Employee was cleaning bolt holes for the grizzly bars and missed the laddy slipper with a sledge hammer smashing their right index finger. Employee was taken to clinic and had the nail removed.

Employee was lying on their back trying to get a rock unstuck from a plugged chute when a larger rock rolled off the belt and hit them on the left knee.

Employee was drilling a hole through a stainless steel box. They reversed the drill and that pushed the drill out, pushing their thumb against the stainless steel edge. Employee was cut on the top of their thumb between the first and second knuckle.

Employee was scrapping frozen material in a coal barge. When doing this EE jarred their back. Was taken to OMP for evaluation for a strained back and given restricted duty.

Employee was cutting grizzly bars and the grizzly bar rolled on employee's hand. Employee was taken to OMP for evaluation of hand.

Employee was stepping over to a platform and twisted their knee resulting in pain. They were taken to OMP and given restricted duty.

Employee was making a change on a bin. When EE went to stand up EE felt a pain in EE's right knee. Employee was taken to OMP on 5/20/24 for evaluation. Given restricted duty of sit down type work.

Employee was changing boards on ski ramp when they felt like their back went out. Was taken to OMP and given work restrictions of no lift over 5 lbs. or forcibly pushing over 5 lbs. Accident under review.

Employee was pulling a chain hoist in from landing a screen. EE thought EE just pulled a muscle in EE's right arm. EE was taken to OMP for further evaluation on 9/15/23 and given restricted duty.

Employee was replacing the center bank winch wire. The winch went into free spool and the handle hit EE's hand. Employee was taken to OMP with a fractured finger and received restrictions of no use of right hand.

Employee was putting screens back in the plant. Doing this the employee pinched the end of this finger and received 5 stitches. Accident under review.

Employee was walking up the steps from the ski ramp and felt a pop in the back of EE's knee. Employee was taken to OMP and received restrictions.

Employee was loading barges and could not see breast wire. EE stepped out of shack to check wire. Doing this they stepped over the take up wire. The wire freed itself and struck employee on the leg. Employee was taken to Dr. and given restrictions of no standing work. Accident under investigation.

Employee was pulling on a cable when EE slipped and lost footing. This caused EE to land on the elbow. Employee was taken to OMP and placed on restrictions. Accident under investigation.

Employee was changing belts on crusher when the ladder kicked out, causing EE to fall and injure elbow. Accident under investigation.

Employee was washing out beside the jaw. As EE was walking around washing out the hose got wrapped around EE's Left leg. This caused EE to trip and break EE's leg. Accident under investigation.

Employee was a vertical seam about EE's head. When EE flipped EE's welding hood up to examine the weld a foreign body entered EE's right eye. Employee was taken to OMP to be observed. Dr prescribed eye drops for the day.

EE was shoveling the floor at scrubber stone plant for morning clean up. EE felt something pull in EE's left elbow. EE was taken to omp and placed on restricted duty.

EE was loading a shot when the boom hit the ground and jarred EE's teeth together causing one tooth to crack. EE has now had a root canal on 5/4/2020 and other dental procedures to come.

Employee was cutting a piece of belting with a utility knife, when the blade slipped from the belt cutting their left upper arm. Employee was taken to local ER and received 3 staples.

EE was kneeling on the veranda of loader using pull rope to raise the fuel line up. As ee was doing this ee felt a sharp pain in back. EE was taken to be evaluated by the omp. EE was placed on restricted duty of no lifting more than 50 lbs

Employee was machining a pin hole underneath a haul truck. While working under the truck they bumped their head causing a stiff neck. Employee received medical treatment and was placed on light duty and received an Rx.

EE was hooking a tow cable up to a stuck haul truck. When EE was placing the cable on the truck the cable spun to the right twisting EE's right hand causing a strain. EE was placed in light duty with no use of right hand.

EE went to pull the rope to start the wash out pump. The rope broke and EE felt a pain in the lower right back. EE was taken to the Dr. and diagnosed with a back strain, put on restrictions, and prescribed a non- narcotic muscle relaxer.

EE picked up the pull down cable to hand it to the deckhand on the empty barge that was going to be loaded. EE heard something pop in right shoulder and felt a sharp pain in shoulder and bicep. EE was taken to be evaluated and was diagnosed with a torn bicep tendon. EE was placed on light duty with no use of the right arm.

Employee was walking down steps of Plant 1 Operator's Shack, and when employee stepped off last step they landed on an uneven spot and twisted the left knee. They went to OMP and is on restricted duty.

