EMPLOYEE WAS WORKING ON THE 4TH FLOOR OF SCREEN TOWER. A GUARD RAIL WAS REMOVED TO LOWER A PIECEOF SCRAP TO THE GROUND. AFTER SCRAP WAS LOWERED,EMPLOYEE WAS BELIEVED TO BE THROWING ANOTHER PIE CE TO THE GROUND WHEN HE FELL FROM THE OPENING TO THE GROUND 56 FEET BELOW. FALL PROTECTION WAS PROVIDED BUT NOT USED.

Employee was removing a screen from a screen deck and contused his hand. Incident was originally not considered reportable. Employee was referred to a specialist after pain. Found to be an aggravation of arthritis and was placed on confirmed lifting restrictions on 8/15/13. Date incident became reportable.

Employee was climbing the ladder to access his haul truck when he bumped his elbow on the access platform.Incident did not meet reportability requirements until 8/19/09 when employee was issued work restrictions following a cortisone injection.

Fall down stairs

Employee was walking down the stairs at the scale house when she slipped and fell. She initially had a bruise which turned into an infection that required treatment on the 6th of March.

Pedestrian had an automobile accident on a street adjacent to our property. He was reported missing on the accident date and was discovered down an embankment near an access road on the outskirts of our quarry. He appearantly died of overexposure. The victim was a tresspasser and this event is non-chargeable as a mine related incident.

EMPLOYEE JUMPED FROM 65 TON TRUCK WHEN THE ENGINE CAUGHT FIRE. FIRE CAUSED BY POSSIBLE FAILURE OF HYDRAULIC BRAKE LINE.

EMPLOYEE WAS GATHERING SMAPLES WHEN THE BUCKET FELL OUT OF HIS HAND. IN HIS ATTEMPT TO CATCH THE BUCKET HE SNAGGED HIS HAND ON A JAGGED PIECE OF METAL CAUSING LACERATIONS TO HIS INDEX FINGER OF HIS RIGHT HAND. SUTURES WERE REQUIRED.
