Employee was removing old hog panel from the mine roof (M8 crosscut #66-#67 entry #2) when debris got into the employees left eye. Employee was seen by Dr. on 12/18/2025 and given a prescription for eye drops for irritation.

Employee was roof bolting (rehabbing) M7 belt-line Entry #5 cross-cut #28 when EE slipped on a flat rock, hyper extending EE's right knee. MRI results from 11/25/2025 showed a torn meniscus.

Employee was cutting out old hog panel and scaling down loose ribs to re-fence the rib-line, when sulfur ball measuring appox. 14 inches wide 16 inches in length 10 inches thick rolled out landing on employees left foot.

Unplanned roof fall occurred on the travelway of M4 xc 38. the fall was approximately 60 foot long and 20 foot wide and approximately 10 foot thick.

Employee was loading a grinder on a fifth wheel trailer and felt their left bicep muscle flex more than it should have. An MRI showed a small tear to the bicep and employee was placed on restrictions.

Employee was putting a scoop battery on charge. While lifting the battery lids, one of the lids closed and smashed the tip of EE's right finger. Employee was treated and placed off work until release from orthopedic surgeon.

Employee was trying to move a bundle of saddles by hand and felt pain in the right bicep. An MRI showed that there was a partial tear in the bicep. Employee was placed on restrictions.

Employee was reclaiming a drive set-up. When dismounting from the tail, the employee stepped on a joint of waterline and twisted the knee. Employee was placed on restrictions.

Employee was walking an air course and stepped in a hole and twisted the right knee. Employee was seen by a doctor when the knee did not seem to get any better. An MRI on 4/18/24 showed a tear to the meniscus. Surgery is scheduled for 4/26/24.

Employee was bent over picking up a timber when the stack fell and three of the timbers landed on the left hand. Middle and ring finger were both fractured.

Employee was using a pry bar to take down a rock when the rock broke and struck the employee in the back and neck. Employee was diagnosed with a left arm contusion and was placed on restrictions.

An unplanned roof fall occurred in the LOXC on the P2-1 in entry 4 at crosscut 18. The fall measured approximately 70' in length, 17' in width and 8' thick.

Employee was walking ""quickly"" towards the miner. When they went through the curtain they hit their hip on the corner of the miner tail, causing them to fall backwards and land on the buttocks. Employee was placed on restrictions for tailbone pain.

Employee was replacing a steering cylinder on the battery hauler when the finger was struck while inserting a pin. Employee received 4 sutures to the finger.

Employee was bolting on unit #2 in 8R when it was decided to use hog panels for the spot. While putting the hog panel on the ATRS it slipped off. When employee bent over to get the panel a rock struck the back of the head. Employee was taken to ER and was diagnosed with a concussion and placed on restrictions.

Employee had drilled a hole and was in the process of inserting the glue into the hole when a rock fell and struck the right shoulder. Employee was placed on restrictions for a shoulder contusion.

Locking mechanism froze due to the weather temperature dropping.

Employee was working on the continuous miner when EE stepped off to get a tool and twisted their knee. Employee has been working without restrictions. A follow up and MRI on 1/19/2024 showed a tear to the right meniscus. Surgery is scheduled for 2/02/2024.

Employee was helping change a top roller on a mainline belt. When the employee pulled the roller under the top belt, they felt pain in their back. The employee was placed on restrictions on 12-26-2023.

Employee was lifting a rail onto the rail setter when it rolled off and smashed the middle and ring finger between the bed of the hauler and the rail. Employee received a fractured finger on the left hand.

Employee was helping hang the miner cable and stepped into a hole, causing pain to the right knee. Employee was placed on restrictions and is awaiting an MRI.

Employee was trying to remove a rock from the scoop and cut the right arm on the rock. Employee was taken to the ER and received 5 sutures.

Employee was roof bolting in entry 4 on unit #1 when a rock fell from the top and struck the leg. Employee received a spiral distal tibia fracture to the right leg.

Employee was changing out a broken belt saddle. While hammering the pin out, the employee struck self in the hand with a hammer and received a contusion to the left hand. Employee was placed on restrictions.

Employee was using a ratchet to loosen a bolt on the flame trap when the ratchet slipped, causing the employee to hit the arm on a pressure valve and cutting the elbow. The employee received 3 sutures.

An unplanned roof fall occurred on the S1-2 at crosscut 22 entry 4-5. The fall measured approximately 80' long by 17' wide and 10' thick. It appears the roof shifted causing the roof support in the area to fail.

