Found an issue with the 5th floor door flag so it was adjusted. They also adjusted the contacts on the flag on the 7th floor. They then rode the elevator to confirm the issue was resolved. Once they were comfortable, they turned it over to the plant operations to resume operations and monitored the elevator form the machine room for some time to confirm everything was working.

Employee sustained a laceration to the left ring finger while attempting to remove a grinding-aid lance from #3 Finish Mill. Employee was taken to the ER for further evaluation and received 4 stitches.
After a series of self-care measures w/out relief, progressive medical treatment includes PT as of 7/31. On 4/17, emp. went to pick-up a piece of 3x3 angle iron and reported discomfort in EE's elbow.
During the process of moving railcars an experienced employee needed to maneuver a rail switch to change the track. The shaft of the rail switch was not easy to maneuver, so the employee applied additional force by stepping on the switch. The switch went into place unexpectedly and the employee felt/heard a ""pop"" in left calf. The Employee was given a prescription medication.

The employee was descending from the 9th floor on the Preheater Tower elevator when EE heard a noise and the elevator stopped. No injuries to the employee.
Employee was carrying a belt down the incline at 311.BC1 when EE slipped on a rock and fell onto their right knee. Right knee is bruised and swollen.
EE was attempting to clean out airslide when EE got material in eyes and nose.
Employee was shoveling gypsum from the top of the #4 Day Bin building. In doing this task EE felt a pain in EE's back. The employee visited the doctor on EE's own and was prescribed a medication.
Employee was working to clear an air slide, when the air slide cleared material came into contact with the employee's face. Employee was wearing safety glasses, but material came into contact with left eye. The employee was taken to the eye doctor who discovered minor burns to the eyeball and prescribed an antibiotic eyedrop.
The employee was lifting the tool bag out of the locker when employee felt a pain in left groin area. The employee had completed morning stretches prior to this event. Employee was seen the next day at the clinic where a 5-10 lbs lifting restriction was placed on employee to aid in the healing process.
The employee was cutting vac hose with EE's pocket knife. EE was not wearing gloves and nicked the bottom of EE's left thumb. 3 stitches were required.
Elevator lost electrical communication causing elevator to stop. Miner attempted to use descending lever and felt elevator was moving at a slow rate of speed. The supervisor on the ground could not definitively tell it was moving. This ceased operation of the lever and emergency rescue was notified.
Employee was walking up the stair when employee felt a pop in right knee. The incident was originally a first aid only, on 6/6/23 the employee was sent for an MRI which showed a tear in the meniscus. Physical therapy was ordered.
The employee was exiting the dozer and employee's foot was caught in the step. Due to the uneven terrain when employee freed foot, the rock around employee gave out allowing ankle to roll. Employee felt a pop, after X-rays employee was diagnosed with a fibular ankle fracture.

Our employee fell in a railcar EE was cleaning and as EE fell the pressure washer gun (who'd trigger was taped) slipped out of EE's hands and the barrel swung around and caused a water cut to EE's left lower bicep requiring 10 stitched and antibiotics for potential infection
Employee got dust in eye, proper PPE was being worn. Employee received a prescription for antibiotics due to an abrasion on left eye.
EE smashed left foot causing broken bones while replacing tilt cylinder seals on 992 loader when left dog bone (bucket link) impacted foot between pin and bucket.
Employee was descending down stairs and missed a step. Grabbed the handrail, twisting left shoulder. EE consulted nurse and treated with first aid. Discomfort persisted, placed on work restrictions 10/5 after clinic visit
EE sustained abrasion to cornea after exposure to dust in eye. EE was given antibiotics. EE was going to take sample from port and had PAPR face shield up at time of dust exposure.
EE got dust in their eye. EE flushed eye at plant, but continued to have irritation. EE was seen at local clinic and foreign body was removed. EE was prescribed anti-inflammatory eyedrops to aid in recovery.
EE got left thumb caught in pinch point while opening access door to equipment. Required stitches. No other injury beyond laceration. Restricted duty only.

