Employee was fatally injured when he was struck by a battery powered coal hauler in Unit 2.It is believed that the employee walked through the last open crosscut to #4 entry and sat down near the inby corner of #4 entry to eat his lunch. A coal hauler rounded the corner and pinned the employee to the rib.

IE was packing 60"" top belt rollers to supply trailer when IE tripped over a stick of 6"" waterline. IE fell into the supply trailer striking IE's left knee and thigh. IE received a 1"" laceration on IE's inner left thigh, which required 6 sutures to close the laceration.

Employee was hanging curtain when a rock fell out between rows of bolts and struck employee in the head and shin.
Employee was throwing cable over and when the employee raised back to the standing position hit head on a brow.
EE complained of pain on 11/6/19. Evaluated and diagnosed with partial AC separation on 11/14/19 due to repetitive trauma. EE returned to work on 11/11/2019 with no restrictions and was to work as tolerated. 11/14/19 EE given temporary restrictions and taken off work. Employee was placing hog panel on the ATRS when EE over extended EE's back muscles
Injured employee was operating a continuous miner when a 15""x10""x4"" piece of cap rock fell out striking the right foot just behind the steel toe causing a fracture to the pinky toe.
On 11/8/2019 an accident report was filled out stating the IE shoulder has been hurting for the past month. After work comp/doctor visits. The IE believes the incident resulted from hanging a cable on 10/10/2019. It wasn't until 12/09/2019 the worker was taken off work pending an mri.
EE was hanging cable in entry 5 @ 27+50 EE felt burning and a small pop in right shoulder.
Employee was in the process of moving the continuous miner cable off of a getman and lost balance causing them to fall into the canopy of the getman.
Employee was reclaiming belt structure and was stacking belt rails on the back of a mini track. While attempting to place a rail the previously stacked rails moved and pinched the left pinky finger
IE was sealing a return stopping. IE had a bucket of sealant slip out of IE's hand and land on the mine floor. When the bucket hit the floor, a watery sealant mixture went into IE's eye.
While driving a ram car on unit 1 the employee bumped their head against the canopy of a ram car. The IE stated that they drove through a hole which caused them to suddenly come up and in contact with the canopy.
IE was running a continuous miner. IE was cutting extra height for a future feeder setup. IE noticed blood on IE's pant leg and realized IE had been cut.
Employee was in the process of installing the extension drill steel needed for drilling an 8ft bolt hole. The extension was coupled with the starter drill steel and the drill head was brought up to insert the extension into the drill head. The steel did not align correctly and kicked out into the injured employees thumb
A rock fell from the roof, striking the employee on the middle and upper back causing them to be pushed into the roof bolter canopy. While waiting on the other operator the injured employee inserted their drill steel into the drill head to check for suction. As soon as the steel was removed and was being placed onto the tray a rock fell striking the employee.
Employee was in the process of installing top belt framing above their head. The piece of framing got caught on a piece of fire deluge pipe while trying to get it into position. When freeing it up the piece of framing slipped off of the belt rail and suddenly fell, which jerked the employee's arm downward causing an injury to the right shoulder
While tramming battery scoop pulling high voltage tub, IE ran over solvent can that exploded and sprayed IE in face and eyes
Employee was helping hang up miner cable and felt a pain in the left shoulder
EE is a Roof Bolter who has worked for 2 years installing roof bolts. The position requires repeated overhead lifting and motion. EE complained of pain in L shoulder in Feb of 2019. EE was evaluated by an Orthopedic Specialist and diagnosed with Distal Clavicle Osteolysis on April 30, 2019. EE RTW under a restriction of 10lb lifting at waist and no overhead 5/1/19.
Employee was building a stopping after a belt move. A block slipped and caught EE's finger between block and rib.(Employee had gloves on.)
Employee was installing bendable 8ft tensioned rebar bolts. Employee felt a sudden pain in the Left arm when the bolt spun inside the hole when attempting to straighten it.
The injured employee turned on the cable reel on a roof bolter, which caused the cable to whip up into the employee's face.
IE was dragging 2 buckets through curtain when ee twisted to right and felt sharp pain in center upper abdomen.
IE was pushing an 8' bolt in the top in low coal and felt a popping and burning in right shoulder.
Injured employee sustained a scratched cornea and was prescribed antibiotic eye drops as a result of being hit in the face with a blast of water and debris. The employees were repairing a water leak on a 2"" Victaulic coupler on a battery barn deluge system. The system pressure was isolated but was not properly bled off.
EE strained the left shoulder when installing an 8ft notched tensioned rebar roof bolt. Strain occurred when attempting to straighten roof bolt and insert it into drill head. EE did not lose days or work restricted due to this strain. Lost time began when surgery was performed 9/20/19 for acromioclavicular joint degenerative disease, inflammation and sub acromial bursitis.