Employee was working on a compressed airline underground. When a hammer was used to make sure the shut off valve was closed completely the valve blew apart. The employee was standing with their left foot in the mud and when the line blew apart the employee turned to the side but the foot did not. Employee received a compound fracture below the knee and fractured wrist.

Employee was handling hog panel in the beltline when EE's right forearm was lacerated. Employee was taken to the ER and received staples and a tetanus shot.

Employee slipped when shutting off a main water valve and cut their hand. Employee was taken to the ER and received 4 sutures.

Employee went to lift a timber and felt the knee pop. On 9/5/23 an MRI showed a left tibial bone contusion. Employee was placed on restrictions.

MSHA inspector was on site testing hoist functions when the safety catch test failed. The hoist was placed out of service and phoned in. The hoist was then tested in the same way that it was commissioned and the safety catches passed.

Incoming power to our substation was lost. When power was restored, the 500 KVA transformer shorted out. Our hoist operator was 50' from bottom doing the daily checks when power was lost. The hoist was manually released to get the hoist operator to the bottom of the shaft. A new transformer was installed.

Employee had just been loaded and left the continuous miner. When traveling to the feeder, employee ran over a rock and struck their head on the canopy. Employee was placed on restrictions.

Employee was cribbing in the intersection when employee reached for a crib block and felt a pull in the shoulder. An MRI on 8/15/2023 showed a strain to the rotator cuff and the employee was placed on restrictions.

Injured was taking tie wire off of a loose truss bolt that had been tied up to the roof. When the truss bolt became loose it struck the injured in the mouth. Injured received 1 suture.

Injured was operating the mini trac and ran over something causing the mini trac to drop down with jarring impact. The injured felt a popping in the right shoulder. An MRI on 8-7-23 showed a tear in the rotator cuff. Surgery is scheduled for 8-11-23

An unplanned roof fall was discovered in entry 3 on the #1 unit at xc2. The fall measured approximately 20' in length, 18' in width and 10' high.

Injured was lowering the drill pot down when something struck the knee forcing downward on the leg. The injured was taken to the ER and X-rays showed a fracture ankle.

Injured smashed finger between the canopy and the rib when swinging out the install the outside roof bolt. Injured received a fracture to the middle finger.

Injured was bolting in 3 right when a rock fell from the corner of the nitch and struck the back of the neck/head. ER doctor said possible concussion and placed on light duty until follow up with family doctor.

The Lilly Sequence went out of sync on the man hoist.

Injured was moving the continuous miner cable by hand during a cut when the shoulder popped causing pain and lack of movement. Injured was put on restrictions for a sprain to the shoulder.

Injured was helping change miner bits and was using a hammer to set the new bits into place. While hammering a piece of metal broke off and flew into the right eye. Injured was taken to ER and had surgery to remove the metal.

Injured was washing a coal hauler when the water splashed back against the top causing debris to get in the right eye. Injured had eye flushed and was giving prescription eye drops.

An unplanned roof fall took place on Unit #1 at crosscut 57, entry 11, inby the loading point. The fall was approximately 20' long, 18' wide and 8-10' thick. An action plan was submitted for the immediate area and a revision was made to the roof control plan.

Injured was using a hammer and a header board to install 4"" waterline. The hammer turned and hit the index finger of the hand holding the header board. The injured received six sutures.

Injured was helping replace a rear end on a battery scoop. When a hydraulic hose was removed the battery was not cribbed tight and moved, this allowed the battery to pinch the lower left leg. The injured received a tear in the distal calcaneofibular ligament, and a partial tear of the posterior talofibular ligament.

Injured was helping build cribs at the M20 drive area when a 6""x6""x4' crib block fell about 5' striking injured in the neck and shoulder. It was determined on 3/6/2023 that the injured had a concussion and was placed off work.

Injured was unloading structure rollers from the scoop bucket when the rollers moved and smashed the middle finger against the side of the scoop bucket. Injured received 4 sutures.

Injured was driving a tractor hooked to a generator. The feeder was tramming in front of the tractor when it came in contact with the lifeline freeing it from the roof. As the injured moved the lifeline away from the injured, it got wrapped around the neck. Injured received slight hematoma to the neck area.

Injured was roof bolting on Unit #1 entry 1 when a rock fell from around the hog panel and hit the leg and foot area. Injured received a contusion lateral lower leg, foot and ankle and was placed in a moonboot.

