Truck Service vehicle lost brake control due to unknown circumstance. Unable to brake causing impact to a rib.

Employee was painting rib markings and as EE was turning around, tripped on a rock. When EE fell, the right hand and wrist landed on a sharp rock. This contact caused a laceration on EE's right hand and wrist.

Employee was using a prybar to assist in moving a gate handle. The prybar slipped causing a contusion and fracture to the left thumb.

At 7:53pm, EE reported a fire behind the mine portal. The fire department responded and used foam to extinguish the fire. MSHA was called at 8:21pm and the fire was extinguished at 8:30pm. No damage occurred to any equipment except two high voltage fuses that blew due to bird activity (a dead bird was found at the origin of the fire).

Employee was operating haul truck when he strained his lower back

Employee was climbing the steps to his truck when he slipped and caught himself causing a strain to his left arm and shoulder. Incident became reportable 7/16/07 when the employee was sent for ""Other Professional Treatment"" (Physical Therapist)

Employee was using a high pressure steam cleaner when he contacted his boot causing a hole which allowed hot water to burn his foot.

Employee was handling a 4'x4'x 1 1/2"" opening screen cloth when the metal cloth cut his finger.

Employee fell and hit his forehead and twisted his ankle.

Employee was using an Oxygen Actylene cutting torch when the Oxygen hose developed a hole in it causing a flashback and subsequent 2nd degree burn to the back of his left hand

As employee was tightning the rear diff. housing bolts, the wrench he was using slipped off of the bolts. This caused stress to his middle back.

Employee was making a plant change by opening a gate when he smashed his finger between the handle and a keeper pin. The employee received 4 sutures.

Employee was removing ice from a conveyor when he smashed his pinky finger between a chunk of ice and a machine guard.

Employee was going to turn on a conveyor when he stepped on a step incorrectly and strained his calf muscle

Employee was washing out under the crusher when a rock fell from the structure and struck him on the shoulder. Employee returned to the Dr. for follow up where it was decided on the 18th of October that he would see a physical therapist for treatment of the contusion.

Employee was removing a fuel cap when he strained his thumb.

Employee was walking back to scaler when he was hit in the head by a small rock that had fallen.

Employee was using a hand tool when it slipped and hit his thumb.

Employee was helping another employee carry a troughing roller when he miss stepped and twisted his lower back.

EE was reaching to turn on the master switch when he wrenched his back.

As he was helping the mechanic remove the hub off of a wheel on a loader, it fell mashing his finger against the inside of the wheel.

ACCIDENT OCCURRED WHEN EMPLOYEE DROPPED A STEEL PLATE ON HIS FOOT BEHIND THE STEEL TOE. HE REPORTED ACCIDENT AT TIME OF OCCURRENCE BUT DID NOT ASK FOR ASSISTANCE UNTIL 12-29-03. ACCIDENT RESUL TED IN RESTRICTED DUTY DAYS.

EE WAS ASSISTING WITH A BELT SPLICE WHEN HE OVEREXERTED HIMSELF AND INJURED HIS KNEE.

EE TURNED TO WALK AND TRIPPED OVER A HOSE REEL STRAINING HIS BACK.

EMPLOYEE FELL OFF OF HIS HAUL TRUCK AND STRAINED HIS BACK.

CLIMBING INSIDE A SCREEN WHEN HE EXPERIENCED PAIN IN HIS RT. SIDE BACK. EE IS UNSURE OF EVENT THAT CAUSED INJURY BUT RESULT IS A FX RIB.

EMPLOYEE WAS PULLING A WATER HOSE AND HE STRAINED HIS SHOULDER.

EE WAS USING A HAMMER AND STRAINED HIS ELBOW.

EE STUCK HIS FINGER IN THE AIR VENT OF HIS LOADER TO SEE IF IT WAS OBSTRUCTED AND PUT HIS FINGERIN THE FAN.

EE COMPLAINED OF HIS LEFT EYE BOTHERING HIM, WE THEN SENT HIM TO THE CLINIC AND CLEANED OUT A PIECE OF RUST.

EMPLOYEE WAS WALKING TO THE FACE WHEN HE STEPPED ON A ROCK AND TWISTED HIS KNEE. DID NOT WANT TO GO TO DOCTOR UNTIL 10/29/02.

THE ELECTRIC IMPACT WRENCH THAT WAS IN USE HAS AN OPENING OF ABOUT 1/2"" WIDE AROUND THE BODY OF WRENCH. THE PLASTIC GRID THAT IS MOLDED ACROSS THIS OPENING HAD A SMALL PIECE BROKEN OUT OF IT A LLOWING HIS FINGER TO COME IN CONTACT WITH MOVING FAN. WRENCH HAS NOW BEEN TAKEN OUT OF SERVICE.

EE WAS OPERATING HAUL TRUCK WHEN HE JARRED HIS BACK.

WHILE REPAIRING COLLECTION BOX SKIRTING EE STEPPED ON A ROCK WHICH ROLLED ONTO FROM UNDER HIS FOOT TWISTING HIS RIGHT ANKLE.

EE WAS STRUCK IN THE EYE WITH A METAL FRAGMENT WHILE RUNNING A POWER GRINDER.

EE WAS OPERATING THE UNDERGROUND CRUSHER WHEN HETWISTED HIS ANKLE RESULTING IN A FRACTURE.

EE WAS CLEANING BIG ROCKS OUT OF A PLUGGED CHUTE. A ROCK JAM BROKE LOOSE KNOCKING OFF HIS HARD HAT AND THEN ANOTHER ROCK HIT THE EE IN THE HEAD REQUIRING SUTURES.

EE WAS LOADING POWDER ONTO THE TRUCK WHEN HE STRAINED HIS BACK. ELECTED NOT TO GO TO THE CLINIC UNTIL 10-1-01.

EE WAS USING HIGH PRESSURE STEAM CLEANER WHEN HE BECAME DISTRACTED AND RAN THE STEAM ACROSS HIS FOOT. HIGH PRESSURE CUT THE BOOT AND HOT WATER BURNED HIS FOOT.

EMPLOYEE CUT THUMB WITH UTILITY KNIFE WHILE CUTTING SKIRTBOARD RUBBER.

EMPLOYEE WAS ATTEMPTING TO DISLODGE FOREIGN MATERIAL FROM THE MUCK PILE WHEN A ROCK FELL ON HIS HAND SMASHING THE RIGHT MIDDLE AND INDEX FINGERS. EMPLOYEE DIDN'T REPORT RECEIVING SUTURES UNTIL 9-21-01 WHEN COMPLICATIONS FORCED HIM TO REPORT TO HIS SUPERVISOR.

EE WAS ATTEMPTING TO DISLODGE A LARGE ROCK FROM A DISCHARGE CHUTE. WHEN THE ROCK WAS FREE IT FELL ON HIS R THUMB.

EMPLOYEE WAS HOLDING AN OPEN UTILITY KNIFE WHEN ANOTHER EMPLOYEE ATTEMPTED TO GRAB IT OUT OF HIS HAND. EMPLOYEE WITH KNIFE JERKED AWAY AND CUT THE HAND OF THE EMPLOYEE WHO WAS ATTEMPTING TO G RAB THE KNIFE. EMPLOYEE WAS UNAWARE THE KNIFE BLADE WAS OUT!

STEPPING OFF OF HIS LOADER, WHEN HE SLIPPED AND FELL ON THE ICE.
