Process employee was attempting to de-head a drum using the Wizard Drum De-Header. As the employee was lifting the top off of the drum he realized that it had not been completely cut off. The employee then tried to tear the lid off of the drum and received a cut on his left, middle finger requiring 3 stitches.
Employee had sampled a waste fuel storage tank. He purged material into a bucket, and as he was walking, tripped on a pipe and splashed material that was in bucket into his right eye. Employee was sent to local doctors office to have eye washed out. Doctor administered tetanus shot and prescribed ointment for eye.
Employee was opening an overhead garage door. The door stuck and employee jerked upward to free the stuck door. He felt a pop in his upper back and then pain.

Employee looking up at return roller on conveyor belt. Roller was slipping & located about 15' above employee. Roller knocked dust loose which irritated employee's left eye (safety glasses were in use). Flushed eye with solution & continued shift (feeling better). Reached home, irritation started again, went to ER to have 3 tiny pieces of metal removed (barely visible).
Employee was walking down steps at truck terminal, as he reached the bottom he twisted his left leg and felt pain in his calf. Employee was using handrail, area was clean and clear of obstructions. 4 days after initial incident, employee was not getting better and his supervisor took him to see a doctor to make sure everything was ok. Employee is recovering.
Employee had face shield & prescription safety glasses on while using grinder. Dust & particles became airborne & irritated employee's eye. First aid was given & employee was taken to the doctor. Released to full duty the same day.
Employee was walking east of Punch Clock on a ground-level concrete walkway that has a handrail. It was raining heavily at the time. Employee's left leg slipped. Right arm caught handrail & temporarily stopped fall. Left leg continued to move & struck bottom portion of handrail at the knee.

Two Contractor employees were driving railroad spikes when both employees swung hammers at the same time, pinching one employees left hand between the hammers. Result was a cut on the outside of the hand between little finger and the wrist. Employee was taken to local emergency room and receive 8 stiches.
Repairing kerosene space heater with fuel tank emptied before starting reapir work. Removed guards but forgot to unplug small space heater blower fan. While reaching down to pull hose hand slipped into rotating blower fan and was cut.
Solid object prevented Truck Terminal automated cement bin dishcharge gate from closing. Employee wearing safety glasses, dust mask + other PPE, attempted to close manual gate above automated gate. Manual gate could not be closed due to solid object blocking opening. Result was a large amount of cement dust in air which got into employee's eye causing discomfort. Rec'd first aid
A customer-hired truck driver arrived at the facility at 9:00 am. At 10:00 am, an employee found him unresponsive in his truck. Emergency Medical Services was called. Driver was deceased. Preliminary autopsy fingings are death was caused by Spontaneous cardiac arrhythmia.
Employee was taking empty, crushed drums off of a roller conveyor and stacking them onto a pallet. As he lifted a drum, he felt a pop in his back followed by discomfort. He went to chiropractor who performed an adjustment and gave him an all-natural muscle relaxer to ease the discomfort.
Swinging a sledge hammer driving pins out of the primary crusher breaker chain - momentum twisted body around causing right knee to twist. Right after incident, EE reported feeing a ""pop"" sensation while walking. He rated pain 7 out of 10 but did not feel symptoms were serious & agreed to home care. Symptoms worsened & he underwent microscopic surgery 12/17/2010.
Employee was trying to get warm near a Nat Gas Garbage Can Style Heater. When the employee turned away from the heater the reflective safety vest caught on fire. Employee used left hand to put out fire. When that was unsuccessful he dropped & rolled to extinguish the flame. First aid was given & employee driven to medical facility. Back to work following day.

Plant was screening clay. Due to wet weather the clay built-up inside transition hopper between 2 conveyor belts. Employee used a poking tool to remove the build-up from the hopper while the belts were shutdown. While poking at the built-up clay he felt a burning pain in his right shoulder. He finished his task and reported the burning pain to his supervisor.
He got into his big haul truck & after he slammed the door he felt something get into his left eye. It was very irritating & he waited as long as he could before seeking treatment. He did not report the incident until 9/17/9 in the afternoon & then he went to the eye doctor - Greene Vision Group. Doctor removed foreign dust or particle from eye.
A plant millwright injured the middle finger of his left hand and the pinky finger of his right hand when placing replacement discharge screens in the Nordberg Raw Mill. The injury occurred at approximately 11:30 am CST on 6/25/09. Both fingers were lacerated and required sutures. There were no fractures. He was released to work the same afternoon with a 20 lb lifting restriction.
Employee was pullling fish tape while installing new condiut - felt pain in shoulder. On Monday, 03/23/09 still having pain in shoulder went to Dr. Dr said no pulling or pushing. Employee to return to Dr. on April 1, 2009
EE used palm of hand to close a Cam lock fitting.EE reported to co-worker that it really hurt.EE did not file a first aid report. Co ruled not work related. EE had surgery on 5/15/09, not reported has lost time.On 12/09 Workman's Comp. determined this injury was work related.

