The IE and other were operating the drill rig and pulling drill pipe out of the borehole. The IE was turning the drill steel carousel by hand and felt a pop in EE's left elbow.

I.E. was helping to put a pulling coupler on a section of 8"" pipe. when EE stood up EE felt pain in EE's lower back.

I.E. was hooking up a tube strap when EE's finger became hung in the hook and when the strap was released EE's finger was pulled in a non-functioning direction.

I.E. was pulling tubes out of the return. When EE was trying to pull them around the fan EE twisted the wrong way and EE hurt EE's back.

EE was in the process of fueling up an excavator at the North Fuel tank area on the refuse disposal area. When the EE stepped down off the track to the ground, their left knee gave out. The EE continued to work until 2/13/2026 when they went off for surgery on their knee.

I.E. was loading structure onto a trailer when EE felt pain in EE's left shoulder and down EE's left side.

I.E. was helping carry water pipe when the person who was carrying the other end suddenly dropped their end causing I.E. to drop their end of the pipe on their foot.

I.E. was hauling bolting materials up to the face to load bolters. While tramming, T-3 channel that they loaded on the scoop came loose and entered the cab striking them in their left shoulder.

I.E. was moving a cable over when they came in contact with exposed leads where the cable had been damaged.

I.E. was dropping miner cable that was hung across the entry. When they cut the rope hanger loose with right hand they caught the cable with their left hand. They then felt a little pain in shoulder but continued to finish the shift. The next morning they woke up with a burning sensation in their shoulder.

I.E was helping load a bundle of mesh onto the roof bolter. The mesh became hung up on the mesh tray. I.E claims that the mesh sprung forward freeing itself. When it did, EE felt something hit EE in the face but does not know what it was.

I.E. stuck a hand over the drill chuck to check the suction on the drill pot. The drill steel then slid out of the hole and pinched a finger. Possible ligament damage.

I.E was helping build a stopping when a rock fell out and struck EE. The rock landed on EE's left leg and EE's right leg was dislocated in the process.

I.E. was exiting EE's personal vehicle when EE slipped and fell on the ice in the parking lot before entering the building. EE worked up until the doctor took EE off work on 2-12-2025.

An Atlas Copco T4W drilling rig was in the process of drilling a hole into the mine that would be used for a turbine pump. All work ceased around 3:00pm on 11/27/2024 and the workers left the site. At approximately 7:15pm that evening, a neighbor heard a loud pop and saw flames at the drill rig, which burned for more than 30 minutes

I.E. was using a grinder to cut a power cable. While cutting the cable the cutting wheel broke apart striking I.E. in the left index finger. I.E. received 4 sutures.

I.E. Was pushing the cable reel compartment cover back on and felt a strain in EE's shoulder. EE continued to work up until the day of EE's surgery.

IE was down the HG-16 belt line at x-cut 102-103. IE was carrying a stick of monorail above their shoulder. IE threw it off their shoulder and tripped during this process and their right arm got caught between the outside top roller segment. IE sustained a fracture to the right elbow.

IE was removing a come a long from the roof mesh and slipped on slick hill and fell onto their back. IE was taken off work on 8/5/2024 to get further imaging/mri.

IE was working on a scoop and struck EE's knee against a cover. IE received a contusion. IE was taken off work on 7/25/2024.

IE was washing underneath the belt line at the dewatering system. During this time IE tripped on the water line and lost balance. IE fell and cut right forearm and received 4 sutures.

IE was sealing a stopping at the mouth of HG-14. IE got sealant underneath EE's rubber gloves. IE got burnt from the sealant and was taken off work on 7/11/2024.

IE had just put two roof bolting steels together and was putting up pressure to drill the hole. The two steels were not completely together, and a portion of the nipple was sticking out. This caused the nipple to break and the steel to swing and hit IE in the right arm. IE was diagnosed with a hairline fracture.

IE had just helped put the bobcat door back on at HG-16 belt line. IE stepped on to the mast of the bobcat and went to adjust their hard hat. While adjusting their hard hat. Their arm got caught between a bottom roller and the belt. IE was diagnosed with abrasions/burns and is waiting additional imaging.

