While removing slide above screw conveyor material inside the hopper moved and dust came out of the gap left from the slide removal and got into employees eyes

While cleaning a chute with water the individual had wet high alkali material get behind EE's safety glasses and into EE's eyes causing minor chemical burns

Employee was climbing into the cab of the bulldozer EE operates when EE's foot slipped off the step and EE fell injuring EE's ankle in the process.

Employee was grinding a beam when it grabbed on the beam and the hand holding the handle slipped off. The grinder slipped back into their wrist as they came off the trigger with their other hand. The disc caught their left forearm and required 4 stitches. EE was wearing proper PPE and all guards were in place.

Employee was cutting a plastic bucket, the knife slipped off the bucket and cut their hand

Employee was cleaning around pulper pit. The steel floor covering the pit Flexed and slipped off of the support causing the employee to fall. EE caught self with both arms. Resulting in shoulder injury. Procedure took place on 11/07/2024 resulting in restricted duty but the restricted duty began on 11/11/2024

Individual was using a rod to change valve position. The valve was difficult to move so they jerked on the rod which rotated and struck them in the face above the left eyebrow causing a laceration.

While emptying the fine coal bin some hot material in the bottom of the fine coal bin became exposed and ignited float coal dust within the bin.

Employee was striking a stuck carry roll on a conveyor. The roll started to spin and pulled employee's hammer in to it. This drew employee's hand across a sharp piece of metal on the framework

While beating on a chute the employees hammer contacted an airline slightly above and behind EE causing it to be redirected which caused it to hit the left thumb of EE's off hand. This created a laceration which required a few stitches to close up.

After prolonged use of a sledge hammer the EE started having pain in arm. Due to inflammation from swinging the sledge hammer EE experienced a pinched nerve in right shoulder.

We believe that due to the age of the motor control center and the brittleness of the wiring the work they were doing may have caused a wire to short out and cause the fire. The motor control center has been de-energized and a new one has been ordered for installation.

Injured employee was accessing tanker, when gangway shifted employee stepped through the gap between the gangway and the tanker twisting a knee.

Accident type, without injuries

Smoke was seen coming from the coal tunnel. Coal was removed from above the feeder below and coal in the feeder was run out onto the ground. There were no flames, only smoke and smoldering coal were observed. Called EE once situation was resolved to ask if it was reportable or not since there was no flame to extinguish and EE said to go ahead and report it.

Employee was using a hydraulic puller to remove a gear from a shaft. EE had a hand on the body of the puller. The puller came loose from the gear and EE's finger was momentarily caught between the arm and the body of the puller causing a fracture to the little finger.

While opening a door a gust of wind swept up debris and something got into right eye. Flushed eye and went back to work. Continued to have redness but no discomfort. Sent to company eye doctor on June 22 for examination where they realized a piece of debris on the eye that needed removal.

Miner was trying to get silo to feed and cement eventually began to flow at a fast rate and got in the right eye of the employee.

Individual was climbing a fixed ladder and lost EE's grip or footing and fell approximately 6 feet to the ground below fracturing the humerus of EE's right arm and dislocating EE's right shoulder. Employee is on restricted duty.

Employee stepped down from dozer and foot slipped on unconsolidated material causing employee to injure knee.

Employee was placing a barricade around some I beams that had been removed when EE walked into the end of one of them that had a sharp edge and cut leg just above EE's boot.

Employee fell through sheet metal roof and was suspended above an operational kiln. Injuries were due to the extreme heat, not from the fall. Burns were primarily to the buttocks and backs of legs.

Employee was removing welding lead from a container. EE was pulling it out from storage and setting on a pallet for relocation to the work area. Did not feel well, when reporting to work the next day said had soreness in groin. Took EE to clinic for evaluation and diagnosed as groin strain

The employee was on the third rung of a 6'-0"" ladder using a harness with a retractable lanyard. The employee was tied off and using a plasma cutter to cut out grizzley bars. While cutting, slag went down the employee's shirt, and EE fell from the ladder. The platform the ladder was on had refractory on it, causing the employee to strain/sprain ankle when EE fell.

Individual was climbing ladder and lost EE's grip. EE jumped back landing on EE's feet. In doing so EE sprained EE's left knee and right ankle.

Employee was operating an off road forklift transporting some scrap metal, when EE hit a bump some debris on the load became airborne and entered the cab and got behind in between EE's face and safety glasses and into EE's eye.

Employee had just finished cleaning the riser duct and lifted face shield and dust fell from the frame of it behind employee's safety glasses into eye.

Employee injured right shoulder on 3-15-2020. Employee was terminated from employment on 5-9-2020 for reasons unrelated to the injury. During the course of EE's employment, the injury did not meet MSHA's part 50 reporting requirements. However, after termination the employee will be receiving surgery on the right shoulder on 7-10-2020.

MINER WAS CLEANING BUILD UP OF FROZEN MATERIAL IN COAL HOPPER. EE WAS ABLE TO LOOSEN MATERIAL, BUT NOT TURN MATERIAL. EE REACHED INTO HOPPER AND TURNED MATERIAL. MATERIAL FELL AND STRUCK MINER IN R HAND BREAKING TWO FINGERS.

MCC went to ground. Power was turned off and breaker was repaired. When MCC was renergized, the breaker blew and caused a flash burn to hand. New breaker was believed to be faulty.

Removing air pad from bottom of cement silo. Silo had been emptied leaving a small amount of cement on the pad. While the pad was being removed a small amount fell onto his face irritating his eyes.

EE was ascending side slope of a limestone stockpile to get elevation of top of slope. He lost footing of lt foot, started to slide. Trying to catch himself his rt leg twisted & collapsed under his own weight. The rt femur was broken at a location of a previous break. According to EE, the bone was previously broken 4 times due to a degenerative bone disease.

Employee was carring powershot tubes to and from 308 bin. He was utilizing the stairs because the elevator was down for repairs. Weather conditons were not a factor and all ppe was worn. The employee complained of a strain/pull in his lower back.

Employee was retrieving a hot meal sample, when an air cannon discharged 30ft below the sample line. This forced some hot air and dust back into the line and out of the opening. The employee was wearing all ppe but the dust still managed to travel under his faceshield, which resulted in his eyes being scratched.

No injuries, dust collector cartridge replaced. This was an unplanned explosion in the coal mill. Mill was designed to absorb and release energy and respond in the correct manner. No injuries & little damage

When lifting mill door into place the anchor point broke free causing a whiplash of lifting sling while the sling was in employee's grasp causing a deep laceration to his left hand.

Employee had 2nd and some 3rd degree burns to his underarm/back area. We believe that he backed up to the muffle oven to warm up when he caught his shirt on fire after being outside getting a slurry sample.

EE was lifting a 90lb steel brace for a conveyor and the brace slipped out of his hands causing his back to jerk, resulting in a sprain of the lower back.

EE's shift was having problems with #1 finish mill. EE went out to see what was wrong with the finish mill. He found that the tipping valve was stuck open coming from dust collector. When he pushed the valve arm down, the counter weight slid down the handle and hit his right hand cutting it.

Employee was helping a co-worker repair a broken port hole door on the 3rd floor of the tower riser when some hot dust flowed through the opening and got in his eyes. He had PPE but the dust went underneath and got in his eyes.
