The employee was using a 6-inch electric handheld grinder with a cutoff wheel. The employee was cutting a steel bar grading walkway when the grinder kicked back and struck EE's right thumb. The grinder caused a laceration just above EE's right-hand thumb knuckle that required 5 sutures to close.
A qualified surface electrician was working inside of an electrical cabinet to close a hole temporarily until the cabinet could be replaced. EE took out a utility knife to cut a piece of plastic. While EE was making the cut, the blade slipped, slicing through the tip of EE's left thumb. The employee was taken to the local ER, where sutures were added to close the laceration.
IE was using a chain fall to assist in raising the lid on the #2 dryer that was in the down position. The IE was pulling downward on the chain when the dryer lid become dislodged and moved suddenly. IE came down suddenly onto IE's right leg. The initial diagnosis was a right quadricep muscle strain. Eventual MRI showed a torn quadricep muscle which required surgery on 4/21/17.
IE was descending the stairs from the cab of a 988G end loader. IE had his left hand on the hand rail he misjudged the last step, lost his balance, and began to fall backwards. He was able to briefly make contact with the handrail at the rear of the machine, which changed the direction of his fall, causing him to fall to the ground on his right shoulder.
IE was helping two other employees change out a section of 6"" poly pipe.The pipe was in a bind because the 90 degree elbow piece above it was on backwards. The three employees were taking the pipe back apart to correct the problem. As they were taking the last bolt out, the section of pipe sprung back striking IE just above his upper lip. The IE received sutures to close the wound.
Contractor employee had climbed up onto a travel motor on a Hitachi 5500 shovel to bleed a hydraulic line to lower the fast fueling arm. After lowering the arm and tightening the line the employee jumped from the motor causing the injury. The height from motor to the ground is 5' 7"".
Employee was examining the steering relief valve on a Cat. 793. Oil dipped down onto the brim of his glasses and entered his eye. He cleaned it out and continued to work. On 9/13/08 he reported that the eye was swelling. Taken to ER, diagnosed with infection. Prescribed antibiotic drops and returned to work.
MRI of his lumbar spine. blging small disc l5,s1,l4-5. some degenerative changes in his lumbar spine.....EE was assisting 2 mechanics install a hydraulic pump. He was in a sitting position beside the pump as it was lowered into position. Other was moving the pump back and forth to lines the shift spines so it would slide into position. Stiffness and pain in lower back
EE originally was injured on 9/6/07. Submission #509993. He had surgery and was released to return to work on 2/5/08. On 2/20/08 he was shoveling drill clippings in a drill hole when he felt pain in the same shoulder. An M.R.I. was performed on 3/4/08, diagnosis was tendonitis, bursitis, and arthritis to the 4 muscles of the rotator cuff group from the previous injury.
Employee was dismounting from D-11 dozer. He positioned his left foot on the pusharm step. As he moved his right foot down to the next step, his left foot slipped off allowing him to drop to the ground unexpectedly. Landing on his feet his left leg hyperextended backwards. He was examined and diagnosed with a knee strain and started missing work on 12/21/07.
EE had been removing tools and items from the warehouse for clean up. Later while welding from a sitting position he felt stiffness in the lower back. On his next scheduled shift he sought medical attention, and was taken off work. After further examination a degenerative condition of the lower spinal area was discovered. Dr. felt condition contributed to the muscle strain.
EE was sweeping out the drill cab. When finished, he reached out with his right hand to grab the door handle, while bracing himself with his left hand against the door frame. As he pulled the door shut, his thumb was pinched between the door and the frame, fracturing his thumb.
After fueling EE entered the cab. He reached for the seatbelt between the seat and buddy seat, which is over the heater/air conditioning unit. The guard for the fan was missing, allowing his finger to come in contact with the fan blades. 6 stitches were required to close the incisions.
Injured was loading powder trucks under the storage bins. He used a 10 lb. dead blow sledge hammer to beat on the chute, to loosen the stuck material. He swung the hammer app. 75 to 100 times, when he felt a pop and burning sensation in the shoulder. He was examined and an M.R.I. revealed an internal roator cuff tear. He was ordered off with no use of left arm.