IE was roof bolting when a piece of rock fell loose and struck IE in the mouth causing a front tooth to break off just below the gum line.
IE was in the process of manually resetting a breaker on a battery scoop. A protective cover had to be removed to gain access to the breaker. After it was reset the IE was re-installing the cover, when it slipped and smashed the end of the middle finger
IE was loading bolter with 3' glue when IE felt right shoulder pop.
IE stepped in a rut while roof bolting. IE felt pain/warmth after incident. IE was treated at ER and released. IE was treated at OISI on 9/25/18 and released. An MRI was performed on 10/12/18 and revealed a tear of medial meniscus. IE followed up OISO on 10/22/18 at which time a non-weight bearing restriction was applied becoming lost time.
As an underground operator, I.E. performed various repetitive task: hanging carious cables, handling roof bolting supplies, building stoppings, shoveling belt, operating equipment, etc. all with vary levels of consistency. A note from Dr. indicates a lumbosacral Disc Degeneration
IE stated they went to help seal an intake brattice. IE was sealing near the top of the brattice and turned from getting sealant to place on the wall when a piece of tie wire hanging from a plate went in the side of safety glasses and poked eye.
While operating 401RB on Rt Side of Unit 2, Entry #9, Inby Spad Location 7+90 IE drilled approx 4' with starter steel then dropped boom to couple extension steel. After coupling steels IE raised boom to seat extension steel into the chuck. IE slipped on a rock and pushed joystick into Up position causing it to strike their hand
I.E. was operating 239CM on the Right Side of Unit #2. I.E. set the CM over to start on the 1st cut out of the Right Side. After doing so, I.E. knelt down in the corner, which had been cut when the slider was turned for Entry 8-7. A rock measuring approx. 6' x 4'4"" x 3"" fell from a height of 6.5', traveled approx. 39"", and struck I.E. in the Right Shoulder.
I.E. was building a crib on Unit #1 in Entry #12 at Spad Location 11+20. While lifting a 3' crib into place approximately 6' above the mine floor, I.E. felt a ""POP"" in the left shoulder. I.E. worked the remainder of the 6.25.2018 shift and the following shift, 6.26.2018. On 6.27.2018, I.E. was seen by a physician and diagnosed with a Torn Rotator Cuff
IE was lifting cable, and felt a sharp pain in the back. IE continued to work, but pain did not go away. IE was transported out of the mine and taken to local physician for further evaluation. IE received a Full Return to Work with No Restrictions and worked for 10 Days. IE received a 2nd opinion on 5/4/2018, and was taken off work.
After finishing the shift on 1/8/2018 a continuous miner operator said IE's back was sore. IE said there was not a specific incident but related the soreness to moving continuous miner cable throughout the shift. IE said the soreness escalated after the shift. IE has been diagnosed with a strained ligament in IE's lower back and began missing work.
2017 · 25 incidents
IE stated while squatting to look at a continuous miner a rock measuring 16.5"" X 5"" fell from the roof and struck IE on the hard hat. When this occurred IE lost balance, fell forward, and reached out to brace self. On 12/27/2017 IE was diagnosed with an elbow strain and given work restrictions.
IE was replacing a roof bolter hyd fitting near the base of the ATRS. IE completed the work, the control side operator approached, did not see the IE and thought maint work was completed. The operator started the machine and swung the boom in pinching IE between the boom and ATRS. IE was treated at the local ER for a broken clavicle and released.
A mechanic was working on a ram car tram motor. IE was having trouble positioning the motor so they used a drill steel to pry with. While prying, the drill steel slipped and they struck themselves in the mouth with it causing a laceration that required 6 sutures.
Weathered roof conditions and potential slip in the roof fall area.
IE was changing a bit on new drill steel at the start of shift. As IE was standing next to the bolter a rock fell from between roof bolts measuring approximately 4'X3'X4' - striking a glancing blow to left shoulder. IE was taken to the local ER for examination and was taken off work pending a follow up visit on 10/16/2017.
IE unsuccessfully tried pulling a pin from a belt splice. EE used a claw hammer to grab the pin then struck the claw hammer with another hammer. As hammer struck hammer a metal shard dislodged, became a projectile and struck the IE's left forearm. The shard was removed at the local ER, two sutures were required to close the opening caused by removing the shard.
IE was carrying an open bucket of sealant preparing to plaster a stopping. When ee set the bucket down liquid that was pooled on top of the sealant splashed up and contacted the employee's left eye. The IE was wearing prescription safety glasses with side shields. Treatment included prescription eye drops.
IE was installing roof bolts as primary support in #6 entry. As IE was preparing to place screen wire on the ATRS a rock fell from the roof striking the screen wire on the free end. The screen wire pulled IE to the left and forward then a second rock fell striking IE at waist level. The rock measured approx. 5'x3'x1-6'.
