EE WAS STANDING TIMBERS. HE TURNED AROUND & HIT A PIECE OF WIRE HANGING FROM A ROOF BOLT.
THE EMPLOYEE STEPPED OFF MANTRIP AND ALLEGEDLY PULLED MUSCLE IN LOWER BACK.
THE EE WAS RUNNING THE MINER BY REMOTE CONTROL, CUTTING COAL ON HIS RIGHT SIDE, WHEN A SHUTTLE CAR STRUCK HIM ON HIS LEFT SIDE.
FAULTED AREA FELL OUT AT 14 & 00 E3 2ND EAST PARALLEL TRAVEL ROAD.
THE EE WAS STRUCK BY A FALLING ROCK, WHICH FELL OUT BETWEEN ROOF BOLT AND RIB LINE.
HIDDEN SLIP CAUSED FALL AT #17 X-CUT 2ND MAINS EAST ENTRY #2.