Injured was hauling from shovel in pit #6. The material was gray mud. When dumping the material would come out all at once, and jar the truck and driver. Injured was bracing his leg against the floor when dumping and the jarring strained his knee. Was seen by a Dr. on 1/24/07, an MRI was done on 1/25/07 and he was released to return to work on 1/26/07.
Employee is a mechanic; on 7/12/06 he was having striking pains that ran through his hands and arms. On various occasions he suffered from hands falling asleep and pain. He was examined by a hand specialist on 8/4/06, and was diagnosed with carpal tunnel syndrome in both wrists. He had surgery to the right wrist on 9/8/06 and surgery on the left wrist 12/18/06.
Mechanic was jump starting a D-10 dozer. After hooking up jumper cables from his service truck to the dozer battery, he mounted the dozer to start. As he stepped up onto the cab platform, one of the batteries blew the top out of it, spraying him with acid. Some entered his right eye. Was flushed out and taken to the E.R. Was released to follow up with an ophthalmologist.
EE was dismounting from the back counterweight step. He had positioned a 6' folding ladder 26"" from the step to assist. As he stepped with his left foot onto the counterweight step, then stepped with his right foot facing the ladder, his left foot slipped through the step opening, which measured 5"" X 11"". He then fell spun around ending upside down, hunging by his leg.
EE was shoveling spilt coal in the R.O.M. & feeder tunnels for approximately 1.5 hrs. He felt weak, dizzy & nauseated. Sat & cooled off & felt better. Finished his shift & reported the next day. At the end of shift on the way home felt the symptoms again. Called family doctor & was sent to the hospital. Admitted for heat exhaustion.
EE was loading a troughing roller into a manlift. He placed it onto the top rail to slide it into the the basket. He was gripping the bolt mounting plate when it unexpectedly fell downward, catching his thumb between the bolt mount and the rail. He fractured the distal bone and numerous stitches were requried to close the wound.
EE crawled out from under drill, felt slight pain just below the knee, noticed a small abrasion. Finished his shift, was scheduled off on 11/17/04. Returned on 11/18/04, worked a full shift but the leg was swollen. Saw company physician on 11/19/04, diagnosis: cellulitis infection. Issued antibiotic and ordered to stay off of it. He returned on 11/23/04.
Pushing down from a 30' mud highwall. Edge broke off allowing the dozer to fall landing on it's blade, then done 1 complete somersault back onto it's tracks. The back window popped out striking ee on the side of the head. He was taken to the E.R., examined, and recieved 3 stitches to the head. He was released and returned for his next scheduled shift. He was wearing his seat belt.
He cut out the flange on the tooth, took a 10lb. sledge hammer and struck the top back edge of the tooth to knock it off of the shank. As he did a piece mesuring 1.5 cm X .8 cm entered his upper leg. He was taken to Gibson General E.R. were it was surgically removed. It took 6 stitches to close the incision. He was released to return to work on his next scheduled shift.
While dismounting, his foot slid into a rut. E.R. diagnosed a slight ankle sprain, and X-ray's indicated a frature of the proximal metaphysis to the 5th. metatarsal. An appointment with an orthopaedic surgeon was on 2/16/04. He was released to return to work with restrictions of limited standing for 2 weeks. He was temporary assigned to an end dump position.
EE He assumed the bolts were there, explaining why he didn't expect the pan to fall. He was transported to St. Mary's hospital, where diagnosis to have dislocated his right elbow, and fractured the 2,3,4 and 5 Metacarpus bones in the left hand. The elbow was reset and surgery was performed on the hand on Jan. 30th.
EE HAD JUST COMPLETED AN OIL CHANGE ON THE UNILOADER. AFTER STARTING THE ENGINE HE WAS CHECKING IT OVER WHEN HE NOTICED THE DIPSTICK WASN'T PUSHED ALL THE WAY IN. HE REACHED IN FROM THE BACKSI DE ABOVE THE FAN SHROUD. HIS COAT SLEEVE WAS GRABBED BY THE FAN, PULLING HIS HAND INTO IT. HIS THUMB WAS SEVERED 1/4 OFF & INDEX FINGER LACERATED. (NEUROSURGEON REATTACHED THUMB).