IE was shoveling on the slope belt. IE took a break and put their hand against the top roller outer frame and their thumb on the roller that wasn't spinning. The belt got loaded with material and spun the segment causing IE's thumb to spin through it.

IE was putting up carrier cable bolts in entry 2 right on HG-17. IE tilted back the platform and some bolts fell off of the tray. When IE tilted back forward. The bolts flipped upwards striking IE in the wrist. IE was given a wrist brace the next day at the doctor.

IE was roof bolting in HG-17 putting up cable bolts. IE went to push the cable bolt up in the hole and contacted the metal with the top side of their wrist. IE received 4 sutures.

Employee was utilizing a cum-a-long to remove a stuck roller, when the roller became free, the employee's momentum carried the employee forward. The employee caught self with left hand injuring the left wrist.

IE was pulling 2x12's out from underneath the cribs and stepped backwards. IE's heal hit a crib and EE fell back and landed on EE's scsr. IE was diagnosed with two broken ribs on EE's right side.

IE was on the longwall setup face putting longwall chain together. IE was using a pry bar to flip over a piece of longwall face chain. IE felt a pop in EE's shoulder. IE was placed on restricted duty on 5/22/2024.

IE was backing up a fletcher tractor into a crosscut. IE struck the rib and a piece of coal rolled out and hit EE's middle finger. IE was treated at the hospital the next day and was given a finger splint for EE's contusion.

IE was prepping whip lines on the longwall face. IE was trying to take a staple out using a pick hammer on the backside of the staple. IE had another hammer tapping on the pick hammer and a piece of metal came off and hit IE in the eye. IE finished the shift and went the next day to have the metal removed.

Employee was removing upper pin from the back leg of the headframe. The employee had placed left arm beneath the leg to catch the pin. When the pin came out, the leg struck EE's left forearm pinching it between the leg and the headframe.

IE was trying to winch the longwall face mesh up out of the way of the roof bolter. IE flagged the roof bolter to stop tramming but the atrs contacted the mesh and spun the boat winch handle. The boat winch's handle contacted IE's right hand which resulted in a fracture.

IE was working in TG-5's CM section. IE was working on replacing bit cones on a continuous miner. IE was holding onto the bit cone when IE was attempting to get the cone started. IE swung the sledge and missed. IE struck the small finger on left hand causing a fracture.

IE was setting a surveyor's string on HG-16 CM section. IE was pushing the exhaust tubing to the side and felt a pop in EE's neck and shoulder on the right side of EE's body. IE was taken off work on 4-23-2024.

IE was bolting 2 Left (HG-17) and had hog panel 3 pieced to help cover the top. IE pinned EE's side than helped EE's partner finish EE's row. They advanced to the next row and as IE got underneath EE's canopy. A rock fell out between the atrs pad and last permanent row. Rock hit the roof bolter tray than hit IE's elbow. IE was diagnosed with a contusion.

IE just loaded roof bolter tray and was lowering the canopy. IE was talking to partner while lowering the canopy and had right hand resting on the roof bolt tray, resting against the bolts. When the canopy got low enough, it pushed down against the roof bolts and IE pinched palm region between two roof bolts that were in the tray. IE received 7 sutures.

IE approached management and said was having pain in upper back, neck and between the shoulders. IE said IE couldn't recall an injury that caused it. But IE was having burning pain sensation throughout neck/shoulder/back. IE was taken off work 4-25-2024.

Employee was setting up to do some maintenance on the feeder during the belt move. The belt crew shut off the 6"" waterline 10 x-cuts outby the feeder. When they took apart the Victaulic fitting on the 2"" water line. It recoiled backwards striking the IE in the back and IE fell to the ground with IE's leg folding behind IE. IE was taken off work pending MRI to the knee.

I.E was picking up a piece of belt when EE felt a sharp pain in EE's right shoulder. I.E was taken off work on 8/12/24 to have shoulder surgery.

IE was using a pry bar to get the roof mesh aligned better on the roof bolters mesh rack. When IE began prying, IE felt a strain in middle back on right side. IE was diagnosed with a strained back muscle.

