Caught in, under or between a moving and a stationary object

The EE was operating the C.M., he was making the first cut in the C.C. left from No. 4 to No. 5 entry. After making the initial cut,EE backed the C.M. into C.C. between No. 4 and 3 entries to set the machine to slab the right side of the cut. As he backed the machine into the C.C., he was pinned between the outby rib of the C.C. and the left side of the cutter head. (See attached)

Caught in, under or between a moving and a stationary object

A Supervisor was found lying in an active roadway intersection with fatal injuries that would appear to have been caused by a coal hauler running over his body. There were no eyewitnesses. The investigation is ongoing.

Caught in, under or between a moving and a stationary object

While doing belt recovery work, employee had removed the bottom rollers from their bracket on one side of the beltline. This put the rollers in a bind, when he pulled hard, the roller came loose, smashing the middle finger of his right hand between the roller and the belt rail. The employee was taken to the hospital and the finger was pinned and stitched.

Struck by falling object

Employee was walking through an underpass on the Old 2A Beltline when a rock 12 inches x 12 inches x 5 inches fell from a brow striking employee. The rock caused a small laceration on the left forearm that required 3 stitches to close. The employee was released to return to work on his next scheduled shift.

(See Attachments)EE had been experiencing back pain. On 1/24/13 the EE exp. more back pain. Notes from a case worker suggest he slipped that day and landed on his buttocks and was also recovering cable with several coworkers. The EE was released to work on 1/25/13. The EE had a microdisectomy in 2/2007. Dr. took EE off work for P.T. 1/28/13.

Fall from machine

Employee was stepping out of battery transport when he struck his head on the mine roof, knocking his hard hat off. As he reached out to grab for his hard hat his momentum carried him forward and he lost his balance. His feet caught on the lip of the deck and he fell out of the cab. As he fell he attempted to catch himself with his right arm breaking his Humerus.

Employee was walking beside the double boom roofbolter when he tripped on some loose rock. As he fell he tried to catch himself with his right arm. He fell onto the floor pulling something in his right shoulder.

Over-exertion (Not Elsewhere Classified)

While working underground on 11/5/12 EE felt pain in his left shoulder. He continued to work until he had MRI which shows possible tendonosis. See Attached

Accident type, without injuries

An unintentional roof fall occurred on the 4th North belt line @ the 218 crosscut (station 173+71) Entry #5. The fall did not affect ventilation, passage of men or cause personal injury. The fall did cause damage to the belt line. The area will be cleaned up and arched. The area had been bolted with 6 foot roof bolts. The fall measured 35'x 20'x 15' high.

Employee was carrying a 6 foot timber with another employee when he wrenched his lower back. The top was low in the area. Employee continued to work until 11/8/12 when the Doctor took him off work and referred him to a neurosurgeon.

On 10/8/12 employee said that he had been having problems with his hands going numb. Employee continued to work until 12/19/12 when he had carpel tunnel surgery on his right wrist. The left wrist will also need surgery, to be scheduled.

An unintentional roof fall occurred at crosscut 270 in the Main North Intake in the #3 entry. The fall had been roof bolted with 6 foot fully grouted roof bolts. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30 feet long x 16 feet wide x 3 foot thick.

Employee stated that he was having trouble with both hands. He said that they are going numb, tingling, and falling asleep. He was diagnosed with carpal tunnel syndrome and started missing work on 12/3/12 when he was scheduled for surgery.

Over-exertion in lifting objects

Employee was in the intake working on a ramcar. He had to lift the screen over the tram motor. Employee stated while standing on the side of the ramcar he lifted the screen and felt a pull in his lower stomach. Around an hour later he felt a small knot in his lower stomach. Employee finished his shift and was later diagnosed with a hernia. Employee is scheduled for surgery.

Over-exertion in lifting objects

Employee was lifting miner cable over the scoop to move a generator. When lifting the cable over the center of the scoop, employee felt pain in his left shoulder. Employee continued to work until doctor took him off on 10/18/2012.

