Employee was found in pick up truck unresponsive. Site employees began First Aid/ CPR / AED measures until EMS and First Responders arrived.

Employee was tightening up a fitting on an air starter. When EE was finishing tightening it EE felt a pop in EE's right shoulder. Employee was taken to OMP and has a strained right shoulder with work restrictions.

Employee was coming down a set of steps and on the bottom step rolled an ankle. Was taken to a clinic and given sit stand restrictions.

EE was cutting a piece of center rubber for a screen deck. When making the last cut on the rubber the box cutting knife slipped and struck EE's left leg causing a laceration. EE was taken to the Dr. and evaluated and received 6 stitches on the cut. Accident under investigation.

Employee was putting on a life vest to go onto a barge. As they were putting the life vest on they were bitten by a small insect. Employee was taken to clinic to be evaluated and prescribed steroids for swelling.

EE was closing the window of a Cat 990H. When EE closed the window the right pinky was smashed in the hinge. EE was taken to OMP and the nail was removed and received stitches.

Contact with hot objects or substances

Employee was torch cutting a bolt lose in a feeder liner. A piece of hot slag popped up underneath employee's face shield and safety glasses and a small piece of slag entered the eye. Foreign object removed by eye dr. No other treatment, medicine, no other observation & no other follow up deemed necessary per eye dr.

Struck against a moving object

EE was hauling from the pit to the jaw crushed. When the loader operator loaded EE's truck a larger rock rolled into the back of truck hitting the side of the bed and jarring the truck, thus causing EE to have some lower back pain. EE was taken to be evaluated and is being treated for a pinched nerve. Dr. prescribed a steroid and physical therapy.

EE was trying to remove a suspended deflector plate when the left index finger came between the lift chain and the plate. This resulted in a broken/smashed index finger. EE was taken to the hospital and treated for a broken/smashed finger and received stitches. EE was also prescribed antibiotics and pain medicine.

EE was helping change screen at the plant. Another employee helping moved the screen cloth and set it on EE's left hand. This resulted in EE's Left hand Ring finger being smashed. First aid was administered at the quarry. The employee received medical attention getting stitches on ring finger on the left hand.

Preparing to repair a dump belt splice on a 18 ft x 72 inch conveyor belt. The belt was locked, tagged and verified then the drive belt guard was removed. Injured was using both hands to pull & push on the v-belt drive belts. EE caught index finger between the v-belt and the pulley. Fracture of the distal phalanx left index finger.

Struck by falling object

EE was cleaning berm from in front of spoil/stripping ledge prior to break time. Material from the ledge slid down the wall onto the bucket down the boom and through the windshield of the cab. Victim exited loader and sought first aid. EE was sent to medical care, and diagnosed with lacerations and contusions. EE was perpendicular to the face and material was rock and mud.

Employee suspects to have re-aggravated EE's back while operating the loader all day the prior to seeking medical treatment. EE cannot identify what or when EE's injury occurred.

Lifting a light fixture from shoulder height to ground felt a pop in lower back. Fixture weighed 30 - 40 pounds.

EE was filling and lifting 5"" bags for powder truck. Felt twinge in back.

Upon leaving the quarry to travel to other mine he felt a burning feeling in his eye. He said the wind was wild because of weather front coming in. He called safety and said something is in his eye. He went to eye doctor - rust speck was removed off the eye. Eye doctor administered Rx eye drops as required by Indiana law. EE wears safety eye wear.

Employee was climbing down the ladder of the CAT loader. When he reached the ground, employee stepped awkwardly on the dirt on the ground, rolling his left ankle.

EE was exiting the forklift correctly. When stepping down from lift onto the concrete his ankle rolled causing a sprain to the ankle. He was given restricted duty work.

EE had removed some metal from the detector on the conveyor, and a softball size rock bounced off of the conveyor that had just recently restarted, and somehow struck him on his neck and shoulder. He was taken to medical care facility for an examination. He was given one day of restrictions until tests came back. He is already back full duty.

Struck by falling object

Using a hydraulic jack mounted in a steel frame to press out a king pin in a 35 ton truck. The steel frame and jack fell and struck him on the right leg and ankle resulting in a bruised and sprained right ankle.

Welding on a piece of angle iron. Removed glove to get a drink of water then picked up hot angle iron before putting glove back on resulting in a burn to his right hand.

Walking to electrical building to get supplies, slipped on ice and fell on left elbow causing a bruised elbow.

While replacing ski ramp planks he stepped through a hole in the ski ramp and fell. He was carrying a plank which dropped on his little finger causing a laceration requiring sutures.

Struck by flying object

Victim was inside a truck, dumping spoil, when an explosive shot was fired in the quarry pit. A piece of fly rock traveled over the highwall and about 1200 ' striking the EE, causing a laceration requiring sutures.

Performing maintenance on a day when the temperature was in the high 90's. Got overheated resulting in heat exhaustion.

EE fell from landout platform striking his head causing a laceration requiring sutures.

Lifting an oxygen cylinder to put on the back of the welding truck and pulled a muscle on his left side resulting in a strained muscle in his groin.

Removing punch plates from a screen, one of the punch plates slipped and mashed the victim's thumb between the plate and frame causing a fracture.

Cleaning parts of a new cone liner, the liner rolled onto his foot causing a broken bone on the top of his foot behind the steel toe.

Picking up a bottle off the floor. When he raised up he hit his ear on drawer of his truck tool box causing a laceration requiring sutures to his right ear.

USING A LOADER TO CLEAN A BARGE FLOOR. THE LOADER BUCKET HIT A PLACE IN THE BARGE FLOOR THAT CAUSED THE LOADER TO STOP SUDDENLY RESULTING IN A STRAIN TO HIS BACK.

A ROCK HIT AND BROKE THE HANDRAIL ON AN R-35 HAUL TRUCK. THE DRIVER WENT TO THE SHOP AND HAD THE HANDRAIL WELDED AND REPAIRED. THE DRIVER WAS GETTING BACK INTO THE TRUCK AND TOUCHED THE HOT HANDRAIL CAUSING A BURN TO HIS LEFT HAND.

Cleaning a barge floor using a loader to push coal to one end. The loader bucket hit a bad spot in the floor causing the loader to stop suddenly causing his left hand to hit the steering wheel resulting in a sprained hand.

Installing bolts in a skiry board on a conveyor wind blew dust into eye behind safety glasses causing a scratch to his eye.

Employee was bringing steel up out of the hole. He saw the dust and started running. A rock fell from ledge and hit employee's ankle.

Victim was cutting a piece of metal. A hot piece fell and burned through his coat sleeve and down into his glove resulting in a burn to his right forearm.
