EE WAS FEELING DIZZY, LIGHT HEADED, AND WAS NOT SWEATING. EE WENT TO THE HOSPITAL, WHERE HE WAS ADMITTED OVERNIGHT.

Employee was working on a shaker pan at the shop when he raised up and hit his head on a piece of angle iron.

Employee was lifting pipe off a flat bed truck. The load swung to the side and tipped the machine he was using. He has a sprained ankle. It is his right foot.

Employee was walking across the shop area. He was wearing his safety glasses, however, they started to fall off and he reached up to push them back on (he had his welding gloves on and shavings fell from the gloves onto his eyelashes. Metal shavings got in his left eye.