Mechanism of injury occurred on 10/26/2021. EE knelt to pick up debris on belt line walkway. Felt discomfort in right knee. Seen at clinic treated first aid. 6/17/22 Pain present. EE diagnosed by MRI slight tear to ACL. No restrictions. 7/7/21 arthroscopy to right knee.
EE went to lift a large tote of grease. EE experienced no pain at the time of the lift. On 9/7/21 the employee reported to supervisor that they had a protrusion near belly button. The employee was taken to the clinic and diagnosed with an umbilical hernia. EE is on restrictions awaiting further care.
EE was lowering mill liners into the mill to stage the liners for installation. While lowering the mill liner into the compartment, the liner became bound while passing through the opening in the mill. The EE began to pull on the liner to free it and allow it to lower through the opening. EE then felt pain in the muscle near EE's right shoulder.
Employee strained right knee while walking in plant on 6/26/21. Was seen by local doctor and treated with first aid. 09/09/21 Pain returned and EE stated the knee was never totally recovered. Was seen by doctor and placed on restrictions.
EE was using 1/2"" impact equipped with a swivel adapter and 1 inch socket. While the socket was not seated on a bolt EE pulled the trigger. The socket came off hitting EE in the front center incisor, cracking it. A medical assessment was conducted on 3/5. Tooth repair did not occur until 5/17.
Area received large amount of snowfall during week. Snow was packed from equipment repeatedly driving over it. EE exited loader. While walking away from loader EE slipped on packed snow. As EE fell backward they went to catch themselves with hands. Sprained left shoulder.
EE with history of chronic back pain bent over to attach chain hoist to a 8"" flex hose to lift it for installation. While bent over EE felt pain in back. EE treated with first aid for 24 hours. On 2/04 pain worsened and employee received medical care at local clinic. EE was prescribed anti-inflammatory medication and 2 weeks light duty.
EE reported illness to management on 01/04/2021. EE stated: was experiencing pain in right shoulder. Pain was said to come from repeated use of computer during week of 12/14/20. Employee spent a few hours each day during the week watching online training modules and taking quizzes. The pain was said to be caused from repeated use of mouse.
EE was pulling scraper out of belt line. EE had ribcage pressed against belt assembly. The weight of the scraper was heavier than employee anticipated. Before EE let go of the scraper EE felt a strain in EE's ribs. After medical assessment it was treated as a first aid. On 1/6/21 the pain was still present and the ee returned to the doctor. Was placed on light duty until 1/10/21.

EE was at a workbench attempting to seat a rubber gasket over a baring race. EE was striking the gasket with a hammer. The tool slipped and hit the tip of EE's left middle finger, causing blood to pool beneath. EE was taken for medical treatment where they drained the blood from under the nail. An X-Ray was performed; no broken bones were present. There were no restrictions.
EE received 2nd degree chemical burns from high PH material that released from the main conditioning tower during plug clearing. The EE was standing ~10 feet away, material fell from the tower, exited the vessel, hit the deck, and splashed onto EE. The material impacted upper chest, neck, cheek, and forehead. Eyes and mouth were protected by a N95 mask and safety glasses.
EE was entering the coal mill to preform work. The trunnion roller assembly was rolled out for maintenance, this created a small void near the assembly. As EE entered the mill left foot became stuck in the void. EE lost balance. EE was able to catch themselves partway through the fall but not before a substantial amount of pressure was applied to their shin breaking their fibula.
Employee was using a porta-power sitting on a stand to push out cooler floor grate plates. The porta-power became cocked on the stand and the stand hit the employee in the mouth, chipping a tooth.

Trying to remove a bolt from an attachment point and when the hammer struck the bolt driver a piece of metal had projected from the point of contact and struck the employees arm.
Employee was preparing a tank for inspection by a third party vendor. As they were exiting the tank their leg contacted a sharp stationary piece of metal causing a cut to their lower leg.
Employee was walking through work area and stepped over a concrete form board, stepping onto a rock causing the employee to roll their ankle and fall to the ground.
Employee was driving a re-bar stake into the gravel to set height for concrete forms when they missed and struck their hand on the top of the re-bar.
EE attempted to move a bolder that had rolled into path of loading haul trucks. EE felt a pop and a burning sensation in left bicep as they pushed the rock. EE was able to operate the loader and continue work without restrictions. A burning sensation continued and surgery was required. EE was not able to return to work as of Jan 2 2020.
While inspecting some old bucket conveyor the employee reached for a broken pin. After grabbing the pin the buckets shifted and pinched their finger.
Employee was with a crew removing demo material. EE was outside the hole removing buckets that the crew inside was filling up. As EE reached inside to remove a bucket the crew inside shouted at EE to watch out because a chunk of refractory material fell from side wall above door. Due proximity of the falling material, EE was unable to avoid being struck in the left hand.
Employee attended the 7 AM safety meeting. After meeting they were walking out, came back in to trailer indicating they could not catch their breath. Called plant safety. Administered first aid, called ambulance, was taken to hospital. Employee was diagnosed with anaphylactic reaction.

