A loader operator was exiting the piece of equipment, a Kawasaki Z7 Front End Loader. When descending the ladder pan, EE missed the bottom step of the loader losing grip and falling to the ground. EE had used their right hand to break the fall.

The Injured Worker was injured when EE fell or slid approximately 30 feet while working on a bench. Based on information available at this time, it appears the Injured Worker injured at least their neck and upper extremities.

Loader Operator was spreading rock in employee parking lot, to add more traction to the icy conditions that morning. He climbed down from his loader to move his personal vehicle. After moving his personal truck, he stepped out of the truck and slipped and fell on his left side.

EE WAS RUNNING PLANT WHEN IT STOPPED SO HE HAD TO GET A CONV. GOING HE SHUT THE WHOLE PLANT DOWN AND STARTED TO WORK ON A CONV. TAILPULLEY WHEN HE WAS WALKING AWAY FROM IT . HIS HEAD ON A PIEC E OF ANGLE.