EE was placing signs in the pit areas when became entangled with the sign post EE was carrying and tripped. Bruising EE's knee and causing small cut. EE waited to go to medical services on 11-28-16 where EE was placed on restricted duty for EE's knee.

EE was entering the stockpile truck and was apparently stung by a bee that EE felt crawling on neck. EE reached up and pinned it against neck and was stung. EE had an allergic reaction and was taken to the ER where EE received a steroid injection and was told to take some Benadryl.

EE was pulling a waste oil cart from under the truck EE was working on. EE slipped and fell causing restricted duty.

Employee was going to reset the metal detector at the scrubber stone plant. As EE was walking through the quarry EE slipped on snow hurting EE's left foot. For clarification, this was NOT a walkway or travelway. EE stepped in fresh snow and slipped - felt pain in EE's foot.

Employee was stockpiling material at Plant 2 when EE decided on their own to try a different route. The short cut had a rut/ditch that EE forgot about the resulted in truck damage and hit EE's face/head on the steering wheel. Employee is being disciplined.

Walking down steps to close grease pit after servicing equipment and stumbled rolling his ankle. (Sprain)

Employee was being loaded in the Euclid R-85 in the pit when he turned to look out of his driver's side window the load being dumped into the truck causes the truck to rock causing him to hit forehead on window of haul truck. Employee was not prepared for this rock/shift in movement.

Employee was adjusting the mirror in his 924 Loader, and the lens broke cutting his right wrist. It was treated with first aid the next day reported he went to doctor to have it check. They cleaned it, and applied steri-strips for closure. i.e. similar to butterfly bandage.

EE stumbled on his truck's floor mat and fell on his right knee. Contusion/swelling of right knee.

Employee went to lift a screen to help install screens and cut his thumb on the edge of the screen. Gloves were available and he chose not to wear them.

Employee was opening an aluminum can of welding rods and his hand slid across edge of can lacerating his hand requiring three stitches.

While employee was climbing up his truck steps, he felt a sharp pain in his right groin.

Upon the completion of cleaning with a wire wheel attachment, EE had discomfort in his right eye. He was sent to the doctor and a foreign body was removed. EE was given prescription eye drops.

Employee was helping carry a screen. The screen slipped from his hand cutting his left index finger.

EE has had back issues work related / personal for many years. He reported it and wanted medical treatment. It has recently been diagnosed as work related. Exact cause is undetermined - he is a Bulldozer operator which does involve vibration/bounce.

WW was washing out around MK-509 feeder when a piece of mud got into his left eye. EE was wearing personal protective equipment.

Employee bent over to pick up a cable to hand it to someone. When he grabbed the cable and stood up, he felt something pull in his lower back.

EE was pulling the breasting wire on the center of TE-5261(river barge) and felt a pull/discomfort in forearm. He returned to normal duty that day, but the injury has progressed and doctor now decided he need Carpal Tunnel surgery (6-12-14.)

Employee was walking on a travel way when he slipped, turning his ankle.

Employee was not paying attention walking alongside of a barge when he stepped on a winch cable, which resulted in a rolled ankle.

Employee was removing a seal from a brake head. His hands slipped off while pulling on the seal, cutting both left and right thumbs.

EE started to walk down a set of steps and missed the first step, then slid on his behind down a few steps.

EE was assisting in the switching out of barges when the barge winch brake was left on, and the boat pulled out with the cable and brake still on. The cable broke and whip lashed back into EE's lower legs. Life Flight was called.

EE was performing accumulator check and steering adjustment, and bled off the pressure. He then removed the valve which still had pressure and it punctured his right arm. He is on light duty with restrictions.

Employee was removing a loader cutting edge out of the 924 loader bucket and smashed his finger. Open fracture and bruised nail bed.

Employee was driving haul truck with slop from pond, and had window down. He removed his safety glasses to wipe sweat and was splashed by crane bucket entering lime slop pond. He got mud/water in his eye and was wanting to be checked out. Only recordable because of prescription eye drops (precautionary)

Employee was handling material to line a chute and felt discomfort in his lower back. He was sent to OccMed for evaluation and was diagnosed with low back strain. Returned to work with restrictions.

Pulling stern wire from dock onto IN87060 barge, and stepped on a fore and aft line on the barge and slipped. His foot slipping jarred his lower back causing aggravation to the muscles of his lower back. Medically diagnosed as a low back strain.

