Struck by falling object

Two employees working on station cleaning out vibrating feeder. One employee was below the station installing a cutting shield hard hat and other employee had started work on cleaning the feeder above EE. As the employee was installing EE's head gear EE came into the path of a rock that was thrown down to the ground striking EE in the head, resulting in stitches to EE's head.

Employee was loading a conveyor belt into the back of a pickup truck and upon setting the end of the shaft on the tailgate, their helper pushed the opposite end of shaft forward, causing employee's thumb to get caught between a sharp metal part of the tailgate and roller shaft.

Struck by falling object

Employee was changing feed box liners on top deck of screen #2.Dew accumulation, and light coating of dust had made the work area slick for footing and a slip occurred while positioning liners in place. The liner dropped and impacted the employee's finger causing a laceration.

Over-exertion in wielding or throwing objects

An adjustment needed done on the gate for the #8 limestone. This is usually performed with a 3/4"" ratchet and hammer. EE was using a sledge hammer which resulted in a mid-low back/front hip strain with right leg radiculopathy.

Employee was stepping down from Off Road Haul Truck, left foot stepped onto a tire lying flat on shop floor, lost balance and fell backwards hitting right leg (above knee) on truck.

Employee claims injured lower back while operating a Komatsu WA600 front end loader.

Removing the drill bit from hammer to replace the blow tube using a hydraulic chain wrench, left hand was on wrench when pulling wrench with hydraulic cylinder thumb on left hand was crushed in cylinder to wrench pinning point.

While welding on sand screw auger, a small piece of metal slag fell into employee's boot and burned his right ankle.

Helping 2 co-workers turn sheave on a pump in the plant. When he was pulling he got his finger between the belt & the sheave.

Enlarging the opening failed then ee used a pry bar. While poking the obstruction the bar glanced off the obstruction contacting the moving conveyor belt kicking the bar and his hand that was holding the end of the bar up against a horizontal support between the skirt boards. This resulted in the end of the bar piercing the palm of his right hand.

Cleaning the deck at head end of conveyor 2. He was picking up a rock to put on conveyor 3 when another rock fell off from conveyor 2 and struck his right pinky finger and smashed it between 2 rocks.

While installing strut bearing on truck, a piece of brass entered his eye behind his safety glasses. He was using a brass hammer.

Fall onto or against objects

While working under haul truck, he lost his balance and fell with all his weight on the left knee landing on a small rock. Never reported because he did not believe it would still hurt 1 1/2 months later. Treatment received 10/3/08 making this now reportable.

Shoveling rocks & mud from under Conv. 8. Strained back. Began injections last week, reason for reporting now.

Employee cut the 4th finger on an AC fan. Wound required sutures and this is the reason for reporting injury.

Caught in, under or between a moving and a stationary object

On conveyor #2 unplugging head box, using a 5' pry bar to wedge between rocks. Pry bar popped out of rocks & smashed little finger between pry bar & steel plate skirt board. Wound required sutures to close, reason for reporting.

While tramming a 1 ton hoist operated by a hand crank pulley system, the load on hoist shifted while being turned over, causing the crank to back lash into employees left wrist.

STEPPED ONTO CATWALK PIECE OF WALK NOT WELDED DOWN, FLIPPED OUT FROM UNDER HIS FOOT. FELL AND TWISTED RIGHT ANKLE. GIVEN PAIN MEDS AND CRUTCHES.

CUTTING WITH ACETELYN TORCH, DUST AND SLAG SPLASHED BACK INTO HIS FACE AND EYES. HE WAS GIVEN RX EYE DROPS

Struck against stationary object

Installing conveyor belt on conveyor. His knee hit conveyor truss. Reported on 4/4/05. Was unaware that he had hurt himself at the time. Fluid had to be drained from knee.

FIRE BROKEOUT IN CEILING OF LUNCH ROOM THE FIRE BURNT FOR AN UNSPECIFIED AMOUNT OF TIME. STRUCTURE DAMAGE ONLY.

Struck against stationary object

The head box of conveyor #32 plugged with a 16"" x 16"" steel plate. He removed the steel plate from head box and while he was climbing out of box he pulled his right foot up into a sharp piece of chute liner, piercing through his work boots onto the top of his foot. Wound required 4 stitches.

EE WAS WALKING FROM HIS POV TO EMPLOYEE LUNCH ROOM TO CLOCK IN. HE SLIPPED & FELL ON HIS LEFT KNEE DUE TO ICE. HE DID NOT REPORT THIS TO US UNTIL 2-26-04.

HE WAS LOADING OLD PUNCH PLATES INTO SKID STEER BUCKET. HE SLID SECOND ONE IN AND THE HOOK STRIP CAUGHT RIGHT MIDDLE FINGER BETWEEN THE PLATES.

HE WAS CUTTING ANGLE IRON WHICH DEFLECTED OFF A CROSS ANGLE & LANDED ON HIS FOOT.

EE WAS REPLACING A BUCKET TOOTH ON PRODUCTION LOADER. HIT THE KEEPER WITH A HAMMER AND A SLIVER OF STEEL BROKE OFF AND LODGED IN LEFT ARM.

LOADER OPERATOR THREW A BIG ROCK INTO HIS TRUCK CAUSING HIM A NECK INJURY. THIS ACCIDENT WAS NOT DETERMINDED TO BE WORK RELATED UNTIL 10-1-02.

EE WAS PROPING A TRUCK TO HELP WITH CLEAN UP IN THE QUARRY. HE WAS STANDING ON THE HOOD WHEN HE STEPPED BACK AND SLIPPED OFF THE TRUCK FALLING TO THE GROUND. HE STEPPED ON A RUBBER MUD FLAP WHICH HAD NO STEEL BACKING, THE RUBBER FLAP GAVEAWAY ALLOWING HIM TO FALL.

HE WAS HELPING REMOVE A LARGE ROCK FROM THE HOPPER AT THE JAW CRUSHER. THE CHAIN WHICH WAS WRAPPED AROUND THE ROCK BROKE, STRIKING HIM IN THE BACK BELOW HIS LEFT SHOULDER.

EE WAS WORKING ON THE DRILL. HE STEPPED ON A PIECE OF PIPE THAT WAS COVERED WITH SNOW. HE HIT HIS ELBOW ON THE CONCRETE AS HE FELL JAMMING HIS SHOULDER HIS TEETH WERE CHIPPED AS HE HE HIT THE GROUND.

EE WAS CUTTING WITH A PLASMA CUTTING TORCH ON A RUSTY TANK. BOTTOM. HE WAS WEARING FUEL FACE CUTTING SHIELD WITH SAFETY GLASSES. AERT RUST MAY HAVE COME FROM BEHIND AND LANDED IN HIS GLASSES O R IT COULD HAVE TURN THE DUST PARTICLES IN THE AIR.