Fall to the walkway or working surface

Employee tripped and fell while changing batteries on his ram car. Employee claims that he felt something pop in his back when he fell. Employee was taken to hospital and follow up with doctor. He continued to work until 10/24/2012 when he was taken off for bulging disc of the L4 and L5.

Over-exertion in lifting objects

Employee was lifting his tool bag out of his tool box when he felt something in his right shoulder pull or tear. Employee came out of the mine and was transported to the Doctors office. He was later diagnosed with a torn rotator cuff. Employee continued to work until 10/22/2012 when he went off for surgery.

Fall to the walkway or working surface

Employee was cleaning in the unit 4 tail area, when she heard a rib crack. She took off running and fell down onto the ground. She landed on her right arm leaving a small cut on it. She was released to come back to work but did not come back because she is experiencing dizziness.

Employee was diagnosed with Carpal Tunnel Syndrome and started missing work on 9-11-12 for surgery on his right hand.

Employee was loading rock dust bags onto the scoop. Employee stated that when he turned to put the rock dust bag onto the scoop he felt pain in his back and right leg. A diagnosis of a lumbar disc was not made until 9/11/2012.

Employee was dragging a curtain to the face of the #3 entry when a rock fell out between the bolts striking him on his left knee. Employee continued to work until 10-15-2012 when he went off to have a torn meniscus repaired.

Employee was under a physicians care in April 2012 for low back pain. The MRI taken at the time indicated degenerative changes (MRI enclosed). The care provided was not a result of a work related event. At work in July, employee stated he had pain in his lower back. He was then off work on July 30, 2012. This is the result of off the job medical issues as well as repetitive motion.

An unintentional roof fall occurred in the 5th north headings MMU 012 return @ 48+15 in entry #5. The area had been supported with 6 foot glue bolts. The fall measured approximately 19feet x 19feet x 8feet high. The fall may be cleaned up at a later date. It did not affect ventilation, cause personal injury or passage of men.

He worked over the next few weeks. EE stated that in 2006 he had degenerative disc problems. On 4/28/12 he was diag. with degen. disc disease.The EE worked until his surgery 5/14/12.

Employee was bending over when a rock fell out of the roof striking him on the back and spine. Employee walked to the dinner hole and felt faint. After this he was taken to Deaconess Hospital where he was kept overnight for observation and released the next day to go home.

An unintentional roof fall occurred in the 2nd West intake @ 42+13 station in the #4 entry. The roof fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20'x 20' x 10' high and had been supported with 6 foot fully grouted roof bolts. The fall has been timbered off and will not be cleaned up.

Struck by flying object

Employee was putting two pieces of conveyor chain together. He had a hammer hitting another hammer putting a link together. A piece of metal flew off striking employee in the right arm. The piece of metal was removed from his arm.

Fall onto or against objects

Employee and 3 other co-workers were working on 242 miner, removing tram reducer, when the roof behind them started working. Employee stated he was bumped into by one of his co-workers knocking him into the front of the miner. As he was falling he reached to catch himself with his left hand. His left hand landed on a sharp rock which caused a laceration that required 11 stitches.

A roof fall occurred in E#3 @ station 30+10 in the Old 5A intake. The roof fall did not affect ventilation, cause personal injury or affect passage. The roof fall measured 27'x 31' x 6'. The area had been supported with 6' roof bolts. This report is being filed to abate a citation, we do not feel that it is reportable. Area had been dangered and timbered out prior to the fall.

Over-exertion in lifting objects

Employee was putting a tire in the back of #15 truck, employee was picking a tire up and twisted a little, he had a little discomfort at the time. Employee said that he misjudged how high to pick up the tire. Employee has a lower back and groin strain. Employee did not start missing work until March 1, 2012.

An unintentional roof fall occurred on the 5A intake entry #3 at x-cut 57 (32+20 station). The fall measured 20'x 20'x 6' high. It did not affect ventilation or cause personal injury. It did fall on the primary lifeline but their are 3 intake entries and the panel is worked out. The intersection had been bolted with 6 foot fully grouted roof bolts.