A roof fall was discovered at 8:34am between crosscuts 16-17 on the 5th Main North. It appears two bolts failed allowing wire mesh and subsequent supported material to impede travel.
I.E. was changing bits on 239CM on the Right Side of Unit #2. I.E. and another employee went to spin the head of the Continuous Miner. While applying downward pressure to spin the head, I.E. felt a sharp pain across the back of the shoulders.
Employee was moving solid 6"" block to build a wall. Employee received a block from another worker and turned with the load bent over and felt a sharp pain in lower back.
Employee was dumping concrete bags onto grate on a gunite pump when another employee tossed a concrete bag on employee's hand smashing it between the grate and the bag. Employee received 6 stitches.
EE was working to unplug the 1st North transfer chute. EE used a cutting torch and trimmed the chute to allow better access to material blocking the chute. A piece of slag dropped between glove and cuff and burned the top of hand. EE continued to work. On 8/23/2017 EE was put on restrictive duty then released to full duty on 8/25/2017.
IE was installing a 10ft cable bolt. It is believed the bolt was damaged during installation. IE said EE installed the bolt, dropped the pot and the bolt spun. A flying particle struck IE's safety glasses and contacted eye. E.R. exam showed a small cornea contusion - no foreign material found in the eye. Prescription ointment was issued as a precaution to prevent infection.
IE said while hanging miner cable with a co-worker, the tie wire broke so IE reached to catch the cable with left hand. IE continued to work but noticed that hand/arm felt different. Note: This employee had a previous injury to the left hand/arm (tear to distal bicep) that required surgery to repair. Surgical repair was performed again following this re-injury.
IE was adjusting the forks on a minitrac. IE lifted on the fork to adjust it. Lifting on the fork rotates the keeper stock located on top of the fork. As the IE lifted the fork they did not realize that their little finger was in a pinch point between the metal stock keeper and the frame. IE received 9 sutures.
IE was removing a cell from a coal hauler battery. The cell top became delaminated causing a gap between the cell top and cell casing. The cell slipped out of IE's hands landing vertically on the floor causing acid to exit the top of the cell. Acid splashed under IE's face shield, around the safety glasses contacting left eye. Prescription eye drops were issued/used 5/31/2017.
A Getman tractor operator pulling an empty trailer ran over undulations in the floor and struck head on a newly installed canopy. EE thought they were fine and continued but said the pain worsened and neck stiffened so they notified the supervisor. Employee was taken for medical examination and released to work full duty but on 5/9/2017 began working under restrictions.
Employee said while preparing to move the roof bolter employee picked up the cable and shoulder popped. Employee continued to work - an MRI was done on 7/17/2017. Initial diagnosis included degenerative issues, tendonitis. Later diagnosis was a rotator cuff tear related to the incident. Employee continued to work until surgery on 11/7/2017.
Borehole with a pvc pipe in it allowed surface water from a field to enter mine at the 2nd East Mains Parallel - entry 11 - approx 17+65 Sta. Water gathered at the end of the 2nd East Main Parallel. It appears that a farmer knocked the top off the pipe. The borehole appears to reach the 6 seam only. Wildcat Hills did not drill this borehole. In response to citation # 9105311
A temp employee was hauling gob with a diesel scoop. EE reported to EE's foreman something hit EE in the back of the head while driving the scoop back to a turn around. The foreman noticed signs of concussion and initiated response. The area involved has been checked twice by an investigation team. It is believed the TE struck EE's head on the top of the seat or on the mine roof.
3-13-2017 after shift- temp emp told a supervisor the temp emp was struck by a rock shoveling beltline. TE said was OK but was reporting per procedure. Supervisor 3 times asked if TE was OK or needed examined etc. TE said was OK. Alleged location was investigated - no rock found. TE began missing work. Mine was informed 6-2-2017 by temp agency work comp was accepting claim.
A roof bolter was installing 8' bolts. Due to bolt length being greater than seam height these bolts must be bent prior to installation then straightened once inserted into the hole. While straightening the bolt, hand slipped off. The bolt sprung back and struck I.E. over the right eye causing a laceration of approximately 2"" that required sutures.
I.E. ducked due to height of the roof as EE walked along the beltline rib. A corner of screen wire that had gapped down from the roof caught EE's ear causing 2 lacerations. (Height from floor to roof - 66"", from floor to screen wire - 59.5"", from screen wire to rib - 21""). Employee was taken to local physician where EE received 10 sutures.
Employee said on 1/16/2017 EE fell as EE stepped off the concrete pad near the take up. At the time of the incident EE said didn't think much of it and continued to work. On 1/17/2017 EE noticed pain. Employee began missing work on 1/23/2017.