IE contacted safety and human resources to let us know that they are having right shoulder pain that they believe to be from wear and tear from their job duties. IE believes this to be from repetitive motion and daily tasks associated with being a longwall mechanic. IE was taken off work for surgery on 6/18/2024.

Moisture accumulated in wired connection to a relay inside the communication box to the Nordberg Main Sinking Hoist.

Motor on main hoist overheated causing Nordberg Main Sinking Hoist to be inoperable. This condition was found on the pre-shift inspection of the hoist after an idle shift. There were no people in the shaft at the time of the malfunction of the main hoist. The shift for the regular crew was set to begin at 6:00 am.

IE was setting up to torch a piece of storage unit component. IE had it sitting on a board and it rolled off the board and IE's left hand was underneath it. IE received sutures to the index finger along with a fracture to the index and middle finger.

Struck by powered moving object

IE just finished hooking up a storage unit component at HG-17 x-cut 5 and told the bobcat operator to move it across the road. When IE was walking out of the crosscut. A truck pulled up and stopped behind the bobcat. During this time frame the bobcat operator backed out and pinched IE between a truck door and the read end of the bobcat. Four fractures to the right hip.

IE was strapping down a load on supply trailer using a 2"" ratchet strap. When IE was tightening it down, they felt a pop in right elbow. IE continued to work until taken off work on 4/11/2024. IE is taken off work for physical therapy.

IE was plastering a wall and when IE smacked a hand against the wall. The liquid from the eagle seal got in left eye. IE was given prescription eye drops.

Accident type, without injuries

A displacement occurred on the east side of 417 impoundment. There were no injuries and work ceased on the east side.

Fall onto or against objects

IE was watering the roads on the NE-mains travel road. IE got out of the tractor to hang up 1.5"" hdpe pipe that IE knocked down. When IE went to hang the lines up IE slipped and hit head against the fifth wheel. IE received 5 sutures and an orbital fracture.

Employee was assisting with pulling conveyor belt when the rope came free it caught their legs causing them to fall and land on their right elbow resulting in a dislocated elbow.

IE was building a return stopping on TG-5. Hog panel was cut out before the stopping was being built. IE was waiting for a block to be handed to IE. At this time IE was resting a hand on top of blocks that were already stacked. A piece of rock fell and cut IE's right index finger resulting in sutures.

IE was picking up a 4 foot roof bolt that was loaded down with chain hangers. When IE bent down to pick it up and lifted it. IE felt a sharp pain in IE's groin that continued throughout the shift.

At approximately 12:30 pm a roof fall was discovered at xc 41, between No. 1 and No. 2 entry, on the TG-5 travel road. The fall was approximately 25-30' long and was 18 feet wide. All approaches were cribbed off and an action plan was created.

IE was on the backside of the return stopping at x-cut 67 in TG-5. The wall fell towards the return and landed on the IE. IE suffered a fracture to the eye and received sutures to the back of the head.

IE was getting out of the bobcat on HG-17. When IE went to exit the cab, IE jammed neck into the lap bar. IE was wrote off work for 3 days with a strain.

IE was hanging 6"" discharge pipe in HG-16 at x-cut 19. IE was was in a bobcat basket using the wheel attached to it for the pipe to sit on. The pipe came off the wheel and landed on IE's finger. IE received 2 sutures.

Struck by flying object

IE was observing a vertical pin keeper being driven in on a relay. When a piece of metal came off and struck IE in right arm above the bicep. The piece of metal was not taken out by the hospital. The cut was glued back together.

Struck by rolling or sliding object

IE was bolting entry 2 right installing 16' straps. The top was uneven and when the operator pushed the ATRS in the up position. The 16' strap hit the top on the operators side causing it to slide down the ATRS. When the strap slid down, it hit IE's middle finger on the right hand. Resulting in a broken middle finger below the first knuckle.

The material crane/hoist experienced an electrical problem resulting in the loss of the VFD #1. The hoist was removed from serviced and secondary lifeline was rerouted to the slope. All mine personnel were alerted of the changes.