Walking from 725 Haul Truck to the fuel dispenser miner stepped in rut in the ground and twisted ankle. Diagnosed with strained or partially torn ligament and put in a walking boot for 2 weeks. Work was restricted as to standing and walking activities but not restricted as to driving or operating equipment.
Employee was preparing to grab a sample of kiln feed on the Pre-Heat Tower when an air cannon fired and blew dust out of the sample port and into the employee's right eye.
Employee was working on unplugging a coal line. In doing so a pulse of compressed air passed through the line and broke the impacted coal fines loose. This resulted in coal fines being blown from the line, striking the employees hand and then being directed towards their face, with some coal ultimately entering their left eye.
Driller and mechanic were performing drill maintenance/repair when driller began to double over attempting to vomit. Chest was sore and left arm was numb. Mechanic rushed them to the ER at Chanute Med. 1 hr later, mechanic was told that driller had a heart attack and was going to Freeman Med in Joplin, MO. Driller was placed in ICU. On 2/17, employee was released.
Employee was sorting 1 1/4"" steel, mill balls used to grind cement in a roller mill when a stray mill ball rolled down the chute incline and ramped up towards the employee who was in the process of securing the chute discharge to the upright position. As such, the steel mill ball struck the employee in the mouth.

Employee trying to free up a stuck rail car door along with another employee. The door was freed up and closed. In doing so the employees left hand had gotten caught in a pinch point of the door mechanism.
Employee was working on installing some Mill Liner Plates in a Finish Mill. They reported soreness in their lower back on the right side. The employee does not recall a specific incident that lead to the injury.
The EE reports that they have been experiencing soreness and numbness in their hands and wrists, which has been occurring off and on for a few months. The employee is unable to provide a mechanism or singular event that lead to this condition. Employee was seen by a medical professional and was diagnosed and treated for Carpal tunnel Syndrome.
Employee received a laceration to their left thumb from broken glass. The laceration severed a tendon which was repaired surgically through an outpatient procedure.
Employee was removing table liners from the coal mill. One of the liners had a strap around it so that it could be lifted with the hoist. The employee was taking up slack from the choker when the liner tilted and fell over, striking the employee's leg.
Employee was lining up a punch and die set in a Piranha Ironworker. In doing so, the set up was not aligned and the punch came into contact with the inner edge of the die. As the employee bumped the machine to recheck alignment the pressure of the punch and die contact caused a small sliver of metal to break off and strike the employee's left cheek.

Employee was walking around skid steer and tripped over the edge of the bucket and feel to the ground. In the process of falling the employee attempted to grab a handle on the equipment and received a laceration to their thumb caused by a sharp metal burr on the handle.
Connecting a hydraulic line when they felt a ""pop"" and pain in the left side of their sternum.
Struck by metal bar while performing tower rodding duties. Material fell inside the preheat tower and struck the end of the bar causing the other end to raise and strike the employee.
Employee slipped and fell (same level) while working inside a rail car and needed assistance exiting the car.
When chipping refractory wall using chipping gun, a small piece of refractor/slag fell hitting miner on tip of thumb.
On 2/25/16, the employee was struck in the mouth by an object affecting the upper right 3rd tooth from center. On 3/16/16, the employee returned to a follow up visit with a dentist and a temporary crown was put in place. As such, making this a reportable injury.

On 1/19/16 the employee slipped on the ice and fell as they were leaving work at the end of their shift. On 1/25/16 the ee requested medical treatment as there was minimal improvement to their condition. The employee was seen by a medical provider and diagnosed with a fractured rib. The employee was given prescription medicine making this a reportable injury.

While grinding during the course of the day, it is believed that a piece of metal landed on employee's eyebrow or eyelash and worked its way into the eye after leaving plant.
Incident allegedly occurred on 10-26-12 while laying out belt in hopper. Incident not reported until Monday, 10-29-12. Doctor appointment scheduled employee seen on 11-1-12. Irritation of the patella bursa sac of left knee. Rx issued restricted days until next scheduled appointment of 11-27-12.
Knee twisted and popped while in awkward position. Originally reported 8/19/12. Employee moved refrigerator for mother on 10/20/12 and reported irritation. Scheduled medical examination on 11/1/12. Outpatient surgery on 11/21/12. Released light duty 11/22/12.
Reported STS hearing Shift. An interpretation to the Audiologist is pending. It will be supplied as supporting evidence to rescind this action, should it support non-work related cause of loss. Hearing report received at plant on 3/27/12.