Employee reported that while boarding the bus at the end of the shift. Employee's lunch box handle came loose and struck self in the mouth resulting in a fractured front tooth. (Medical treatment was on 12/21/2016) Employee has continued to work.
Temporary employee was hanging curtain and a rock fell from between bolts striking the employee on the top of the foot. The rock was approx. 5'X 2' and about 1-4"" thick. This individual was diagnosed with a broken metatarsal bone and started missing work on 11-11-2016
approximately 2' of deteriorated black shale fell out in the area causing the bolts installed to fail, However the main roof was intact and sound. This area was mined in April, 2006.
On 9/13/2016 a supply man driving a getman tractor backed into a cross cut to allow another vehicle to pass. When ee did so a rock fell from the rib striking neck / upper back. IE was taken for a medical examination and released to work full duty. EE continued to work until ee had surgery on 4/4/2017
IE said they repositioned a ram car at quitting time to make room for another piece of equipment. As they dismounted the ram car they struck their right shin on the cab causing a laceration that required 5 sutures. (no lost work days)
Employee was moving a high voltage PLM plug on 8/23/2016. EE placed the cable over shoulder to better pull/drag the cable. While pulling the cable EE ran out of slack - this caused the plug to jerk EE back and EE began experiencing shoulder pain. EE performed full work duties and missed no work until Surgery was performed.
IE was installing 48 inch conveyor belt framing. IE stated EE momentarily lost EE's balance while moving top roller assembly and pinched EE's left ring finger. IE was taken to local physician and received 5 sutures.
Employee told supervisor that they think they strained their back loading glue onto the scoop. Employee was examined/released and continued to work with no restrictions until they had surgery. IME states: I suspect lifting glue sticks caused the disc hernations seen on the MRI although it is possible they aggravated a pre-existing stenosis.
Mantrip 100004 did not make a turn exiting the mine and struck the rib. A passenger said they experienced back pain. The employee was examined at the local ER and taken off work.
Employee said while examining on a permissible ride in the 3A panel, they ran over a rock in the path of travel. When this occurred the steering wheel turned unexpectedly. The employee stated they did not let go of the steering wheel and began having shoulder pain. It was determine the employee had a torn rotator cuff and had surgery on 9/20/2016.
A roof bolter walked out-by to an intersection to get supplies. A CM operator was changing bits nearby. The CM operator stuck the bit with a hammer causing a metal shard to project striking the front chest/shoulder area of the bolter - approx. 5ft. away. Dr. at local ER determined the shard would not be removed. On 9/9/2016 the shard was removed.
3/23/16 IE said ee tripped over a CM cable and twisted knee. IE finished the shift, was examined the next day and returned to full duty. Follow up MRI was done 6/2/16. IE was returned to full duty scheduled for orthoscopic surgery 6/21/16 for loose body removal. Post Op. notes revealed chondromalacia deterioration grade 2&3 but also an acute traumatic meniscus tear.
A mechanic was driving a maintenance vehicle on a travelroad. Evidence shows a rock fell and broke after striking the canopy in front of the driver. It then continued through the windshield opening striking the employee on the right side of EE's face. Employee began missing work on 3/23/2016.
Employee stated that they injured left shoulder, on 3/4/2016, getting out of the scoop. Employee has some notable degenerative changes. (Full narrative and some medical documents are attached to the file.)
The IE was in #10 entry to wash the rock dust out of the bed of a coal hauler. EE squatted down to pick up a water hose. As EE stood up EE turned to their left twisting and pushing off the right knee. During this motion EE felt something pop in their right knee. This incident resulted in a torn meniscus. The incident occurred on 01/12/2016. IE started missing work on 01/20/2016.
Employee said EE was sitting on the battery end of a scoop. While dismounting EE said their foot slipped off the steering jack. EE said they fell and caught themselves with their left arm injuring EE's left shoulder. Employee began missing work on 3/21/2016.
2015 · 18 incidents
EE walked into a piece of wire mesh, caused him to fall backwards. Rt knee began to hurt. Eval. detected torn meniscus. Opinion of surgeon who repaired knee was that tear was more likely to have happened because of the extensive pre-existing degeneration. Degeneration process from working 37 years has weakened meniscus, making it more susceptible to injury. Dr. note is included.
Employee was grabbing a roof bolt plate off his roof bolter tray when a rock fell from the roof and struck him on the left hand fracturing his ringer finger.
EE was at the slope head where he and a contractor service were changing out liner plates in a chute. The EE needed a specific liner plate that was on the bottom with four of them on top of it. As the EE was removing the other liner plates he felt a pain in his left shoulder. The incident occurred on 10/10/2015. EE started missing work on 01/28/2016.
The IE struck his head (hard hat) against an 8' roof bolt end sticking out of the roof in a cross cut.
Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low-dust area on the mine.
EE and another mechanic were working on CM233. The EE was in the process of installing the cover back on the demister, as he was performing this task the other mechanic was removing a rub-rail to check permissibility on a panel. As he removed the rub-rail it fell striking the EE on the left ankle.
As the IE stepped down out of the feeder with his left leg, he also struck the back of his hard hat on a chain hanger. The combination of these two incidents caused the IE to lose his balance, causing him to strike his right knee on the hopper end of the feeder.
The EE was lifting a cover, when one end of the cover slid forward and the other end fell backwards into a motor compartment twisting the IE right arm clockwise injuring his shoulder.
The IE was traveling the 6th North travelroad when a rock fell from the roof measuring (3'L x 2.5'W x 2.5'T) entering the cab of his Tomcar, striking him on the right hand, chin and upper chest. The IE was transported to hospital where he was treated for his injuries and was diagnosed with a break in the little finger at the last digit
IE was unhooking the tow bar attached to a water trailer from a Getman tractor (ST101) in the pit. As the IE lifted the tow bar up off the pintle hitch on the back of Getman tractor, the water trailer rolled forward and held the IE between the back end of Getman tractor and the water trailer.
A roof fall occurred in the 1st North Sub-Mains in entry #5 at 7+40. It is 25' long and 16' wide.
A roof fall occurred in the 4B Belt line at 27+15 ft. station, entry #6. The roof fall fell on top of the 4B forty eight inch belt. The fall is estimated to be 12' long x 10' wide x 6 - 8' high. The area had been mined approximately 6 months ago and was a hazard zone.
The injured employee was loading coal with a continuous miner at the face entry. He had loaded two cars out of the left side of the entry on his box cut. When he was struck in the head by a projectile, knocking him unconscious.
On 5/14/15 EE reported that on the previous day 5/13/15 that he experienced discomfort in his elbow while shoveling. EE had surgery on 3/3/16.
The IE stepped over a 3/4"" hose and it caught his left foot causing him to trip and twisting his left knee
On 5/4/2015 employee reported to his supervisor that he bumped his elbow. On 7/28/2015 employee was diagnosed with Tennis Elbow (lateral epicondylitis). The employee has 8 years of repetitive motion of the elbow performing activities such as swinging a hammer, shoveling, pushing and pulling levers, pulling cables, etc. Employee was given work restriction on 7/28/15.
The I.E. was exiting his ramcar using the grab handles on the canopy. As he was climbing out, his right hand slipped off a handle causing all of his weight to be on his left arm injuring his bicep. The I.E. had an MRI on 5/13/2015 and was taken off of work for a torn bicep.
The IE was sealing a stopping. While picking up a bucket of sealant he dropped the bucket and the liquid splashed in his eyes. He was given prescription eye drops by the physician.

