Struck by falling object

EE was fabricating frame work in the fab area. A piece of steel fell over while EE was kneeling and grinding on the frame. The piece struck EE on the back of the head between the top of the welding helmet and the strap requiring stitches due to head laceration.

EE was working on an engine mount when the wrench slipped and struck the EE in the R eye causing a laceration and bruising.

EE was walking on 2"" rocks next to rail cars and felt a pain in the bottom of EE's foot. EE is in a walking boot that is for immobilization at this time causing it to be reportable. EE is performing EE's job functions. Possible stress fracture- waiting for further appointments.

Struck by falling object

Employee was welding on the bottom of a railcar. All PPE was being worn but a piece of slag dropped from the weld and rolled down EE's forehead and into EE's right eye coming to a rest on the bridge of EE's nose. Reportable due to prescription eye ointment applied. Released to work 4/22/2025

Fall from piled material

After shoveling out the 100 mesh pit employee slid into the pit and landed on their right knee. This jarred their back causing pain that showed up the next morning.

Pry baring door open on hopper car, gate was stuck and trying to break free. The bar slipped and EE fell and hit the ground.

EE was cutting zip ties with a knife and when EE cut through the zip tie EE jabbed self in the forearm. EE needed stitches and was released back to work with no restrictions.

EE was underneath the surge bin trying to free frozen material by hitting the bin with a sledgehammer. On one of EE's swings the sledgehammer bounced back at EE striking EE in the right shoulder.

Fall to the walkway or working surface

Employee was walking to the back of their truck to get something out and did not lift a foot high enough and tripped over the railroad tie that was used for parking spots in the parking lot. They fell on their right shoulder hurting it.

EE states EE got a broken hand on 8/21 but did not report it until 8/26 after termination was decided due to attendance. EE states EE hit EE's right hand on a rib while torquing a ground support nut. EE went to the ER and had a fracture on 8/24 but did not call anyone until 8/26 at 8:45 am post no call no show.

Over-exertion in lifting objects

While replacing the head bearings on BL_CV05 EE thought they did something to their back. EE was in the process of lifting the bearing from their belt level to mid-chest when this happened.

EE was cleaning out a grizzly and the scalper fell and caught 2 of EE's fingers. EE broke the tips of middle and index finger on left hand. Reportable due to fracture. No Missed or Restricted days.

EE was driving a semi and had a pain start in EE's lower right side. EE has been removed from work by the Dr. until 4/22 when a CT is scheduled. Undetermined if personal or work at this time. EE states roads were very rough and thinks injured self. To be determined but filed due to the 10 day rule.

EE was lining up an industrial pipe and used the back of EE's leg to help align it. EE felt a pain in left knee. Not reportable to MSHA until 2/21/24 when EE received a cortisone injection and medical treatment. Prior was able to work normal job duties and no medical treatment.

WHILE PUSHING SCREENS UNDER SEMITRUCKS FOR SAND GRADING THE SCREENS KEPT FALLING OFF AND NEEDING TO BE PUSHED BACK. THIS REPETITION CAUSED EE'S WRISTS TO HURT. EE HAS A SPLINT FROM THE DR.

While pulling the lid back on rotex 6 EE stepped in an open spot under the rotex and caught self with EE's left elbow on top of the machine. Did not seek medical attention until 11/20. No restrictions to job duties, but medical treatment (Therapy) was ordered 12/11/23. Opened due to therapy for nerve in the elbow.

EE was cranking a come-along that was attached to a floor panel. The floor panel broke in the Dryer and the come-along hook came back hitting EE in the mouth. Stitches, Broken Teeth and Fracture to bone in mouth.

EE reported that EE's right heel was sore when walking on rough/hard ground at work. Went to the Dr. 4/14/23 and will not have missed work days pending an MRI due to a walking boot and not being able to wear proper PPE.

E did not want to see the Dr. until 4/18/23 when EE called and said it was sore and wanted to go to the Dr. EE was climbing off the cat-walk of the truck when the side bracket that holds the 2 steps together broke and EE fell from the bottom step to the ground. Seeing Dr. on 4/19/23.

