Fall to the walkway or working surface

The Employee slipped and fell on the ice. The Employee has been treating since that time thinking it would heal with exercise and therapy. In the end they had to go in and repair a small tear causing him to become a lost time on 08/29/2008. Employee is expected to be back on restricted duty on 09/08/2008.

Treating pond. Opened a container of caustic soda when wind picked up and blew pellet onto eyebrow. Melted by sweat and ran into right eye.