The IE was setting the continuous miner to the other side of a cut, when he stepped in a rut twisting his left knee.
The IE was putting oil into the gathering head of a miner, when a rock fell from between the bolts forcing his head into the runyon can he was using. The IE received a laceration to his upper lip. He received sutures in his lip.
A wheel unit and tire broke off of a coal hauler, causing the operator to be bounced into the canopy of the hauler injuring his back.
A rock fell from between the bolts striking the operator in the head and back pushing his face onto the bolter tray, cutting his nose. He received sutures for the laceration.
A repairman was driving a golf cart to retrieve his tools at the start of shift and caught his left foot on some curtain that was lying on the bottoms causing his leg to twist fracturing his tibia.
The I.E. caught his leg between a top belt roller frame and a pull bar while lapping belt.
A rock fell out striking the IE on the left shoulder while servicing equipment.
The IE was opening a set of equipment doors and felt a pain in his left bicep and forearm.He worked until 1-5-15 and was taken off work for surgery.
The IE was pulling miner cable slack with one arm, and felt pain in his right shoulder. He continued to work until 9/29/14 and was taken off work by his doctor.
Employee was changing tire on a battery scoop. Scoop dropped four inches pinching employees hand between top of tire and the fender on the scoop.
While unloading bridge boards out of a sled, the IE felt a pop in his left knee.