Struck against stationary object

Employee was getting a line curtain from the rib in entry #4 between 56+95 and 57+65, when he turned around he ran into a roof bolt plate that was hanging down about 8"" from the roof. He suffered a laceration to the forehead requiring 5 stitches.

(See Attachments) EE has been a mechanic who had years of repeated pulling and pushing on wrenches and pry bars. On 1/25/12 he felt pain in his right shoulder. He was diag. on 8/31/12 with rotator cuff tendonitis, degen. changes of the joints.

Employee was closing air lock doors on the Main North at 246 x-cut. He had closed the west door, when he went to close the east door he tried to catch it so it would not slam shut. His right hand got caught between the two doors causing a dislocated wrist and torn ligaments.

An unintentional roof fall occurred on the 5B belt line at 106 crosscut it measured 20'x20'x15' high. The fall did not affect ventilation, cause personal injury or affect passage. It will not be cleaned up.

Struck by falling object

Employee was observing roof bolting on the Main North Travel road @ x-cut #59. As the bolter was drilling a hole, the injured employee was squatted down with one hand on the mine floor watching the bolter drill, a rock measuring about 2.5"" x 17"" x 24"" fell out of the roof striking her left hand causing a fracture in it. The hand was put into a hard cast.

Caught in, under or between a moving and a stationary object

Employee was hooking up his material trailer to the back of the scoop. When he swung the tongue around to hook his trailer up, he smashed his left hand ring finger. The employee received a laceration which required 5 sutures to the finger.

Employee has been complaining about numbness and pain in his left hand and wrist. He has operated a continious miner for years and examined for years, using both levers on the miner and remote and stearing a ride through air courses. He was diagnosed with Carpal Tunnel Syn. from repetitive motions of operating machinery. Employee did not start missing work until 2/17/2012.

While doing a daily hoist inspection, employee found a broken guide rope on the Gallitan County North Fan emergency hoist. The rope was replaced.

Struck by powered moving object

The employee was changing scoop batteries in a crosscut on the Unit 4 travel way, when another employee was driving out battery-first on a ram car and struck the front of the scoop bucket, causing the scoop to roll backward. When the scoop rolled backward, the injured employee's right leg was pulled into the area between the scoop tire and the frame of the scoop.

Employee was stopped and ask by another employee to hang miner cable. He shut off the scoop and upon exiting the scoop his right knee popped, employee stated that his right knee was burning and it hurt to straighten it out. Employee continued to work until 1/12/2012 when he had surgery for a torn meniscus of the right knee.

An unplanned roof fall was discovered by MSHA on 11/6/2011 during evaluation of a near by fall. The fall measured 19'x 40'x 6-8' high. The fall was in the 1st West Submain off of the 1st North Submain in Entry #6 @ station #29+95. The fall did not affect ventilation, passage of men or cause personal injury.

Employee was hand dusting a cut out of 7 Left when a rock measuring 30.5""x 35""x 3.5"" thick fell from the roof between the rib and first row of roof bolts and struck him on the head and right forearm. The employee was taken to Harrisburg Medical Center where he was diagnosed with a fracture to his right forearm.

Employee claimed that he was eating dinner at the dinner hole underground with his safety glasses on when he felt something in his right eye. Employee was seen that after noon by Harrisburg Medical Center and followed up the next day with an Optometrists where a piece of debris was removed from his right eye.

A mechanic was in the process of re-installing a cover on the roof bolter when it slipped out of his hands and struck him on both shins. He was taken to Dr. office where he received 6 stitches in his left shin. He was placed on restrictive duty by Dr.

Employees were loading onto the cage, EE could not hear what the foreman was saying, so he moved closer but did not realize his right hand rested on the rail of the cage. A co-worker shoved the door to close it pinching ee thumb between the door and the framing.

