Struck by falling object

Oil leak was noticed during operation of the end loader. Foreman instructed operator to block the machine & went to get oil. Returned & found EE under the machine looking for the leak. Loader had been raised by tilting the bucket on the ground. Foreman got under machine to help look. Machine came down on top of them shortly after that, just as foreman saw that it was not blocked.

Employee was pushing a steel belt bar into a roll of belt with left foot when they experienced pain in left calf. The injury resulted in a tear to their Achilles tendon.

Struck by flying object

Employee was assisting with unrolling conveyor belt when at the end of the roll the chain holding it came loose allowing the bar to fall and strike EE's left hand resulting in a laceration. The injury resulted in 13 sutures.

Employee was tightening the compression ring on a bearing with a co-worker. The co-worker was using a punch hammer and sledge hammer when a piece of metal broke off of the punch hammer and went into the injured employee's left arm.

Strands were found broken on the regular lay slope hoisting rope. Rope was taken out of service and corrected. This was the secondary escape way and was rerouted underground to another hoist until rope was corrected.

A methane ignition occurred on the 12WLW MMU-059 at approximately 7:45 pm on 06/08/2016. The ignition was on the headgate drum while shear was mining to the headgate end. This 7000-1 is being submitted to terminate citation #9103414.

Employee was going through man door with hand tools in both hands when he noticed the door was about to slam shut. He tried to stop it and got his finger caught between the door and the frame. He received 10 sutures.

An unplanned roof fall occurred at the 1585' mark in the 10 west tailgate entry number 2. The area had been mined in October 2012 and the fall occurred in a three way intersection. The area had previously been supported with 8' torque tension bolts in t-3 channel as the primary roof bolt with 12' cable bolts between the existing rolls of the primary bolts.

EE was operating a continuous miner on MU 3 section finishing the clean up in 4 right. According to unit EE's the injured EE was under supported top when found. The injured EE was concentrating on the clean up prior to the accident. Apparently a rock from the mine roof located at 4C fell out bounced off the floor then flipped over striking EE.
