Employee was walking to EE's haul truck when EE stepped on a frozen puddle of ice. EE slipped and fell straight backwards and injured EE's shoulder.

Employee was using a pipe wrench to install a hydraulic line on a rock breaker. The wrench slipped striking the employee in a front tooth.

Caught in, under or between a moving and a stationary object

Employee was attempting to clear clay grates with a steel bar. When a section of material broke loose, the employee's finger was pinched between the steel bar and steel clay grate. This required 6 sutures to close the wound.

Employee was driving a bearing out of a loader frame with shop bar. When bearing came out it ee hit wrist on the bore causing laceration

Employee grabbed a hammer drill box from the back of a service truck at the end of his shift. As he lifted the box, he turned and the box struck his left knee. The employee continued working for two weeks but noticed some discomfort in his knee when he would bend it a certain way. He sought medical treatment for the first time on 9/22/15.