EE was cleaning sand away with a mud buggy and the bucket caused the buggy to kick back hyper extending their knee.

Employee was driving a semi tractor trailer. Driver started to complain of back back from being jostled around in cab.

EE was walking from the office and when EE stepped down the steps and turned, EE felt pain in the inside lower part of the left knee. EE did not feel EE needed medical attention until 1/30/23. EE notified HR on 1/27 while EE was on vacation that EE's knee hurt and wanted to see a Dr. when EE got home. Released to full duty but given a brace for the knee at this time.

EE hit a pothole while driving a semi-truck and bounced in the seat. This jarred EE to the left and EE hit shoulder against the driver's side window.

Caught in, under or between a moving and a stationary object

While guiding the truck & trailer back to unload another car- ee crossed between the rail trailer and 2 rail cars that were already placed on the track. The cars on the track rolled toward the trailer pinching ee's right thigh.

Unloading rail car, grabbed the hook to unhook the rail car and set it on the trailer. Stepped off trailer and lost balance and fell to the ground on right side.

While changing screens sand fell past EE's safety glasses and got in eye. Only reportable due to a prescription ointment for EE's eye.

Stepping out of a 650 exc. EE missed footing when stepping on the track and missed with foot. EE fell to the ground on right side/back.

The operator lost control. Root cause is unknown at this time due to the on-going accident investigation. We are waiting for the monetary amount from the insurance company and the mine operator.

REPAIRING A SAND LINE CRUSHING BELT CAUSED SOME SAND TO FALL OFF AND GET INTO EE'S EYE CAUSING IRRITATION. EE DID NOT FEEL NEEDED MEDICAL ATTENTION UNTIL THE NEXT DAY WHEN IT STILL HURT. 9/8/22 EE SAW DR. AND GOT A PRESCRIPTION FOR EYE AND NO FOLLOW-UP WAS NEEDED. RELEASED TO FULL DUTY. FILED WITHIN 5 DAYS OF MEDICAL TREATMENT TO MSHA 9/12/22.

EE went to a personal Dr. for carpal tunnel syndrome on 7/20 where the injury deemed was most likely work-related due to the repetitive motion of EE's wrist while fabricating. There were prior reporting's for personal concerns with EE's wrist before 7/20, but no injury or cause was determined until 7/20. EE at Duluth stated to use DOI as 7/20 and not any prior date.

WHILE CHECKING THE BOTTOM OF A RAIL CAR EE FELL BACKWARDS. WHEN CATCHING SELF EE HIT ANKLE ON A ROCK.

While hanging mesh from the JLG onto the mine ceiling EE overreached and stretched EE's shoulder causing injury. Removed from work on 6/28 to have therapy and a sling.

EE was grinding and when they raised their face shield to use the hoist after completing grinding some debris fell from the rim of their hard hat and into their eye. Full release, no restrictions. Received an ointment in the eye at the Dr.

EE was changing out a pipe and was pushing it with EE's right arm to get it into the right location while reaching to move another pipe with EE's left arm. EE heard a pop and felt pain on EE's right side.

Struck against a moving object

While operating 3331 600 loader EE's back began to hurt and right leg went numb. Did not want to see the Dr. and continued to work as EE saw improvement. On 3/10/2022 EE was in pain and wanted to see a Dr. EE went to the ER on 3/11 and was removed from work at that time. Became MSHA reportable with missed day on 3/11/22.

EE believes they stepped wrong on a rock or rolled a window up and turned wrong hurting back. EE did not notify anyone until 2/7/22 stating they wanted to see the Dr. EE is now seeking medical treatment of therapy. Saw Dr. 2/7/22 and the therapy ordered made it reportable.

LIFTING SCREENS TO REPLACE FOR REPAIR IN THE MEGATEXTES AND FELT A TINGLING IN EE'S BACK. MEDICAL APT. ELECTED TO GO ON 1/21/22.

