The injured was helping dismantle belt structure in the slope, he was pulling on a section of structure that had been cut free and it fell to the ground and bounced up and struck him between his lower lip and chin. He received two sutures to close the wound.
The injured was operating a continuos miner in Unit #1, entry #6, at 13+70, when a piece of roof rock measuring 38"" X 24"" X 3"" to 6"" thick fell from the mine roof between the last two rows of bolts striking the injured on top of his hard hat. The injured has been diagnosed with a fracture to C6 and C7 in his neck.
The injured was in the process of changing batteries on a 488 scoop located at the Unit #2 battery station. He exited the cab of the scoop and was walking over to disconnect the jumper cable, when he stepped on a piece of roof rock measuring 4"" X 3"" X 3"", causing him to twist his left knee.
The mine encountered a roof fall at 19+25, entry #5, of the Main Northwest. The fall measured 4'L X 18'W X 4' thick. The event was called in at 7:30PM. 103(k) issued at 7:55AM on 1/5/2009. The area was developed 7/10/2002, bolted with 12' point anchor and truss bolts. The fall will be cleaned up.

Employee was riding out of the slope in a diesel mantrip when the trip started to slid backwards down the slope. After several attempts to stop the trip, the driver turned towards the arches to stop the ride. The impact caused EE, who was sitting in the last seat behind the driver to have contact the back wall of the mantrip.
The injured was making a splice on a miner cable. While stripping the outer jacket, the knife he was using slipped out of the cable striking his leather gloved left hand causing a laceration that required 11 sutures to close.
In the process of setting a hietzman jack in an area of unsafe roof the injured was struck on the left elbow by a piece of roof rock measuring 10"" X 8"" X 2""; the injured suffered a chipped bone near his elbow. The incident occurred on the Main North, Entry #3, Sta. 6+40.
The mine encountered an unintentional roof fall on the Main South, entry #4, at the 3+60 intersection. The fall measured 60' X 18' X 8' thick. It did not affect ventilation or the passage of men, no personal injuries. The area was developed on 6/16/07, supported with 8' point anchor bolts. This fall will not be cleaned up.
The injured was trimming a piece of belt to be used for a skirt rubber on the tail piece for Unit #2. He laid the belt across his right leg, and as he was cutting, the utility knife he was using slipped out of the belt causing a laceration / puncture to his right thigh. The injured required three sutures to close the wound.

The injured was brushing float dust from the outside rail of the belt structure, the wire brush he was using came in contact with the outside barrel of a top belt roller, causing the wooden handle to strike his left index finger. The injured received three sutures to close the wound.

The injured was standing at the operators controls of the roof bolter when a piece of roof rock measuring 53""L X 34""W X 14"" thick, fell from between the rib and first pin. The rock caused the injured to fall to the mine floor with the rock across his legs. The injured was diagnosed with a fracture to the right fibular head.
The mine experienced a roof fall in the 5th Northwest Panel at the 8+10 XC between entries #2 & #3. The fall did not affect ventilation, but did involve the secondary escapeway. The injured was escourting MSHA inspector, he slipped in some mud, as he fell he caught himself with his right hand injuring his right wrist. The incident was reported but no medical treatment was sought. On 4/22/2008 the injured had x-rays that revealed a partially healed fracture. No work missed until surgery performed on 5/15/2008.
The accident occurred in the slope approx. 1260' from the surface when the examiner was heading down the slope in a Wallace 6 man diesel mantrip. The injured lost control of the vehicle, and struck on of the supporting arches with the front drivers side of the mantrip. The injured walked to the surface and called for help.

The starter steel fell from the mine roof pinching his middle finger of right hand between the two pieces of drill steels. Required 5 sutures to close injury. The injured was working on the cats of the #1 Continuous Miner when a piece of roof rock measuring 4'6"" X 4'8"" X 1-6"" thick fell out of a small slip. The rock struck the injured's left leg causing two fractures to his fibula. The injured required surgery to repair.

The injured was in the process of loading curtain boards on the return side bolter located in Entry #1, S.S. 2+70 of Unit #1. When a piece of roof rock measuring 27"" X 12"" X 2"" fell from the mine roof striking the employee on top of his hard hat. The injured finished his shift on 10/10/2007. On 10/11/2007 the injured sought medical attention, diagnosed with a neck strain.
The injured was setting timbers in the Main North, near the West seals. After completion he and another employee sat down for a few minutes when he felt something bite/sting him on his right upper back. On 10/5/2007 the injured showed the area to management and was sent to the E.R. on 10/6. The injured was treated for an infection at the bite/sting location.
The injured was holding an 11' timber while another employee was getting ready to place the wedges to secure it. A piece of roof rock fell striking the injured across his forearms, and a piece struck him in the mouth causing a laceration to his upper lip that required 6 sutures to close.

