While tasked to fuel pit equipment, the employee crested ramp going to 5 pit when EE lost the front brakes. This caused the truck to descend ramp faster than normal with just the rear brakes working. The employee experience neck pain later in the shift.

A mechanic was using a service truck crane to install pull down chains on an overburden drill mast. He had tied a nylon rope to the chain and then to the crane. As he was pulling the chain/rope, the rope broke and made contact with the mechanic. He was taken to the ER and give 11 sutures in his lip and nose.
The operator of the Hitachi EX700 excavator slipped while walking on the track. He fell 43 inches to ground resulting in torn ligaments in his left knee. His first shift missed was on 01/10/2014.

The victim was clearing debris from the underside of a drag line. The victim was working on one of the sections when the plate liner fell. The plate was not just bent upwards into the section as usual; it was broken completely and only being held by debris. As the plate fell towards him it struck him on his shoulder and neck area.

An employee was injured while helping to load a dozer radiator and nose assembly onto a flatbed trailer. As the assembly was being lowered, the crane cable broke causing the assembly to drop several inches and fall forward, pinning the employee against a previously loaded motor. Employee was treated and released from the ER.

As the CAT 777 end-dump driver was travelling from the pit floor to the top of the coal seam (approx. 8 foot incline), he was jarred inside the cab of the truck resulting in a compression injury to his lower back.

Injured employee was in the dragline house, walking on a catwalk when his leg made contact with a 16""x53"" motor cover that had been removed while the crew was changing out motors. When he made contact with the cover, it fell into the walkway causing him to trip and fall on it. The injury resulted in a 2 inch laceration below the right knee which required sutures.
Employee noticed discomfort in his right shoulder while installing a bearing at the preparation plant. The employee has had two previous surgical operations on the same shoulder.
Injured employee was troubleshooting the alternator on a CAT D( N doxer when he instructed the operator to reenter the cab and start the dozer so he could check the voltage on a multi-meter. He then used his right hand to check the alternator belt tension at the same time the dozer started resulting in the partial amputaion of his right index finger.
The injured employee was stepping down and off the school bus manlift when he slipped on the bus steps. When he attempted to catch himself, he landed on his left hand causing a laceration/contusion to the left ring finger which required 8 sutures.
A dozer operator was attempting to enter the dozer push by travelling over a 6 foot berm. He misjudged the distance between the end of the push and the berm and tracked down the spoil wall approx. 40 feet. When the dozer stopped abruptly, his forehead struck the radio equipment mounted overhead, causing lacerations that required 17 sutures. The operator was wearing his seatbelt.

Employee backed over a large rock while operating a D11 dozer causing him to be jarred in the cab. He was wearing a safety belt at the time. The following day he had some discomfort in his neck and was scheduled for an appt. An MRI evaluation later indicated a cervical disc displacement and he was ordered off work effective 10/24/08.
Employee was returning tools to his truck after repairing a light plant. When he turned to begin walking, he tripped and fell over the tongue of the light plant landing on his left elbow. The area was well lit. X-rays on 3/21/08 were negative. He had an MRI on 4/21/08. He underwent surgery to repair an avulsion of the left triceps tendon on 4/25/08.
While attempting to align a dragline hoist motor to a motor base (<1 inch) using an overhead crane, the motor moved 42 inches horizontally until it made contact with an air valve with the employee's gloved left hand caught in between. The impact resulted in a fracture to the left index finger.
Windblown debris struck EE in the back while he was bent over doing general site clean up. THis resulted in a fractured bone in EE's back.

The mine experienced a premature detonation of a single deck in a single blasthole while performing the necessary checks on the digital detonator system. The blast area was evacuated prior to performing the checks.
As employee was driving a Euclid R170 end dump up an incline, he noticed an engine noise, so he stopped the truck and shut off the engine. The truck began to roll back a distance of 75 yards before he turned the truck into the berm, causing it to roll onto the truck's cab side. The impact caused a window to shatter. Glass caused a laceration to the right side of his face and eye.
The #7 main hydraulic pump on the Hitachi EX3500 excavator had a catastrophic failure. Evidence shows that the pump exploded and started a fire. The fire began near the right engine compartment and spread quickly. The operator was able to safely exit the machine.

As an employee was mounting an end-dump, he lost his footing and gripped the handrails tighter to secure his position on the ladder. In doing so, he felt a ""pop"" near his right elbow. Although he did not fall off the ladder, he suffered a ruptured tendon of the right triceps. The conditions were muddy at the time. He had surgery on 12/28/05 which was his first day missed.
While replacing a rotary breaker drive chain, employee was using a large sledge hammer to brace the back side of the drive chain while co-worker was driving a pin on the opposite side with a 3lb. sledge hammer. While co-worker was swinging the 3lb. sledge, the head of the hammer slipped off the wooden handle making contact with the left index finger of employee causing a fracture.
Employee was using a pry bar, it slipped and hit him in the head and side of his face.
Employee was repairing an electrical problem on a CAT 777B end-dump and as he exited the cab he slipped and fell 10 feet 4 inches between the opened door and the handrail to the gravel below. His boots were muddy from steam cleaning the end-dump.The fall resulted in a fractured right wrist and a compression in the lower back.