Employee was assisting with putting the sheave back on the screen shaft. Employee's finger got caught between the sheave and the shaft resulting in a laceration and a fracture on the right middle finger.

EE misstep on the barge and twisted his ankle.

EE was power-washing a truck when material got in his eye.

Employee was getting off the loader when he stepped in to a low spot. Felt a dull pain in his right leg.

Employee was digging out a plug up in the hopper when he felt a burning sensation in his back.

EE was working on transmission housing filter when the socket slipped off hitting his left hand on a piece of angle iron.

EE was unplugging a feeder with a bar. When the rock came loose the bar forced his down cutting three of his fingers on top of his right hand on some metal.

EE was shoveling to clean up area. He pulled something in his back but is unsure of exactly when it happened.

Employee was loading a barge and when he was walking he slipped and hit his back when he fell.

Employee was lifting a gate up under crusher and pulled a muscle in his lower back.

EE was getting ready to put the right rear wheel hub on an R-85 haul truck. As he picked up a stack of shims he cut his right little finger on a 0.005 thick shim. He went to the doctor and received stitches.

Employee was replacing a wear plate inside MI-0503 when the plate slipped he instinctively tried to catch it. The plate weighing 70# smashed his finger (3rd digit on left hand)between the plate and the ground. He was immediately sent for medical evaluation.

EE was grinding on a crusher part when particulate flew in behind his faceshield and safety glasses causing eye pain. He was sent to the medical provider where they removed a foreign speck and prescribed eyedrops for preventative purposes.

Lubrication technician was turning wrench when the wrench slipped allowing his hand to strike frame causing immediate pain and discomfort. Employee failed to report it until 4-21-10. He was sent to doctor, and diagnosed with FRACTURE of 5th metacarpal of left hand. He was able to return to his normal job with a follow up appt. with doctor. No restrictions.

Shoveling rock with a long handle shovel and strained his back.

walking across the yard, slipped on some packed snow and fell, causing a strain to his left hip.

Climbing down ladder on an R-60 truck. When he stepped to the ground he twisted his knee resulting in a bruised left knee.

Employee was changing a screen in a screen deck. She was bent over and when she raised up she hit her back on the frame work causing an abraision to her back.

Employee was loading barges at the wash dock. He had an asthma attack and may have passed out. He fell on his right side causing a bruise to his back.

Using a hammer to drive a pin out of a loader bucket. Missed the pin and struck the inside of his left knee resulting in a bruised knee.

EE was pulling on a screen with a hooker bar. The hook came loose from the screen and the bar hit him in the mouth, causing a broken tooth.

Victim was using a torch working through a small opening in a chute. He had cut a hole & put the torch in his left hand. He reached in the hole with his right hand and his arm passed in front of the torch causing a burn to the top of his right forearm.

Shoveling debris off catwalk, hit right knee on conveyor frame resulting in a strained right knee.

Changing screens in a screen deck. Lifting a screen up to the top deck pulled a muscle in her neck resulting in a neck strain.

Employee was climbing down from his truck and slipped and fell to the ground landing on his feet causing a broken toe.

Victim was walking by a conveyor when a rock fell off the conveyor striking him on the shoulder resulting in a bruised shoulder.

Stepping from cab of truck to ground. Shoe string came untied and he stepped on shoe string causing him to trip and fall resulting in a contusion to his left hip.

Using a prybar to adjust the feed rate to a crusher. The prybar slipped and struck him in the mouth resulting in a chipped tooth.

EMPLOYEE WAS STARTING A FIRE IN A COAL STOVE WHEN THE FIRE FLARED UP AND BURNED HIS HAND, CAUSING A BURN TO HIS FINGERS & HAND.

REMOVING A TRUCK BED FROM TRUCK. CUTTING AN EXHAUST COUPLER FROM THE BED. THE COUPLER FELL BEFORE HE WAS READY & THE COUPLER FELL ON HIS HAND CAUSING A LACERATION REQUIRING SUTURES.

Victim was picking up a pail of #2 rock for testing and strained his back.

EMPLOYEE WAS REMOVING ROCKS FROM A DUMP HOPPER & GOT HIS FOOT CAUGHT BETWEEN THE HOPPER BUMPER & A PIECE OF MOBILE EQUIPMENT RESULTING IN A LACERATION REQUIRING SUTURES.

Climbing steps on r85 truck and felt a pop in his back resulting in a muscle strain to his lower back.

Victim was helping another ee lift and carry an air conditioning unti and strained his back.