Injured was running the continuous miner on Unit #2 when injured was struck above the right ear. Upon inspection it appeared to be a piece of carbide. Injured was taken to the ER to have the carbide removed and received 1 suture.

Injured was trying to remove the tail roller keeper with a hammer and a pin hammer. The injured swung, missed, and contacted the hand holding the pin hammer. X-rays showed a transverse fracture of the 2nd metacarpal.

Injured was building cribs in the intake on Unit #2 xc42 when they felt something pull in the stomach area. On 1/26/2023 a CT scan was done and showed the injured had a hernia. Surgery is scheduled for 2/22/2023.

Injured was roof bolting on unit 1 in entry 6. The drill steel slipped from the left hand and landed between the top of the left foot and the drill boom that was being lowered. This put enough pressure on the foot to cause a fracture of the 2nd metatarsal.

Injured was helping mechanic work on the drill feed pot. As they were lining up the holes to install a bolt, the pot shifted and smashed the tip of finger. The injured received 3 sutures to the tip of the right hand middle finger.

Injured was tramming a roof bolter on unit #2 entry 3 crosscut A. As they turned the corner the roof bolter became a little unstable from a drop in the mine floor. As the injured reached for the top of the canopy to help with their balance a crossover pad fell from the canopy and struck injured on top of the hand. This resulted in a fracture of the lower part of the middle finger.

The north gate switch went bad.

Injured was working in the beltline when EE stepped in a hole and twisted EE's knee. On 12/06/2022 MRI results showed a meniscus tear in the right knee. Surgery is scheduled.

Injured was joining the starter and the pusher drill steels together. When the pressure was applied the steels did not align correctly and the steels popped the injured on the hand. The injured received 4 sutures between the thumb and the finger.

While setting timbers in the M3 Intake, the injured stepped in a hole and strained the right knee. On 11/22/2022 an MRI showed a torn medial meniscus of the right knee.

Injured was at the M11 seal area building walls. At the end of the shift, EE felt a slight pain in the abdomen area. The next day there was swelling in the abdomen. On 11/22/2022 a CT scan showed EE had a large left inguinal hernia.

Injured had reached for roof plates on the bolter when the operator trammed forward. The left foot was in front of the tire as the bolter moved forward. Injured was diagnosed with a fractured left great toe.

Injured was driving a mantrip on the haul road. Injured was pulling into a crosscut to get out of the way of an oncoming piece of equipment. They had their hand/fingers through the opening of the steering wheel when the tire ran over a rock and the steering wheel spun. Injured was diagnosed with a 4th and 5th metacarpal fracture.

An unintentional roof fall happened in the face of entry 8 located between crosscut 21 and 22. There was a slight start in elevation change and the coal seam was starting to pinch off.

Injured was working on the Mine No2 fan. While EE attempting to open the mandoor at the back of the fan, EE lost EE's footing and fell off the concrete ledge (46"" high). Injured fell to the ground and landed on EE's left side fracturing four ribs.

Injured was helping build cribs in entry 2. A curtain board (2X4) was broken and hanging from the top in the way. Injured was prying on the board when it broke off and hit injured in the mouth knocking a tooth loose. A dentist ended up removing the tooth.

Injured was drilling the inside hole. The 2nd steel became clogged causing it to bend. Injured attempted to bring said steel back down, breaking the steel to steel insert and spinning the steel catching the injured across right forearm causing a laceration to the right arm.

Injured was bending a stamped 8' roof bolt on the cab of the roof bolter. The bolt rolled and slipped off striking the injured in the lip. Injured received stitches to the top lip area.

Injured was climbing out of a coal hauler when injured's feet slipped and felt pain in right inner thigh. Injured was diagnosed with a groin strain on 6/27. Injured was seen again on 7/14 and 7/28 for same injury. On 7/28 a CT scan was ordered. On 8/9 it was determined that the injured needs hernia surgery.

Bad storm caused a power outage. When power was restored the PLC had to be re-programmed.

Power loss (flicker) caused the D/C drive to fault

During a non constructive test, a distortion was discovered on one of the ropes. The hoist was removed from hoisting men underground.

Injured was cutting down 3"" waterline to reclaim. When IE cut the wire holding up the waterline the roof fell out around the bolts, landing on IE pushing IE to the ground. IE cut right hand open and fractured L5.

injured was putting 2 drill steels together. The steels were not all the way together when pressure was applied. This caused the steels to pop and strike injured in the hand causing injured to receive stitches.