Employee was walking in crosscut between #2 & #3 entry @ 24+75 and a rock fell from between bolts and struck on top of head and knocked him to the ground. He was x/rayed and has two minor compression fractures.
The employee was roofbolting, when a piece of rock between the rib and screen wire he was installing fell striking him on the right shoulder.
Employee was changing out a turn table gear case on726 Getman grader. The old gear case was removed and as he was putting in the new one it shifted over and cut his small finger on left hand requiring 4 sutures.

Employee was installing a 8' bolt. As he was adding a 4' extension steel on the initial steel, he dropped the pod and it came down on the steel he had resting on the boot of his right foot. This caused the steel to come thru the top of his boot, smashing the middle toe of his right foot. Toe was amputated on 10/22/13.
Had been unloading arches. When his leg was smashed between 2 I-beams, when they had been caught by a scoop pulling away.
IE stated he was tramming coal hauler from the face back to the feeder and he ran over a crib tie and bounced up into the canopy injuring his neck. Due to several doctors and tests not finding anything wrong, we have waited until a IME was performed. The doctor from the IME verified his problems could have come from the accident.
Employee was setting a jack pipe and when he let go to the mud seal for the top of it, the mud seal came down and smashed his small finger right hand between the seal and screw jack handle.
Was building brattice wall Lifting a concrete block above his head and it dropped. Smashing his right index finger in between the concrete block and wall.
Got off mini-trac to center arch by hand. When lifting and pushing arch, strained lower back.
Employee was making a splice on miner cable and said he heard a pop in his right wrist. He worked until thru 6/28/13 and was taken off work by a orthopedic surgeon waiting on surgery. Due to a compensation investigation this has just became a lost time injury.
Employee was tapping drive gear into a tram shaft with a crib tie and felt pain in his right arm and shoulded. Had MRI and was diagnosed with a torn rotor cuff and torn bicep muscle.
EE was building a stopping for return air. He lifted a block and felt pain in his lower back. He continued to work and returned to work the next day. During the latter shift he felt much pain. He went to a doctor the following day.
The injured was cleaning a rock fall from the back of a roof bolting machine. Another piece of rock fell during the cleaning activity. This rock pinned his hand on top of the rock bolting machines.

I.E.stated as he was putting a roof bolt in the top he hit his rotation while his hand was on the bolt and it jammed his ring finger on his left hand.
Walking up to mantrip to hook up tow rope. Not knowing the ram car was in reverse coming toward him. He was pinned in between the ram car and mantrip.
Injured employee walked up behind a man using a pry bar. The employee with the pry bar turned the bar and when he did he spun it around and hit the injured employee in the mouth. Required 5 sutures to close the wound.
An unintentional fall occurred at the #82 x/cut, on the 2nd west travelway. The fall was 20' X 20' X 10'.
Employee was starting to seal a stopping. He was kneeling down and starting to apply plaster, when a rock measuring 20"" X 26"" X 5"" fell about seven feet. The rock struck employee on the head. The roof control and spacing was good.
An unintentional fall occurred at #20 crosscut on the Main North entries @ 14+50' between #5 & #6 entry. the fall was Approximately, 18' wide by 20' long by 10' high.
Employee was hitting with a hammer and a piece of the hammer came off and went into his arm. Surgeon had to removed the piece of the hammer. He received 6 sutures.