As employee was using a 4 foot pry bar to loosen a CAT D11 alternator, the bar kicked out and his left middle finger was smashed between the bar and the engine. The result was an avulsion of the nail bed which required sutures.
Ee was manually pulling out a 12 inch drift pin out of a bolt hole that was stuck, at which time it freed itself and came out striking him in the mouth. His right upper front tooth was chipped.
WHILE STANDING IN THE ENGINE COMPARTMENT OF A CAT 789 END-DUMP, EE WAS REPAIRING A FRAME CRACK WHEN HE TURNED TO HIS RIGHT TO REACH AN AIR CHIPPER. AS HE TWISTED, HE FELT A POP IN HIS BACK AND IMMEDIATELY NOTICED DISCOMFORT IN HIS LOWER BACK AND LEFT LEG.
EE was attempting to push an 80 gallon air tank through a bulkhead in the BE 2570 revolving frame when he lost control of the tank, smashing his left hand between the tank and the bulkhead. The result was a laceration/fracture to the left hand. He was wearing gloves at the time of the injury.

EE HIT HIS RIGHT ELBOW ON A DRYER IN THE PREPARATION PLANT AND LATER BUMPED IT AGAIN AND BEGAN TO NOTICE SWELLING.
AS EMPLOYEE WAS REMOVING A LARGE NUT FROM A THREADED BOLT, THE NUT & BOLT FELL SMASHING HIS LEFT LITTLE FINGER BETWEEN THE NUT AND THE DRAG CASE HE WAS WORKING ON. THIS INJURY RESULTED IN A FR ACTURE AND LACERATION WHICH REQUIRED 15 SUTURES.
AS EE LIFTED A 75 LB. CAT D7 TRACK ROLLER FROM THE FLOOR TO AN ELEVATION OF APPROXIMATELY 4 FEET. WHILE HE WAS LIFTING HE TWISTED AND IMMEDIATELY NOTICED DISCOMFORT.
THROUGHOUT THE COURSE OF EMPLOYEE'S SHIFT, HE WAS INADVERTANTLY EXPOSED TO WELDING & CUTTING PROCESSES THAT WERE TAKING PLACE AROUND HIM. THIS EXPOSURE RESULTED IN WELDING FLASH IN BOTH EYES. HE WAS TREATED THE FOLLOWING DAY.
AS EE WAS LIFTING A LARGE METAL PLATE USING A PAD EYE AND OVERHEAD HOIST, THE WELD ON THE PAD EYE FAILED CAUSING THE PLATE TO FALL APPROXIMATELY 12 INCHES AND LANDING ON HIS LEFT FOOT. XRAYS T AKEN AT THE GIBSON GENERAL ER INDICATED A FRACTURE TO ONE METARSAL IN THE LEFT FOOT.
AN UNPLANNED DETONATION OF A BLASTING CAP &/OR PRIMER OCCURRED AS A BE 2507W DRAGLINE WAS DIGGING OVERBURDEN FROM A PREVIOUSLY DETONATED CAST SHOT. THE UNPLANNED DETONATION OCCURRED APPROX. 5'ABOVE THE SURFACE OF COAL & APPROX. 65'BELOW DRAGLINE BENCH. THE BLAST SEPARATED A TOOTH FROM THE BUCKET. THE LAMINATED SAFETY GLASS OF THE UNOCCUPIED LEFT OPERATOR CAB WAS ALSO DAMAGED.
AS EE WAS HANDING A COWORKER A PIECE OF PLYWOOD, A WOODEN SPLINTER ENTERED THE BASE OF HIS LEFT HAND. THE SPLINTER APPEARED TO COME BACK OUT CLEANLY BUT ON 3/4/03 HE WAS STILL NOTICING SOME DI SCOMFORT SO WE SOUGHT MEDICAL ATTENTION. AT THAT POINT, HE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS PERFORMING MAINTENANCE IN A BE 2570 DRAGLINE PROPEL ROOM WHEN A GUST OF WIND SENT SENT DUST PARTICLES AIRBORNE WHERE THEY MADE CONTACT WITH HIS RIGHT EYE. HE WAS NOT WEARING EYE PROTECTION AT THE TIME. HE WAS TREATED AT WABASHVALLEY EYE CENTER AND ISSUED A PRESCRIPTION.
AFTER EE CUT OFF A 30-LB. METAL GUESSETT, HE CARRIED IT 3-FT. UP A LADDER AND PLACED IT ON A PLATFORM. AS HE LIFTED THE METAL CHEST-HIGH HE NOTICED DISCOMFORT IN HIS LOWER BACK.