Employee was operating a Getman Tractor pushing a rock dust pod into a cross-cut. He had his right hand resting on a pinch point warning sign. When he articulated the machine, the center-section stops smashed his right ring finger at the pinch-point fracturing the tip of the finger.

Employee was walking toward the face, slipped on the mine floor, fell, and struck his forearm on a sharp rock. The employee finished the shift, but later went to a doctor who applied 9 sutures to close the wound. The employee was released back to work without restrictions.

EE was removing covers from top of cont. miner. When he was dismounting miner, his foot slipped off side rub rail, causing him to strike lt leg against rub rail of machine. EE received small abrasion on lt shin. EE felt pain in lt leg the following day. Dr diagnosed him w/cellulitis. Further visits determined EE had contracted MRSA bacterial virus. MRSA is a contagious virus.

Employee was standing on a conveyor take up frame beam when he stepped off the beam onto a water pipe and rolled his left ankle. The employee was restricted to surface work on 8/22/2011.

Struck by falling object

Employee was moving a miner from the face into position for the next cut. A rock fell from between the bolts and struck the employee on the back. He fell to the ground and rock hit his left heel as well. Subsequent testing revealed two cracked spiny member od L1 and L4 vertebra and a fractured heel. Employee was taken off work on 8/22/2011.

An unintentional roof fall occurred on the mmu 015-0 working section entry #5, 3rd left panel off 8th west submain. The fall measured 25'x18'x6.5'and will be cleaned up. It did not affect ventilation, passage of men, or cause personal injury. The area had been roof bolted with 6 foot fully grouted roof bolts.

Over-exertion in pulling or pushing objects

Employee states he was helping the miner operator remove a rock from the machine. He braced himself and started to push, when he felt a ""pop"" and pain in his left calf muscle. Diagnosis was a torn calf muscle and the employee was placed on restricted duty.

Struck against a moving object

Employee ( coal hauler operator) has been exposed to repeated jarring of his neck while operating his hauler. Finally, after a bouncing occurrence on the date of the report, the employee requested to be taken out of the mine for evaluation. The diagnosis was a neck strain due to the repetitive jarring of the hauler.

EE was loading muck bucket with 630 Mucker rock fell off of mucker dipper striking him on 3rd finger of his left hand.

An unintentional roof fall occurred in the 2nd right panel off 5th North Main (2A) left side return aircourse entry #7 @ 5+25 (crosscut 7). The fall did not affect ventilation, passage of men or cause personal injury. The fall area had been roof bolted with 72"" fully grouted roof bolts.

Extra drill steel fell and hit our employee on the back of the head and left shoulder.

Employee states he was installing a resin roof bolt. He bent the bolt, inserted the glue tube, then pushed the bolt in the roof. He failed to completely insert the bolt past the bend, and raised his boom and started to spin the bolt prematurely. The bolt plate spun around and cut his left elbow. The cut required 5 sutures.

An unintentional roof fall occurred on the 5A travelroad at 52 crosscut on the 8th West panel. The fall measured 25 feet x 20 feet x 6 feet high. The fall had been bolted with 6 foot fully grouted roof bolts. The fall did not affect ventilation or cause personal injury. The fall was in the travelway so the secondary escapeway had to be re-routed.

EE was riding cage out of shaft had his hand on the outside causing it to get smashed.

Struck by falling object

Employee states he was putting a metal cover on a continuous mining machine when a rock fell from between the bolts and struck his right hand. This caused a laceration to the top side of his hand and required 8 sutures. This is a medical treatment injury (beyond first aid).

While shoveling the 5C head his shovel got caught in belt and kicked back. When the shovel kicked back it dislocated pointing finger on his right hand and tore the tendon. Employee continued to work until 7/11/11 when he went off work for surgery on his finger.

The employee (a Mine Examiner) states he was walking on the back side of the 4th North belt when he tripped over a water line coupling, fell, and struck his forehead against the belt structure. The injury required 12 sutures.

EE was running 630 Mucker when bullhose blew apart and caused dust. Ee was dodging bullhose and landed under mucker bucket when it fell and pinned him.

