EE slipped and fell on EE's right hip

EE was entering the building on EE's way to EE's office when EE tripped on the steps going up to the door. This was on Friday. We considered it first aid. On Tuesday EE went to the Clinic for an X-ray. X-ray showed a fracture in EE's right foot. EE came back with restrictions. This is when it became an MTI and recordable to MSHA

EE experienced Heat Exhaustion due to temperatures in the 90's range

EE was removing a wire from the #5 rotary valve when the valve turned and pinched EE's left middle finger, removing the fatty tip of the finger.

Employee was working in #1 kiln baghouse helping to change bags. The employee was kneeling to pick a broom up off the tube sheet. While kneeling the employee overextended self trying to reach the broom and employee's weight shifted to left arm and wrist twisted on the blow down tube that employee had ahold of. Employee felt a pop at this time in left wrist.

Employee was changing silo hammer line valves when EE experienced a sharp pain in EE's Left shoulder. Employee reported the incident but did not seek medical treatment at that time. Employee tried home self-care but the shoulder did not get better. Employee sought medical treatment and was referred to physical therapy, making it a reportable injury.

Employee was walking around the cooler fans when EE hit head on a yellow handle causing their head to lift slightly and caused a small laceration without complications in EE's upper lip.

Employee was on knees on FM1 beating the hatch loose on the secondary end so we could drop and pull this hatch to spin balls out. As employee was swinging down, index finger on right hand was pinched between the sledge hammer handle and a liner bolt that was just to the north of the hatch.

Employee was pulling an unloading hose (4"" diameter) from Bay #2 to Bay #6 the hose caught on an I beam. IE turned to see what the hose was struck on, pulled hard, turned to continue toward Bay #6 and tripped over another hose. IE fell on left knee. Landed on concrete that caused a contusion to left knee.

Employee was walking to EE's car after clocking out EE fell going to EE's car and dislocated EE's left pinky. There had been a heavy down pour minutes before the incident. Employee's pinky was put back in place and EE returned to full duty the next day. On 7/27/2020 EE was re-examined and the pinky was not set correctly. Pinky was reset and employee was placed on restricted duty.

EE was boarding 772G haul truck when EE felt a pop in right knee.

Employee was helping to remove a door from the #1 kiln hood when the door shifted and caught a finger between the door and the door frame

Employee was moving a bucket of cement and when EE sat it down the cement splashed up and under EE's safety glasses and into EE's right eye.

A brick broke and clipped the left pinkie of a brick layer breaking EE's finger tip

While attempting to blow out debris from the direct burn line fuel splashed out onto my PPE and unknowingly on my boots. After a couple of hours my foot was tingling and would not stop. I pulled off my boot and noticed material had soaked through my boot to my sock. Pulled off sock found that it made contact with my foot, causing chemical burn.

Employee was using a box cutter knife and placed it on a pallet of sand knee high with the blade exposed. The employee walked away and as ee walked back toward the pallet ee ran knee into the knife causing a laceration of the left knee.

Employee was operating a 12 ton electric hoist. EE placed EE's hand on top of the block while the hoist was traveling up. EE's pinky finger on EE's right hand was pinched between the shiv and the cable lacerating the top of the finger and breaking the pinky finger.

Employee was working to put mill rods into the raw mill when ee felt a sudden pain in left shoulder. The next morning the pain had increased and went to the doctor to be evaluated.

Employee complained of back pain as a result of a reoccurrence of a previous condition. (See E-Document 2130606)

Employee was using a standard metal sledge hammer to remove the teeth off of a loader bucket when a piece of the tooth fractured and struck the employee in the arm. The piece of metal hit the employees left forearm causing a laceration and the metal piece to stick in the arm. The metal piece that fractured was approximately 1/4"" (long) X 1/8"" (wide) X 1/16"" (thick).

Employee was attempting to adjust a fan damper when the ladder they were on slipped and they fell to the ground. As the ladder came to a stop at the bottom the employee jumped to the right and felt a pain in both ankles.