Injured was a passenger in a 2 person mantrip. The driver ran over a timber in the road and caused the mantrip to run into the rib. The injured was brought back outside with right ankle and knee pain. MRI on 3/16 showed torn ligament on side of ankle and fracture in back of ankle.

Injured had just drilled a hole in the top. As EE twisted to grab the glue and roof bolt, the knee popped and dislocated.

Injured was unlatching fence hook on ATRS pad and got fingers caught between ATRS pad and hook handle. The ATRS was being raised when the pad came in contact with the top and the hook latch smashed fingers

Injured was getting the next hog panel to install that was leaning against the rib, a rock came loose from the edge of a previously installed panel and stuck injured in the back of the head.

Injured was pulling a roof bolt from the working tray when they came in contact with the edge of a roof plate. This resulted in a laceration the the left forearm, which the injured received stitches for.

Injured was building cribs on unit 2 in entry 6 when Injured picked up crib block & strained Right Shoulder. Injured was sent for an MRI on the shoulder on 12/8/21, then on 12/15/21 MRI Results revealed a tear of Rotator cuff.

Pre-shift examiner discovered a roof fall on the #1 Unit at the 33xc of the S1H in the #10 entry. The fall was cribbed off and additional supplemental support was added to the surrounding areas.

An unintentional roof fall occurred on the S1L at the 15xc in the #7 entry in the last open XC. The fall was discovered by the examiner during the pre-shift examination for the on-coming shift. The fall was approximately 20' x 20' x 10'

Injured claims hit head on the canopy of the mantrip as the ride was coming out of the mine. Injured reported the incident the following day. Injured was sent for observation and diagnosed with Myofascial strain and placed on restrictions.

Injured went to pick up a Roof jack off of the mine floor to install along the travel way of the M14 and when EE picked up the jack EE felt a pop in EE's forearm. Injured continued working and the following morning had a knot in the forearm of EE's arm. Injured was sent for medical attention and then sent for MRI. For possible bicep muscle tear.

Injured fell and hit elbow on the coal rib and the mine floor causing elbow to become dislocated. Injured was transported for medical attention and and elbow was put back in place.

Injured was swinging sledge hammer while installing bit inserts for the miner. Injured said the next morning EE had trouble lifting EE's arm. On 10/11/21 injured filled out accident report and was set up for appointment to be seen for shoulder and on 10/13/21 injured was placed on restrictions.

Injured was putting crib blocks under the miner because it was stuck in the intersection of the #10 entry. While putting the cribs under the machine EE smashed two fingers between the crib block and the ground. X-rays revealed EE had fractured both fingers.

Injured was cleaning walkways along the M8 belt line and bent over to pick up rail and strained lower back. Injured was taken for observation and placed on restrictions for a lower lumbar strain.

Injured was bending over and putting waterline together and when EE stood up said EE felt a pop in EE's knee. Injured continued working and on 8/27/21 injured reported the incident and was set up for doctor appointment on 8/30/21. On this date injured was placed on restrictions and is awaiting an MRI.

An unintentional Roof fall occurred on the S1J at the #2XC. The fall was approximately 30-foot long by 30-foot wide and over 10-foot high. The fall was discovered by the examiner.

Injured was installing 8' bolt and bent and smacked hand. This caused a sprain to EE's right hand. EE was placed on restrictions on 8/10/21.

At approximately 4pm the hoist man received a call from the work deck to bump up on #2 work deck winch. The hoist man reportedly heard a noise coming from #2 work deck winch, EE checked the winch and seen that the 1"" cable was unspooling from the drum and falling into the shaft. The cable completely unspooled to the work deck below due to mechanical failure of coupler.

Mantrip tire blew out while on slope. This caused the mantrip to hit the archways. Individuals involved were jostled inside the mantrip. Body contact with mantrip caused injuries.

Mantrip tire blew out on the slope. This caused the mantrip to hit the archways. The passengers were jostled around in the mantrip. Injuries occurred due to bodily contact with mantrip.

Injured was taking side cover off of the miner and lost grip on the cover and smashed finger between the cover and the machine. This caused a non-displaced fracture on EE's finger.

Injured was driving in a battery lift pin on battery hauler and while driving it in EE felt a pop in EE's shoulder. Injured had an MRI and results revealed EE had torn articular tear.

