Employee was roof bolting (rehabbing) M7 belt-line Entry #5 cross-cut #28 when EE slipped on a flat rock, hyper extending EE's right knee. MRI results from 11/25/2025 showed a torn meniscus.
Employee was cutting out old hog panel and scaling down loose ribs to re-fence the rib-line, when sulfur ball measuring appox. 14 inches wide 16 inches in length 10 inches thick rolled out landing on employees left foot.
Unplanned roof fall occurred on the travelway of M4 xc 38. the fall was approximately 60 foot long and 20 foot wide and approximately 10 foot thick.
Employee was loading a grinder on a fifth wheel trailer and felt their left bicep muscle flex more than it should have. An MRI showed a small tear to the bicep and employee was placed on restrictions.

Employee was putting a scoop battery on charge. While lifting the battery lids, one of the lids closed and smashed the tip of EE's right finger. Employee was treated and placed off work until release from orthopedic surgeon.
Employee was trying to move a bundle of saddles by hand and felt pain in the right bicep. An MRI showed that there was a partial tear in the bicep. Employee was placed on restrictions.
Employee was reclaiming a drive set-up. When dismounting from the tail, the employee stepped on a joint of waterline and twisted the knee. Employee was placed on restrictions.
Employee was walking an air course and stepped in a hole and twisted the right knee. Employee was seen by a doctor when the knee did not seem to get any better. An MRI on 4/18/24 showed a tear to the meniscus. Surgery is scheduled for 4/26/24.
Employee was bent over picking up a timber when the stack fell and three of the timbers landed on the left hand. Middle and ring finger were both fractured.
Employee was using a pry bar to take down a rock when the rock broke and struck the employee in the back and neck. Employee was diagnosed with a left arm contusion and was placed on restrictions.
An unplanned roof fall occurred in the LOXC on the P2-1 in entry 4 at crosscut 18. The fall measured approximately 70' in length, 17' in width and 8' thick.
Employee was walking ""quickly"" towards the miner. When they went through the curtain they hit their hip on the corner of the miner tail, causing them to fall backwards and land on the buttocks. Employee was placed on restrictions for tailbone pain.
Employee was replacing a steering cylinder on the battery hauler when the finger was struck while inserting a pin. Employee received 4 sutures to the finger.
Employee was bolting on unit #2 in 8R when it was decided to use hog panels for the spot. While putting the hog panel on the ATRS it slipped off. When employee bent over to get the panel a rock struck the back of the head. Employee was taken to ER and was diagnosed with a concussion and placed on restrictions.
Employee had drilled a hole and was in the process of inserting the glue into the hole when a rock fell and struck the right shoulder. Employee was placed on restrictions for a shoulder contusion.

Employee was working on the continuous miner when EE stepped off to get a tool and twisted their knee. Employee has been working without restrictions. A follow up and MRI on 1/19/2024 showed a tear to the right meniscus. Surgery is scheduled for 2/02/2024.
Employee was helping change a top roller on a mainline belt. When the employee pulled the roller under the top belt, they felt pain in their back. The employee was placed on restrictions on 12-26-2023.
Employee was lifting a rail onto the rail setter when it rolled off and smashed the middle and ring finger between the bed of the hauler and the rail. Employee received a fractured finger on the left hand.
Employee was helping hang the miner cable and stepped into a hole, causing pain to the right knee. Employee was placed on restrictions and is awaiting an MRI.
Employee was trying to remove a rock from the scoop and cut the right arm on the rock. Employee was taken to the ER and received 5 sutures.

Employee was using a ratchet to loosen a bolt on the flame trap when the ratchet slipped, causing the employee to hit the arm on a pressure valve and cutting the elbow. The employee received 3 sutures.
An unplanned roof fall occurred on the S1-2 at crosscut 22 entry 4-5. The fall measured approximately 80' long by 17' wide and 10' thick. It appears the roof shifted causing the roof support in the area to fail.
Employee was working on a compressed airline underground. When a hammer was used to make sure the shut off valve was closed completely the valve blew apart. The employee was standing with their left foot in the mud and when the line blew apart the employee turned to the side but the foot did not. Employee received a compound fracture below the knee and fractured wrist.
Employee was handling hog panel in the beltline when EE's right forearm was lacerated. Employee was taken to the ER and received staples and a tetanus shot.
Employee slipped when shutting off a main water valve and cut their hand. Employee was taken to the ER and received 4 sutures.
Employee went to lift a timber and felt the knee pop. On 9/5/23 an MRI showed a left tibial bone contusion. Employee was placed on restrictions.
MSHA inspector was on site testing hoist functions when the safety catch test failed. The hoist was placed out of service and phoned in. The hoist was then tested in the same way that it was commissioned and the safety catches passed.
Incoming power to our substation was lost. When power was restored, the 500 KVA transformer shorted out. Our hoist operator was 50' from bottom doing the daily checks when power was lost. The hoist was manually released to get the hoist operator to the bottom of the shaft. A new transformer was installed.
Employee had just been loaded and left the continuous miner. When traveling to the feeder, employee ran over a rock and struck their head on the canopy. Employee was placed on restrictions.