Employee was working in the #1 kiln dust collector re-bagging. EE noticed arms becoming irritated so put on a long sleeve shirt to keep dust off. The following morning reported arms at the bend of the elbow were showing signs of irritation.

Employee was working on a pickup truck in the maintenance shop, helping another mechanic to lift the bed off of a 1/2 ton pickup truck. Employee stated that ee ""felt a ""pop"" in lower back but felt like it would go away"". The next morning the employee reported the incident to supervisor and that back felt stiff.

Employee tripped and fell while measuring a silo. When EE lifted the cover, the silo was blowing dust out. EE turned head quickly to get out of the dust and tripped. EE fell forward onto the silo lid that is a 3'X3' box 18"" off of the ground. EE's shoulder hit the lid first then the right side of face.

Employee was walking down a flight of stairs wearing cleated over boots when EE lost their footing. The stairs were made of a perforated grating for grip. The cleat caught in the grating and EE's momentum caused EE to lose footing. EE hit the bottom but said was ok. EE was sent to the clinic as a precaution. EE was given a 25 lifting restriction until 2/2/2015.

Employee was attempting to seat a valve on a diverting plate by striking it with a hammer. EE missed intended target and EE's hand struck an adjacent piece of metal lacerating the pinky finger requiring 3 stitches.

Employee was operating a 988F end loader. The air suspension compressor for the operators seat had a blown fuse. The repetitive bouncing through normal operation of the loader caused lower back pain.

While doing routine work employee felt discomfort in left shoulder. He was sent to a health clinic for evaluation and was returned to work the same day with no restrictions. After continued discomfort, surgery was deemed necessary on 1/29/2016. This did not become reportable until surgery on 01/29/2016.

While pulling rods out of the Raw Mill an employee got their fingers pinched. The crew was using a cable sling to bundle mill rods together and hoist them out of the mill. As the sling came up the rods rolled and one of them slid on top of the employee's hand pinching the two middle fingers on the left hand.

A laborer was attempting to check an oxygen bottle to see if it had anything in it when he was burned. He was standing on the right side of the bottle and used his right hand to open the bottle. There was a flash and the employee's right side and right forearm were exposed to a flash fire. The burns covered an area from his wrist to his upper bicep.

Employee reported a chronic pain in his left hand, possibly attributed to operating rocker switch for forward and reverse on 988H Loader. Employee was sent to physician, placed on restricted duty (avoid using left hand)to see if symptoms improved

Employee was walking between cooling tower & tank 7. The plant had endured numerous snowfalls and multiple days of sub-zero temperatures. Snow had been cleared but employee stepped on a patch of ice, slipped and landed on his hip, also hitting his head in the fall. Employee taken to physician, placed on restricted duty until re-evaluated. Released to full duty 2-1-2014

Employee was attempted to open a diverter gate on #2 kiln clinker drag conveyor. The gate is operated by a 4' handle. When he applied tension to the handle, it broke free unexpectedly and struck him in the forehead. He was taken for medical treatment, received 5 stitches, and was returned to work with no restrictions.

Employee was cleaning debris from a Raw Mill feed hopper when the seat of his pants got wet from contact with material on the wall. Employee later complained of an irritation of his right buttock. Employee was sent to Dr. for eval, given an ointment to treat mild chemical burn and returned to work w/ no restrictions.

Injured employee was removing a shaft from a speed reducer. As he drew the shaft out, he nipped the tips of three fingers on his right hand cutting the tip of his R middle finger, treated on site, sent to Dr. for further evaluation where he received a few stitches on his fingertip and placed on left handed duty only until follow-up appt on 7-14

Driver was raising dump bed and truck and trailer turned over

A contract truck driver was unloading clinker, when the trailer tipped to the right,also turning the tractor over. Driver was wearing seatbelt,and climbed out of cab. Mine personnel administered first aid.Driver had small cut on right ear.EMS took driver to ER-received a few stitches in his r.ear.Note:outside driver-some info-exact DOB,experience has been estimated

Employee had filled a bucket w/ CKD dust. When he set the bucket down, impact w/the ground caused the dust to slosh out of the bucket, getting in his eyes. On-site personnel flushed eyes. Employee sent to phys. for further irrigation.phys. removed dust particles from eyes, employee was given prescription eye drops & returned to work w/ no restrictions.