EE HAD JUST REPLACED A FENDER LIGHT, AS HE STEPPED FROM THE FENDER ONTO THE CAB DECK, HE RAISED UP STRIKING THE BOTTOM OF THE MIRROR BRACKET WITH THE TOP OF HIS HARD HAT. THIS CAUSED HIS TEETH TO STRIKE TOGETHER, WHICH BROKE APPROXIMATELY 1/8TH INCH OFF OF HIS TWO FRONT INCISOR TEETH. NO MEDICAL TREATMENT WAS NECESSARY, BUT HAS SCHEDULED AN APPOINTMENT WITH FAMILY DENTIST.
EE WAS REPAIRING A CRACK ON THE FRAME RAIL. AS HE WAS GRINDING FROM ABOVE HIS HEAD, HE FELT DEBRIS STRIKE HIS EYE. AT THAT TIME HE FLUSHED IT OUT & FELT BETTER. THE NEXT MORNING IT WAS RED & IRRITATED. HE WAS TAKEN TO AN EYE CENTER, WHERE AN EMBEDDED PIECE OF METAL WAS REMOVED. HE WAS ISSUED A PRESCRIPTION & RELEASED TO RETURN TO WORK. HE WAS WEARING SAFETY GLASSES.
EE WAS HELPING POSITION FLOOR PANELS AS THE WELDER WAS WELDING THEM DOWN. HE FELT BURNING SENSATIONS IN HIS EYES WHILE TRYING TO SLEEP THE FOLLOWING DAY. HE WENT TO E.R. DIAGNOSED WITH FLASH B URN. HE WAS ISSUED A PRESCRIPTION.
EE HAD ENTERED THE CAB OF THE TRUCK TO ANSWER THE RADIO, WHEN HE DISMOUNTED HE STEPPED ON A ROCK CAUSING THE ANKLE TO ROLL. HE WAS TAKEN TO GIBSON COUNTY E.R. TO BE EXAMINED. X-RAYS WERE NEGAT IVE, AN ANKLE SPLINT WAS ISSUED AND HE WAS RELEASED TO RETURN TO WORK WITHOUT RESTRICTIONS.
EE WAS ATTEMPTING TO LIFT HYDRAULIC TANK UP FROM DOZER. HE WAS USING A 3 LEGGED CHOKER. UNAWARE THAT THE DANGLING HOOK HAD GOTTEN CAUGHT ON THE EDGE OF THE DOZER'S FENDER, HE APPLIED LIFTING P RESSURE. AS HE DID THE DANGLING HOOK POPPED FREESTRIKING HIM IN THE SHIN.
EE WAS IN THE PROCESS OF MOUNTING A RADIO IN HIS END DUMP TRUCK, THE BRACKET FOR THE RADIO DIDN'T MATCH UP WITH EXISTING MOUNTING BOLTS. EE HELD THE BRACKET IN HIS LEFT HAND AS HE ATTEMPTED TO POKE A HOLE THROUGH THE BRACKET WITH A POCKETYKNIFE. AS HE APPLIED PRESSURE THE BRACKET BROKE, CAUSING HIM TO RUN THE KNIFE BLADE INTO THE PALM OF HIS HAND. 3 STITCHES APPLIED.
EE HAD MOUNTED THE GRADER WHICH WAS PARKED IN THE WEST SHOP BAY. HE THEN STEPPED ONTO THE FRONT TIRE OF THE REAR TANDEM TO MOUNT A CB ANTENNA ONTO THE CAB. AS HE DID HE LOST HIS BALANCE AND JU MPED FROM THE 5' TALL TIRE TO THE SHOP FLOOR LANDING ON HIS FEET. PAIN WAS FELT IN THE LEFT FOOT. HE WAS TAKEN TO GIBSON GENERAL E.R. EXAMINED, X-RAYED, DIAGNOSED WITH FRACTURED CALCANEOUS BO
EE WAS WALKING TO THE WASHPLANT, STOPPED AT THE EE PARKING LOT TO GET A PAIR OF GLOVES OUT OF HIS TRUCK. AFTER CLOSING THE DOOR HE TURNED AROUNDAND WALKED INTO THE HANDLE OF A GARDEN TILLER HE WAS HAULING IN THE BED OF THE TRUCK, WHICH OVERHUNG THE TRUCKS BED. HE WAS TAKEN TO GIBSON GENERAL E.R. WHERE 5 STITCHES WERE PLACED TO CLOSE THE WOUND.