Employee received a small metal splinter to the right forearm on 9/14. He completed the company incident report on 9/22/10. At that time it was thought the splinter would work itself out. On 10/7/10, employee was seen by doctor and had splinter removed.
Employee missed last step on stairway, landed on right knee. Continued to experience pain had routine medical examination found minor tear in meniscus. Outpatient surgery on 4/23/10. Returned to full duty on 4/26/10.

Foreign material in right eye. Didn't report event until next shift. Treated on 9/26/09.
EE involved in minor vehicle accident on 9/19/09. Reviewed at time, had no complaint of injury. EE described back spasms on 9/26/09. EE attended by doctor on 9/29/09 (days off) event became reportable.

EMPLOYEE WAS CLIMBING DOWN FROM A 6' HIGH SCAFFOLD PLATFORM WHEN HE SLIPPED AND FELL ABOUT 3' TO THE GROUND. HE STATED HE LANDED ON HIS BACK AND RIGHT SIDE THEN ROLLED ON TO HIS STOMACH WITH H IS LEFT WRIST STRIKING AGAINST A PIECE OF ANGLE IRON LAYING ON A PALLET.
EMPLOYEE WAS ATTEMPTING TO ADJUST THE BLADE GUIDE OF BANDSAW WHILE THE SAW WAS RUNNING. HIS GLOVE WAS CAUGHT BY THE BLADE AND PULLED HIS RIGHT THUMB INTO THE GUIDE ROLLERS AND BLADE. EMPLOYEE RECEIVED A FRACTURED RIGHT THUMB AND SUTURES WERE ALSO TAKEN.
EE AND CO WORKER WERE HANDLING A 40' SECTION OF 2"" PIPE. AS THEY WERE PASSING THE PIPE THROUGH A RESTRICTED AREA BETWEEN TWO STAIR STEPS, CO WORKER PULLED TOO HARD ON HIS END OF THE PIPE, CAUS ING THE OTHER END OF THE PIPE TO FALL STRIKING EE ON THE TOP OF HIS HAND.
EE WAS USING A CUTTING TORCH TO CUT A CHANNEL IN TWO SUPPORT BEAMS WHEN HOT METAL SLAG FELL INSIDE THE TOP OF HIS BOOT. HE DID NOT REPORT THE INCIDENT TO THE SAFETY DEPT. UNTIL 5-1-01 WHEN IT WAS APPARENT THE THERMAL BURN HAD BECOME INFECTION.

EE WAS WORKING OFF OF A 8 1/2' SCAFFOLD PLATFORM. HIS FALL PROTECTION LANYARD WAS ATTACHED TO AIR CHUTE VALVE ASSEMBLY LOCATED OVERHEAD. HE AND CO WORKERS HAD JUST FINISHED DETACHING AN AIR SL IDE FROM THE VALVE ASSEMBLY WHEN THE ASSEMBLY FELL DOWN FROM ABOVE. BECAUSE EE WAS TIED OFF TO THE ASSEMBLY. IT PULLED HIM OFF THE SCAFFOLD ONTO THE FLOOR.
EE WAS CUTTING CONCRETE USING A MAGNESIUM ROD CUTTING SYSTEM WHEN A PIECE OF CONCRETE FELL FROM A SCAFFOLD PLATFORM LOCATED ABOVE THE NEXT FLOOR LEVEL. THE PIECE OF CONCRETE STRUCK HIM ON THE RIGHT SHOULDER AREA.
EE WRAPED HIS FALL PROTECTION LANYARD AROUND A DIAGONAL WIND BRACE & CLIMBED TO THE TOP OF IT TOTHE NEXT LEVEL OF THE PREHEATER STRUCTURE (~20')AS HE ATTEMPTED TO CLIMB UP ONTO THE FLOOR FOR T HAT STRUCTURE LEVEL HE LOST HIS GRIP, FELL BACK,AND BEGAN SLIDING BACK DOWN THE BRACE. HE FELL ~8' LANDING ON RIGHT FOOT.
EMPLOYEE WAS HELP BUILDING BALL PENS UNDER THE MILLS, AND ONE END OF THE RAILROAD TIE WAS DROPPED WHILE EMPLOYEE WAS STILL HAD THE OTHER END AND SPRAINED HIS LOWER BACK. EE FOLLOWED UP BY GOIN G TO A CHIROPRACTOR.
EE WAS ON HIS KNEES, PULLING ON A PIECE OF STEEL CHECKER PLATE THAT HAD BEEN CUT TO FIT UNDER A GEAR BOX AND CONVEYOR. AS HE PULLED THE PIECE OF PLATE TOWARDS HIMSELF, THE TIP OF HIS FINGERWAS PINCHED BETWEEN THE PLAT AND A SUPPROT FOR THE GEARBOX.