Struck against stationary object

EE WAS REMOVING A HOUSING CAP WITH A BREAKER BAR ON THE FLETCHER DRILL WHEN THE CAP LET LOOSE AND EE'S ELBOW STRUCK THE BOLT ON THE ENGINE DOOR. EE FELT FINE AND DID NOT NEED MEDICAL ATTENTION. 4/15/22 EE REPORTED PAIN IN ELBOW AND THAT EE HAS BEEN HAVING TROUBLE SINCE INCIDENT WITH FEELING. SAW THE DR AND WAS ISSUED THERAPY 5/9/22. FILED WITH MSHA 5/11/22.

EE was knocking a rock that was stuck in the chute of the screener loose with a sledgehammer. When EE was done doing this EE's arm went numb and had pain.

EE stepped onto a rocky surface and rolled EE's right ankle causing a fracture.

Fall from scaffolds, walkways, platforms

While shoveling sand EE turned and leaned on a metal rail on a platform at a height of about 15 feet. The railing snapped and EE fell from a platform straight down and hit the cement. EE has missed work days at this time only.

Absorption of radiations, caustics, toxic and noxious substances

EE was stung by a bee exiting EE's work truck. EE had an allergic reaction and had to go to the ER. EE received medication to stop the reaction and was removed from work for 1 day due to the side effect of drowsiness.

EE was pushing bolts and felt a pop in EE's back. EE did not have any pain immediately but EE's back started hurting overnight. EE reported 7/7/21 that EE was sore and wanted to see a Dr.

While hammering on a brushing mill head a piece of metal came off the mill head and embedded in finger. Did not have medical treatment until 5/7/21 to remove it. Released post removal with no restrictions.

EE was walking across the parking lot and slipped on packed snow.

EE was assisting in loading holes for blasting and when done loading holes right shoulder was sore. This incident was not reported to a manager or HR until 3/4/21. After they went to personal Dr. and had been in therapy for 2 weeks. Due to therapy we are reporting.

EE was climbing out from underneath a loader and felt a pinch in back. EE did not notice anything else until the next day when they had pain. EE did not seek medical attention until 11/23/20 where they had therapy and light duty. Reportable 11/23/20

Struck by falling object

EE was placing a pin on to a shelf when EE's hand slipped off the pin. The pin hit the shelf and proceeded to fall off and land on top of EE's right foot.

While EE was performing work of replacing a hose on a piece of equipment and also driving a pin back in EE got material in eye causing irritation. After multiple flushes it was still irritating EE and EE was taken to the doctor to have it examined- no foreign body was currently found or abrasion. EE received drops for eye and was released to full duty.

EE was placing a bolt on a railroad tie, a co-worker was using a hammer to place the bolt and inadvertently struck the EE between the eyes about 1 in. above the eyebrows causing a laceration that required stitches. Return to work full duty. Reporting due to stitches.

EE notified HR on 9/4/20 when HR called to verify EE was not continuing employment that EE received stitches on 5/12/20. EE was unable to provide the location of the incident or if there were any witnesses. EE was cutting and clearing trees and stated the saw was turned off but a limb kicked it back and struck EE in the leg giving EE 5 stitches.

EE was pounding a spike into a railroad tie, EE had the plate and spike placed on the tie. When EE swung the hammer, the plate and spike shifted causing EE to strike left thumb at the joint.

EE was unloading a wire spook from the back of a truck and as the spool rotated it placed a metal sliver from the wire in wrist/palm area. EE requested medical attention on 4/3/20 and was released to full duty. EE received 3 stitches making it reportable.

EE was using a maul to hit the side of a railcar hopper that was retaining sand. On a swing EE's finger was caught between the maul and the railcar.

EE was moving the genie lift when they hit a bump or rock. The jack leg came sliding across the basket. EE tried to catch themselves, but missed and hit left wrist on the bar.

EE was pushing bolts when EE began to feel soreness in right shoulder.

EE jumped down into a railcar to remove a piece of debris. EE landed on a rock and rolled EE's ankle.

EE was walking on the cat walk in the Rock Cut and felt their lower left calf muscle pull.

While EE was using a hammer a piece of the hammer broke off and cut EE's lower left arm. EE received 4 stitches and returned to work.

EE was on restricted duty due to a personal injury to shoulder. EE felt a pop in shoulder climbing on the Titan. Investigation by Workers Comp. determined that the injury did not arise out of, or within the course and scope of employment 9/10. On 9/26 HR was notified by Workers Comp. that further investigation caused them to reverse the decision; injury occurred at work.