Injured was walking the M2 belt line and EE's knee locked up and EE fell to the ground and when EE fell EE's left knee hit a piece of C-Channel. The employees left knee was x-rayed and it was discovered EE had broken EE's knee cap.

Injured was installing roof bolts and while bring pot up to match with roof bolt bent the bolt and struck injured on the left wrist. Injured was taken and diagnosed with a left wrist strain and was placed on restricted duty.

Injured was recovering belt structure and while carrying a belt saddle and slipped and fell into standing support along the belt line. This caused them to smash a finger between the standing support and the belt saddle which after being checked caused a small fracture at the tip of the index finger.

Injured was pulling on miner cable on the date listed above and continued to work and then on 5/18/21 injured had an MRI done and it was detected that the injured has sustained a small infraspinatus tear and was referred to an Orthopedic.

Injured was helping install rail bars in the belt line and while handling the rail bar EE lacerated EE's 5th finger across the Dip Joint. The laceration required stitches.

Injured was installing roof bolts and when EE pushed bolt into the roof EE hurt stomach area. On 4/29/21 injured had a CT scan and Hernia was discovered. On 4/30/21 injured was placed on Restrictions.

Injured hit head on canopy of battery hauler after hitting an uneven surface while operating the hauler. Continued to work until 7/12/21 when injured was brought outside with neck pain. On 7/13/21 injured was placed on restrictions for the injury.

Injured was lifting a timber and hurt EE's left shoulder. On 4/1/21 injured was seen and placed on restrictions and is awaiting an MRI.

Injured reported on 3/16/21 that EE had fallen and hurt shoulder but could not remember when it was when it had occurred exactly. Injured was sent to dr and on 3/24/21 was reported EE had a torn rotator cuff.

injured was installing bolts in two left blow through, when installing the outside bolt of the last row the bottoms which the foot jack was sitting broke apart causing the canopy to come down approximate 8-10 inches and striking injured on EE's head. Injured continued to work but neck become sore. Injured was taken for observation and placed on restrictions

Injured was using a pipe wrench to tighten a fitting and strained left shoulder, on 1/20/21 injured was placed on restrictions from doctor and reported the incident to the safety department

Injured was moving a battery cell at the battery station when it slipped from EE's grip causing EE to strain EE's left lower back. Injured continued to work EE's shift then on 12-21-20 went for medical observation and at that time was placed on restricted duty.

Injured picked up a piece of belt structure to move on a belt line and when EE went to toss it felt a pop in EE's shoulder. MRI results EE suffered a partial labrum tear in EE's Right arm.

An unintentional roof fall has occurred on the working section of the #1 Unit. The fall occurred in the #7 entry of the 17th-XC of the S1-G submain.

Injured was helping pick up cable to hang between entry 3 of the #3 unit and claims ankle buckled. Injured was taken for medical attention and was told had a sprain and was placed on restrictions.

Injured was bringing up the second steel when it caught and struck EE's hand. This caused a dislocation of EE's thumb and caused EE to be placed on restricted duty.

Injured was carrying a piece of tin for ventilation controls and tin got hung up and then buckled back and struck the injured in the face. This resulted in a laceration that required stitches.

Injured was using a pry bar to maneuver a pot into place for a miner and pry bar slipped striking the left hand of the injured. This resulted in a non-displaced fracture of the 5th metacarpal of EE's hand.

Injured was getting fencing for the unit and when EE went to cut the metal bands that were holding the fence together the band backlashed and lacerated the hand of the employee. Employee had to have stitches for the laceration.

Injured hit an elbow on the canopy of the bolter, on 7/31/20 injured went to have elbow looked at and had an olecranon bursitis on elbow. The elbow had fluid drained and injured was placed on restrictions on this date.

Injured was walking and stepped on uneven ground and twisted left knee. On 7/20/20 MRI results show EE suffered a Posterior Horn Medial Meniscus Tear.

Injured was in the process of picking up a cable and as EE was picking up felt a discomfort in EE's bicep muscle. Injured was taken for observation and discovered EE had torn EE's bicep muscle.

Injured was helping lift a piece of metal and cut the middle finger on left hand. Injured was taken for medical care and received stitches for the injury.

Injured bent over to pick up a piece of fencing off of the ground and when EE did EE said EE felt a sharp pain in EE's back. Injured was placed on restrictions on 4/10/20 for a muscle strain in back.

Injured twisted EE's knee on 3/27/20 but continued to work. Injured still had pain and was sent for an MRI and discovered on 4/20/20 that EE had suffered a meniscus tear in EE's knee.