EE WAS DRILLING THROUGH A 3/8"" PIECE OF METAL WITH A HAND DRILL USING A 1/2"" BIT. AS HE WAS DRILLING, THE BIT HUNG CAUSING THE DRILL TO SUDDENLY SPIN OUT OF HIS HAND. AS IT SPUN IT CAUGHT AND BENT HIS 4TH FINGER. EE WAS TAKEN TO GIBSON GENERAL E.R. EXAMINED, X-RAYED AND DIAGNOSED WITH A FRACTURE OF THE 4TH METACARPAL BONE. A SPLINT WAS APPLIED ALONG WITH A PRESCRIPTION FOR LORTAB.
EE WAS IN THE PROCESS OF REMOVING THE FRONT RIGHTSIDE STRUT. A CONTRACTOR MECHANIC WAS REPAIRINGTHE FRONT LEFTSIDE STRUT MOUNT, WHICH REQUIRED WELDING AND GRINDING. THE NEXT MORNING EE FELT DI SCOMFORT IN HIS EYE, IT WAS SWOLLEN AND RED. HE WAS TAKEN TO WABASH VALLEY EYE CLINIC, WHERE A PIECE OF EMBEDDED METAL WAS REMOVED. HE WAS ISSUED A PRESCRIPTION OF OCUFLOX (ANTIBIOTIC SOLUTIO
EE WAS ATTEMPTING TO REMOVE A HEADLIGHT THAT WAS INOPERATIVE AFTER HE UNBOLTED IT, HE PULLED ON IT TO BREAK IT LOOSE FROM THE BRACKET. AS HE DID HE RUBBED HIS HAND AGAINST THE BRACKER CAUSING THE LACERATION. HE WAS TAKEN TO GIBSON GENERAL E.R. WHERE THE LACERATION REQUIRED 7 SUTURES TO CLOSE. HE WAS THEN RELEASED TO RETURN TO WORK.
EE WAS SPOTTING FOR CHRIS AS HE POSITIONED HIS WSWERVICE TRUCK ONTO THE SERVICE PIT. AFTER OTHER HAD PARKED THE TRUCK, HE WALKED AROUND TO THE REAR OF THE TRUCK AND STEPPED INTO THE 38 INCH WI DE, 5 FOOT DEEP PIT. HE WAS TAKEN TO GIBSON GENERAL HOSPITAL E.R. AND DIAGNOSED WITH A SMALL ABRAISION ON THE LEFT FOREARM AND A SPRAIN TO THE RIGHT KNEE.
EE WAS PUSHING OVERBURDEN. WHEN HE CURLED THE BLADED CONTROL LEVER, PAIN RADIATED UP HIS ARM TO THE ELBOW. HE WAS TAKEN TO A FAMILY CLINIC AND WAS DIAGNOSED TO HAVE A FOREARM MUSCLE STRAIN. H E WAS RELEASED TO RETURN TO WORK, BUT WAS ISSUED A WRIST IMMOBILIZER TO LIMIT THE TWISTING OF THE WRIST.
A SEMI COAL TRUCK HAD GOTTEN STUCK ON THE COAL PILE. EE HOOKED A 12' X 3/8"" CHAIN TO A D-8 DOZER TO PULL THE TRUCK OUT. AFTER THE DOZER OPERATOR TIGHTENED THE CHAIN, EE BACK AWAY APPROXIMATELY 35'. AS PRESSURE WAS THEN APPLIED, THE HOOK ON THE CHAIN BROKE, SENDING THE 1 1/2"" FRAGMENT FLYING AND STRIKING EE ON THE BRIDGE OF THE NOSE. HE HAD 2 LACERATIONS AND A FRACTURED NOSE.
EMPLOYEE WAS IN THE PROCESS OF HAVING HIS TRUCK LOADED WITH REFUSE. WHILE BEING LOADED HE STEPPED UP APPROX 18 INCHES ON THE FRONT OF THE CAB TO VIEW THE LOADING. WHEN HE STEPPED DOWN, HE TWIS TED AND FELT A SHARP PAIN IN HIS LOWER BACK. THE FOLLOWING DAY HE WAS TAKEN TO FAMILY CLINIC AND DIAGNOSED WITH STRAIN OF THE LUMBAR REGION. PHYSICAL THERAPY.
