Over-exertion in lifting objects

Employee was helping move a stand that a cone crusher was sitting on for repair. When employee was moving the stand they felt a pop in their back. Employee was taken to clinic and prescribed a muscle relaxer for the strain.

Employee was having back pain. Was taken to OMP and given restricted duty. Incident under review.

Struck by falling object

EE was cutting liners out of chutes. As EE was lifting one out it slipped out of EE's hands landing on the top of EE's foot. Went to OMP and received restrictions of no climbing.

EE was removing drive line on MP0227 so EE could repair parking brake. EE was knocking out U joint when a piece broke off and stuck in the left wrist. Sent to clinic and had piece removed and was given tetanus. Removal of foreign body. 30CFR50.20-3(a) 1(ii) EE was wearing PPE.

EE was unplugging the air hose from the hose reel, when EE's right hand was injected with metal, dirt, and air. EE was taken to the Hospital for care

Employee was cutting a piece of steel when the torch hose caught fire causing a burn to EE's abdomen. EE was prescribed Rx and restricted duty.

Employee was changing a bin pulley on MJ-0201. When the pulley got stuck between two uprights. EE pulled on it causing it to swing back and strike EE's left hand causing restricted duty.

Employee was walking around a water pump on the ground in Pit #2 when he slipped and fell injuring his right ring finger.

Employee climbed up in his truck after doing some work at the primary while the plant was down and when he sat down his back/right shoulder was hurting. Employee did not report this accident until 1/25/2012.

EE was climbing up front end loader when he stepped on lowest step to pull himself onto the equipment he felt a strain/pull in his lower back.

Employee was assisting with changing screens, and while removing screen from hoist gravity brought the screen down striking the back of her left hand resulting in a laceration needing sutures to close the wound. Gloves are provided but were not being used.

Employee was using a chain saw clearing brush in an area. He tripped on some brush and fell to the ground. The chainsaw struck him on the leg causing a laceration requiring sutures.

VICTIM WAS USING A CUTTING TORCH TO CUT SOME PAINTED STEEL. THE FUMES FROM THE CUTTING MADE HIM SICK CAUSING LUNG CONGESTION.
