Flash burns (welding)

IE was welding in a haul truck bed installing new bed liners. EE sustained corneal flash burn because the digital welding shield's batteries died, which went unnoticed until the next morning when EE woke up in pain. EE's wife took EE to the emergency room, where EE was prescribed Cortisporin ophthalmic drops and released after receiving medical treatment beyond first aid.

Fall from machine

The EE was climbing down the ladder of the Haul Truck they were operating to check out the equipment for a possible fluid leak and missed a step as they were descending down the ladder and fell to the ground. The employee injured their knee when they hit the ground.

Caught in, under or between a moving and a stationary object

A miner was looking down inside the Jaw to guide the operator of the rock hammer down onto the rock that was stuck. The rock breaker slipped off of the rock causing it to strike the right hand of the miner in between the back of the jaw crusher and the hammer while guiding the hammer into place. A severe hand injury occurred resulting in the loss of 3 fingers.

Fall onto or against objects

A miner was unplugging the Block Heater from the Cat 775 Haul Truck when EE bumped the back of EE's head on the truck fender causing a laceration. Injury resulted in the miner receiving staples to close the wound on the back of the head.

A miner stepped off the haul truck and felt EE's knee pop when EE twisted to walk away from the truck. A MRI was conducted and a torn ligament was found requiring surgery.

Employee was operating haul truck in pit when EE pulled over on level ground to take a quick stretch and walk around the machine to do a mid-day equipment check. When EE reached the ground level from the ladder, EE turned to the right to begin walking but EE's right foot was against material on the ground. When EE rotated laterally, EE felt/heard a pop followed by knee pain.

Struck by falling object

Rigging scale safety rail on the shop floor with telehandler. While doing this, rail shifted, EE tried to move out of the way, but was struck by the rail, falling to the ground, rail landing on EE's knee

Fall from machine

Greasing the loader. Several grease fittings weren't taking grease, needed to be changed. Employee climbed ladder, attempted to open door from too far down. Employee lost grip as they opened the door with the right hand, causing them to lose balance and fall, landing on right hip.

While installing saddle liner in 5 1/2 ft cone crusher, the part became wedged. While freeing the part it rolled back, pinning employee's LT hand between the part & the main frame of the crusher.

The door on side of Stedmon Mill was frozen. Employee was trying to pry it open. He lost grip and the door sprung back catching tip of his left little finger between the door frame and the door. The door is constructed of 3/4"" steel plate. The frame is also steel. Employee received a fracture to his left pinky finger.

Employee was the Stedmon plant operator for the day. Employee was opening the feed gate on the feed hopper when it came down unexpectedly catching employee's left ring finger between the handle and a guard. Employee's left ring finger was dislocated. Was taken to ER. Is restricted. No lost time.

Rubbed or abraded

On 1-18-13, employee was working on a cone crusher assembly after repair. Was working on his knees. His left knee started to hurt him. Used ice & Heat - got no relief. Reported to his supervisor on 1-23, went to his family doctor on 1-24. Was sent to company clinic on 1-28-13. Diagnosed with pain/sprain to his left knee. Is restricted.

Employee was using a 20-ton jack to lift the bottom of the Symmons Cone Crusher in order to replace some worn rubber pads. The jack shifted, or failed, and shot out from under the crusher and stuck employee.

The employee was inside a screen cutting a piece of rubber. The rubber was resting upon the employee's leg while making the cuts. On the 4th cut, the employee slipped and cut his leg with the utility knife. The condition contributing to the accident was the body positioning (lying down) inside a screen which is a tight area to work.

EE WAS CUTTING CONVEYOR FRAMEWORK WITH TORCH WHEN A SECTION OF THE CONVEYOR CAME LOOSE & BUMPED THE TORCH INTO HIS RIGHT FOREARM. IT BURNED THRUHIS JACKET & CREATED A 2ND DEGREE BURN TO ARM.

EMPLOYEE WAS SWITCHING LOADERS. WHEN HE REACHED FOR THE FIRST STEP ON THE LADDER HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS HELPING INSTALL A FEEDER BOTTOM. WHILE SLIDING THE NEW BOTTOM INTO PLACE, HIS RIGHT HAND WAS PINCHED BETWEEN THE OLD AND NEW PLATE. ON 4/24 HE SOUGHT MEDICAL ATTENTION AND WAS ADMITTED TO THE HOSPITAL FOR INFECTION.