Injured was roof bolting on the #2 Unit when a piece a draw rock fell and struck EE on the right lower back. Injured was placed on restrictions for a deep muscle bruise.

Injured was roof bolting on the #1 unit and when EE went to connect EE's pusher steel into the roof it buckled and struck EE's hand. EE's hand was x-rayed on 2/24/20 and a fracture was revealed.

Injured was walking around coal hauler and stepped on uneven surface and sprained left ankle. Injured was placed on restrictions on 2/3/2020 for the ankle.

Injured stepped on uneven surface and twisted ankle on 1/24/2020 but continued to work regular duties until 2/18/20 when employee was still having discomfort in the ankle. On this date the employee went for medical observation and an MRI and was placed on restricted duties.

Injured was connecting split steel and drill pot caused steel to bend and strike injured on the index finger. X-rays revealed finger was broken and injured was placed on restrictions.

Injured claims on 11/22/19 injured hurt shoulder lifting crib block but did not seek any medical observation until 12/11/19 where injured was placed on restriction for the shoulder.

Injured was roof bolting and was putting in split steel and caught the lip of the drill pot and steel bent and hit their thumb. This caused a dislocation of the thumb.

An unintentional roof fall has occurred on the S2-J at the 25th xc of the #8 entry. The fall occurred in an intersection and was approximately 18'x 18'x over 8' high.

An unintentional roof fall occurred on the S2J in the #9 entry of the 21st Xc. The fall was approximately 25' x 18' by 8'. A plan was implemented to add supplemental support to the area.

Injured was walking backwards and slipped and fell, straining left knee. On 10/21/19 MRI results came back showing injured suffered a meniscus tear and was referred to a Ortho for further consult.

An unintentional roof fall has occurred on the S1B at the 7th xc in the number 6 entry. The fall was discovered by the weekly examiner. The fall was approximately 20'x20'x8'.

Injured strained back after throwing cable over underground. Was taken for medical attention and was placed on restrictions.

Injured was moving a tire and felt something pop in upper arm, injured was taken for observation at medical facility and scheduled for an MRI to determine if injured had tore the bicep muscle. Injured was placed on restricted duty and awaiting MRI results.

Injured was roof bolting on the M4 travelway and while installing a roof bolt pinched EE's thumb between the wrench and the roof plate. Injured received a laceration from EE's injuries which required stitches.

An unintentional roof fall has occurred at the XC-12 of the S2H in the #6 Entry. The fall was approximately 15' x 17' X 10'. The fall was barricaded off and a site specific action plan put in place to rehabilitate the roof in the area.

An unintentional roof fall has occurred at the M4 Xc-32 in the #4 Entry. The fall was approximately 25' x 20' x 12'.

Injured was trying to unplug drill pot and strained a wrist while attempting to unplug it. Continued working but was still having discomfort in the wrist. On 7/19/19 went to have wrist looked at and had a partial tear in a tendon in the wrist.

Injured was installing a roof bolt and as injured was pushing the bolt into the roof felt a pop in shoulder. Injured was given an MRI and discovered injured had a torn rotator.

injured was building a stopping and stood up and felt a pop in EE's knee. Went for an MRI and on 6/7/19 results were given that EE had a tear on EE's meniscus.

Injured strained back while lifting a Roof jack. Injured was placed on restrictions for the strain.

Injured was tearing down belt structure at the S4-C drive area and got a foreign body in EE's left eye. The foreign body had to be surgically removed from EE's eye.

Injured was pulling cable off EE's shuttle car at the end of the shift and strained EE's shoulder. Injured was placed on restricted duty for EE's injury.

Injured was handling materials for building an undercast and strained back, on 5/6/19 injured was seen by Doctor and placed on restrictions for the thoracic lumbar strain.

Injured was cutting a PVC pipe and suffered a laceration on the left forearm and received stitches for the injury.

An unintentional roof fall has occurred at the 23xc of the S4-P in the #9 Entry. The fall was approximately 20' long by 16'11"" wide by 4' to 5' thick. The approaches to the fall were barricaded and flagged off.

Injured Reported on 3/14/19 that EE had been working on a conveyor chain linkage and while EE was hammering on it that a piece of metal struck EE's leg. Injured was sent for x-rays and had a piece of metal surgically removed from leg on 3/18/19.

Injured was bringing pot up to insert pusher steel and caught the lip of the chuck and caused the steel to bow and strike hand. X-rays were taken and injured suffered a broken finger.

