TH, DR. PUT HIM IN PHYSICAL THERAPY.

EE WAS PULLING CABLE ON THE RIVER. HE INJURED HIS LEFT LOWER BACK. BACK STRAIN.

EE WAS REMOVING A PONTOON. THE BRACKET HOLDING THE PONTOON BROKE & CAUSED THE PONTOON TO ROLL TO PORT SIDE & PINNED EE BETWEEN HANDRAIL.

EMPLOYEE WAS PULLING ON A RATCHET. THE RATCHET SLIPPED AND HIT HIS THUMB ON THE RIGHT HAND. RESULT FRACTURED THUMB.