EE was stepping back to grab a radio and tripped self. EE fell back caught self with right hand. EE has been issued a splint from the DR. for a sprained wrist.

EE was shoveling on the west side of bucket elevator 11 on dry plant floor when EE's left arm scraped against the tail bearing guard cutting arm.

EE was attempting to clean out material built up in the tail pulley. While trying to free up the drop door dropped down knocking over the EE and landing on their leg causing injury.

EE possibly getting dust in eye causing an irritation. EE received eye drops making this reportable. EE stated EE worked in an area where dust was present then 3 weeks later notified HR that EE wanted to see the Dr. This was not reported immediately and is under review with Work Comp at this time. Date reported was 5/15/19 and dr was seen 5/16/19.

EE had to chip out frozen material in conv. 11's tail pulley. Due to the fact the cover does not come off EE was on the ground working overhead while pushing the electric chisel into material.

EE was shoveling at the Aggregate Crushing Line and felt lower back pain.

EE was jarred in the seat when EE's piece of equipment was bumped by another piece of equipment.

Using an impact gun to install transmission mounts, the gun pulled EE's arm into the transmission causing EE to strike EE's elbow against the transmission.

EE was cleaning out shotcreter when EE lifted the grate up and slammed it back down to knock off shotcrete. EE's finger got caught between the grate and grate support bracket as it came down.

EE and another co-worker pulled mesh to the kobota for them to take it to the heading in 22AX. When arrived at 22 they got out and unhooked the mesh, got back in the kobota to park it. EE then proceeded to walk to the JLG lift when groin started hurting on the right side.

EE picked up a cone and felt pain in EE's abdomen when sitting or bending over.

EE was pushing bolts. When lifting the bolt in the back to push it with the jackleg. As EE was lifting EE's shoulder started hurting. Now has a sharp pain in shoulder.

EE and another coworker were picking up and moving ballard's. When ee put down a ballard ee felt a sharp pain in lower back.

EE WAS LOADING A FACE WITH ANFO AND THE PRESSURE BUILT UP IN THE HOSE DUE TO ANFO NOT FLOWING PROPERLY. THE HOSE CAME OUT OF THE HOLE AND STRUCK EE ON THE FACE CAUSING A FRACTURE TO THE NOSE

EE was wearing safety glasses & when they removed the glasses they got something in their eye. Safety glasses had some sand on them as well as their gloves. Some sand got into the eye casing an abrasion on the eye. EE received a prescription for the eye.

EE was performing work under a vehicle and due to the limited space, EE was required to remove hardhat. Upon exiting EE lifted EE's head and struck head on the bottom corner of an open door. Results were 6 staples on top of EE's head. No restrictions or loss time.

EE was attempting to secure a second clamp for a wire mount when the drill bit gripped the strap causing it to strike EE in the face and upper lip. EE received stitches on the lip and was released to work with no restrictions

EE was replacing a sheer pin on a semi-trailer 815. EE caught finger on the bolt which resulted on laceration. EE received stitches and a splint.

EE was cutting a dust guard when the knife slipped and cut their knee. EE received stitches and returned to work with no restricted activity.

EE was exiting a skid loader and when EE was close to the bottom and stepping down EE's anchor foot slipped and EE fell forward towards to ground. As EE hit the ground EE tripped over the track on the skid loader causing EE's knee to pop and EE fell to the ground. First aid was administered onsite. EE notified HR that EE wanted to seek medical attention on 3-26-18.

While stepping off of a ladder EE stepped onto a small rock that was at the base of loose material on the ground causing EE's foot to roll. First aid was provided at the scene and EE was taken for follow up medical evaluation.

EE was running a jackleg performing bolting when the pressure of the jackleg pushed on the basket of the JLG and tipped it towards the ground. When the basket impacted with the ground the jackleg struck EE in the right shin causing a laceration. First aid at the scene and EE was treated at clinic with staples and released. No restrictions or missed days.

Following milling and scaling, the employee was working at the face to prepare a heading for a shot. While performing the work, a rock fell from the face, striking the employee in the leg. First aid was provided at the scene and the employee was then transported to the local hospital for further treatment.