Injured was throwing over miner cable and strained shoulder and on 2/12/19 injured went to doctor and was given a prescription for strained muscle making this a reportable incident.

Injured was stepping out mini trac and twisted EE's ankle. Injured was placed on restrictions and was given a prescription for pain.

Injured was hand dusting the roof and ribs when they stepped on a rock and twisted an ankle. Injured continued to work and the following day was still experiencing discomfort in the ankle. Was sent for x-rays and was discovered they had an Fibula comminuted fracture, distal, non-displaced and tibia avulsion fracture non-displaced.

Injured was throwing cable over and felt a pop in their lower back. Injured continued to work but was experiencing discomfort in the lower back and leg area. Injured went for evaluation at Doctor on 12/6/18 and was placed on restrictions and given medication for the injury

Injured hit self in the mouth with drill steel chipping front tooth out.

Injured was connecting split steels and brought the drill pot up to reconnect and caught the drill steel and caused the steel to bend and strike the palm of the injured. Injured was taken the next day for evaluation and x-rays and it was discovered they had suffered an avulsion fracture at the base of the thumb.

Injured was bolting in the #2 entry of the P1-F, while they were installing the middle bolt, injured dropped the drill pot on top of their right foot. Injured continued working and when arrived on surface the foot had began to swell and injured was taken to ER for evaluation. It was discovered with x-ray they had small fractures on top of the foot.

An unintentional roof fall has occurred on the M12 at the 32nd Xc of the number 4 entry. The area was in the process of being re-supported when the failure occurred the fall was approximately 35' x 40' x 12'. A site specific plan was submitted and the area is having additional support installed.

Injured was struck by a piece of draw rock approximately 3' x 4'x4"". Injured was taken to hospital for medical treatment and was discovered EE suffered fractures in EE's back and foot.

An unintentional roof fall occurred at the Xc-31 in the #10 Entry of the #2 unit. The fall was approximately 40' x 18' x 12'. The fall was barricaded off and a site specific plan to rehabilitate the immediate area was put in place.

Injured was struck by a piece of draw rock on the left forearm while bolting on the #2 unit.

Injured was moving screen wire to place on the Roof bolter and their hand was smashed causing a fracture of a finger and thumb.

Injured was turning to walk and twisted ankle and fell down while in the #8 entry of the #1 unit. Injured was taken to have ankle looked at and was originally told it was a sprain. On 6/15/18 x-rays were reexamined and it was discovered ee had a fracture.

Injured reported on 6/11/18 that ee had sprained wrist while changing out an insert on 6/8/18. Injured was taken for medical treatment and was given prescription for the injury and placed on restricted duties.

An unintentional roof fall has occurred in the #9 Entry at the 6XC of the S1C. The fall was approximately 30 feet in length and 18 feet wide by 14 feet high.

Injured fell when EE tripped over the power cable and fell on EE's back and landed on EE's SCSR. Injured was placed on restrictions for a lower lumbar strain.

Injured was walking and twisted their knee after stepping awkwardly, continued to have pain. On 2/26/18 had an MRI and discovered has a medial meniscal radial tear in the left knee. Was placed on restrictions on this date.

Injured was swinging a sledge hammer and strained a muscle in lower back, on 11/9/2017 injured was seen by doctor and prescribed a muscles relaxer for back.

Injured got a foreign body in left eye, went for treatment for injury to eye doctor and was given a prescription for injury.

Injured was building a stopping and stood up and stepped back and twisted left knee. Injured was taken to Hospital and had an MRI and it was discovered injured had a bucket tear of Meniscus of left knee.

Injured was using a grinder with a cut off wheel while working on a tail gate when the grinder kicked and struck the injured in the knee. This caused a laceration which required stitches.

Injured broke the tip of 4th finger on right hand, after hitting it with a miner bit. While trying to clean out the clip area for holding the miner bits in the insert, the injured hit finger with a bit causing a small fracture on the tip of finger.

Injured strained a muscle in their back while performing routine maintenance on the roof bolter. The injured was seen on 8//28/2017 by doctor and prescribed a script for a muscle relaxer for the muscle.

Injured strained back on 8/4/2017 and continued to work on 9/28/2017 injured had an MRI performed and it was discovered injured had a herniated disc in back.

Injured was struck by the battery end of a coal hauler, as employee was working on miner cable along the rib-line. The injured suffered bruising along their side with no fractures or other injuries.

