Caught in, under or between a moving and a stationary object

Technician was working with a man lift and it became stuck. Technician was pushing at the end of the man lift to get it unstuck and the slide was out a bit and when pushing on that it slid in and it cut/split their finger while servicing a 775 Cat at the start of the shift.

Accident type, without injuries

Mid American Electrical insulator failed causing an arc flash, power wire fell to the ground. No damage/injury to the mine site or employees.

Employee fell into the frame of a conveyor while removing tail pulley guard.

Over-exertion in lifting objects

Employee was changing return rollers when the employee felt lower back pain. Physical Therapy and restrictions were prescribed by the clinic. Injury became reportable on 07/23/2024.

Employee was using a utility knife while performing patch work on a conveyor belt. While cutting, the utility knife slipped, and cut the employees left thumb just below the nail. The employee received 3 stitches.

Employee was dragging an ANFO hose along the front of the heading and stumbled. As they regained balance, they strained lower back. Employee began physical therapy on 8/20/21 making this a reportable injury.

Caught in, under or between a moving and a stationary object

On the evening of Thursday, March 25th, 2021, the night shift was changing drive belts on the (secondary) crusher. As they were spinning the sheave putting the last belts on, the employee's hand got pinched between the sheave spoke and the motion sensor.

Mine operator was notified approximately 4 hours later of a customer truck driver that slipped and fell on the ground.

Fall from machine

At 4:35 AM on 8/26/20 EE arrived on site prior to shift start to retrieve items from 769D Cat haul truck. EE was at 2nd rung from top of ladder, lost grip w/left hand on grab bar located below windshield. EE swung out towards the right & fell off of haul truck, landed on ground. Compression fracture to T12 vertebra.

Employee was installing a hood on a 336F excavator. The hood shifted when employee was installing it and it bumped employee and knocked employee into the truck. Employee braced self with R. hand which jammed shoulder. Employee has not gone to the doctor yet, but will be soon.

Employees working to install new backstop and speed reducer for underground conveyor. While lifting machine out of shipping crate with hoist, the component rotated, causing a bolt to catch an employee's right hand against the shipping crate, lacerating the skin under the glove.

Employee slipped and fell to the ground landing on knee injuring the knee cap. Employee required physical therapy effective 11/21/2016 making this a reportable.

Employee was changing screens and using a hammer. EE missed the target with the hammer and hit right ring finger on a screen wire causing a fracture in finger.

EE stepped on uneven ground and fell back onto right elbow. EE was issued a sling to stabilize arm.

Struck against a moving object

Employee veered into the face with haul truck causing the steering wheel to jerk back and forth. This caused a hairline fracture in EE's wrist.

Employee was swinging a 4# sledgehammer and came into contact with his left middle finger causing a small fracture under his nail.

Employee was climbing down off of his haul truck and lost his three points of contact and missed the last rung of his ladder. He fell into the side of the truck injuring his left shoulder.

EE was loading holes at an underground bench and slitting sticks of powder. The utility knife slipped penetrating a kevlar glove causing a laceration at the base of his left thumb. The injury required 8 stitches to close.

Struck against a moving object

EE was cleaning mud on the decline to the portal. Some of the mud spilled on the road and ee was cleaning it up with a skid loader. Bucket was too low and it hit the steel grate over the road. EE was jerked to a complete stop and dislocated his right shoulder. This incident was a first aid only incident until 5/1/2009 at which time he was sent to see a physical therapist.

EE was holding a hydraulic hose that had a pin hole in it. When the hose pressurized it shot oil into the employee's right thumb.

EE was cleaning a catwalk and became overheated and began to feel sick to his stomach.

Struck by falling object

Injured employee was helping install a guard and a coworker dropped the guard hitting him on the top of his left forearm causing a three corner laceration requiring thirty two stitches to close.

Injured employee suffered a pinched finger. His finger was xrayed and no bone fracture was found. He was released to full duty with a follow up visit scheduled for two weeks. After repeated follow up visits to the doctor with no medical treatment, he was then scheduled for physical therapy. At this time we feel his condition became reportable.

