Two team members were installing a guard on a conveyor after repairs had been made. As one team member was attempting to install bolts into the guard it shifted smashing a finger. Stitches and outpatient surgery were needed to properly repair the finger. No broken bones.
EE turned to walk to the mechanics truck 10 feet away, to pick up a tool. As ee stepped over power cords and tripped causing ee to twist/roll and fall. EE immediately reported incident to Supervisor. Injury developed as a contusion on the right foot requiring prescription strength anti-inflammatory.
EMPLOYEE WAS WALKING ACROSS THE PLANT YARD, SLIPPED ON ICE AND FELL, CAUSING A LOWER BACK MUSCLE STRAIN.