Employee states he was roof bolting, and stuck his drill steel wrench in the roof. He hit the wrench with a hammer and the wrench fell from the roof smashing his left little finger between the wrench and the drill pot. 3 sutures were required.

EE was assisting with grinding, Employee felt like he had something in his eye, employee stated he did not know if it happened at work or at home.

An unintentional roof fall occurred on the MMU 011 working section at station 7+35 Entry #8 intersection and the crosscut between Entry #7 and #8. The fall measured 50 feet x 19 feet x 9-10 feet high. The fall did not affect ventilation, passage of men or cause personal injury.

Employee states he was helping get a continuous miner out of a cut when the machine was hung up. He states he was tossing crib blocks under the tracks of the machine, when a rock fell from beyond supported roof and struck him on the back and left arm. The employee suffered a laceration of the left upper arm (22 sutures) and a fractured left wrist.

Employee ( a roof bolter operator) was waiting in a cross-cut outby the face loading his boom tray with supplies. He said he started to walk toward the back of the machine when a small rock fell from between the bolts, struck the front of his hard hat. The front brim of his hat cut his forehead requiring 6 sutures.

EE stated he hurt his leg will walking the overland belt. On 12/15/11 the ee had back surgery. The operator is reporting this as an illness because the surgery repaired degenerative issues and does not have an event problem.

An unintentional roof fall occurred @ 52+50 (x-cut 86) on the 5B travelway. The fall measured 20'x 22'x 7'. The fall did not affect ventilation or cause personal injury. The fall did affect passage of men because the fall was in the secondary escapeway. The stopping line had to be set over to make a new travelway.

Employee was shoveling on the belt line when a rock approx. 2 feet around and 12 inches thick fell from the roof and struck him on the head causing strain and pain to his neck.

Employee and others were hanging miner cable early in the shift. Employee is not certain of the time of any injury, but felt pain in his groin later in the day. Employee was diagnosed with an inguinal hernia from repetitive motion of entering/exiting the cab of his hauler and hanging cable. Employee was released for work, but began missing 3/12/2011.

Employee is a roof bolter and stated he had a rock fall on his ATRS. He was preparing to clean the rock, turned toward the rear of the machine, when a rock fell from between the bolts and struck the back of his head and shoulder. He fell forward and landed on his left hand, fracturing the tip of his small finger and requiring 2 sutures.

An unintentional roof fall occurred on the 2nd West,station 63+73 in entry #4. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30 feet x 20 feet x 7 to 8 feet high. The intersection had been bolted with 6 foot fully grouted roof bolts, approximate date of development was 5/20/09.

Employee stated that he went to kick battery breaker off to lock and tag out for next shift. It was hard to kick off so I pushed real hard, something popped and burned in my left shoulder. Employee later went off work to have surgery. While off work, he had surgery on both shoulders.

An unintentional roof fall occurred at 75+30 in entries 1 and 2 in the 4H panel. The fall did affect ventilation because it took out a stopping between the intake and return. The fall did not affect passage of men or cause personal injury. The unit has been idle and is in the proccess of moving. The fall measured 100 feet x 18 feet x 4-6 foot high.

Injured cannot point to a specific incident. He states that due to the repetitive work of his job(mechanic), his right shoulder started hurting over time and kept getting worse. His Dr. took him off work to rest his shoulder on 04/08/2011.

Employee was driving a small mantrip into a crosscut and attempted to manuever past a coal hauler battery when his left front tire contacted the left rib. This caused the steering wheel to jerk and spin suddenly. The spoke of the steering wheel struck the top of his hand. A subsequent MRI indicated a fracture, and the employee was taken off work.

Employee stated he was operating a diesel ""mini-trac"" to get rock dust on the 4H road. He pulled the machine into a cross-cut as he thought the engine was getting hot. He dismounted the cab, started to walk toward the rear of the machine when he stated the radiator cap blew off and hot water sprayed him in the face and left arm. He received medical treatment beyond first aid.

