Struck by falling object

Employee was removing a motor and reducer. Had motor rigged up with motor puller and chain and was helping lower the motor to the ground when the shaft slid out of the broken section of the auger jerking the employee forward. Declined initial treatment. Went to the clinic on April 10, 2026 due to shoulder pain and was diagnosed with a right shoulder strain and deltoid tendinitis.

EE stepped backwards in JLG man basket, heel came down on a piece of strut, injuring heel.

EE was sweeping and shoveling material off of floor, felt pain in right forearm and elbow. 2/10/2025 was a report only. EE asked to see Dr on 3/17/2025, at which time it became a recordable event.

Struck by flying object

Employee torch cutting activities, was done for the shift, and felt something in right eye. Next day went to doctor and had foreign object flushed from eye. Received prescription drops for eye.

Employee was working at work bench cutting belt flashing and twisted left knee wrong and heard/felt a pop in kneecap.

Worker slipped on black ice that was not visible between office building and East scale area. Pad had been removed of snow and ice, needed more ice melt applied to area. Fell on right side bruising back and chest area. Few days later worker went to doctor for follow up because of pain. Worker was given treatment for muscle relaxer and inflammation.

Individual was parking a company vehicle overnight in a building and fell on icy parking lot when getting into personal vehicle. Individual went to doctor in morning with pain and was treated for broken ribs.

Individual was holding a shaft driving punch tool while coworker was striking sledgehammer. The tool bounced; employee lost grip causing left hand little finger to be pinched between handle and hammermill casing. Tip of finger was smashed and required stitches.

While using a chain hoist to open the door on a hammermill the door moved more than expected after passing center balance point causing the employees hand to get pinched between the door and the clevis attached to the hoist

Stepped wrong descending staircase and twisted knee resulting in a sprain

Struck by powered moving object

Person was injured during routine cleaning of pin mixer. Machine was locked out, operator did not wait until it came to a complete stop before opening door. EE used EE's foot to get through buildup in front of the door and a pin attached to the spindle struck EE's foot. Broke some bones, caused a large laceration. One toe had to be surgically removed.

Changing belts on hammermill. Rotating pulley by hand when it moved unexpectedly causing the employee to move forward and strike chest against the guard frame

EE felt a ""pop"" in left shoulder while using a rubber mallet to strike a bag house; resulting in left shoulder strain.

Snow/ice led to a slip/twisted ankle. Undiagnosed injury; employee did not initially report injury to management, called in to work due to injury, and reported injury upon return the following day. Employee declined medical treatment.

Pulled muscles in left while team lifting a large speed reducer onto an auger drive shaft.

Fall from machine

Fell off of last rung on a ladder while exiting a crane. Did not maintain 3 points of contact until on the ground as EE was trained to do. EE was roughly 12 - 18 inches off of the ground. Landed on left arm which caused EE to sprain shoulder.

Got right foot caught in the bottom rung of fixed ladder while coming down, fell backwards and twisted right ankle.

Over-exertion in pulling or pushing objects

Employee was performing housekeeping duties, racking material from under a bin and pulled something in lower left side of back. Took to doctor next day, and found out that EE had pulled a muscle. Was given a muscle relaxer prescription.

Over-exertion in lifting objects

Employee was shoveling loose materials around a piece of equipment and felt something pull/pop in left upper shoulder area. MRI taken and showed deep bruising. Pain pursing and tingling in fingers, sent employee back to doctor to be evaluated by orthopedic doctor. Restricted duty given from first MRI, but Ortho states no work until second MRI is completed.

Accident type, without injuries

Unknown. Fire started in Pellet Plant, Old Lignin room. Total loss of lignin storage room (batch treatment area) and majority of plant operations.

Struck against stationary object

Worker hit right elbow on frame of forklift while getting off machine. Safety notified on 12-24-15 that swelling on elbow wasn't going away. Sent to doctor on 12-29-15 and had fluid remove and steroid injection of the bursitis on elbow.

Worker was cleaning out a plug air locking device under bag house unit and air lock engaged catching his hand while removing blockage amputating two ends of his right hand fingers.

Struck by falling object

Worker was caring a section of piping with his co-worker and caught the pipe on a cross section I-beam causing them to loose there grip and dropping pipe. Pipe fell less than 3 feet on upper section of right foot tow, above steel toe guarding which caused swelling and a deep bruise.

Tripped on door jam and fell on left knee. Didn't report injury was work related to supervisor. Employee assumed he knew it happened at work and company didn't found out until private doctor called on 2-23-15 for billing information for work related injury.

Lift bag of material from pallet and placing into hopper unit. Strained right shoulder/dislocated.

Was putting tools away after repairing belt and left wrist gave out/locked up.

EE fell on wet surface when going outside to fill forklift propane bottle. He indicated was alright and refused medical treatment. 2-24-14 EE requested to see doctor for back pain and was sent to medical provider. Sprain, lumbar sprain. modified duty with 30 lbs lifting restriction and ok to drive forklift. Follow up appointment on 3-17-14 with doctor.

EE was shoveling snow in alley when slipped on snow covered ice and fell onto his back. Suffered an ""acute lumbar sprain"" to his back. Treatment was take over counter ibuprofen and perform exercise and walking at home for treatment. Return 2-27-14 follow up.

EE was using belt knife to cut a conveyor belt. Knife slipped cutting EE's left hand index finger, required 6 stitches.

Employee was carrying a bucket of pins and strained his right elbow. Did not report the injury until noon on 7/19/2012.

EE was using a belt knife to cut belt. Knife slipped cutting EE right hand. Required 10 stitches.

Struck against stationary object

EE was blowing off the dryer and felt the heat by his hand so reflexively pulled his arm back bumping his right elbow. He did not report it until the next day and declined to see a doctor. IT was still bothering him on 7/9 so we sent him to a doctor. He missed work on 7/9 and returned to work on 7/10.

In the process of stacking pallets, pallets shifted. Employee was not positioned well and injured his left shoulder. Damage includes possible torn muscles and hyperextension of left elbow.

Taking liner off door of Hammermill equipment. It was wedged, so he used a hammer to beat it loose while using hands to hold the liner of the door. It popped loose, but was heavier than he thought, as this was the first time doing this task. So the weight forced his hand downward with the liner in hand between the door. Causing laceration to the left middle finger.

Doing some remodeling of Lignin Room. Removing the plywood floor to which EE was now walking on to perform task. He had a piece of 4x4 plywood in his hands and stated he didn't pay attention and walked right into a spot where he removed the plywood roof (flooring), falling through opening and catching himself before falling through. Scuffed arms, legs, as well as back.

Removing Calcium oxide from a bin. Dust cloud formed when it went into wheelbarrow. Some got into left eye. EE was wearing safety glasses but not goggles which were available.

WORKING WITH BAGGING FOR ABOUT 1 WEEK FELT SORE ON RIGHT SIDE OF CHEST, SHOULDER & BACK UNTIL FRIDAY. WORK WITH SUPER SACK THEN STARTED TO FEEL PAIN BEFOR WORK WAS OVER. PAIN GETTING WORSE.