EE WAS DRILLING OVERBURDEN WHEN THE DOOR POPPED OPEN. HE REACHED AROUND WITH HIS LEFT HAND TO PULL THE DOOR SHUT. AS HE DID HIS HAND SLIPPED FROM THE HANDLE ALLOWING HIS FINGER TO GET BETWEEN THE LATCH AND THE DOOR FRAME. HE WAS TAKEN TO GIBSON GENERAL EMERGENCY ROOM, X-RAYS INDICATED A FRACTURE OF THE DISTAL BONE OF THE LEFT RING FINGER, WITH A SMALL LACERATION.
EE WAS WELDING ON SPRAY BARS IN A RAW COAL SHAKER SCREEN. AFTER CLIMBING OUT HE STEPPED ON A PIECE OF 2"" ANGLE IRON LAYING ON THE FLOOR. HE FELT PAIN IN THE ARCH OF HIS LEFT FOOT. HE CONTINUED WORKING YHIS SCHEDULED SHIFTS, BUT THE DISCOMFORT CONTINUED AND ON 2-24 HE WAS TAKEN TO HOSPITAL ER FOR AN EXAMINATION. THE WAS WAS SRAYED AND IT WAS DETERMINED TO BE NEGATIVE. THE EE CONTINU
EE WAS CLOSING THE SLURRY VALVE ON THE HEAVY MEDIA CYCLONE. HE WAS USING A 30"" PRYBAR, STICKING IT THROUGH THE WHEEL HANDLE FOR LEVERAGE. WHEN HE APPLIED PRESSURE DOWNWARD TO SEAL THE VALVE, T HE PIPE SLIPPED OUT ALLOWING HIM TO FALL FORWARDTO THE GROUND LANDING ON HIS HANDS. AT THAT TIMEHE FELT A SHARP PAIN IN HIS LOWER BACK. HE WAS TRANSPORTED TO THE CLINIC, DIAGNOSED WITH STRAIN
EE WAS ASSISTING WITH THE LOADING OF BLAST HOLES. WHILE HE WAS SHOVELING IN ONE OF THE HOLES, TWO OF HIS FELLOW WORKERS DECIDED TO ENGAGE IN HORSEPLAY. ONE SHOVED HE BACKWARDS AS THE OTHER TRI PPED HIM UP CAUSING HIM TO FALL TO THE GROUND. THE ADJUSTABLE RATCHET KNOB ON HIS HARDHART SUSPENSION STRUCK THE BACK OF HIS HEAD AS IT MADE CONTACT WITH THE GROUND. THIS IMPACT CAUSE TO TO L
AS EE WAS DISMOUNTING FROM THE DOZER IN THE RAIN. HIS FOOT SLIPPED ON THE TRACKS CAUSING HIM TO FALL. HIS LEFT LEG BECAME LODGED BETWEEN THE TRACK-RAIL AND THE BLADE PUSHARM. HIS MOMENTUM CARR IED HIM OVER THE ISDE OF THE PUSHARM. HIS LEG SUPPORTED HIS WEIGHT, BREAKING THE FALL BUT CAUSED THE KNEE TO BE TWISTED, CAUSING THE INJURY. HE WAS TAKEN TO EE. S FAMILY.
WHILE REACHING TO MOUNT GRADER, WIND BLEW THE DOOR SHUT ONTO EE'S HAND CATCHING HIS THUMB BETWEEN THE DOOR AND THE DOOR JAM RESULTING IN THE INJURY. EE WAS EXAMINED AT HOSPITAL, XRAYS SHOWED A FRACTURE OF THE DISTAL BONE. HE WAS GIVEN A BRACE AND RETURNED TO WORK LATER THAT DAY.
EE WAS BACKING A BULK POWDER TRUCK BETWEEN 1ST &2ND ROW OF DRILL HOLES TO BE LOADED FROM THE OUTSIDE EDGE OF THE DRILL BENCH PAD. HE WAS BETWEENTHE ROWS. PROCEEDING BACKWARDS HE WAS UNAWARE OF THE 1ST ROW ANGLED INWARDS TOWARD THE 2ND. THE RIGHT TANDEN WHEEL DROPPED OFF THE EDGE. AS THE TRUCK OVERTURNED HE LEAPT FROM IT LANDING ON THESPOIL BANK & SLID UNDERNEATH ROLLING TRUCK.