The injured EE was walking between entry # 1 & entry #2. He was going to the roof bolting machine entry. He stumbled, twisted an ankle and fell. He felt something pop in his back when he hit the ground. He was transported out of the mine at 8:40 p.m. He received further medical attention thereafter. He remains off work.
Employee was handling cable and strained his right shoulder. Worked for the next 8 days and went off due to pain in arm. Compensation denied his claim and after Multiple MRI's and further diagnosis the injury was related back to the injury on 10/29/11. This happened on 3/13/12.
The injured was setting bits on a continous miner. A rock fell from between the bolts. The rock struck her on the head. She complained of nausea and dizziness. She was removed from the mine at 1:30am and was transported to medical treatment.
The I.E.stated that while hand dusting the roof, some of the dust blew back into his eye.
IE was sent outby the section to get hog panels for the roof bolters on the section. He used a wooden handle brass handle to secure the strap attached to the coal hauler that he was driving. When pulled the wooden handle snapped and a piece of the wooden handle broke, hitting the IE in the lower lip causing a small puncture which required 2 stitches.
An unintentional fall occurred in the 7+00' x/cut between the #5 & #6 entry's on the 2nd west submain. The fall was 35'L X 19'W X 8' h.
An unintentional fall occurred on the 2nd west travelway at the #38 intersection. The fall was due to deterioration of the roof above the bolt anchorage. Fall was 45' long and 18' long and 7' high
Injured employee was pushing a dead coal hauler out of the way with his ramcar and the hitch on his went into the bucket of the one he was pushing. This car went over a small hill in travelway and raised his off the ground. When it came loose and his coal hauler hit the ground it threw him up into the canopy of his ramcar, causing multiple injuries.
The injured was building a stoppage utilizing 6"" solid blocks between the #5 & #6 entries. A coal haulage operator with a load drove through the curtain & struck the construction. The resultant collision knocked the blocks onto the injured. He suffered a severely lacerated right calf, a broken fibula & a broken patella. The injured was air & land evacuated to the U of L hospital.
An unintentional fall occurred at the 7+00' crosscut in the 2nd west between #1 and #2 entrys. The fall was14' wide by 10' long by 7' high. Fall area was timbered off.
An unintentional fall occurred due to adverse conditions above bolt anchorage at the #40 crosscut 28+00', on 2nd West Travel way.
Unit was in transition area of a fault and as they were mining 2 bolts fell out of the last row. 3' w X 4'L by 3' high.
An unintentional fall occurred in the 2nd west entry's at the 27+30' of the #3 entry. The fall was 19' X 19' x 10'. The fall will be timbered off.
Employee was putting roof bolts plates on the sides of the bolter and a rock fell from between the bolts and hit him in the head and back, knocking into the bolter.
Employee had left his scoop bucket out in travel way. A coal hauler came along and struck his bucket and knocked it around into the rib. He was between it and the rib. Broke bones to sacrum and tail bone areas.
Injured employee was getting off the coal hauler and fell landing on his right hand breaking it in 2 places.

An unintentional fall occurred in the main north #10 entry at the 17+30'. The fall was 19' X 19' X 10' in the return aircourse.

An unintentional fall occurred between the #4 & #5 entrys of the Main North entries at the 55+40'. The fall was appoximately 17W X 17L by 8' high. The fall occurred do to deteriation of the roof above the bolt anchorage.
EMPLOYEE HAD BEEN BEATING ON AN AREA THAT WAS WELDED ON A RAMCAR AND A PIECE OF SLAG CAME OFF AND WENT INTO HIS ARM. THIS HAPPENED ON 11/22/10. EMPLOYEE WORKED UNTIL IT WAS TAKEN OUT ON 1/21/11. SLAG WAS REMOVED AND HE RECEIVED 6 STITCHES, AND WAS TAKEN OFF WORK UNITL 1/31/11.
Employee was walking by the miner and a rock that was on it, broke and fell down hitting him on both shins and his feet. No broken bones, but doctor put him off due to swelling and pain in his right ankle.
An unintential fall occurred in the #7 2nd West Main entry at the 8+40'. The fall was approximately 15'X 15'X 8' on each side of the crib setting on the corner of the intersection. The fall was due to adverse conditions above bolt anchorage.
Injured employee was bolting and a rock fell from the roof and hit him on the left shoulder and elbow, causing a contusion and pain. Started missing work on 9/23/10.
An unintentional roof fall occurred at this mine on this date at approximately 1722 hrs. The fall fell due to deteriation of roof above the bolt anchorage. The fall was 20 long by 20 wide by 15' high.
EE was carrying a fire extinguisher in muddy conditions to replace one that had been used and slipped in mud and when he caught himself to keep from falling, he strained his lower back.
Employee strained his back when he picked chain hangers up off a mantrip, the chain hanger were strung on a 3' bolt, that had 25 hangers on it. He injured his back when he turned, holding onto the bolt.
This employee was standing beside a roof bolter and his buddy turned on the bolter and the cable took his feet from under him, and he landed on his back injurying it. This did not become a lost time till 6/23/10.
Employee was operating ramcar and ran over rock in roadway and hit his head on canopy of ramcar, straining his lower back.
Employee was injured on 11-03-09 and worked until he had surjury on 4/26/10. Injured struck his right elbow on a piece of metal when he was jerking on hose on a service center. Torn Tricep.
Employee was working on a bit lug on miner and a rock fell from the roof and hit his hardhat. The rock broke and a piece of the rock came down and lacerated his jaw on the right side of his face.
Employee was getting out of truck and slipped on some mud twisting his left knee. Employee worked until taken off work on 4/5/10 due to a torn meniscus.
An unintentional fall occurred just inby #9 crosscut on 2nd west travelway. The fall was 14' X 14' X 5'.
Employee never had an incident at the mine. Employee has carpal tunnel due to repetitive motion over the years. Dates and time above is from the day he started missing work due to this.
An unintentional fall occurred in the main north #10 entry at the 40+50'. Fall was in an entry not a intersection, fall was 30'L X 14""W X 15'H.
Employee went back to employment dust box on bolter and put his hand up on control lever cober and a rock fell out and hit his left hand breaking 2 metacarpal bones in his hand.

