Struck against a moving object

Truck Service vehicle lost brake control due to unknown circumstance. Unable to brake causing impact to a rib.

Fall onto or against objects

Employee was painting rib markings and as EE was turning around, tripped on a rock. When EE fell, the right hand and wrist landed on a sharp rock. This contact caused a laceration on EE's right hand and wrist.

Employee was using a prybar to assist in moving a gate handle. The prybar slipped causing a contusion and fracture to the left thumb.

Accident type, without injuries

At 7:53pm, EE reported a fire behind the mine portal. The fire department responded and used foam to extinguish the fire. MSHA was called at 8:21pm and the fire was extinguished at 8:30pm. No damage occurred to any equipment except two high voltage fuses that blew due to bird activity (a dead bird was found at the origin of the fire).

Employee was climbing the steps to his truck when he slipped and caught himself causing a strain to his left arm and shoulder. Incident became reportable 7/16/07 when the employee was sent for ""Other Professional Treatment"" (Physical Therapist)

THE ELECTRIC IMPACT WRENCH THAT WAS IN USE HAS AN OPENING OF ABOUT 1/2"" WIDE AROUND THE BODY OF WRENCH. THE PLASTIC GRID THAT IS MOLDED ACROSS THIS OPENING HAD A SMALL PIECE BROKEN OUT OF IT A LLOWING HIS FINGER TO COME IN CONTACT WITH MOVING FAN. WRENCH HAS NOW BEEN TAKEN OUT OF SERVICE.