The employee slipped and fell on rough and frozen ground as EE stepped from a telehandler, resulting in a strain to EE's left ankle and shoulder. EE did not seek medical attention until 2-21-2018.

Struck by falling object

EE was moving a screen and when pushing a different screen one came back down and landed on another screen hitting EE's tail bone.

EE was loading the mobile conveyor, EE was chaining the front of the conveyor back by the tires on the lowboy trailer and as EE went to step off the back of the lowboy EE slipped and fell. 11/27/2017 Therapy was ordered and MSHA was filed.

EE was pulling a screen to life into a telehandler and pulled back while performing this task. EE did not seek medical attention until 11-20-17. Notified HR 11-20-17 that ee wanted to see the DR. Filed 11-22-17 within 10 day time frame.

EE was kneeling down on steal grating to get some bolts out of the bolt bin and EE felt something in knee pop as EE knelt. EE did not seek medical attention until 8/2/17.

EE was pulling self out of a rail car and heard shoulder pop. EE did not seek medical attention until 10/24/2017. We are filing this 11/2/2017 within the 10 day reporting window.

EE was opening railcar lid, as EE raised the lid, the lid got stuck on the spout causing the lid to bounce and hit the top of EE's right hand.

About 1 hour after EE started cleaning out rail cars started feeling light headed and dizzy. EE realized the heat was causing issues and stopped sweating. EE got help and was taken to the ER for heat exhaustion. Released back to work the next day.

EE used brake cleaner and Krol oil to clean a shaft. The product pooled in the key way and when EE inserted the key; the product shot up into EE's eyes. The chemical went under EE's safety glasses and prescription glasses. Reportable only due to prescription medication for eyes.

Un-banding switch ties. Cut one band and was repositioning self to be clear for the next one when band snapped prematurely, causing ties to roll onto left foot.

EE was loading some 4x4's into trailer of the semi and when EE turned to walk towards the trailer EE tripped and fell down striking the left elbow on the ground.

EE reported on 2-15-17 that shoulder was sore and that it is from doing the same movement every day. 2-14-17 was when it hurt the most. EE did not want to see the doctor and felt it was going to be ok. 4-10-17 EE reported to HR that EE was still having issues. An apt was scheduled for 4-11-17. EE received restrictions and return to work as of 4-11-17. Report filed 4-17-17

EE was scaling when trying to get a loose rock out the scaling bar slipped out of the pitch point causing EE to pinch finger between the scaling bar and the railing on the basket and EE was wearing gloves.

When the crane was lowered to the ground when it was done being used, it came to rest in what (at the time) was thawed snow/mud. Overnight the thawed snow/mud had become frozen. When the crane operator began to use the crane, it was observed that partial ice had formed and was not cleaned as well as thought. Falling ice/mud chunk hit and broke hard hat.

A roof fall occurred in the Wille Mine at the intersection of 19AD. Date and time are unknown. Due to conditions that prevent access to the fall, actual dimensions are unknown at this time. No mining activities were being conducted in the area of the fall.

EE was operating the Telehandler with the window open but due to high/humid temperature, EE stated that he was not feeling well. We took EE to ER for evaluation. Diagnosis- Heat Fatigue.

Document # 2066018- explains Heat Exhaustion suffered by EE (Illness). Post illness; EE reported on RTW date that his back hurt from when he fell from the haul truck due to heat exhaustion. EE went to Dr. 7/17/15 and was put on restrictions. MSHA requested we release Document #2066018 due it being an illness and report the back injury separately.

While operating a haul truck the EE noticed the A/C was not keeping up and was taking it to the shop for proper tag out procedure. When he got to the shop he stepped out and fell off the stairs because he passed out- Heat Exhaustion- remove from work for 24 hours

A roof fall has occurred in the Wille Mine in the intersection of 22AE. Approximate dimensions are 50' x 75' x 8'. The mine has been closed since 6/17/15 and no entry has been made since that time. The affected area was observed from outside the mine at the Hwy 22 portal. No investigation has been initiated because entry into the mine is not allowed.

