Fall to the walkway or working surface

While taking a lunch break the employee was sitting on their lunchbox. when they stood up they lost balance falling backwards onto the floor. The following day they complained of knee pain and went to a clinic to get looked at. Resulting in restricted duty.

While walking up an inclined catwalk, carrying a piece of steel, the employee felt a sharp pain in left calf. The employee went to Urgent care, then to Orthopedic doctor. Determined EE had a tear in the calf muscle.

Opening a sliding fence gate. Pinched right pinky in roller guide. Required an X-ray and stitches. Hairline fracture at base of pinky.

Absorption of radiations, caustics, toxic and noxious substances

EE1 arrived at Hanson Harding Street, checked out a 775 haul truck and drove it over to the restroom. EE2 noticed EE1 slumped over steering wheel. EE3 stopped to check on EE1 and EE1 was unresponsive and EE3 called 911. Ambulance crew gave EE1 medicine to revive EE1, then took EE1 to the Hospital.

Employee hit right elbow causing contusion on metal frame while using hammer to dislodge built up material from screen.

Employee was lifting liner plate into chute above crusher, when wind caught plate turning it into metal guarding causing dust and dirt to come down onto EE and into EE's eye.

Employee was trying to knock loose stuck rock from a catwalk with a hammer when loose rock came back and hit EE in mouth damaging EE's left front tooth.

Employee was airing up a tire on a 40 ton haul truck when the tire blew. EE was taken to local hospital and received treatment for two fractures on the neck and two on the back and one on EE's knuckle.

Employee stated they slipped and scraped right lower leg on track of Cat 330 excavator.

Crane was holding the lock post cylinder 8""-10"" above the platform as the emp was kneeling beside it EE was hitting the lock post cylinder rod trying to extend it out when the snap ring holding the top cap in place came out and allowed the cylinder body to come loose, hit the platform and fall onto the emp knee.

Emp was operating haul truck hauling shot rock to the primary crusher. While being loaded, the bucket of the loader contacted the truck bed and jerked the truck. The emp complained of back pain and told the super about incident but refused med attn at that time. On 9/21 the emp told the super was in a great deal of pain and needed to seek med attn for back pain.

Employee was changing oil in screen 2 at the stone plant. When he had completed the job he was attempting to lift the 5 gallon pail of used oil out of the back of the screen. He attempted to lift the pail while also turning to the side. During this motion, he felt a pinching sensation in his lower abdomen resulting in a groin strain.

Bent over cutting up old conveyor truss with a cutting torch and a piece of rust popped up and went behind employee's cutting shield and safety glasses getting into his right eye, treated with eye drops.

Employee was manually removing material from C3A Conveyor beltline. Employee performed job and after job was finished said that his back was tight. Employee said Monday at work he had no complaints and back felt better. Monday evening approx. 6:30pm his back started hurting badly. He reported the injury to his supervisor on Tuesday August 5, 2008 in the morning.

Employee was grinding a bore hole on a standard crusher mainframe. The employee put the grinder down, picked up the polisher, looked up to not hit his head on the mainframe and steel fell off his grinding shield. The steel fragments went behind safety glasses and into employees right eye.

Pulling electrical cable with pick-up truck for relocation, employee's right ankle became entangled, causing compression fracture to right ankle.

EE was pushing pile of dirt with the D8T dozer, he had his coat hanging in the back of dozer which created a blind spot, he went into reverse and backed up about 10 feet, when he back up he didn't see other ee coming in the A40D hual truck with a load of dirt to unload, he backed up into the haul truck, which caused the tracks of the dozer, to collide with front end truck.

Employee was digging out the primary crusher. A rock rolled down from the feeder and struck the employee in the left lower back.

Employee utilizing Oxy Thermal cutting torch to remove wear parts from impact crusher; while cutting, a piece of slag landed on the employee's left knee and burned through his pants & subsequently burned the outside of left knee. Slag traveled down pant leg and burned top of left foot.
