EE was checking on plant status while operating. While walking back to the operator shack EE fell and contacted EE's head on the sidewalk. No specific cause of fall was identified by EE. Conditions were dry and only slight slope present. EE received 3 stitches to forehead.

While removing the bucket from the skid loader EE's left foot was struck by the bucket resulting in fracture.

EE was operating a Cat 769D haul unit from the pit to the primary crusher. On his 9th trip the employee stopped and told his supervisor that he was experiencing some lower back pain. Diagnosis was back strain. EE was found to be operating the truck too fast for the conditions. The haul road was smooth but EE was taking dry water crossing too fast.

Employee was cutting a section of belt with a belt knife when it slipped and cut his right thigh requiring seven stiches to close.
Employee was ""training"" a belt using a 3/4"" ratchet wrench with a 1 5/8"" 122 point socket. Pulling downward, the socket slipped off the worn adjusting bolt and the wrench and employee's gloved right hand impacted a 3/8"" X 5"" X 5"" structural angle iron behind the take up pulley.

4 EMPLOYEES WERE MOVING AN ELECTRICAL PANEL INTO POSITION IN A NEW ELECTRICAL CONTROL ROOM. DUE TO MISCOMMUNICATION ONE EMPLOYEE PUSHED WHEN HE THOUGHT EVERYONE ELSE WAS PUSHING. UNFORTUNATELY THIS EE WAS THE ONLY PERSON PUSHING. HE FELT A PAIN IN BACK. THE NEXT DAY AT HOME EE DID'T FEEL MUCH PAIN UNTIL HE SNEEZED. TAKEN TO HOSP. & DIAGNOSED W/FRACTURED RIB. DELAY IN DIAG=LATE REPO

CONTRACTOR ELECTRICIAN WAS STANDING ON A FOUR FOOT STEP LADDER SLIGHTLY ABOVE AND IN FRONT OF A 200 AMP 600 VOLT 3 PHASE DISCONNECT BOX. REALIZING THE BOX WAS ENERGIZED HE PUSHED DOWN ON THE D ISCONNECT LEVER WITH HIS FOOT WHICH CAUSED THE BOX WHICH WAS UNDER LOAD TO ARC OUT.THE ELECTRICIAN SUFFERED 1 ST AND 2ND DEGREE BURNS TO LEFT SIDE OF NECK AND RIGHT HAND AND ARM.