This was a unintentional roof fall in a X-cut between the travelway and left return entries. We had to stop production, call MSHA and a 103 K issued. MSHA then modified it to timber off all approaches and another modification to re-route the lifeline and build stopping to re-route the travelway. Employee states he was beside the feeder standing a timber up when he slipped and fell striking his elbow against the feeder or the ground. He was treated at a medical center and taken off work with a contusion.

Employee stated he was relieving the left side bolter and had finished installing one bolt. He then was walking back to the controls to move the machine up, when a rock fell from between the bolts and struck him on the back of the hard hat and neck and slid off of him. Transported to ER where X-rays showed a fracture of the posterior spiny member at C-7 vertebrae.

Employee stated he had lifted up the Cont. Miner trailing cable to get it over against the rib out of the roadway more. As he was walking the cable over he stumbled on some loose gob and fell toward the rib. He stuck his arm out to catch himself from falling into the rib and felt sharp pain in his shoulder.

Employee has had previous back pain and on this night he was working on a battery breaker box lid, putting it back on. Lifting the lid and positioning it caused his back to hurt more. He received medical services and taken off work for a MRI. Received the MRI today the 23rd, which showed lots of degeneration of several disc and arthritis (Illness).

Employee stated he was Rockdusting on a conveyor belt line when he turned around to walk out of a x-cut and struck his head on a brow injuring his head/face and fracturing his top rib. He was treated at a medical center and released.

Employee states he was exiting the cab of a scoop when his right foot slipped in mud and reported pain in his right knee. Employee has continued to work, but with pain. Employee was taken off work on December 1 for arthoscopic evaluation and according to the operative report, a torn Chondral Flap was repaired.

Employee stated he had prior shoulder problems and when he pulled on a ventilation curtain it started hurting worse. The MRI shows Tendinosis, Joint Arthrosis. This is an illness that's caused by repetitive motions. This doesn't just develop in a few days.

Employee stated he was cleaning on a belt drive take up when a rock fell from the roof, striking him on the head and knocked him to the ground hurting his knee and low back.

Employee stated she was getting out of the cab of a Ramcar, when she apparently got her leg caught on something, which caused her to fall to the ground. She really didn't know what happened. She was treated in the ER for a leg injury and transfered on to other medical facilities. Injury was a severe (Hematoma) that had to be drained.

The injured employee reach in to the controls to swing the boom of the bolter, when a rock fell from the top and struck employee driving him under the boom of the bolter. His right leg was broken in two places below the knee.

Employee was in beside a Ram Car, in a sump, washing the machine off. He finished what he was doing, shut the water of and threw the water hose and valve up on the bank. The handle to the shut off hit just right and caused it to open. When this happened it sprang back under pressure and struck the person in the mouth. He's off work, lost 6 teeth.

Employee stated he was operating the Cont. Miner and had just set the machine over in the cut and was waiting on a car to come in to load. He stepped up beside the machine and looked back to watch for the car coming in, when a piece of slate slid off the top on the machine and struck his foot. Employee stated while bending a 6' roof bolt to get it in the hole, he pulled something (muscle) and the pain went into his back also. He worked the rest of the shift and stated there was even discomfort pulling drill augers out of the holes. He was treated by a family dr. and taken off work.

Employee was attempting to drive his vehicle around a parked Shuttle Car that had been left out in the middle of an intersection. There was gob on the opposite corner of him and when his front tires hit this gob, it caused the vehicle to slide sideways into the car. The victim was pinned between the dump end corner of the car and the transmission housing.

Employee stated he was standing in a notched in place, when a rock fell from between the roof bolts and struck him on the head and shoulder. Employee has continued to work until he was taken off until a MRI is done. Last day worked was 03/19/2010.

Employee stated that he was carrying pin boards to the scoop when he stepped in a uneven place in the mine floor and fell on his knee. He continued to work for a couple hours and asked to be taken to Med. services. He then was taken off by his family doctor.