Employee was helping coworkers manhandle a section of screen into a screen box. Injured employee lost his balance and cut his left hand with the hook on the sceen section. He was not wearing gloves at the time of his injury.

EE WAS WORKING ON DRILL THAT HE RUNS IN THE SHOP. HE HAD BEEN BENT OVER & WHEN HE WENT TO STAND UP HIS PANT LEG CAUGHT & HE LOOKED & THEN LOOKED BACK. WHEN HE LOOKED BACK HE RAN HIS HEAD INTO THE BOOM OF THE DRILL CAUSING A LACERATION TO HIS LEFT EYEBROW THAT REQUIRED SUTURES TO CLOSE.

EE WAS REPLACING A PLATE THAT CLOSES THE SECOND SCREEN DECK WHEN HE SUFFERED A GROIN PULL. UPON CLOSER EXAMINATION RESULTING FROM PROLONGED DIS COMFORT THE EE WAS DIAGNOSED WITH A INGUINAL HERNIA. THE EE RECEIVED SURGERY TO REPAIR HIS CONDITION 8-6-03.

EE WAS GETTING OFF OF HIS STOCKPILE TRUCK WHEN HE SLIPPED ON ICY GROUND & HIT HIS LEFT KNEE ON THE FUEL TANK STEP. THIS RESULTED IN A CONTUSION. ORIGINALLY THIS INCIDENT WAS THOUGHT TO BE A FIRST AID ONLY BUT HE HAS BEEN DOWNGRADED TO MEDICAL/RESTRICTED DUTY. 20 DAYS INTO THE RESTRICTION THIS EE WAS LAID OFF.

EE WAS LOADING THE BOTTOM HOLES OF A FACE SHOT WHEN HE ALMOST TRIPPED WHILE MOVING BACKWARD. IN HIS ATTEMPT TO KEEP FROM FALLING THE EE STRAINED HIS RIGHT HIP AND SURROUNDING TISSUE.

EMPLOYEE APPARENTLY SUFFERED BACK STRAIN FROM REPETIVE LIFTING OF EXPLOSIVES BAGS AND BOXES. EMPLOYEE DID NOT INITIALLY REPORT HIS INJURY TO MANAGEMENT. HE SOUGHT MEDICAL ATTENTION FROM HIS PE RSONAL DOCTOR WHO PLACED HIM ON WORK RESTRICTION. HE THEN REPORTED HIS INJURY, WAS TAKEN TO THE COMPANY DOCTOR WHO TOOK OVER TREATMENT.

EE WAS HELPING REMOVE HUNG UP MATERIAL FROM THE PRIMARY IMPACT CRUSHER WHEN HE WAS STRUCK BY A ROCK CAUSING A WOUND THAT REQUIRED STITCHES TO CLOSE.

HAND SCALING OUT OF MANLIFT. LARGE LOOSE FELL ONTO MANLIFT BOOM. BOOM & GASKET WERE PUSHED DOWNBY THE WEIGHT OF THE ROCK & THEN CATAPULTED EE INOT THE BACK. EE HIT BACK WITH HEAD (HARD HAT HAD FALLEN OFF) CAUSING 3 LACERATION TO THE TOP OF HIS SKULL

2 EMPLOYEES WERE CUTTING TREES WITH A CHAIN SAW. 1 EE WAS PUSHING ON THE TREE WHILE THE OTHER EE WITH THE CHAIN SAW TRIED TO FREE IT FROM TREE. WHEN THE TREE BEGAN TO FALL THE EMPLOYEE PUSHING ON THE TREE TURNED AWAY FROM THE TREE AND DROPPED HIS GLOVED HAND INTO THE PATH OF THE CHAIN SAW BLADE RESULTING IN 3 LACERATIONS TO HIS RT WRIST REQUIRING 10 STITCHES.

EE WAS CLIMBING ON THE STEPS OF HIS HAUL UNITWHEN HE FELT A PAIN IN HIS BACK AS HE LIFTED HIS LEFT FOOT TO STEP ON THE ACCESS LADDER EE HAD A PREXISTING CONDTION WHICH WAS APPARENTLY AGGRAVA TED BY CLIMBING INTO HIS HAUL UNIT.