Employee was pulling plastic standing on a metal bar to get better leverage for removing plastic. While pulling plastic out of the pan, his right foot slipped off the bar. While stepping down with left foot, contacted ground, rolling ankle.
Employee went to move a 70lb gear box from bench to floor and strained shoulder. Though injury took place on 9/5/08 - did not go to doctor until 10/ 01 /08 when shoulder did not get better . Doctor placed employee on lifting restriction of 30lbs for 10-days.
3rd party driver for customer delivering fuel quality waste to Systech was walking back to his truck along a sloped gravel area. He lost his footing and apparently broke his ankle. Driver has over 5 years driving experince. Currently (Coal City Cobb 972-923-7508) for 3 years 11 months. As he is not a Systech employee, will not be able to confirm return to work status.
Employee was using a personnel blow off station to blow dust off of himself and clothing. Dust got into his right eye. Eye flushed immediately. Taken to the doctor's office, diagnosed with minor ""lime"" burns. He was given a prescription for antibiotic eye drops.
Employee was using a pry bar to remove mud buildup from a roll inside a rock spreader. A conveyor belt that runs thru the spreader was in operation when this was being performed. The employee did NOT shut down & lock out the conveyor belt. No guard was in place where pry bar was used. Employee received a fracture of right hand. Hand was pinned between bar & spreader frame.
Installing brick (Kiln 1). When reaching for next brick, a brick fell from the pallet & landed on his left hand. He experienced pain & swelling in his left hand & was taken to dr. for x-ray. Dr. said there was no fracture & he received a tetanus shot due to a minor scrape on his hand. During follow up visit on 1/29/08, the dr. said it was fractured.

Employee was crossing a work area that was being cleaned and slipped on wet floor that resulted in a fall causing a sprain to his back, hip and elbow.
Employee was going restart a compressor. He was in a hurry and lost his balance. He stumbled forward and tried to run forward to catch himself. When he reached the point to where there is a 4"" step down in the floor, he lost control even more. He ran into a switchgear cabinet causing a laceration to his right elbow that required stiches.
EE was pushing drum into dumper carriage holding drum by the cut rim of the drum. The drum jerked suddenly lacerating his right palm. Medical treatment included 5 stitches to the palm of right hand and a round of antinotic, light duty until stitches removed on May 10, 2006.

Employee put the parifin wax back onto the hot plate after using it without turning the hot plate off. He then noticed it smoking and went to take it off. When he removed the wax, it flash set on him causing wax to get onto his hand causing 2nd degree burns. He went to the hospital and returned on the same day.
Developed a cough after inhaling fumes coming from Systech while loading railcars. Went to doctor on 11/4/05 and was released with no restrictions. Cough still existed so he saw a throat and lung specialist on 11/29/05. He was prescribed medication at this time therefore making this a reportable incident.
Employee was walking along the railroad tracks checking rail cars when he stepped into a hole and twisted his right knee. He seen a doctor, but was released without any treatment. In February, it was determined that he needed surgery. The surgery was on 2/17/06, and he returned to work on his next regular scheduled day of 2/20/06.
Employee was removing insulation from an electrical connection using a utility knife when the knife slipped and made a 3/4"" cut in the palm (near little finger) of the left hand. Employee was taken to ER for stitches and a tetanus shot.
After taking slurry sample, injured was exiting the slurry tank pit w/sample in left hand and right hand used for handrail. When he reached the top set of stairs, he fell forward striking his left knee on the bottom edge of the doorway cutting his knee open. He received stitches for his wound. Addendum: Leg that wasn't cut swelled up & EE admitted to hospital.
EE WAS PULLING A PIECE OF CONDUIT WHILE LEANING OVER AND USING CHEST TO HELP HOLD SELF FOR MORE LEVERAGE WHEN EE FELT SOMETHING ""POP"". INJURY OCCURED ON 3/3/05 BUT DID NOT BECOME RESTRICTED DUTY UNTIL 3/7/05. EE SAID WAS STILL HAVING SHARP PAIN IN THE AREA. DR PUT EE ON RESTRICTED DUTY FOR 2 WEEKS
INJURED & OTHER EMPLOYEE WERE ON BREAK IN THE GREEN AREA NEXT TO THE PAVILLION. THEY BEGAN WRESTLING ON THE GRASS. THEY BOTH FELL TO THE GROUND & INJURED'S LEFT LEG WAS BROKEN IN THE PROCESS.
Pulling on a chain, he felt pain in lower right back.

