The victim was operating a battery-powered, rubber tired, personnel carrier on the 2nd 48 supply road. As he traveled outby, for a presently undetermined reason, the golf cart veered suddenly to the left before overturning on top of him. Co-workers initiated CPR and continued to the surface. Attempts to revive him were unsuccessful and he was pronounced dead at 1:59pm.

While bending a roof bolt ee felt a burning in right shoulder. Then ee went to put the glue into the hole and ee felt shoulder pop. ( strained shoulder)

Struck against stationary object

EE was walking between cross cut #4 and #5. There was a shuttle car traveling the straight of #5 which was anchored in #4. While the cable was moving across the entry, ee tried to step over the cable, which caught boot, causing EE to do the splits. (EE has a bruise of the right hamstring.)

Was carrying oil cans in each hand, while walking. EE felt something in left knee. ***(Started missing on 03/17/2016. Orginal diagnosis was a cyst(non work related). After further test, as of 04/08/2016 changed diagnosis to strain.)

He is 6 ft. 5 inches tall walking down a entry that is 5 ft. 6 inches tall. While walking he had his head tilted to one side, when he hit his ear on the end of a roof bolt. (he received stitches)

While throwing over the miner cable, he slipped and fell, he tried catching himself and pulled something in right shoulder. ***Began restricted duty on 11/19/2013***

There was a unintentional roof fall that occurred on #2 units 3rd belt at crosscut #8, (width 25'X 25' 10' height, in intersection) We are not planning on cleaning this fall up. This area was supported with 72"" tension rebar roof bolts.

While installing roof bolts and hog wire to the mine roof, ee used his left hand to hold up the hog wire, the hog wire got wrapped around rotating steel, pulling his left arm around the steel.(causing the amputation of his left arm just below the elbow)

There was a unintentional roof fall located on the 8th 48 beltline, between x-cuts 29 and 30, in the straight. It was approximately 50 ft long, 18 ft. wide and 8 to 9 ft high. 84 inch tension rebar bolts was used in this area.

He was scooping out header hole, (scoop did not have a canopy) Material fell out from roof, and struck him on left knee.( caused cut on knee required 3 stitches )

An unintentional roof fall has occurred in front of the #1 seal at the Route 1 Seals xc 27 entry #9 (6th 48 Main Return). We are not planning on cleaning it up, we are going to add 3 seals around fall. This area was supported with 72"" tension rebar fully grouted roof bolts and additional cribs, props and timbers were added to the affected area.

An unintentional roof fall has occurred at xc 3 on the 8th 48 travelway, entry #6. We are not cleaning this fall up, we stepped the travelway over. The area that fell was approximately 20' x 20' and was bolted with 72"" tension rebar roof bolts. The affected area was re-supported with 84"" bolts, brow bars, cribs, props and timbers.

Was standing on opposite side of bolter, putting bits on stringer, when operator trammed bolter backward, running over top of left foot.

while getting in shuttle car, there was screen wire hanging down. The screen wire caught his right ear and cut it (received 5 stitches)

While dusting the beltline he bent down to remove a damaged bottom roller. When dragging the roller he felt a strain in his groin area, causing him to have surgery to repair a hernia. ***Began missing work on 8/26/13***

While loading the rock dust trailer, he was trying to lift himself up on the trailer with his left arm to pull the plastic off the bag. He felt his right shoulder pop and then give out causing all his weight to come down on it. He immediately was scheduled for a MRI and the results were positive for a tear in his rotator cuff. ***He began missing on 8-14-12 for surgery.***

While backing the bolter out of #9 Entry, the bolter began to sink in the bottoms and slide towards the rib. After hitting a big rock, the bolter jumped, throwing the operators foot out of the cab and smashing it between the rib and the bolter. He received a fracture in his left foot. ***He began missing on 06/04/2012.***

While kneeling down working on the miner, a piece of the coal rib rolled out striking him on the back and right shoulder. He continued to work full time without restrictions until the doctor put him off so they can do therapy and injections. ***He started missing work on 07/09/2012***

While bending an 84"" tension rebar roof bolt, he felt a sharp pain in left shoulder. He continued to work full time without restrictions with pain until he was diagnosed with rotator cuff tear. ***He began missing work on 7-25-12.***

The roof bolter operator was cleaning the gob off top of roof bolter, his hand slipped and cut right wrist on piece of sharp slate rock.( had to have 4 stitches in right wrist)

While tramming the scoop he hit a hole, making him hit his head on the canopy, jamming his neck and right shoulder. He continued to work full time until he was put off work for physical therapy. ** Began missing work 01/30/2012**

He continued to work full time without restrictions until 06-07-12, where it was determined that surgery would be needed to repair a small tear in his rotator cuff.

The mechanic was in the middle of fixing the foot switch on a golf cart when he dropped his supplies on the cart engaging the tram pedal. EE was pinned between the golf cart and mantrip, deep bruising his right thigh. He was taken to the Doctor and was put off work until Sept. 2, 2011.