Injured was operating the mini trac and ran over something causing the mini trac to drop down with jarring impact. The injured felt a popping in the right shoulder. An MRI on 8-7-23 showed a tear in the rotator cuff. Surgery is scheduled for 8-11-23
An unplanned roof fall was discovered in entry 3 on the #1 unit at xc2. The fall measured approximately 20' in length, 18' in width and 10' high.
Injured was lowering the drill pot down when something struck the knee forcing downward on the leg. The injured was taken to the ER and X-rays showed a fracture ankle.
Injured smashed finger between the canopy and the rib when swinging out the install the outside roof bolt. Injured received a fracture to the middle finger.

Injured was using a hammer and a header board to install 4"" waterline. The hammer turned and hit the index finger of the hand holding the header board. The injured received six sutures.
Injured was helping replace a rear end on a battery scoop. When a hydraulic hose was removed the battery was not cribbed tight and moved, this allowed the battery to pinch the lower left leg. The injured received a tear in the distal calcaneofibular ligament, and a partial tear of the posterior talofibular ligament.
Injured was helping build cribs at the M20 drive area when a 6""x6""x4' crib block fell about 5' striking injured in the neck and shoulder. It was determined on 3/6/2023 that the injured had a concussion and was placed off work.

Injured was running the continuous miner on Unit #2 when injured was struck above the right ear. Upon inspection it appeared to be a piece of carbide. Injured was taken to the ER to have the carbide removed and received 1 suture.
Injured was trying to remove the tail roller keeper with a hammer and a pin hammer. The injured swung, missed, and contacted the hand holding the pin hammer. X-rays showed a transverse fracture of the 2nd metacarpal.

Injured was building cribs in the intake on Unit #2 xc42 when they felt something pull in the stomach area. On 1/26/2023 a CT scan was done and showed the injured had a hernia. Surgery is scheduled for 2/22/2023.
Injured was roof bolting on unit 1 in entry 6. The drill steel slipped from the left hand and landed between the top of the left foot and the drill boom that was being lowered. This put enough pressure on the foot to cause a fracture of the 2nd metatarsal.
Injured was helping mechanic work on the drill feed pot. As they were lining up the holes to install a bolt, the pot shifted and smashed the tip of finger. The injured received 3 sutures to the tip of the right hand middle finger.
Injured was tramming a roof bolter on unit #2 entry 3 crosscut A. As they turned the corner the roof bolter became a little unstable from a drop in the mine floor. As the injured reached for the top of the canopy to help with their balance a crossover pad fell from the canopy and struck injured on top of the hand. This resulted in a fracture of the lower part of the middle finger.
Injured was working in the beltline when EE stepped in a hole and twisted EE's knee. On 12/06/2022 MRI results showed a meniscus tear in the right knee. Surgery is scheduled.
Injured was joining the starter and the pusher drill steels together. When the pressure was applied the steels did not align correctly and the steels popped the injured on the hand. The injured received 4 sutures between the thumb and the finger.
While setting timbers in the M3 Intake, the injured stepped in a hole and strained the right knee. On 11/22/2022 an MRI showed a torn medial meniscus of the right knee.
Injured was at the M11 seal area building walls. At the end of the shift, EE felt a slight pain in the abdomen area. The next day there was swelling in the abdomen. On 11/22/2022 a CT scan showed EE had a large left inguinal hernia.
Injured had reached for roof plates on the bolter when the operator trammed forward. The left foot was in front of the tire as the bolter moved forward. Injured was diagnosed with a fractured left great toe.
Injured was driving a mantrip on the haul road. Injured was pulling into a crosscut to get out of the way of an oncoming piece of equipment. They had their hand/fingers through the opening of the steering wheel when the tire ran over a rock and the steering wheel spun. Injured was diagnosed with a 4th and 5th metacarpal fracture.
An unintentional roof fall happened in the face of entry 8 located between crosscut 21 and 22. There was a slight start in elevation change and the coal seam was starting to pinch off.
Injured was working on the Mine No2 fan. While EE attempting to open the mandoor at the back of the fan, EE lost EE's footing and fell off the concrete ledge (46"" high). Injured fell to the ground and landed on EE's left side fracturing four ribs.
Injured was helping build cribs in entry 2. A curtain board (2X4) was broken and hanging from the top in the way. Injured was prying on the board when it broke off and hit injured in the mouth knocking a tooth loose. A dentist ended up removing the tooth.
Injured was drilling the inside hole. The 2nd steel became clogged causing it to bend. Injured attempted to bring said steel back down, breaking the steel to steel insert and spinning the steel catching the injured across right forearm causing a laceration to the right arm.