The injured employee stepped out from under the canopy of a Fletcher RRII bolter to go to the back of the machine to get some supplies when he was struck by a piece of roof coal measuring 21"" X 15"" X 5"". The impact drove his hard hat onto his safety glasses causing a laceration to the bridge and left side of his nose that required 12 sutures to close.

The injured was loading bolting supplies onto the back of the #4 Fletcher bolter; he had bent down to pick up a bundle of resin when a slab of roof rock measuring 5' X 3' X 3"" fell from the roof striking him on the back of the head and neck. He was transported to local E.R. where all x-rays and cat scan were negative and he was diagnosed with a neck strain.

The mine encountered a roof fall on Unit #1 (MMU 001) at 37+45 in entry #2. The fall was immediately reported to the MSHA 800# as required. The fall measured 40'L X 17.5'W X 8'high. A verbal 104(k) was issued over the phone from a representative of District #8.
The injured was cleaning/shoveling on the 3rd Main Northeast beltline. The material was wet and muddy with coal fines and chunks of coal in it . As he was shoveling, he said he suddenly had a pain in his right side, that radiated to his groin area. He was diagnosed with a groin strain.
On 7/25/2007 at 11:35 am, the mine experienced a roof fall on the 3rd main Northwest at S.S. 54+60, including the 54+60 crosscut. The Secondary escapeway (haul road) was rerouted to an adjacent entry to allow the travel of men.
The mine examiner located a roof fall located outby the #1 Seal of the 1st Southwest seals, off of the 2nd Main Northwest. Instructions from Distrct #8 is to monitor the area near the seals hourly until the fall can be cleaned to check the condition of the seal. Approaches to the area will be resupported with 12' bolts and cable bolts.
MMU-001 experienced an unintentional roof fall at S.S. 29+05, entry #0. 103(k) issued, and modified to allow additional roof support to be completed.
The injured was in the process of retrieving a B.O. Getman tractor out of the slope. In the process, the tractor started sliding and struck the arches in the slope causing an avulsion to the back of his head.
Injured was shoveling section belt tail and sustained a hernia.
The injured was exiting the #7 Stamler coal hauler that he was operating, and his CSE SR-100 rescuer caught on the side of the car causing him to fall to the mine floor. He tried catching himself using his right arm/hand, which caused him to injure his right shoulder.
Injured employee was using a standard utility knife to cut a piece of mine belt. The injured employee was using both hands pulling the utility knife towards himself, when it slipped out of the cut and lacerated his right leg just above his knee. The laceration was approximately 3/4"" long. He received 4 staples to close the wound.
The mine experienced an unintentional roof fall at S.S. 65+40 of the 3rd Main Northwest. The Secondary escapeway(haul road), has been re-routed to an adjacent entry to allow the travel of men.
IE was roof bolting in #2R at 00+60 crosscut on Unit #1. IE had sudden onset of pain just above left hip bone. IE asked to be taken out of mine and sent to Emergency Dept to be seen by a doctor. IE was taken to Provenas' Emergency Department, and diagnosed with a hernia.
The mine experienced an unintentional roof fall on the MMU 001 working section, at S.S. 22+80, entry #2 of the 2ndNorthwest Panel. A 103(k) was issued at 2:00am.
The injured was installing 6' resin bolts in the face of Entry #6, on Unit #2. While trying to install the bolt he he felt some discomfort in his left shoulder. At 6:45 he requested to seek medical attention. He was diagnosed with a left rotator cuff strain. The mine was unable to meet his restrictions.
The injured was attempting to sand paint off of the new 7/8"" drill steels that he had taken underground. He placed a piece of emery cloth in his leather gloved hand, and placed the drill steel in his drill pot, placed his hand around the drill steel and became entangled. His glove was ripped from his left hand, he sustained compound fractures to his left ulna and radius.
The injured employee was walking past the corner of a pillar that had a slip running across the corner of it. As he walked by a piece of rock fell from the roof striking the employee knocking him to the ground; a second piece of rock fell striking the back of his left ankle. Diagnosed with tear to his ligaments in ankle.
Encountered fall of roof at Station 10+50, Entry #3 (Haul Road), on the Main Northwest. The fall measures 17.5' x 50' x 12' thick. Secondary escapeway has been routed to Entry #4.
Employee was digging a hitch in rib, soreness started developing & swelling.
The injured employee was installing 6' fully grouted bolts; in an effort to lift the bolt onto his drill pot he felt a pinch in his lower abdomen. He was diagnosed with a hernia that will be surgically repaired.