Employee was cleaning hard build up from the bottom of #1 Kiln Clinker elevator with small chipping hammer. Employee was wearing approved safety glasses, and dust mask. When he activated the handle on the chipper the 5/8"" air hose came loose from the hose barb on the quick coupling, and struck the employee in the right eye.

Employee was descending ladder from a haul truck and stepped off ladder on to a baseball sized piece of shot rock on the ground with his right foot. This caused his right knee to pop and immediately he felt pain. The haul truck was located West of the fuel pump station.

During the unloading of a Montana Minivan that was loaded with parts stolen from the storage barns an employee's right hand was pinched between the forks of a pickup truck and a clinker cooler spindle when the spindle rolled. The employee said that his right hand was throbbing and he had minor abrasions on the top of his hand.

Employee was using Flexane 80 putty to seal around bolts on the SPF hopper screws. Vapors being released from product caused some type of allergic reaction on the arms of employee. Employee noticed itching and removed himself from area. A poison ivy type rash developed on arms.

Employee was exiting the hopper door on the #2FM, #1 DC screw when the stepladder he was using tipped over and he fell approx 5ft to the floor. He landed on his back and struck the back of his head on the floor. Employee was evaluated at Company Physician. Evaluation determined head contusion, no other injuries were noted. Employee was placed on a work restriction of no climbing.

There was fire inside the Pier 6 switchgear room on #1 kiln at 12:36 on 10/18/10 that took down the #1 kiln. The turbosonic compressor breaker caught on fire and tripped the main feed in the sub at Pier 4. The smoke detector in Pier 6 switchgear activated and gave the location of the fire in the control room.

Employee was using a chain hoist to lift the cover from a hoist gearbox on the P&H crane. When the cover broke free from the gearbox, the beam above jumped slightly, causing dust to fall into the employee's right eye as he looked up. Employee was treated by company physician and returned to work the same day.

Employee was replacing gudgeon shaft and hangers in a 9"" conveying screw. He was using a bar to lower the screw back down into the housing. The bar slipped and caught his right index finger between the bar and the screw housing. Fracture to tip of right index finger.

Employee was at the top of the ramp, shaking out gallon plastic bags that are used for lab samples, and he accidentally hit his right index finger on the top of the handrail. A few days later his finger because swelling and became very painful. His range of motion is also impaired. Employee reported this accident on 5/27/2010 to his supervisor and on 5/28/2010 to the HR Manager.

Employee was packing cement and one of the packing spouts activated on its own causing a stream of cement to hit the employee and dust entered the employee's right eye. Employee's eye is red and irritated. Employee was treated and issued eye ointment and patched.

Employee was using an air powered impact wrench and a universal joint and a 1 5/16"" socket. While reaching to remove the bolt his finger energized the trigger of the wrench and the socket flew off the universal joint striking employee in the center of his upper lip.

Employee was standing beside the electric water pump while it was running when the 2"" ball valve came apart on top of the pump. He was sprayed with water and then the cam lock end of the discharge hose came apart and hit him in the nose. The cam lock struck him in the safety glasses and nose. He received a cut on each side of his nose and his nose was broken.

Employee was hooking up the fuel cart between the shop and the silos and as he was cranking up the dolly, the tailgate on the truck fell and struck employee's back and right elbow. Employee reported pain and saw comp physician on 12/21/09 for evaluation. Employee was later scheduled for physical therapy on 1/19/2010 and was not previously on restrictions.

Employee complained at shift change that his left eye felt like there was something in it. At 01:30 he reported to the shift foreman that his eye was really starting to irritate him. Used eye wash, but did not work. Injury visited the doctor to look at his eye. Dr. found a piece of dirt under the eyelid. The doctor put some drops in his eye.