EE WAS IN PROCESS OF PUTTING ON PPE WHEN EE TRIED TO OPEN WINDOW TO SPEAK WITH ANOTHER EE. INJ EE PUT PRESSURE ON WINDOW BECAUSE IT WOULD NOT OPEN THE WINDOW BROKE LACERATING HIS LEFT HAND. 10 STITCHES WERE USED TO CLOSE WOUND.
Ee was overpacking drums. The drum that was being lifted slipped from the grabber and smashed two fingers (little & ring) and wrist between/against the overpack. Hand was x-rayed (no fracture). Ee was told by physician not to use hand until Monday, Sept 27, 2004 and wear wrist brace.

EE WAS CLEANING MATERIALS FROM A PIECE OF EQUIPMENT, WHEN EE PULLED HER BACK. EE FILLED OUT A 1ST AID REPORT. ON THE FOLLOWING TUESDAY 9/16/03, EE WENT TO DR. THE DR. TOLD HER TO STAY OFF WORK FOR 1 DAY 9/17/03. EE RETURNED TO WORK 9/18/03.
CONTRACTOR WAS PULLING VACUUM HOSE TO INSERT INTO TOP OF RAIL CAR, FELT HIP/BACK POP. DR. PRESCRIBED PAIN KILLER AND ANTI-INFLAMATORY, ORDERED ONE WEEK OFF WORK THEN RETURN TO DR. FOR EVALUATI ON. HE IS DUE BACK TO WORK ON SEPT. 22ND. HE WILL BE ON LIGHT DUTY FOR APPROXIMATELY ONE WEEK.
EMPLOYEE TRIED TO CLEAR AN ELEVATOR WITH A METAL BAR WHILE ELEVATOR WAS RUNNING. HE FAILED TO SHUT DOWN AND LOCK OUT. THE ELEVATOR CAUGHT THE BAR, THE BAR CAUGHT HIS LIP AND RIPPED THE SKIN OF HIS FACE.

EMPLOYEE WAS ASSISTING IN MAKING A HIGH-PRESSURE HOSE. HE WAS HOLDING THE HOSE WHILE ANOTHER EMPLOYEE WAS CUTTING THE NEW HOSE WITH A HACKSAW. THE SAW SLIPPED AND CUT THE OPERATORS FINGER TIP. IT REQUIRED FIVE STITCHES TO THE MIDDLE FINGER OF THE RIGHT HAND AND AN UPDATED TETANUS SHOT.
EMPLOYEE WAS GETTING READY TO DO SOME WORK IN THE KILN, CRAWLING THRU HANDRAIL TO GET TO THE LOCATION IN THE KILN, WAS TIED OFF, AS HE STEPPED THROUGH HIS KNEE GAVE OUT, EE DID NOT GO TO THE DOCTOR UNTIL 7/25/02. STRAIN TO RIGHT KNEE, DOCTOR PRESCRIBED ARTHROTEC AND HAD HIM WEAR A KNEE IMMOBILIZER.
EE WAS MOVING A TRASH BARREL. THERE WAS A SHARP EDGE WHERE THE LID HAD BEEN CUT OUT, CUT THROUGH GLOVE INTO RIGHT FINGER. REQUIRED SUTURES.
EE AND ANOTHER ELECTRICIAN WAS REPOSITIONING OVERHEAD LIGHT, ONE WAS WORKING DIRECTLY ABOVE THE OTHER, AFTER HE HAD REPOSITIONED THE LIGHT AND MOVED, THE LIGHT FELL AND HIT EE, CAUSING BRUISE AND SMALL PUNCTURE WOUND TO RIGHT SHOULDER/COLLARBONE.
EE WAS TRYING TO UNPLUG THE SECONDARY CHUTE WITHA PIPE, THE LOAD CAVED IN, HITTING THE PIPE AND JERKING IT OUT OF HAND, WHEN IT FLIPPED UP AND STRUCK EE IN THE JAW. BROKE OUT A FRONT TOOTH.

EE WAS POKING ROCK THAT WAS PLUGGING UP THE CHUTE AT THE ROCK TOWER. HIT HIS ELBOW ON THE TRANSITION SOFT TISSUE INJURY/INFECTION

EMPLOYEE OPENED AIR VALVE AND A HOSE THAT WAS UNCONNECTED WHIPPED UP HITTING HIM ON THE LEFT SIDE OF HIS CHIN. LACERATION, REQUIRING SUTURES.
EE WAS WALKING OUT OF THE MILL BUILDING AND SLIPPED ON THE ICE. POOR WEATHER CONDITIONS. EE DID NOT GO TO THE DR UNTIL 2-25-00. WENT TO CHIROPRACTOR/DIAGNOSIS GRADE 1 SPRAIN TO RIGHT SHOULDER.
CLOSING HANDLE ON RAILCAR. THE CAP WAS HANGINBG DOWN AND SMASHED FINGER BETWEEN VALVE HANDLE & CAP.
