Fall down stairs

Employee fell down the last few steps of the stairwell of the kiln feed area. Worker received lacerations to face and head requiring stitches to head and nose.

A maintenance millwright was walking down the stairs in the raw mill from the 1 1/2 floor and fell down the stairs hurting EE's knee.

Employee was using an air lance with 90 psi air to clear a chute plug in the #4 Finish Mill feed chute. The employee lost grip causing the air lance to hit them in the mouth. They initially declined medical attention. On 10/24/2025, they requested to be seen at the emergency room due to mouth pain.

Shift repair was working on the ""small mills"" FK pump to repair an air leak. While walking from the repair truck to the west side of the FK pump, they tripped on uneven floor plates and fell hitting their right knee. Employee was taken to the emergency room and was diagnosed with a fractured right patella (kneecap).

Employee was removing liners from ball mill when they dropped the liner striking them in the foot. Receiving a fracture in the foot.

Employee was cutting cardboard on the floor with a utility knife in the maintenance garage. Their left hand was holding the cardboard, and right hand was cutting. The cardboard slid on the floor and the knife slipped lacerating their left wrist.

While stripping a ProfiBus wire with a safety utility knife, the blade slipped off the cable and sliced their left-hand thumb. Employee was taken to local emergency care and received 5 stitches in left thumb

Employee was operating a jackhammer to remove material and as piece of material fell- hitting the jackhammer and pinning left index finger against port opening on the container causing injury.

Employee was assisting the shift repair at the finish mill 3 vibrating pan. The belt to the motor of the vibrating pan had seized and once the equipment was locked out, employee was cleaning to get material clear to start up vibrating pan. Equipment still locked out, employee pulled belt to get unstuck-pinky finger injured between the sheave and the belt.

Contact with heat

Employee started feeling unwell, and it was called over the radio that the employee needed help. Supervisor got ice packs and water and headed up the stairs in the tower. It was hot in the tower. The employee was observed walking unsteady and was showing symptoms of heat exhaustion.

Employee was working in the chemical lab and was washing a 800 ml beaker and it slipped out of their hands and hit the edge of the sink. The glass from the beaker cut their left forearm causing a laceration. Employee required sutures to close the wound.

Injured employee was shoveling clinker into a floor hatch that was opened for clean-up. Employee lost footing and fell partially into the opening, catching themselves and landing on, and dislocating right elbow. Employee was taken to ER, shoulder was reset, and they returned to work with restricted duty (one handed) until follow-up.

While installing a new junction box for the #2 cooler fan, employee felt pain in right shoulder. Employee felt pain on 1/29, while working with supervisor, but did not report injury until 1/31. EE did not seek medical attention until 2/4/22.

Over-exertion in lifting objects

On 9/28 @ approx. 3am, ee felt pain in back when dragging hose. EE did not report it until 9/30 @ 11pm. EE tried self-care until seeing Dr. on 10/3. EE taken off work on 10/4 and placed on restricted duty. Incident was not reportable until EE missed work on 10/4.

Over-exertion in lifting objects

Working from a ladder, EE was bringing a light down to repair it and felt pain in lower back. EE proceeded to the Elec. Shop to repair the light. After repairing the light, EE went back up to the top of the Homo Silos to reinstall the light. EE said the pain became worse after installing light fixture and putting ladder away on top of the Homo Silo.

While working on a ladder, approx. 52"" high, EE experienced personal medical event causing them to fall to the ground. Co-worker notified emergency services and performed CPR until EMTs arrived. EE was pronounced DOA. Cause of death unknown at time of report.

EE was transporting a 10 ft. probe in the silo elevator. The probe became stuck while sticking out of the roof. EE attempted to retrieve it, but it fell, striking EE's rt thumb. EE received x-ray which determined that the thumb was broken.

Employee lost footing while exiting locomotive as EE stepped onto concrete working surface. Employee landing on right knee and right forearm. Employee was seen by local doctor and was determined to have a muscle strain in right quadriceps. Employee received limited duty work restrictions for one day.

Struck against stationary object

Employee used bare left hand to brush away the material from the area of an unknown leak. A bolt had backed out from the screw and created a hole in the housing. Employee was unaware of the bolt. Bolt tore nail off of left index finger.

Employee was removing wrapping tape from the seat of new conference chairs. Employee gathered paper to cut and remove, and cut the end of left middle finger with scissors. Skin was lacerated on three sides of the tip, remaining intact on the side closest to thumb. Employee received 6 stitches to aid in healing of the lacerated skin.

Employees used shotgun to breakup material buildup in 3rd stage preheater tower. EE held the stock in right hand which became sore and blistered the next day. EE requested medical treatment & was given prescription 9/28, making incident reportable.

While removing the #3 stator from the motor with a come along, employee was bent down ratcheting the come along and stood up feeling a pain in employee's lower back right side with shooting pain down the back of employee's right leg.

Employee was changing spray nozzles in the tower. On the third to last one, EE's back began to spasm. EE then proceeded to a call to the D-13 area. EE's back began to spasm again as EE finished taking the coupling apart and then EE reported it to the supervisor. Employee did not wish to seek medical attention. Employee was evaluated and given prescriptions 3/13/17.

Employee was breaking up material clogging a chute with a bar. Employee felt a pop and pain in employee's left shoulder. Employee denied medical care at the time, but requested to be seen on 1-23-2017.