IE was pushing a metal band through the bundle of hog panel that was stuck. The metal band cut the inside of the palm of EE's right hand. IE had the cut glued together but was taken off work.

The north side of the 417 impoundment was found to have an unstable condition. This condition resulted in visible cracks of the north embankment of Coal Refuse Disposal Facility No. 2 (RDA2).

IE was walking around the mine front entrance. IE was stung by a wasp on the left forearm.

IE was lifting and pushing on t-channel when EE's hands slid off the piece EE was pushing. EE's forearm came in contact with the metal end of the t-channel. IE received 6 sutures.

IE was working on the longwall airlock doors to repair a broken shaft. IE let the tension off the shaft and heard the spring unwind. One side completely unwound while the other didn't fully release. IE began to take the bolts off the side cover. When IE took the last bolt off. The cover rotated and cut IE's forearm in two spots. IE had a orthopedic surgeon fix the lacerations.

IE was carrying trash to the truck. IE tripped over a cable bolt laying on the ground. IE fell and tried catching self with arm extended. IE has a rotator tear in the left shoulder.

IE was driving a tractor down the North mains travel road at crosscut 15. IE hit a bump and bounced towards the door latch area of the tractor. IE bumped EE's chin and ended up with a cut that required 4 sutures.

IE was helping load the bolter and got a contusion on EE's ring finger on EE's right hand. The doctor advised EE should take one day off of work so EE could rest and ice EE's finger.

IE was rebuilding a wall at x-cut 45 in TG-5. IE was standing on the ground handing block to the foreman on the ladder. IE turned around to grab a block. When they turned back around they saw the wall falling. IE received abrasions and a contusion to the the left elbow.

IE was rebuilding a stopping at x-cut 45. Area was wet and muddy. The wall was approximately 10' when the wall started leaning towards towards IE. IE felt the wall falling and jumped off the 4' ring of a ladder. IE had block land on EE's right leg. IE was diagnosed with a sprained wrist and a contusion to EE's right lower leg.

IE knocked the air crosscut stopping on the longwall. IE had started to clean up the area. While cleaning up the area of block and debris. A cinder block fell that was stuck to the top and came down and IE in the shoulder. IE was diagnosed with a contusion and was given a sling.

IE was standing on shield 5 pulling in shields. After the shields had been pulled and pan pushed forward. IE stepped backwards off shield 5. IE stepped into a hole that was 4-6 inches deep. IE rolled an ankle. IE was diagnosed with a sprained ankle.

IE was washing the slope at the 2200'. EE had the fire hose on top of shoulder and was transitioning it to under EE's arm. The fire nozzle caught EE's bibs or hit EE's hip causing it to turn on. The IE lost balance and fell to the ground striking EE's head on the floor. Witness thought EE was knocked unconscious but IE didn't believe EE was. IE was diagnosed with a concussion.

IE was changing parts out on B-East screen bowl. Taking chute out of building with a 5 ton hoist. When the chain slack went out of the building it whipped hitting the chain bucket causing it to fall 2' hitting IE in top of the head. IE was taken off work until July 7th. Injury/illness is still unknown.

IE was working in Hg-14 at crosscut 49 trying to get a bobcat unstuck. During this process IE cut EE's left knee. IE went to the doctor on 7/16/2023 to be treated for an infection.

Fall from ladders

IE was sealing a stopping on HG-16 x-cut 68 between entry 2 and 3. IE fell from a 6' ladder two different times. The second time EE hit EE's head. IE was diagnosed with a concussion.

IE was dropping ventilation tubing in the second open next to a sump. When IE dropped the tube IE had their body between the sump and the ventilation tube. One of the tubes was about to come down on IE so IE reacted and stepped back. IE fell in the sump. Fell approximately 5 feet on to back. IE was wrote off work with contusion/soreness.

Caught in, under or between running or meshing objects

IE was pinning and got the top steel stuck in the roof after removing the bottom steel. IE couldn't get the top steel unstuck so IE ran the chuck up to the stuck steel trying to align it. IE accidently hit the wrong lever while having hand on the chuck. IE smashed left hand's (index) finger between chuck and steel. Index finger was amputated at first knuckle.