Employee was entering oil shop through the north door of the maintenance shop. Door was partially raised because the Oiler was pumping used oil from mobile equipment oil drain pan to used oil storage container in the oil shop. Employee thought he had ducked low enough to clear the door way, but he didn't and hit his head into bottom of door, which caused his neck to jam.

EE was coming down the P&H crane stairway and when he was almost to the bottom of the stairs, he heard a bang from the cement elevator on #2FM, he looked over that way and misjudged the last stair and landed on the ball of his left foot, a little past the last step. This is larger than the rest of the steps. When he did, his left ankle started to hurt.

He stood to the side of the tank and regulator reached his right arm and hand across the front of the regulator and slowly cracked opened the oxygen tank valve. A loud sound like a shotgun blast occurred when the back of the regulator blew apart. Pieces of the regulator struck his lower right forearm causing it to bleed.

While welding, a hot piece of slag burnt through EE's glove and burnt his hand. He took his glove off and the skin where he was burnt came off. The burn is on his right hand on his pinky knuckle and his ring finger knuckle and is about 1"" long and 1/2"" wide. He went to the first aid room and cleaned the burn, put on antiseptic, and dressed the burn.

Employee was pulling old motor leads from #1 ID fan drive - cutting the wires off with a Sawzall. I was holding the large wires (3/4"" copper stranded) in my left hand and running the saw with my right hand. I was bending the wire with my left hand when the saw drew my hand in. I was wearing my cotton work gloves.

Clinker Elevator for #2 Kiln would not run. Found out the Chute going to cross apron was plugged up with dust and large chunks of clinker. Two co-workers and I were trying to get the chute unplugged using a Jack Hammer. While I was using the Jack Hammer inside of chute the material broke loose and pinched my finger between elevator housing and the jack hammer.

There was some smoke leaving the #2 coal tank that couldn't be seen from the ground. We notified MSHA at their 800 number to report a possible fire in our #2 coal tank. We then ran a fire hose up to the top of the coal hopper from the fire hydrant. The foreman and a laborer flooded the coal inside the tank with fogging spray.

Employee had his tape measure hooked onto a piece of metal and was pulling it out to measure the length. As he was pulling the tape back it sliced into his left index finger. Tape was old and well used. The edge was worn down and sharp.

EE was walking on the catwalk used to sample trailers. After checking the load his foot slipped causing him to fall and hit his knee. His knee had a small scratch and 20 minutes later he had fluid on his knee causing it to swell up to the size of a golf ball. Failure to use the stairway provided. Ladders are there only for emergency exit off the catwalks.

Employee was using new utility knife blade to cut through dust collector bags and sliced through the bag easily and on the hand motion towards his body he cut into his left knee making a 1/2 inch cut on the knee cap.

EE was in the bottom of #1 clinker cooler checking for loose rack and T bolts. There was still some clinker in the compartment and it sloped down toward the middle. He felt himself slide down the clinker and reached up to grab something to steady himself. He grabbed ahold of a pipe brace and felt something pull in his back as he caught himself.

EE has reported to his supervisor that he has pain, tingling, and numbness in both hands, claiming that it has been going on now for a couple of months or more, perhaps starting as early as January. He stated that he notices the pain and numbness when he uses his hands in maintenance.

He was cutting balls out of the discharge screens in #1 Raw Mill with the plasma cutter. A hot piece of slag rolled down into his glove. causing a burn to rt palm below the thumb. On 4/22 was sent to the dr., put on restrictions and given an antibiotic for his infection, which resulted in an allergic reaction, and he was sent back to the dr. on 4/29.

Employee was tightening bolts, locking together two open-ended wrenches to increase torque. Employee was alternating between using the wrenches and using a laser aligning tool, and was not wearing gloves at the time. One of the wrenches slipped off and open end of the wrench punctured the palm of his left hand, requiring six sutures to close the wound.

