Struck against stationary object

Using a pry bar to free up a stuck stone. Employee's hand slipped contacting a steel guard. Contact resulted in a cut requiring 2 sutures.

Caught in, under or between a moving and a stationary object

Employee noticed the rip rap chute was plugged. Plant was stopped. While clearing the material with a pry bar a piece of RR became dislodged striking the pry bar and forcing the bar downward, pinching the tip of employee's left pinky finger which required sutures from the laceration.

EE finished EE's pre shift inspec. & was notified that the plant would not be ready to run for a short time. EE was cleaning up a roll up garage door and while handling the roll up door, a galvanized steel bracket struck EE's right forearm causing a cut. EE reported to a clinic which EE was administered sutures.

EE WAS CLEANING THE APRON OF THE DUMP TRUCK WITH THE BED RAISED WHEN SOME WET LIME MATERIAL FROM THE BULKHEAD FELL, STRIKING HIS LEFT SIDE.

HE WAS PERFORMING ELECTRICAL SERVICE ON A CONVEYER MOTOR. THE MOTOR AND REDUCER TORX ARM BROKE RESULTING IN THE MOTOR STRIKING ELECTRICIAN IN HIS SIDE. HE DID NOT FELL MEDICAL ATTENTION WAS REQUIRED AT THIS TIME. HE DID NOT REPORT TO WORKON 10-13-03 SEEKING MEDICAL ATTENTION.
