Employee was operating a CAT 980H shipping loader when at approximately 7:40 pm he asked to talk to his foreman. The employee climbed down the ladder and when he stepped onto the ground he felt a burning sensation in his foot. The employee was diagnosed with a foot strain on 4/27/06, and at his reexam was placed on restrictions on 5/2/06.

Fall to the walkway or working surface

Rainstorm and muddy conditions resulted in employee slipping while in the process of erecting a tarp over work area. The employee sustained a back strain from the slip and fall.

Worker, with the help of a co-worker, was manually pushing a stacker conveyor when he felt a pop in his right heal. Worker snapped his achelies tendon in two.

EE WAS DOING A PRE-SHIFT INSPECTION OF A LOADER.AS HE WAS LIFTING THE COVER OFF THE LOADER HE SLIPPED AND STRAINED HIS LOWER BACK. HE WENT TO THE CLINIC FOR TREATMENT AND WAS RELEASED TO COME TO WORK WITH RESTRICTIONS.

WHILE EE WAS CLEANING FROZEN MATERIAL FROM A CHUTE UNDER THE JAW, USING A SHORT PRYBAR AND HAMMER, HE FELT PAIN ACROSS HIS UPPER BACK. ON 2-5 HE SOUGHT MEDICAL ATTENTION AND WAS RETURNED TO WO RK WITH RESTRICTIONS. ON 2-15 EE WAS RELEASEDTO FULL DUTY STATUS.