Discovered a roof fall at station 6+40 of entry #2 of the 1st N.W. Panel off the 3rd N.E. Main-Unit #1. The fall measured 120'long X 18'8""wide X 6'-8'thick.
At approx 12:10 am on 12/1/2006, a fall was discovered in 0 entry at station 52 + 10 of the 3rd main northwest. No injuries or property damage and no impairment to ventilation.
On 11/28/2006 at 3:00 am we incurred a roof fall at 1 + 80 cross cut of #6 entry of the 3rd SW panel off the 3rd MNW in unit #2 (MMU 002-0). The roof fall measured 40'L x 19'W x 12' thick. The fall involved unit scoop #3 being covered from the battery end to the cab of the scoop. The scoop was unattended at the time of the fall.

On 11/13/2006 a roof fall was discovered at station 56+30 x-cut of #0 entry of the 3rd M.N.W. The fall was called in 11/14/2006. This area was developed on 11/9/2006. The fall measured 18' 18' x 8'.
At approx 3:15 am on 11/9/06 a roof fall occurred at station 7 + 70 crosscut on the 2 mwm at #1 entry.
At approx. 10:00 pm on 11/2/06 a roof fall occurred at station 44+60 crosscut on the 3rd Main Northwest between entries #3 and #4. The fall involved the beltline in #4 entry.

On 9/26/06 EE reported that his back was continuing to give him problems. He was not clear what he had done to it. After being evaluated, an MRI was conducted on 11/9/06 and he was diagnosed with degenerative disc disease. On 4/11/07 back surgery was performed. Documents to support degenerative disc disease attached.
While setting up to cut the straight of #3 entry at 17+40, the miner operator backed the miner out to sight it in when a rock measuring 2'x1'x4"" fell from the roof striking the back of his left hand which was on the remote box for the miner (the rock also broke the box's handle). Initial x-ray indicated bruising only; follow-up determined that a bone in the wrist was fractured.

After changing oil in the feeded the EE was reinstalling a cover on the side of the machine. One bolt had been installed and he was preparing to insert the second bolt when the cover fell off striking him on the left foot.
EE was operating a coal hauler when he ran over a rock. This caused him to strike his head on the machine canopy. Later in the shift his back began to hurt.
EE was moving a hydraulic cylinder in the ware- house when he struck his elbow on an upright shelf brace.
The employee was roof bolting at the time of the accident. The roof bolter had been advanced to the next row to be installed. While the ATRS was being set the injured stepped up to the operator's drill station. When the center support of the ATRS contacted the mine roof a rock was dislodged that struck the employee on the left leg as it fell to the ground.
Employee was helping the miner operator change bits on the machine when the accident took place. While holding a bit in his right hand he used both hands to pull the miner head downward. In the motion of moving the head his right middle finger met with a stationary part of the head. The result, despite wearing full-fingered gloves, was a smashed and lacerated finger.
The injured was operating the continuous miner in #6 left when a large piece of rock fell onto the mach. He then turned to run and was struck by a second piece of rock that fell from between the last two rows of bolts. Rock was 75""long by 52""wide by 6""thick. A ROOF FALL OCCURRED IN ROOM #7 S.E. PANEL RETURN ROOMS, SURVEY STATION 0 + 00 TO 0 + 80. THE FALL MEASURED 80' LONG X 18' WIDE X 8' THICK. THE AREA HAD BEEN BOLTED WITH 8"" DOUBLE LOCK & 6' FU LLY GROUTED BOLTS. ALL APPROACHES TO THIS AREA HAVE BEEN TIMBERED OFF.
EMPLOYEE WAS HAND DUSTING THE RIB OF A WORKING FACE AND GOT ROCK DUST IN HIS EYE. PRESCRIPTION EYE DROPS WERE DISPENSED BY THE DOCTOR RESULTING IN A REPORTABLE ACCIDENT.