EE WAS CHANGING HEADS IN A CRUSHER. EE WAS HITTING A WEDGE INTO PLACE WITH A HAMMER. A PIECE BROKE OFF THE WEDGE AND STRUCK EE IN THE FACE CAUSING A CACERATION REQUIRING SUTURES

Using a shovel to shovel rock on a flat surface and strained his back.

EE WAS WORKING ON A BARGE, PULLING WIRE CABLE, AND STRAINED HIS BACK. REPORTED INJURY ON 12/06/04.

VICTIM WAS DRIVING A STOCKPILE TRUCK THROUGH THE YARD WHEN THE TRUCK SHIFTED INTO A HIGHER GEAR. IT JERKED CAUSING HIS BACK TO JERK RESULTING IN A STRAINED BACK.

VICTIM WAS OPERATING A NEW HOLLAND BROOM/SWEEPER. WHEN FINISHED WITH THE JOB HE REACHED OVER TO UNBUCKLE HIS SEAT AND STRAINED HIS BACK.

HANGING WATER PIPE, WORKING FROM A LADDER, HE LOST HIS BALANCE AND JUMPED FROM THE LADDER ABOUT FOUR FEET TO THE FLOOR RESULTING IN A SLIGHT FRACTURE TO HIS ANKLE.

VICTIM WAS USING A POCKET KNIFE TO CUT A PIECE OF RUBBER BELT. THE KNIFE FOLDED UP STRIKING HIS FINGER CAUSING A LACERATION TO HIS RT. INDEX FINGER REQUIRING SUTURES.

EE OPENED HIS TRUCK DOOR TO GET OUT OF THE TRUCK. THE DOOR STRUCK THE LOADOUT PLATFORM FRAMEWORK AND BOUNCED BACK, STRIKING HIS RIGHT KNEE AND CAUSING A BRUISED KNEE.

USING A 3 FOOT PRY BAR TO LOOSEN A ROCK IN A CHUTE. THE BAR SLIPPED OFF THE ROCK CAUSING HIS FINGER TO STRIKE A STEEL PLATE RESULTING IN A BRUISED FINGER.

WALKING ON THE GROUND BETWEEN 2 BUILDINGS. GROUND WAS COVERED WITH SNOW & ICE. HE SLIPPED ON TH E ICE TWISTING HIS KNEE RESULTING IN A STRAINED KNEE.

VICTIM WAS LIFTING A SCREEN AND HANDING IT TO ANOTHER EE WHEN HE PULLED A MUSCLE IN HIS SHOULDER CAUSING A STRAINED SHOULDER.

GETTING OUT OF THE TRUCK, GOING DOWN LADDER, FOOT SLIPPED ON STEP, SHE CAUGHT HERSELF WITH HER RT. ARM CAUSING A STRAIN TO HER RIGHT ARM.

STEPPING FROM TRUCK TO A PLATFORM WHICH IS 6"" LOWER THAT TRUCK, TWISTED LEG CAUSING A STRAIN TO HIS RIGHT KNEE.

HELPING CHANGE SCREENS USING A HAMMER TO DRIVE PINS TIGHT, SHE STRUCK HER FINGER WITH THE HAMMER CAUSING A LACERATION REQUIRING SUTURES TO LEFT INDEX FINGER.

PULLING ON WIRE CABLES TO SECURE BARGES AND PULLED A MUSCLE IN HIS BACK CAUSING A STRAINED BACK.

VICTIM WAS GREASING CONVEYORS IN THE PLANT. HE BECAME LIGHT HEADED AND FELT SICK. HE WAS TAKEN TO AN AIR CONDITIONED LOCATION AND THEN TO A DOCTOR. HE WAS DIAGNOSED AS HAVING HEAT EXHAUSTION.

VICTIM WAS PULLING WIRE CABLE ON A BARGE WHEN HE SLIPPED ON SOME LOOSE ROCK ON THE BARGE DECK. THIS CAUSED HIM TO FALL ON HIS LEFT KNEE AND LEG CAUSING A CRACKED KNEE CAP.

VICTIM WAS LIFTING A CHUTE AND FELT A PULLING AND BURNING IN ALS SIDE, RESULTING IN A STRIAN TO HIS GROIN AREA.

VICTIM WAS WALKING ON THE GROUND AND STEPPED ON A ROCK AND TWISTED HIS ANKLE RESULTING IN A SPRAINED ANKLE.