Injured was bringing up the pot to connect to the split steel when the drill steel caught the chuck and buckled, and struck the injured on left thumb. Injured was taken to clinic for x-rays where it was discovered injured had dislocated thumb.

Injured was checking curtains at the start of shift when a piece of draw rock slid off the side of the fence and struck the injured on injureds lower back. Injured was taken to hospital where it was discovered injured had a transverse process fracture in injureds lower back.

The injured's left leg became wedged between a battery coal Hauler and the coal rib on the #1 unit after the miner backed into the hauler and pushed it back into the rib. After transporting employee to the hospital it was discovered employee suffered a fractured leg.

Injured had a hole in the canopy jack hose and attempted to lower the canopy with their hand wrapped around the hose, so the oil would not spray them. This caused a pin hole laceration on a finger and allowed the oil to be injected into the finger. Injured have to have the finger medically cleaned out.

Injured was cleaning rocks of the top of the miner and as employee was pushing the rocks off the employee lacerated the right hand and received 5 stitches.

A 51 year old miner operator with over 30 years of experience was struck by a battery hauler, while making second cut in the #3 entry to the right. The injured was transported to local hospital for treatment of injuries.

An unintentional roof fall has occurred on the P3-1 at the 16th XC in the #9 entry. The fall was approximately 20'X 20'X approximately 13'plus high

Injured was lifting a cable and strained shoulder. Continued to work but had discomfort on 12-23-2016 injured had MRI performed and on 12-28-2016 was informed that injured had a torn rotator cuff in the shoulder.

The injured was in the process of setting jacks when the bands on a new bundle of jacks were cut off the load of the jacks shifted and the injured attempted to stop the load when EE's finger was caught between two jacks causing a laceration. The injured received stitches to EE's finger.

The injured was assisting another employee with moving a belt bar. The Injured smashed EE's right index finger between the belt bar and a pallet of block causing a fracture to EE's index finger.

An unintentional roof fall has occurred on the P1-1 Panel at the #15xc in the entries of 9 and 10. The fall was approximately 100 feet long by 17.5 wide.

Injured hyper extended left knee while moving miner cable on the #2 unit. The injured was placed on restricted duties which prohibited EE from doing normal job duties.

The injured was packing supplies for cribbing and railing on the M2 area, when they twisted their right ankle. The injured was treated at a local facility for a sprained ankle but was placed on restrictions which kept them from their regular job duties.

An unintentional roof fall has occurred on the M3 at the #7XC in the #3 entry, the fall was located between a set of equipment doors going to the return. The fall was approximately 30' by 18' and over 12' high. The approaches were timbered off and additional support is being added to the area.

An unintentional roof fall has occurred on the M2 secondary escapeway at the number 30Xc in the #4 entry. The fall was approximately 30 feet in length and 17.5 feet wide. The fall was dangered off and a site specific plan was put in place to rehabilitate the immediate area.

Injured was struck by a piece of draw rock while turning a left hand slab, the rock hinged back off a piece of hog panel. The rock struck the injured on the left shoulder and back area.

An unintentional roof fall has occurred on the M-2 at the 4th XC of the #4 entry, the fall was approximately 80' in length by 17 3/4 wide by approximately 14' high.

The injured was roof bolting, reached up and pulled drill steel from the mine roof and felt a pop in right shoulder. The employee was seen by a doctor and placed on restrictions.

The injured was operating a roof bolter when left eye became irritated. The injured went to the doctor the next day and was prescribed medication for eye due to scratches on the eye.

The injured stated that EE had a sharp pain in EE's right shoulder after loading supplies on a roof bolter. The injured was seen by a doctor and placed on restrictions.

The injured was bolting with 2 piece steels and connected the second steel and raised drill pod up, the drill steel caught the pod causing the steel to bow, striking the injured in left hand. On 2/24/2016 the injured was given restrictions for left hand.

The injured was hanging power cable with tie wire when the tie wire can in contact with EE's left. This resulted in a abrasion to the corneal of EE's left eye.

The injured was in process of building a crib. When a crib block was handed to EE, EE lost grip of the crib block dropping it. When EE attempted to catch the crib block EE's pinky finger was smashed against another crib block resulting in a fractured finger.

The injured was operating a miner when a rock fell off the rear curtain side of the miner striking the injured left leg. The injured continued to work until 1/4/2016 when he went to the doctor and it was discovered he had a fractured fibula in his left leg.