Employee's glasses were steamed up and dirty and she thought she had reached the bottom of the ladder, when she slipped. Employee missed the two bottom rungs on the ladder coming down from the hydrophobe tank. This caused right knee pain and swelling.

While exiting the elevator at the silos following a silo change this morning employee grabbed the bottom of the interior door to assist in opening the door. While pushing up on the bottom of door his right thumb popped. He noticed his thumb started to hurt and went to shop and took his gloves off to look at his thumb. His right thumb had swollen up within minutes.

Working in the cooler handling grates, passing grates into the cooler. Strained lower back.

Employee was using a pocket knife to push in a jog button for the belt #1 at the crusher to help in determining the problem with the scale readout. As he was pushing on the knife the blade came closed on top of the employee's right hand index finger at the knuckle area. This resulted in a cut approx. 1/2"" long and deep enough that it was necessary to go to doctor for stitches.

The shaft on the feeder had broken and part of it was still in the gear reducer. Employee and co-worker were attempting to drive the shaft out with a hammer and drift. When the co-worker struck the drift a piece of it broke off and flew into employee's left forearm.

Employee fell while descending a stationary ladder. It is estimated that he fell between 6-10 feet. It appears that he hit his back on a guard for the nearby conveyor belt. Employee stated that he was carrying tools while climbing down. He felt a pop in his back and had immediate pain in his neck, left elbow and left shoulder.

Contractor was welding on the #1 Kiln Shell. Employee was performing his normal duties, causing him to walk on the catwalk beside the kiln. A small piece of hot slag, approximately the size of a pellet, rolled around the kiln shell, was deflected off the pier and landed on the back of his neck. This caused 2nd degree burns to the back of his neck.

The door on the Mill Building Crane will occasionally become unlatched due to wear and vibration. While EE was attempting to use his right hand to shut the door, he failed to notice that his left hand was between the door and the door jam. This pinched his left hand, causing a fracture and lacerations to his (L) middle finger and lacerations to his (L) ring finger.

Employee was lifting a heavy chain.

EE was lifting a heavy chain, which at the time we thought resulted in a pulled muscle or possibe hernia, only to discover over a period of time that it was actually a herniated disc, requiring surgery to repair.

Standing on ground the employee began to climb up to work area and placed his foot on a brace that had some ice and snow which resulted in his slipping and falling.

Employee was using a cutting torch overhead, holding the torch with right hand. A slag of liquid metal fell and burned through his coat sleeve, and onto the top of his left lower forearm/wrist area. The metal stuck to the employee's wrist watch and melted his watch. Employee received severe (3rd degree) burn to his arm.

Employee was helping remove rocks from the primary crusher after it had plugged up with stone and dirt. As Ken and another employee were trying to pick the rock up off the ground to load it into a wheelbarrow, Ken strained his back muscle by trying to lift one end of the rock at an awkward positon.

Employee was changing a roll of plastic on stretch wrap machine. After completing that task he reached for the walkway door to close it with his left hand. His hand was not clear of the door when closing it causing him to pinch his left hand between the door and the door frame. He then jerked his hand back to get it out causing cuts and scrapes to his hand.

While tightening bolts on the autoclave the torque wrench slipped and her thumb slammed against the pressure relief valve.

An employee was using Bobcat to haul the plasma cutter to #1 finish mill. EE was having trouble getting it pulled out of the bucket. Another EE rolled the bucket down to make it easier to pull out, not realizing the EE's foot was under the front edge of the bucket. EE suffered a brusie/contusion to the top of his right foot when it was pinched between the bucket and the floor.

Two employees were replacing a piece of piping on Tank # 2. One of the employees was tightening a hose clamp with a flat blade screwdriver. The screwdriver slipped off the clamp and punctured the palm of his left hand. The punture was deep enough to require two stiches at Logansport Memorial Hosptial ER.

Employee was tightening bolts on coal mill roll and wrench slipped off of bolt, causing finger to get hit between spring and wrench.