A roof fall occurred in the Wille Mine at the intersection of 13AC. Due to conditions, an accurate measurement of the fall could not be determined, but the area could be up to 35'X80'X7', extending a short distance toward Hwy 12. No mining activities were being conducted in the area of the fall. All personnel were removed and the mine portals were bermed to prevent entrance.

A roof fall above bolt anchorage occurred in the Wille Mine in the intersection of 25AH only. The fall measured approximately 36'X36'X8'. All access points were immediately bermed off to prevent entry. There was no mining activity in the area at the time of the fall, ventilation was not impaired, nor was normal passage to/from the mine impeded.

EE was stepping off the machine's ladder. Ladder was on top of a berm. Boot rolled and EE fell to the ground.

EE was using a pry bar to dislodge a rock from the back chute area of RSC-4. While trying to use the pry bar his thumb hit something that was sharp and lacerated his thumb.

EE's right leg fell through a floor grate and cut and scraped right knee. EE thought knee was ok and finished the shift. EE came to work the next day and requested medical treatment and was taken to the hospital

EE was attempting to close frozen door on rail car with rail bar. The bar slipped out of the door and EE fell over backward and his leg went the wrong way under his back when he landed on the ground.

Apron on the HSI dropped, they shut down HSI, EE opened inspection door, hammer rolled and fell on EE's wrist and top of left hand.

An employee was backing up a loader and didn't see a haul truck that the EE was driving. He backed into the haul truck and the EE injured his back.

EE was opening the VSI lid and while closing it it wasn't moving freely. He had to work the lid open with bars after trying to pull it open. He didn't feel any pain until 6 am and then finished his shift with out any problems. EE didn't report injury and fill out report until 6-20-14. Doctors put on restrictions on 6-23-14 & Filed with MSHA 6-23-14.

EE was moving a JLG Geni when the jack leg slid off the railing and hit him. This knocked him down and the jack leg landed on his right knee.

EE was behind the mesh in 2CA South and had put in the rib bolt using 3162. EE observed some loose material & moved under the mesh to hand scale. It seemed to be a small amount to remove, but a large rock came loose and landed in the basket. The rock struck EE in his left shoulder.

EE war removing a section of a conveyor and had a chain attached in 3 sections. EE was lifting the chain in the middle section and was apparently caught on the floor guiding. When EE lifted the weight off the middle section the weight of the conveyor came swaying towards him where the lower section hit him on his left side causing him to hit the wall on this right side.

EE slipped on ice and fell on his back walking from his car to the control office. He said he was ok at first but on 1-8-13 he wanted to see the doctor. This report was filed 1 day after he was went to the doctor and received a prescription.

One employee was moving the strap from one pin of the hydraulic quick coupler to the one on the other side; he was having troubles lining the strap loop with the pin. EE came over and grabbed the strap. EE got his finger stuck in a pin hole. He smashed the tip of his finger and the skin and meat were removed down to the bone.

EE was on top of a rail car closing the lid and tagging the car. Another employee began to move the rail car unaware that he was still positioned on top. EE fell off the rail car and injured his ankle. Proper Fall Protection was being worn.

EE was loading shot on 11AY with the Ampho Hose and a small piece of rock came off the face at his back and struck him on the neck. Initially he didn't feel he had any injury, but the next day he was sore. We took him to the doctor for review and he was diagnosed with a neck strain.

EE was backing up with the mud buggy and as he was walking backwards he didn't see the cat skid loader and tripped over it. As he fell backwards he kept ahold of the drive lever and ran over his left leg breaking it.

EE was called to repair an air line on the crushing line. They identified the leaky pipe, shut off the conveyor, bled the line and removed the pin. EE was monitoring air release and stated he couldn't hear or feel air coming out. Cap blew off striking him in the face.

A couple days later he told his supervisor he thought it was still in there and reported it to the safety manager. He then went to the eye doctor and medical doctor and had the material removed.

Upon inspection of new porthole, Mine manager found approx: 2 ton of material had slabbed off the face and into new established porthole pulling down a bolt. Mine Manager called immediately reportable hot line at 6:11 am and was on hold for 11 min and 55sec. At this point he hung up and recalled and got through to some one and reported his finding.