EE SPRAINED HIS CHEST & BACK WHILE LIFTING A LG.METAL TABLE HE WAS FABRICATING. HE WAS TREATED AT MED. CENTER IN EVANSVILLE, IN.
AS EE ATTEMPTED TO DRIVE A 6"" CHISEL INTO A D11 EQUALIZER BAR, THE CHISEL KICKED BACK AND STRUCKHIS FOREHEAD. HE RECEIVED 4 SUTURES AT THE E.R.
EMPLOYEE RECEIVED A SECOND DEGREE BURN ON HIS LEFT HAND WHEN HE MADE CONTACT WITH THE HOT END OF AN OXYGEN/ ACETYLENE TORCH. HE WAS GIVEN A 5 LB LIFTING RESTRICTION WITH HIS LEFT HAND.
EE WAS MOVING A 3/4"", 4'X8' METAL PLATE RESTING ON TWO 4' SAW HORSES WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. HE WAS TREATED FOR LOW BACK PAIN AND RETURNED TO WORK WITH THE FOLLOWING RESTR ICTIONS: LIFTING ALLOWED UP TO 10 LBS., LIFTING WAIST TO SHOULDER, CHANGE POSITIONS AS NEEDED FOR COMFORT, NO BENDING OR STOOPING. HE WAS DIAGNOSED WITH A HERNIATED DISC ON 6-20-02.
AS EE WAS LOOSENING A BOLT ON A DRYER SCREEN WITH AN IMPACT WRENCH, THE WRENCH SLIPPED OFF THE BOLT CAUSING HIM TO LOSE HIS BALANCE AND FELL ON A SHARP EDGE. THIS RESULTED IN A LACERATION TO H IS RIGHT MIDDLE FINGER AND RIGHT SHIN. HE WAS TAKEN TO THE ER WHERE HE RECEIVED 10 SUTURES IN HIS SHINE, 11 SUTURES IN HIS FINGER AND A TETANUS SHOT.
EE WAS PULLING A FORD F700 SERVICE RUCK'S HYDRAULIC HOSE FROM THE FRONT OF THE TRUCK TO THE BACK WHEN HE FELT A POP IN HIS CHEST. THERE WAS ONLYA SMALL DEGREE OF DISCOMFORT INITIALLY SO HE REPORTED IT TO HIS SUPERVISOR BUT INDICATED IT WOULD BE O.K. AND RETURNED TO WORK. FIVE DAYS LATER THE DISCOMFORT RETURNED AND HE WAS TAKEN TO HOSPITAL TREATED FOR STRAINED RIBS.
EE WAS LIFTING A PIECE OF 4'X1' METAL PUNCH PLATE WHEN HIS LEFT HAND WAS PUNCTURED BY THE EDGE OF THE METAL. HE WAS TAKEN TO GIBSON GENERAL ER IN PRINCETON, IN WHERE HE RECIEVED ONE SUTURE AND A TETANUS SHOT.
AS EE WAS ATTEMPTING TO MOVE A D11N TRUNION BALL, IT SHIFTED AND ROLLED AGAINST ANOTHER TRUNION BALL. EE'S MIDDLE FINGER WAS PINCHED BETWEEN THE TWO, RESULTING IN A FRACTURE TO THAT FINGER. TH E TRUNION BALLS WEIGH APPROX. 2503 EACH. THE ACCIDENT OCCURRED IN THE MAINTENANCE SHOP.
EE ALLEGEDLY SPRAINED LFT KNEE WHILE DESCENDING LADDER OF INGERSOLL-RAND BLASTHOLE DRILL. THE DISTANCE FROM GROUND TO BOTTOM STEP IS 18"". THE LADDER SLID OUT FROM UNDER HIM AND AS HE AND THE LADDER HIT THE FLOOR, HIS FORE HEAD MADE CONTACT WITH A RUNG ON THE LADDER. HE WAS DRIVEN TO THE EMERGENCY ROOM AND RECEIVED 6 SUTURES FOR THE CONTUSION/LACERATION.
EMPLOYEE WAS REPAIRING A CAT 777B END-DUMP WHEN A PIECE OF RUST FELL BETWEEN HIS Z87 APPROVED SAFETY GLASSES AND HIS FOREHEAD SETTLING ON HIS RIGHT EYE. HE WAS TREATED AT EYE CARE CENTER.
AFTER SERVICING A CAT 789B END DUMP, EE WAS TIGHTENING A BOLT ON THE REAR HUB WITH A 3/4"" BREAKER BAR. AS HE WAS APPLYING PRESSURE, THE BOLT ON THE HUB STRIPPED AND HIS RIGHT LITTLE FINGER WAS PINCHED BETWEEN THE BAR AND THE HUB. HE WAS THEN TAKEN TO HOSPITAL, AND TREATED FOR A LACERATION AND A FRACTURE.
EMPLOYEE AND CO-WORKER WERE MOUNTING A TOOTH BASE ON A 100YD DRAGLINE BUCKET USING A SMALL SERVICE CRANE ON A MAINTENANCE TRUCK TO POSITION IT. CO-WORKER WAS OPERATING THE CRANE ABOUT 20FT AWA Y FROM THE TOOTH BASE AND INJURED WAS ABOUT 5FT AWAY, WHEN THE END OF THE CRANE SLIPPED OFF THE BASE STRIKING EMPLOYEE'S LEFT HAND. RECIEVED SUTURES IN 2 FINGERS AND SPLINT FOR FRACTURED THUM
EMPLOYEE WAS HANDLING A LARGE THREADED BOLT WHEN A PIECE OF METAL PENETRATED HIS LEATHER GLOVE AND ENTERED HIS LEFT HAND. THE METAL CAME FROM THE DAMAGED THREADS AT THE END OF THE BOLT. THE PIECE OF METAL WAS REMOVED AT ST. MARY'S WARRICKHOSPITAL.
EE WAS USING AN IMPACT WRENCH TO REMOVE A BOLT FROM A PIPE WHEN DUST WAS SENT AIRBORNE BY THE EXHAUST ON THE AIR GUN ANDMAD CONTACT WITH HIS RIGHT EYE. EE WAS UNLOADING CABLE FROM A DELIVERY TRUCK WHEN HE STEPPED ON A NAIL THAT WAS PROTRUDING FROM THE BED OF THE TRUCK. AS A RESULT, HE SUFFERED APUNCTURE WOUND TO THE RIGHT FOOT.