Employee was putting up an angle pin, when the employee was coming back down off the bolter boom, the angle plate caught their left arm.

IE was in the process of chocking wheels on a trailer. IE put down a cinder block and got a finger between the block and the ground. IE was diagnosed with a jammed finger. IE was given a splint to the middle finger on the right hand.

IE was standing on a ladder on the opposite side of the stopping being built. The wall was almost to the point it could be capped off. The wall fell over and knocked IE off the ladder. IE landed on top of the block. IE was diagnosed with a lower right leg (calf area) bruise/contusion.

IE was pulling up waterline with a shuttle car. IE threw the waterline out of the way. IE then proceeded to get back in the shuttle car. IE didn't duck their head low enough and hit their head on the canopy. IE was written off work with a sore neck.

IE was raising up a high voltage cable over EE's head when EE felt a pain in EE's right shoulder. IE was taken off work for surgery on 9/25/2023. IE had surgery to repair a torn rotator cuff on EE's right shoulder.

IE was carrying oxygen tanks to strap on the rib and a fellow employee dropped EE's tank on IE's left foot. IE was diagnosed with a bruised left foot.

IE was packing monorail down the belt line and spreading for the next power move. When IE tossed a piece of monorail, the metal cut the index finger of EE's left hand. IE received 5 sutures.

IE bent down and moved the miner cable laying next to the roof bolter tracks. When throwing over the miner cable, IE's lower back began to spasm. IE worked until 4/13/2023 when IE was taken off work and waiting results on MRI.

IE was trying to get a drill steel unstuck from the drill chuck. IE used a four foot bolt to push the drill steel out from the underneath side of the drill pot. IE put down pressure on the four footer but also hit rotation. It rotated the four foot bolt which struck the IE's left hand. IE received a fracture right above the pinky knuckle.

IE was helping maintenance change out a roof bolter pot. IE was lifting the drill pot into place along with another person. When they lifted the drill pot up, the IE felt a strain in EE's lower back. IE was diagnosed with a sore lower back.

IE was setting bits on the shear at the end of the shift. IE hit the bit wrench with a hammer. A small piece of metal came off and went into left side of IE's face. Above the check. IE had the metal removed and received two sutures.

IE was adjusting the limit switch on the feeder. IE lost balance and fell backwards. IE tried to catch self with arm extended. IE was diagnosed with a navicular scaphoid fracture to the left hand.

IE was adding belt on TG-5. IE was pulling belt over with channel locks. The channel locks slipped off the belt. IE fell backwards catching self with EE's arm extended against the rib. IE was diagnosed with a torn rotator cuff in EE's left shoulder and was taken off work on 5/15/2023.

IE was working in the HG-14 bleeder fixing an air pump. IE was lifting on the pump to pull it out of the mud and strained abdomen. On 4/26/2023 IE was taken off work and is diagnosed with a hernia.

At approximately 9:58 am on 2/13/12023 a member of management found a broken guide rope on the emergency escape hoist. The mine was evacuated using the secondary escapeway. The broken guide rope was found at the bottom of the shaft.

A pre-shift examiner discovered a roof fall on HG-15 entry 2 crosscut 118. The roof fall was approximately 27' long by 27' wide. The primary roof support was 8' bolts with cable bolts installed in the intersection.

IE was pulling a loop of miner cable out of the road and had a pain above the belly button. IE was diagnosed with a hernia.

IE was getting ready to install EE's last outside bolt of EE's last row. EE swung EE's boom out and had EE's hand towards the front of EE's controls. A piece of rock measuring 8"" wide 8"" long 3"" thick slid off the hog panel and hit the bolter before hitting IE's left hand. IE was taken off work and diagnosed with a swollen hand.

IE bent down to pick up the shear cable while moving power. As IE picked it up and put it on top of shoulder. IE felt a pop in the lower right side of back. IE was taken off work with a strain to the lower back.