Employee had thrown cable over out of the way and when he turned he walked into a bit bucket bolted to the roof.
Employee never had a incident at the mine. This accident occurred due to what we feel was from accumulative work over years in the mine
Employee was lowering canopy on bolter and had his right hand on tray and brought canopy down on his finger, crushing the ring finger just below the nail on the right hand. Tip of finger was amputated below nail of right hand at the knuckle.
An unintentional fall occurred in the main north escapeway at #19 x/cut of the #7 main north entry. The fall was 50' long by 30' wide by 8' high.
An unintentional fall occurred in the 2nd main west #4 entry at the 63+00'. The fall was 18' wide by 25' long by 11' high.
An unintentional fall occurred in the #8 entry of the 2nd main west at the 59+50'. The fall was 18' X 35' X 11'. Fall was due to deterioration of the roof above bolt anchorage.
Employee was going to mantrip at end of the shift and hit his head on low hanging brow. He continued to work until 7/21/09, this is the first day he missed.
Employee was lifting cable over his head with 2 other employees. Cable was very tight, he felt a pop in left shoulder. Arm gave out and cable knocked him back. Employee worked until 7/23/09 and went off work on this date.
Employee was struck in chest while changing out fire valve, that still had pressure on it. Employee worked until 6/11/09 when DR. took him off work due to pain from costochondritis seperation of 3 ribs.
An unintentional fall occurred in the 2nd main west Intake escapeway. The fall was 20' long by 19' wide and above the anchorage of the 6' tension rebar bolt. This fall was timbered off.
An unintentional fall occurred in the old 2nd west panel in the #3 entry at the 9+60'. The fall was approximately 18' wide by 45' long and above anchorage of 6' tension rebar bolt.
Employee got cut by razor knife. It was glued at the emergency room. On 1/21/09 after working for 2 days, employee went back to Dr. for a check up. The laceration was opened back up and Dr. used 4 sutures to close wound back up.

An unintentional fall occurred in the 2nd main west between #2 and #3 entry at the 5+60'. The fall was 8' high by 18' wide by 30' long. Area was bolted with 6' tension rebar bolts.
Miner operator was hit on shin and ankle of left leg by rock. Causing a sprained ankle and contusions to shin on left leg.
EE has had 3 sessions of cortisone shots from October of 2007 til August of 2008 for Lateral Epicondylitis (Tennis Elbow). He went in for surgery on 10/13/08 for his left elbow. This illness is caused by reported twisting movements of hand, wrist or forearm done during everyday activity.
An unintentional fall occurred in the 2nd west return just inby the 13+30'. The fall was 18' wide by 30' long by 11' high.
An unintentional fall occurred in the main north, entry #11 at the 23+60' The fall fell in a 3 way intersection. The fall was 20' long by 25' wide by 11'high.
Employee was bolting in #5 entry, putting up wire mesh panel. The panel was crooked and they backed to the last row of bolts for him to straighten the panel and a rock fell out smashing his left hand, breaking 2 bones in his left hand.
An unintentional fall occurred due to adverse roof condtions above bolt anchorage. The fall was at the 23+10' in the #2 entry of the 2nd main west entry's. Fall was 25' long X 18' wide X 8' high.
Employee was installing a bolt into bolt hole and a burr or something else caught his glove pulling it off his hand. He got a laceration to his left ring finger that required 4 stitches to close, and also fractured same finger where finger joins palm of his hand.

An unintentional fall occurred between #6 and #7 entry of the 2 west panel at the 15+00' on unit 1. The fall was 18' wide by 30' long by 6.5' high.
An unintentional fall occurred in the #1 entry of the 2nd west main at the 6+80'. The fall was 18' long by 18' wide by 6' high.
An unintentional fall occurred on the main north belt at the #59 x/cut 41+80'. The fall was 40' long by 40' wide by 10' high.

An unintentional fall occurred in the 1st N. Submains. The fall was 80' X 18' X 8'. The fall fell in the 24+20' intersection and went outby for 80'.