Employee was spraying contact cleaner on a pulley for the roll up gate on the man lift elevator to clean it. The pulley was gummied up with sticky residue. While spraying, the contact cleaner reflected back into the employee's eye. The employee was wearing his PPE.

Employee was using jack hammer in cooler to bust up clinker. Clinker got wedged in the clinker breaker hammer. Dust was stirred up from air going thru exhauster jack hammer. Relief fan was running pulling dust upward and out relief stack causing dust to get in employees eyes.

The crank arm on # 1 clinker cooler had fallen off. Shift repairmen locked out cooler drive, removed guard & attempted to put cooler arm back on. Arm got into a bind and would not go on all the way. They then removed their locks, stood back away from the cooler arm assembly & tried to bump the jog button to move the crank arm. The steel bushing broke and struck employee.

Employees were taking barrels of clean-up material up and down on air chain from basement of the clinker cooler. The hole where the barrels were going through one of the hooked sides came loose causing the barrel to become dislodged from lowering hooks. The barrel fell and employee was under the barrel and was not quite out of the way when the empty barrel came falling down.

Another employee was lowering the door counter weight into the weight guide and injured was guiding the cable over the guide pulley. The employee lowering the weight dropped it forcing injured's finger to be drawn into and around the cable guidle pulley.

Employee was pulling a 4"" corrugated plastic pipe with a blunt end steel pipe attached to the end across a catwalk with floor grating. The pipe was caught on the floor grating, and when he pulled it towards himself it jumped, and he was expecting more resistance, but the pipe came back hard, and hit himself with the pipe in the goin (scrotum) area.

EE and 2 coworkers were trying to move a portable welder by hand to hook it up to forks on fork truck. Coworkers were pushing the welder forward and EE was lifting the welder tongue. The welder tongue was near the forks and EE's hand got pinched between the fork and the welder hitch.

Hauling dust and tanks went empty and slurry splashed over glasses and got into eye when looking to see how full the truck was.

EE was cutting on crosshead beams. Slag ran down welding jacket and burned his arm in 4 spots.

Employee was cleaning up in mill building stairway and shoveling material into new wheel barrow. He went to pick up on the handles of the wheel barrow and felt sharp pain in the lower back.

Adjusting the holding brakes out because they got hot and EE was standing in an awkward position and slipped when he went to catch himself he felt a pop in his back.

While lifting bag of old premixed cement for disposal she turned to take a step felt a hot jab in lower back with pain radiating down left leg.

Employee was stepping across the west rail dike wall when he tripped and fell to the ground.

EE and co-worker were stretching extension cords so they could plug in block heaters for yard truck and track mobile at Waste Fuels inside of unloading docks. EE stepped inside of rectangular box made of angle iron and expanded metal. When he was stepping out of the box his right foot got entangled and he tripped and fell on his right knee and left rib cage.

EE was walking across the parking lot and slipped on some gravel rock and fell on his hands.

Employee was trying to purge the diesel lance on #2 kiln with nitrogen. As the diesel fuel was sprayed into the kiln it vaporized and ignited causing a flash which blew open the hood doors burning the employee on the left arm and ear.

EE was sitting on a bucket adjusting the oiler on a compressor. He felt pain in his lower back when he stood and turned to walk away.

EE WAS TURNING #1 SEPARATOR AND WAS TURNING THE BLADES SO THEY COULD FIND THE TOLERANCES ON THE BIG GEAR. HE WAS USING HIS FEET WHILE SITTING ON THE OPENING OF THE BLADE AREA, WHEN THEY WERE READY O TURN THE BLADES AGAIN, HIS FOOT SLIPPED OFF THE BLADE AND WENT IN FRONT OF THE BLADE. FOOT AND ANKLE WERE PINCHED BETWEEN THE BLAD AND THE ENTRY OPENING.

EE HURT HIS BACK STRAIN WHILE HANDLING AND LIFTING STEEL FOR THE CRUSHER. HE LIFTED SOME PIECES THAT WERE TOO HEAVY FOR HIM.