During longwall recovery. IE was lifting a waffle beam onto the shield tip and felt a pain in left shoulder. IE went off work for rotator cuff surgery on 6/1/2023.

IE was helping use a turn table to unroll the belt. The belt got tangled so they used a foundry hook to pull the belt slack. When backing up the shield hauler the foundry hook broke the chain which allowed the foundry hook still attached to the winch rope to bounce over top of the operator's tire and hit IE in the knee. IE was taken off work 1/11/2023. Fracture and torn meniscus

IE was working in the setup face in HG-15. IE picked up a compressed air bottle and felt a pop in right shoulder. IE was taken off work pending mri.

IE was following the shear to the HG straightening the pan. IE stopped to wait on the srb to catch up. IE's position was at shield 35 on the longwall face. IE put a hand on the handrail and a rock fell from in between the shields and hit IE's left hand. Shields were set and not in motion. Rock measured 6""x8"" and was 1""thick. IE was diagnosed with a swollen hand.

IE was in the process of roof bolting entry 1. IE was installing straps and cable bolts. After IE drilled EE's initial hole and was removing the steels. One of the steels fell from the top and struck the IE in EE's right hand. IE was diagnosed with a swollen hand.

A roof fall was found in HG-14 crosscut 26 in entry #1. The fall was in front of the longwall head drive. The fall was bolted with 8' bolts. The size was 8' to 10' wide and 8' to 10' long. The area that potted/domed out ranged from 3' to 9'.

IE was traveling from entry #2 to entry #1. IE was hugging the outby rib line since the miner cable was on the inby rib. When IE went to turn outby towards entry #1 towards the feeder. IE's shuttle car cab hit the rib where a 4 foot bolt/plate was attached to the rib. The plate broke off and hit IE in the check/neck area. IE received 7 sutures and a broken tooth.

Employee could not recall a specific accident date. IE reiterated that they have had a lingering problem from roof bolting. IE has been roof bolting for 6 years.

IE was carrying a monster shaft down the longwall face. IE was stepping over a shield foot when IE tripped and fell to the ground. The monster shaft landed on IE's finger. IE received 5 sutures to the ring finger on left hand.

On 8/31/2022 IE reported a strain to the abdomen that was caused by throwing over a miner cable. IE was taken off work for hernia surgery on 5/24/2023.

IE was walking down the last open crosscut on the north east mains. Traveling from entry 4 to entry 5. IE stepped on a 4"" rock and strained the top of EE's foot. IE was taken off work on 8/25/2022. IE was diagnosed with a strained foot.

IE was working on IE's utv in the TG-4 travel road. IE was in the process of replacing the serpentine belt. IE was using a short flat headed screwdriver prying the belt onto the utv. The screwdriver slipped off and punctured the right index finger on the underneath side of the second knuckle.

IE was driving down the HG-15 travel road in a tractor. At crosscut 119, IE hit a bump and jarred head into the top of the canopy. The IE was diagnosed with a brain concussion and cervical spinal cord concussion.

At crosscut 46 in the north mains a roof fall was found between entry 2 and 3. The fall was approximately 35 feet in length by 18 feet wide. The fall was supported with 8' bolts. The entry height was 10.75'. The height of the fall varied throughout the fallen cavity. The thickest portion of the fall was 9' thick.

IE was taking the discharge line apart on the cyclone refuse pump because IE thought it was plugged with material. IE removed the bolts from the flange on the discharge line. When the line was pulled apart, material came out of the pipe and hit IE. The material inside the pipe was heated due to the pump not priming. IE received prescription burn cream.

Over-exertion in wielding or throwing objects

IE was swinging a sledge and busting up coal chunks so they could shovel them on to the belt. IE strained their neck and continued to work until 3/18/2023. IE was than taken off work for surgery on their C5-C6 in their neck.

Upon examination of the NE-Mains primary escapeway. A roof fall was found at crosscut 27 in entry number 5. The roof fall was contained to the intersection. All sides were barricaded and flagged.