EE WAS WALKING ACROSS THE EE PARKING LOT WHEN HESLIPPED & FELL ON A PATCH OF ICE. HE COMPLAINED OF PAININ HIS RIGHT LOWER LEG & ANKLE. HE WAS TRANSPORTED TO WIRTH HOSP. & XRAYED & DIAGNOSEDS W ITH A FRACTURE OF FIBULA & ANKLE WITH POSSIBLE TENDON DAMAGE HE WAS IMMEDIATELY TAKEN BY AMBULANCE TO ST MARYS HOSP OF EVANVILLE ILL WHERE SURGERY WAS PERFORMED TO REPAIR ANKLE & LEG
EE WAS DISMOUNTING FROM THE HITACHI 2500 WHEN HESTRUCK HIS LEFT ELBOW ON THE HANDRAIL WHICH RESULTED IN A CONTUSION. HE WAS TAKEN TO THOMPSON FAMILY CLINIC. THE ELBOW WAS XRAYED & RESULTS WERE NEGATIVE. HE WAS PLACED ON RESTRICTED DUTY FOR 2DAYS
EE WAS IN THE PROCESS OF REMOVING AN ENGINE FROM THE DOZER. THE DOZER WAS DRIVEN UP ON THE 10""X10"" CRIB BLOCKS FOR EXTRA CLEARANCE UNDER THE DOZER. TO REMOVE THE BELLY PAN EE RAN HIS CRANE CAB LE DOWN BETWEEN THE TRACK RAIL AND FRAME UNDER THE BELLY PAN AND ATTACHED TO THE TRACK RAIL ON THE OTHER SIDE. THE CABLE IS TIGHTENED TO SUPPPORT THE PAN WHILE THE BOLTS ARE REMOVED, THEN IT
EMPLOYEE REMOVING RIGHT SIDE PUSH ARM ON A D11 BULLDOZER, STARTED TO PICK UP DOZER BLADE PYSH ARM WITH SERVICE TRUCK CRANE TO LET DOZER BLADE SLIDE FORWARD. CHAIN WAS NOT IN LINE, EMPLOYEE ATTEMPTED TO ADJUST THE CHAIN WHEN LOAD SHIFTED CAUSING THE PUSH ARM TO JUMP OFF TRUNION, EMPLOYEE WAS THROWN BY FORCE OF PUSH ARM INTO THE BUMPER OF HIS SERVICE TRUCK.
WHILE LIFTING NOSE OFF D9R AND LEFT LIST CYLINDER CAME AROUND HITTING FOREARM BETWEEN ELBOW AND WRIST. CONTUSION TO FOREARM.