EE HAD JUST STEPPED OUT OF HER CAR AND TURNING TO WALK INTO THE OFFICE BLDG WHEN SHE SLIPPED ON LOOSE STONES AND FELL. SHE BRUISED HER PALMS, SCRAPPED BOTH KNEES, RT WRIST, AND RT SHOULDER PAI N.

TWO EMPLOYEES WERE PULLING ON LANCE AND INJURED WAS PUSHING. HAND WAS CAUGHT FLANGE AT END OF LANCE AND RELIEF STACK.

EE WAS PULLING A STRAINER BASKET OUT OF THE FEED PUMP FILTER POT WHEN HIS RIGHT SHOULDER POPPED. HE FELT A SHARP PAIN.

EMPLOYEE WAS PACKING TYPE S ON PORTLAND WHEN TWO BAGS FELL OFF PACKING MACHINE ONTO THE CONVEYOR BELT BELOW. EMPLOYEE TRIED TO HOLD BACK ONE OF THE BAGS WITH HIS RIGHT FOOT TO PREVENT THE TWO BAGS FROM PLUGGING THE TRANSFER POINT. WHILE STRETCHING DOWN AND EXTENDING LEG TO HOLD BACK THE BAGS, HE FELT A POP IN LOWER BACK.

EE SLIPPED ON MUD ON THR FLOOR WHILE WORKING ON A ROLLER FRAME.

EE WAS IN THE BOTTOM SECTION OF #2 PRECIPITATOR FIELD WELDING IN A VERY DUSTY ENVIRONMENT. WHEN MOVING AROUND, DUST FELL FROM THE FRAMEWORK AND GOT BEHIND THE LEATHER HOOD INTO HIS LEFT EYE.

EE WAS REPLACING THE GRATING ON THE NORTHWEST TRAILER PAD LAUNDER. HE HAD ONE FOOT IN THE LAUNDER & THE ONE FOOT ON THE PAD. WHEN HE PULLED A GRATE INTO PLACE HE FELT A POP IN HIS LOWER BACK.

WORKING IN #3 COMPARTMENT OF #2 CLINKER COOLER. POKING CLINKER THROUGH THE TIPPING VALVES TO EMPTY COMPARTMENT WITH A ROD. HEIGHT OF COMPARTMENT AND LENGTH OF ROD BEING USED CAUSED THE EE TO S TRAIN HIS LOWER BACK WHILE COMPLETING HIS WORK.

WHILE TIGHTENING DOWN ON BOLT, WRENCH SLIPPED & EE PINCHED FINGER BETWEEN WRENCH & BOLT. INJURY REQUIRED CLEANING & SMALL SPLINT. RESTRICTED USE OF FINGER FOR SEVERAL DAYS.

EMPLOYEE WAS CLEANING OUT SILO #1 BIN VALUE THAT WAS FULL OF CHUNKS. ANOTHER EMPLOYEE WORKING WITH HIM CLOSED THE VALUE WITHOUT CHECKING FIRST & VALUE CLOSED ON HAND. EE WAS INSTALLING AN AIR CLEANER HOUSING ON A GENERATOR. EE STEPPED DOWN TO GET A TOOL AND LOST HIS BALANCE AND FELL. WHEN HE FELL HE HIT HIS HEAD ON THE WALL AND LANDED ON THE FLOOR. THE HEIG HT OF THE FALL WAS 19"". HE WAS WEARING A HARD HAT.

EE WAS WALKING ALONG #2 COAL MILL AND STEPPED ON UNEVEN CONCRETE CAUSING HER TO TWIST HER ANKLE.

WHILE CHECKING BEARING FOR VIBRATION LITTLE FINGER WAS CUT BY ROTATING HEAT FLINGER.