Bad storm caused a power outage. When power was restored the PLC had to be re-programmed.
During a non constructive test, a distortion was discovered on one of the ropes. The hoist was removed from hoisting men underground.

Injured was a passenger in a 2 person mantrip. The driver ran over a timber in the road and caused the mantrip to run into the rib. The injured was brought back outside with right ankle and knee pain. MRI on 3/16 showed torn ligament on side of ankle and fracture in back of ankle.
Injured had just drilled a hole in the top. As EE twisted to grab the glue and roof bolt, the knee popped and dislocated.
Injured was unlatching fence hook on ATRS pad and got fingers caught between ATRS pad and hook handle. The ATRS was being raised when the pad came in contact with the top and the hook latch smashed fingers
Injured was getting the next hog panel to install that was leaning against the rib, a rock came loose from the edge of a previously installed panel and stuck injured in the back of the head.

Pre-shift examiner discovered a roof fall on the #1 Unit at the 33xc of the S1H in the #10 entry. The fall was cribbed off and additional supplemental support was added to the surrounding areas.
An unintentional roof fall occurred on the S1L at the 15xc in the #7 entry in the last open XC. The fall was discovered by the examiner during the pre-shift examination for the on-coming shift. The fall was approximately 20' x 20' x 10'
Injured claims hit head on the canopy of the mantrip as the ride was coming out of the mine. Injured reported the incident the following day. Injured was sent for observation and diagnosed with Myofascial strain and placed on restrictions.
Injured went to pick up a Roof jack off of the mine floor to install along the travel way of the M14 and when EE picked up the jack EE felt a pop in EE's forearm. Injured continued working and the following morning had a knot in the forearm of EE's arm. Injured was sent for medical attention and then sent for MRI. For possible bicep muscle tear.
Injured fell and hit elbow on the coal rib and the mine floor causing elbow to become dislocated. Injured was transported for medical attention and and elbow was put back in place.

Injured was putting crib blocks under the miner because it was stuck in the intersection of the #10 entry. While putting the cribs under the machine EE smashed two fingers between the crib block and the ground. X-rays revealed EE had fractured both fingers.

An unintentional Roof fall occurred on the S1J at the #2XC. The fall was approximately 30-foot long by 30-foot wide and over 10-foot high. The fall was discovered by the examiner.
Injured was installing 8' bolt and bent and smacked hand. This caused a sprain to EE's right hand. EE was placed on restrictions on 8/10/21.
At approximately 4pm the hoist man received a call from the work deck to bump up on #2 work deck winch. The hoist man reportedly heard a noise coming from #2 work deck winch, EE checked the winch and seen that the 1"" cable was unspooling from the drum and falling into the shaft. The cable completely unspooled to the work deck below due to mechanical failure of coupler.
Mantrip tire blew out while on slope. This caused the mantrip to hit the archways. Individuals involved were jostled inside the mantrip. Body contact with mantrip caused injuries.

Injured was walking the M2 belt line and EE's knee locked up and EE fell to the ground and when EE fell EE's left knee hit a piece of C-Channel. The employees left knee was x-rayed and it was discovered EE had broken EE's knee cap.
Injured was installing roof bolts and while bring pot up to match with roof bolt bent the bolt and struck injured on the left wrist. Injured was taken and diagnosed with a left wrist strain and was placed on restricted duty.

Injured picked up a piece of belt structure to move on a belt line and when EE went to toss it felt a pop in EE's shoulder. MRI results EE suffered a partial labrum tear in EE's Right arm.
An unintentional roof fall has occurred on the working section of the #1 Unit. The fall occurred in the #7 entry of the 17th-XC of the S1-G submain.
Injured was helping pick up cable to hang between entry 3 of the #3 unit and claims ankle buckled. Injured was taken for medical attention and was told had a sprain and was placed on restrictions.
Injured was bringing up the second steel when it caught and struck EE's hand. This caused a dislocation of EE's thumb and caused EE to be placed on restricted duty.
Injured was carrying a piece of tin for ventilation controls and tin got hung up and then buckled back and struck the injured in the face. This resulted in a laceration that required stitches.
Injured was using a pry bar to maneuver a pot into place for a miner and pry bar slipped striking the left hand of the injured. This resulted in a non-displaced fracture of the 5th metacarpal of EE's hand.

Was sent for x-rays and was discovered they had an Fibula comminuted fracture, distal, non-displaced and tibia avulsion fracture non-displaced.

Injured hit self in the mouth with drill steel chipping front tooth out.