Employee was walking to the truck after the shift. EE met another employee coming in. Other employee said ""watch it, it is slick"". Employee then proceeded to parking and was in front of the truck taking small steps when EE was looking where EE was stepping and heard a pop and then a crack when EE fell to the ground. Employee fell backwards. EE had pain in right shoulder.

Employee was clearing the D-16 to D-11 chute from a flat rock that became lodged in the chute. Employee pulled the e-stop lanyard on D-11 and got on the 3 step platform from the south side of D-11. Employee used a bar to pry the rock from the chute. The rock came loose allowing the bar to drop suddenly and employee's pinky finger got between the bar and the metal skirting.

Employee was water blasting equipment when hot water splashed back onto leg causing a burn. Employee developed blisters on skin and requested medical treatment where EE received a prescription medication.

Employee was walking in the west end of the tapping tunnel checking the area for flooding. Employee saw a piece of trash when employee bent over to pick it up, right foot went into an unseen hole and twisted right knee. Employee did not need to see the doctor at this time. An MRI on 10/5 revealed meniscus damage. Surgery took place to repair the damage on 12/8/2016.

While trying to get material to withdraw, employee put an air lace into a ¾ inch port hole on the side of the hopper. The air lance was 6 foot long. The material finally started to flow. Some of the material came out around the lance some dust got into right eye. Employee seen doctor and received prescription. No work restrictions.

Employee slipped while traveling down a set of stairs causing left foot loose grip on the stair. Employee maintained balance and did not fall. Employee refused medical treatment at the time of incident.

While using a 60' manlift and lowering to ground, the basket got caught on a bolt head. When basket came free of the bolt, the basket jerked and one of employees was ejected and landed on the ground. The employee fell approx. 33 feet. Employee was wearing harness with double lanyard but appears to not have been tied off at time of incident. Employee still being evaluated.

Haul truck was struck by FEL when operator inadvertently used wrong lever and lowered bucket against top of truck bed. Truck operator noted no initial report of injury after incident, but reported pain/soreness in neck/shoulder areas the next day. Initially cleared for duty, but then received restrictions on 3/20/16 when EE went to doctor on own accord.

Injured worker was driving a forklift truck in the plant garage when he drove over the boards that were placed over the mechanic's pit. The forklift broke through the boards and pinned the injured workers right leg between the forklift truck and the wall of the mechanic's pit.

Injured Work was removing snow and slipped on the ice spraining his left ankle. Was sent to the doctor on 12/04/2014 and was placed on restrictions.

Employee was changing a light bulb in the craneway. Material entered both eyes because of the wind. Was seen by the eye doctor. Both eyes were cleaned out. Prescribed antibiotic.

Employee was driving a Cat 777 Haul Truck and felt a twinge in his lower back. The twinge continued to get worse. Employee was taken to the clinic. Doctor said that employee has a lower back strain/spasm. Placed on restrictions.

Injured worker was walking from his car to the main change house to report to work. He slipped on ice and sprained his right shoulder. Was taken to E.R. to be checked out and returned to work. On 3/13/14 requested to see the doctor because his shoulder was continuing to bother him. The doctor took him off of work until an MRI was completed.

Mechanic was lifting a cylinder rod into the barrel of a wheel loader. The load shifted causing the rod to come out of the barrel and swing around landing on his left foot. Then the load block truck crane swung around and hit him in the head knocking his hardhat off causing a laceration.

Injured worker was cleaning in the area of 302 drag conveyor. He slipped on wet material and tried to catch himself, when his left hand went into hot material, causing 2nd degree burns. Injured worker was wearing gloves at the time of the accident.

While moving plywood onto forks of the fork lift, injured worker slipped and fell on his tail bone. On 8/16/09 he went to the ER because of sorness. ER Doctor said that he had a bruised tail bone. On 8/17/09 he went to the clinic and the doctor there said that he had a fractured coccyx. Doctor placed him on work restrictions.

Injured worker stepped on a piece of material and rolled his left ankle. He was seen by the doctor on 5/18/09. He was not put on any formal restrictions. He went back for his check up on 5/28/09. His conditions were unchanged so he will be starting physical therapy 3 times per week for 2 weeks.

Injured worker was ratcheting a 1.5 ton come-a-long when he felt a pull in his left tricep muscle. Later in the day his muscle became sore and started to swell. He went to the doctor and was placed on restrictions. Avoid use of left arm until 5/13/09.

Left ear at 2K had a 25db loss in annual hearing test. Employee's hearing was re-checked and showed hearing loss. Was sent to Audiologist. Had MRI done on left ear. Audiologist results from the MRI was that the hearing loss was caused by sharp loud noise. Employee stated that he didn't know when it happened and he didn't remember any loud noise that caused damage to his ear.

While employee was tightening bolts on lower A-arm on #7 Toyota pickup, felt a pop in his right shoulder. On 4/7/09 employee was working on a dingo, experienced pain in his right shoulder. Made an appointment to see Dr. and was diagnosed with A/C Separation. Placed on restricted duty.

Wireman was installing a control transformer in motor starter compartment using a hand threading tool to tap threads into two existing holes in the back wall of the compartment. The tap's pointed end penetrated the wiring which ran along the back side of the compartment metal wall thereby causing the short circut from the energized wire to the ground via the thread tap.