WHILE WORKING ON MILL EMPLOYEE GRABBED RETAINING RING TO ENTER MILL WHEN RING GAVE WAY CAUSING HIM TO FALL BACKWARDS. HE REACHED BACK TO GRAB THE SAFETY HANDRAIL BUT MISSED AND FELL APPROXIMAT ELY 15 FEET TO GROUND LANDING ON HIS TAILBONE. 911 WAS CALLED AND HE WAS TAKEN TO HOSPITAL. ON RESTRICTED DUTY.

CREW WAS INSTALLING BRICK IN KILN USING A PNEUMATIC RIG. BRICK LAYER WAS PLACING BRICK ON PLUNGER AND BRICK SLIPPED AND FELL APPROX FIVE FT) TO PLATFORM WHERE LABORER HAD HIS HAND ON TOP OF THE BRICK. HE WAS PREPARING TO HAND TO BRICK LAYER AND BRICK HIT THE TOP OF HIS HAND.

WHILE ATTEMPTING TO REPLACE THE TOP COVER TO THE CEMENT COOLER. THE COVER WAS CAUGHT BY THE MOVING SUPPORT ASSEMBLY FOR THE RIBBON SCREW. THE COVER TWISTED IN THE EMPLOYEE'S HAND CAUSING HIS H AND TO BE PINCHED AND CRUSHED BETWEEN THE COVER AND THE OUTER RIM OF THE ACCESS HOLE.

EE ENTERED THE 32 RAW MILL TO BURNOUT THE HATCH. THE MILL WAS HOT AND HUMID. THE EE WAS USING A TORCH WHICH CREATED ADDITIONAL HEAT. HE BEGAN FEELING OVERHEATED AND HAD DIFFICULTY BREATHING. H E EXITED THE MILL. HE PASSED OUT FOR 5-10 SECOND. EMT WAS CALLED AND HE WAS REMOVED TO THE LOCAL HOSPITAL.

EE SLIPPED WHILE EXITED THE #1 CLINKER COOLER & FELL ON HIS RIGHT SIDE. HE REPORTED ON MARCH 19 THAT HE HAD RE-INJURED HIS BACK FROM AN EARLIER INCIDENT REPORTED TO MSHA (DEC 21,2000).

EE WAS TRAVELING TO NEW ASSIGNMENT WHEN HE PASSED A FORKLIFT IN THE PACKAGE WAREHOUSE FORKLIFT OPERATOR FAILED TO COMPLETELY CHECK BEHIND HIM BEFORE BACKING UP & TURNING, EE WAS STRUCK BY FOR KLIFT BEHIND RIGHT FOOT

WHILE EXITING #1 CLINKER COOLER DURING REPAIRS. HANDRAIL WAS MISSING ON PLATFORM. EE FELL HITTING LOWER HANDRAIL WITH BODY. INJURY TO LEFT SIDE RIB CAGE.

EMPLOYEE WAS CLIMBING DOWN FINISH MILL #2 LADDERSTEPPING TO TO BOTTOM HE ROLLED HIS ANKLE & FELLTO THE FLOOR. HE HEARD HIS ANKLE POP. IT SWELLEDUP IMMEDIATELY.

EMPLOYEE WAS GRIDNING ON MILL SCREEN PLATES IN #2 FM. HIS GUARD CAUGHT ON THE SCREEN SLOT AND GRINDER STRUCK HIM IN THE FACE, LACERATING HIS RIGHT CHEEK AND NOSE AND CHIPPING SOME TEETH.

CLIMBING LADDER FOR ENTRY TO CRANE OPERATORS STATION & HIT LEFT KNEE ON RUNG, CAUSING A FALL AND AWKWARD LANDING TWISTING KNEE.

EE WAS WORKING AROUND #1 KILN PIER #1 CHECKING BEARINGS WHEN SOME DUST BLEW INTO HIS RIGHT EYE. HE FLUSHED IT OUT WITH WATER AT EYE WASH STATION, BUT IT STILL FELT LIKE SOMETHING WAS STILL IN IT. CHECKING WITH THE DR HE HAD A RUST RING DEVELOPED.