Employee was working in the Battery barn and was letting the lid down on this battery. He unhooked the rod that held it up and the lid fell shut. When it slammed shut it threw a small rock up and it struck the victim on his front tooth. It broke 3/4 of the tooth off.

Employe was trying to pull scubber door shut by pulling on chain attached to door. He grasped the chain with both hands to pull door shut. He felt pain in his stomach and belly button as he pulled the door shut.

He continued to work and then had a MRI done. His doctor took him off work on 12/16/2009. MRI showed a prior ACL surgery and degeneration throughout the meniscus.

Employee was changing batteries on 516 scoop. He was trying to pick up the charge battery, when the jumper cable got pinned between the scoop and the battery. The employee then tried to remove the jumper by hand. The cable arced and the employee's left arm was burned.

Employee was drilling a hole in the roof with a roof bolting machine. As he was retracting the drill steel out of the roof a piece of rock slid down the drill steel causing a laceration to the his right upper arm. He did not miss anywork but did require stitches.

Employee was drilling a hole in the roof with a roofbolter when a piece of rock broke loose and slid down the drill steel. The rock flew off of the drill steel and struck employee in the mouth causing him to receive stiches in his bottom and top lips.

Employee was using a 3/4"" drive ratchet taking bolts out of a Wheel Unit. The ratchet malfunctioned and broke loose while he was applying pressure. When this happened, he said both wrists started hurting. He was seen by a specialist and diagnosed with Carpal Tunnel. He started missing work on 10/27/2009.

Fall onto or against objects

Employee was unloading a big bag of rockdust when the corner of the bag gave way causing the employee to fall backwards onto the scoop. This caused a injury to the lower lumbar area.

An unintentional roof fall occurred on the old 1A return @ station 31+80 between entries 9 and 10. The fall did affect the ventilation on the #4 unit which was idled until all work was completed and ventilation re-established. The fall did not affect passage of men or cause personal injury. The fall had been bolted with 6 ft. fully grouted roof bolts and 12 foot cable bolts.

The fall measured 40 foot x 19 foot x 8 foot. The area had been supported with 6 foot roof bolts prior to the roof fall.

An unintentional roof fall occured on the 4C travel road at 46 crosscut. The fall did not affect ventilation or cause personal injury. The secondary escapeway was re-routed around the roof fall area. The fall measured 30 foot x 30 foot x 12 foot high. The area had been bolted with 6 foot fully grouted roof bolts and 12 and 16 foot cable bolts.

Employee was walking to location to monitor CM tools on Joy 14-15 miner. While en route, he experienced a severe cramp in his left foot. He noticed he was walking differently and went to the doctor on 7/1/09 who said he injured his L5 vertebrae.

The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20foot x 20foot x 6foot high. It occurred in an 4-way intersection and will not be cleaned up.

The fall did not affect ventilation, passage of men or cause personal injury. The fall occurred in a 4-way intersection. The fall had been bolted with 6 foot roof bolts.

Struck against a moving object

Mantrip slid into hole while employee riding in back, bounced him up, he landed on rescuer, pain left hip/lower back. Dr put him on physical therapy, didn't miss any work from accident, later sent to receive spinal injections, injection sites began to leak fluid, patches placed over holes,started missing 9-14-09. patches applied on 9-16-09. We feel this qualifies as an illness.

Employee stated that he was having some tingling and numbness in his right hand for quite some time. After years of repetitive use of the domanant hand he was diagnoised with Carpal Tunnel Syn. and started taking off work on 06/22/2009.

Employee was removing a conveyor (belt) cutter after cutting the belt for splice installation. As he removed the hold down the other employee was cranking the cutting blade out of the way and the injured employee got his little finger under the lip and in the way of the knife being moved to the side. The blade struck his little finger and caused a severe laceration to the tip.

Employee stated he was walking & apparently slipped & said he hurt his lower back. After investigation, he has had back problems for some time according to him. CT Sacn shows mild bulging disc which everyone has. It also showed degenerative spurring, spinal stenosis & degenerative facet. Also Arthritis.