PULLING ON A WIRE CABLE TO TIE OFF A BARGE AND PULLED A MUSCLE IN HIS SHOULDER CAUSING A STRAIN TO HIS RIGHT SHOULDER.

VICTIM WAS HELPING CHANGE SCREENS WHEN SHE PULLED A MUSCLE IN HER NECK CAUSING A NECK STRAIN.

OPERATING A FRONT END LOADER MOVING FROM ONE PLACE TO ANOTHER TRAVELED ACROSS A PLACE IN THE ROAD THAT WAS UNEVEN CAUSING THE LOADER TO BOUNCE RESULTING IN A STRAIN TO HIS GROIN AREA.

HAULING LIME TO A SPOIL PILE HE HIT AN UNEVEN SURFACE IN THE ROAD AND BUMPED HIS ELBOW ON THE DOOR FRAME OF THE TRUCK BRUISING HIS ELBOW.

VICTIM WAS WALKING UNDER A CATWALK WHEN A PIECE OF ROCK FELL BETWEEN HIS HEAD AND HIS SAFETY GLASSES CAUSING A SCRATCH TO HIS LEFT EYE.

VICTIM WAS USING A RAZOR BLADE KNIFE TO REMOVE AN O-RING FROM A PUMP. THE KNIFE SLIPPED AND CUT HIS RIGHT THUMB CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS REPLACING A ROLLER IN A CONVEYOR SYSTEM. WHILE LIFTING THE ROLLER INTO POSITION IT SLIPPED OUT OF HIS HAND AND HIT HIS FINGER BETWEEN THE ROLLER AND A BRACKET CAUSING A LACERATION T O HIS FINGER REQUIRING SUTURES.

VICTIM WAS USING A WEED KNIFE, SWINGING IT FROM SIDE TO SIDE CAUSING HIM TO TWIST HIS LEGS RESULTING INA STRAIN TO HIS LEFT LEG.

VICTIM WAS HELPING LIFT A CONVEYER GUARD AND STRAINED HIS BACK.

VICTIM WAS MOVING A LARGE ROCK WITH THE LOADER WHEN THE LOADER BUCKET SLIPPED OFF THE ROCK AND JARRED THE OPERATOR CAUSING A LOWER BACK STRAIN.

VICTIM STEPPED OFF A FORKLIFT STEPPED ON A ROCK CAUSING HIM TO SPRAIN HIS LEFT ANKLE.

VICTIM WAS USING A UTILITY KNIFE TO CUT A RUBBER SCREEN WHEN THE UTILITY KNIFE SLIPPED OFF THE SCREEN AND CUT HIS INDEX FINGER ON HIS LEFT HAND CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS RAISING THE LID COVER ON A BATTERY BOX. THE LID SLIPPED OUT OF HIS HAND FALLING ON HIS LEFT HAND CAUSING A SPRAINED LEFT WRIST.

VICTIM WAS HELPING TO PLACE A SPOOL OF CABLE ON A STAND WHEN HE GOT HIS FINGER BETWEEN THE SPOOL AND THE STAND CAUSING A LACERATION REQUIRING SUTURES.

VICTIM WAS GRINDING ON A PIECE OF STEEL AND GOT A PIECE OF DUST IN HIS RIGHT EYE CAUSING A SCRATCH TO HIS EYE.

VICTIM WAS PICKING UP A PIECE OF STEEL PLATE AND CUT HIS THUMB ON A SHARP EDGE OF THE STEEL CAUSING A LACERATION TO HIS THUMB. REQUIRING SUTURES.

VICTIM WAS PULLING ON A ROPE AND PULLED A MUSCLE IN HIS CHEST CAUSING BRUISED CHEST.

VICTIM WAS PERFORMING MAINTENANCE WORK LOOSENING A BOLT. THE BOLT BROKE AND HE FELL AGAINST HIS ARM CAUSING A BRUISE TO HIS ARM.

INSTALLING A PLANETARY ON A TRUCK WHEN A PIECE OF METAL BROKE OFF & STRUCK THE VICTIM ON THE HAND CAUSING A LACERATION

VICTIM WAS CUTTING WIRE CABLE WHEN A PIECE OF HOT CABLE FLIPPED AND STRUCK HIM ON THE WRIST CAUSING A BURN TO THE WRIST.

VICTIM WAS LEANING OVER A STEEL CHUTE INSTALLING A FLAPPER. GOT ALL OF HIS BODY WEIGHT ON HIS CHEST CAUSING A CRACKED RIB.