EE WAS REMOVING A BROKEN OFF WHEEL STUD. HE WAS WELDING THE STUD, BUILDING IT UP IN ORDER TO GET A NUT ON IT, SO HE COULD IMPACT IT OUT FOR REPLACEMENT. WHILE AIR CHIPPING THE SLAG FROM THE WE LD, A PIECE RICOCHETED OFF HIS BROW, OFF HIS SAFETY GLASSES LENS AND LODGED INTO EYE. EE WAS TAKEN FOR TREATMENT, OBJECT WAS REMOVED. HE WAS ISSUED AS PRESCIPTION OCUFLOX, FOR INFECTION.
EE WAS LAYING ON HIS SIDE WHILE HOLDING A 1"" IMPACT WRENCH, TIGHTENING THE IDLER BOLTS ON A D10 DOZER. AFTERWARDS, WHEN FINISHED, HE WAS GETTING UP WHEN HE FELT PAIN IN THE LOWER BACK. HE WAS EXAMINED AND DIAGNOSED TO HAVE ACUTE SPASM OF THE LOWER LUMBAR SPINE.
EE WAS LOADING OVERBURDEN WHEN HE SMELT HYDRAULIC FLUID. HE STOPPED THE LOADER TO INVESTIGATE. HE LIFTED THE DECK LID ABOVE THE ENGINE DEPARTMENT. AS HE DID IT SLIPPED FROM HIS GRASP, IT FELL, CATCHING HIS FINGER BETWEEN THE LID AND THE LATCH. XRAYS WERE NEGATIVE, BUT REQUIRED 3 STITCHES TO CLOSE THE WOUND.
THERE WAS NO SPECIFIC EVENT REPORTED THAT CAUSED THE INJURY. EE'S REGULAR JOB DUTY IS AS END DUMP DRIVER, BUT HE WAS TEMPORARY PLACED AS A FUELER TO HELP OUT THE MAINTENANCE DEPT. THE PERSISTE NT MOUNTING & DISMOUNTING EQUIPMENT IRRITATED HIS LEFT KNEE WHICH CAUSED PAIN AND SWELLING. HE WAS DIAGNOSED TO HAVE A SPRAINED KNEE.
EE ENTERED THE PLANT SHOP AREA TO DISCUSS WORK DUTIES WITH A FELLOW EE. NO WORK WAS GOING ON IN THE SHOP SO HAD REMOVED HIS SAFETY GLASSES. STANDING NEXT TO A PORTABLE STAND, HE PROCEEDED TO P ULL A SCREWDRIVER FROM HIS POCKET AND LIGHTLY PRY ON THE METAL SLAD WHICH HAD FORMED ON THE STAND. AT THE TIME A SMALL PIECE SNAPPED OFF AND STRUCK HIM IN THE LEFT EYE. DEBRIS LODGED IN CORNE
EE WAS HELPING THE POWDER CREW, BY SHOVELING IN THE LOADED HOLES. WIND WAS BLOWING GUST UP TO 40 MPH STIRRING UP DEBRIS, WHICH BLEW INTO EE'S FACE. AT THE TIME HE WAS WEARING SAFETY GLASSES BUT THEY DIDN'T PROVIDE ADEQUATE PROTECTION FOR THE CONDITIONS THAT WAS PRESENT. HE WAS TAKENT O THE ER THE EYE WAS FLUSHED AND PERSCRIPTION OF RX
EE WAS PERFORMING HIS DAILY DUTIES, WHICH ARE REQUIRED AFTER THE LOADING OF THE TRAIN. GENERAL CLEAN UP INCLUDES SHOVELING SPLIT COAL, GREASING MACHINERY, & HOSING THE LOADOUT & DRAWOFF TUNNEL DOWN. WHILE DOING SO EE EXPERIENCED SLIGHT PAIN & DISCOMFORT IN THE CHEST, SHORTNESS OF BREATH & LIGHTHEADNESS. AFTER RESTING A FEW MIN WAS TAKEN TO HOSPITAL-DIAG SUFFERED FROM A HEART ATTACK
