Employee was cleaning bolt holes for the grizzly bars and missed the laddy slipper with a sledge hammer smashing their right index finger. Employee was taken to clinic and had the nail removed.

Employee was lying on their back trying to get a rock unstuck from a plugged chute when a larger rock rolled off the belt and hit them on the left knee.

Employee was drilling a hole through a stainless steel box. They reversed the drill and that pushed the drill out, pushing their thumb against the stainless steel edge. Employee was cut on the top of their thumb between the first and second knuckle.

Employee was scrapping frozen material in a coal barge. When doing this EE jarred their back. Was taken to OMP for evaluation for a strained back and given restricted duty.

Employee was changing boards on ski ramp when they felt like their back went out. Was taken to OMP and given work restrictions of no lift over 5 lbs. or forcibly pushing over 5 lbs. Accident under review.

Employee was pulling a chain hoist in from landing a screen. EE thought EE just pulled a muscle in EE's right arm. EE was taken to OMP for further evaluation on 9/15/23 and given restricted duty.

Employee was replacing the center bank winch wire. The winch went into free spool and the handle hit EE's hand. Employee was taken to OMP with a fractured finger and received restrictions of no use of right hand.

Employee was putting screens back in the plant. Doing this the employee pinched the end of this finger and received 5 stitches. Accident under review.

Employee was walking up the steps from the ski ramp and felt a pop in the back of EE's knee. Employee was taken to OMP and received restrictions.

Employee was loading barges and could not see breast wire. EE stepped out of shack to check wire. Doing this they stepped over the take up wire. The wire freed itself and struck employee on the leg. Employee was taken to Dr. and given restrictions of no standing work. Accident under investigation.

Employee was pulling on a cable when EE slipped and lost footing. This caused EE to land on the elbow. Employee was taken to OMP and placed on restrictions. Accident under investigation.

Employee was changing belts on crusher when the ladder kicked out, causing EE to fall and injure elbow. Accident under investigation.

Employee was washing out beside the jaw. As EE was walking around washing out the hose got wrapped around EE's Left leg. This caused EE to trip and break EE's leg. Accident under investigation.

Struck by falling object

Employee was a vertical seam about EE's head. When EE flipped EE's welding hood up to examine the weld a foreign body entered EE's right eye. Employee was taken to OMP to be observed. Dr prescribed eye drops for the day.

EE was shoveling the floor at scrubber stone plant for morning clean up. EE felt something pull in EE's left elbow. EE was taken to omp and placed on restricted duty.

EE was loading a shot when the boom hit the ground and jarred EE's teeth together causing one tooth to crack. EE has now had a root canal on 5/4/2020 and other dental procedures to come.

EE was kneeling on the veranda of loader using pull rope to raise the fuel line up. As ee was doing this ee felt a sharp pain in back. EE was taken to be evaluated by the omp. EE was placed on restricted duty of no lifting more than 50 lbs

Employee was machining a pin hole underneath a haul truck. While working under the truck they bumped their head causing a stiff neck. Employee received medical treatment and was placed on light duty and received an Rx.

EE was hooking a tow cable up to a stuck haul truck. When EE was placing the cable on the truck the cable spun to the right twisting EE's right hand causing a strain. EE was placed in light duty with no use of right hand.

EE went to pull the rope to start the wash out pump. The rope broke and EE felt a pain in the lower right back. EE was taken to the Dr. and diagnosed with a back strain, put on restrictions, and prescribed a non- narcotic muscle relaxer.

EE picked up the pull down cable to hand it to the deckhand on the empty barge that was going to be loaded. EE heard something pop in right shoulder and felt a sharp pain in shoulder and bicep. EE was taken to be evaluated and was diagnosed with a torn bicep tendon. EE was placed on light duty with no use of the right arm.

Employee was walking down steps of Plant 1 Operator's Shack, and when employee stepped off last step they landed on an uneven spot and twisted the left knee. They went to OMP and is on restricted duty.

EE was placing signs in the pit areas when became entangled with the sign post EE was carrying and tripped. Bruising EE's knee and causing small cut. EE waited to go to medical services on 11-28-16 where EE was placed on restricted duty for EE's knee.

Absorption of radiations, caustics, toxic and noxious substances

EE was entering the stockpile truck and was apparently stung by a bee that EE felt crawling on neck. EE reached up and pinned it against neck and was stung. EE had an allergic reaction and was taken to the ER where EE received a steroid injection and was told to take some Benadryl.

EE was pulling a waste oil cart from under the truck EE was working on. EE slipped and fell causing restricted duty.

Employee was going to reset the metal detector at the scrubber stone plant. As EE was walking through the quarry EE slipped on snow hurting EE's left foot. For clarification, this was NOT a walkway or travelway. EE stepped in fresh snow and slipped - felt pain in EE's foot.

Employee was stockpiling material at Plant 2 when EE decided on their own to try a different route. The short cut had a rut/ditch that EE forgot about the resulted in truck damage and hit EE's face/head on the steering wheel. Employee is being disciplined.

Employee was being loaded in the Euclid R-85 in the pit when he turned to look out of his driver's side window the load being dumped into the truck causes the truck to rock causing him to hit forehead on window of haul truck. Employee was not prepared for this rock/shift in movement.

Employee was adjusting the mirror in his 924 Loader, and the lens broke cutting his right wrist. It was treated with first aid the next day reported he went to doctor to have it check. They cleaned it, and applied steri-strips for closure. i.e. similar to butterfly bandage.

EE stumbled on his truck's floor mat and fell on his right knee. Contusion/swelling of right knee.

Employee went to lift a screen to help install screens and cut his thumb on the edge of the screen. Gloves were available and he chose not to wear them.

EE has had back issues work related / personal for many years. He reported it and wanted medical treatment. It has recently been diagnosed as work related. Exact cause is undetermined - he is a Bulldozer operator which does involve vibration/bounce.

WW was washing out around MK-509 feeder when a piece of mud got into his left eye. EE was wearing personal protective equipment.

Employee bent over to pick up a cable to hand it to someone. When he grabbed the cable and stood up, he felt something pull in his lower back.

Pulling stern wire from dock onto IN87060 barge, and stepped on a fore and aft line on the barge and slipped. His foot slipping jarred his lower back causing aggravation to the muscles of his lower back. Medically diagnosed as a low back strain.

Employee was replacing a wear plate inside MI-0503 when the plate slipped he instinctively tried to catch it. The plate weighing 70# smashed his finger (3rd digit on left hand)between the plate and the ground. He was immediately sent for medical evaluation.

EE was grinding on a crusher part when particulate flew in behind his faceshield and safety glasses causing eye pain. He was sent to the medical provider where they removed a foreign speck and prescribed eyedrops for preventative purposes.

Lubrication technician was turning wrench when the wrench slipped allowing his hand to strike frame causing immediate pain and discomfort. Employee failed to report it until 4-21-10. He was sent to doctor, and diagnosed with FRACTURE of 5th metacarpal of left hand. He was able to return to his normal job with a follow up appt. with doctor. No restrictions.