Employee was on the head Gate #15 conveyor belt picking up trash and when EE was getting off the belt EE placed EE's left hand on a pair of cutting shears cutting self on the blade. EE was not wearing gloves at the time of the accident. The injury resulted in a laceration that required three sutures.

IE was helping clean up the roof fall at HG-14. One of the steel sets feet was loaded out. IE went and picked it and cut wrist and hand. IE received staples to palm and forearm.

IE was putting up Q decking on the skid steer basket and felt a pain in lower back that also shot down EE's leg. IE was diagnosed with a strained muscle. IE was taken off work on 7/20/2022.

On July 7, 2022 a roof fall was found on HG-14 crosscut 30 entry 2. All sides were barricaded and flagged off. Lifeline was rerouted from entry 2 to entry 3 from crosscut 25 to 31. The top ranged from 8' to 10'. The area was bolted with 8' bolts.

IE was dropping cable above the triple vac when the cable slipped out of their hands and hit them in the neck. IE began receiving chiropractic treatment on 7-11-2022.

IE went to throw the chute on the B East dryer. IE was standing and using both feet and both hands to push off of. That is when IE felt a pop in left leg. IE suffered a tear to the left achilles tendon.

IE was hooking up a tow bar from the generator to the battery side of the scoop. IE tried to swing the tow bar to the scoop battery. The scoop operator bumped the steering controls while the IE was hooking up tow bar. IE suffered a fractured thumb.

A roof fall occurred on the TG-4 travel road at cross cut 58 entry 2. The fall was approximately 35-40 foot in length and was the width of the entry. The height of the entry ranged from 8' to 9'6"". The roof was supported by 8' bolts. The fall was cribbed/flagged off on all approaches.

IE has been been experiencing pain/tingling/numbness in both hands and forearms. IE believed this illness is a result of continual exposure to IE's work duties and work history. The duties include continual and repetitive grasping, lifting and use of hands. IE had carpal tunnel surgery on 6/15/2022.

IE was in the roof bolting progression. IE was drilling a hole when the drill steel became clogged up. IE removed the steel from the hole and attempted to unclog the drill steel. IE was striking the steel against the platform when it came in contact with IE left foot (big toe) area. IE sustained a fracture to the big toe.

IE was carrying buckets of broken concrete down the stairs and slipped/twisted EE's knee. The stair case the IE was carrying the broken up concrete was near the top of the plant. IE was diagnosed with a torn meniscus.

IE was washing the hydraulic tank at the load out. Shortly after, IE felt a sting on the right hand. IE removed the glove to check, but found nothing. By the end of the shift, IE's hand had swollen. IE was later treated for a spider bite.

During MSHA quarterly examination of the North Intake Shaft- Mechanical Escape Facility, a damaged guide rope was discovered at approx. 10:15 AM. This would not allow the escape capsule to deployed to bottom of shaft. The guide rope has been replaced and capsule operational as of 4/28/2022 1:15 PM.

IE was preparing to repair a hose on LW Shield 156. The IE electrically unlocked the shield to allow the shear to continue mining. IE reached over the base lift halo to retrieve EE's hammer. The vertical pin broke allowing the base lift halo to jump up striking the IE in the chest when EE was stretched out reaching for EE's hammer.

Management was informed of an accident on 3/30/2022 that occurred on 03/24/2022. IE stated they were operating a tractor and bumped an elbow on the frame of the tractor in the cab area. The resulting injury was chipped bone spur to the left elbow.

IE was helping to toss crib block at the LW Face. A rock fell from between Shields 2 and 3 striking the IE's Left index and middle fingers. This resulted in a fracture to both fingers.

IE was installing a 6' roof bolt into the bolt hole when EE felt a ""pop"" in the left shoulder and neck area. This resulted in some numbness and tingling in EE's arm and fingers.

IE was installing belt structure for a belt advance on HG15. IE and a partner were maneuvering a top roller with frame through the belting and the IE's left middle finger was pinched against the belt rail causing a laceration.

IE was loading belt rollers on the structure trailer at the LW LOXC. IE lost control of a bottom belt roller and pinched right middle finger between rollers. This resulted in a fractured finger tip and laceration to the finger nail.
