Employee was welding on a chute at the stoker loadout. Employee was in a basket attached to a CAT telehandler. The basket was raised six feet off the ground when it unattached from the telehandler falling to the ground. Employee sustained a fracture to the left tibia.
Employee changed oxygen tank at a permanent station. Attached hose to tank proceeded to open valve on oxygen tank. The regulator failed and blew apart the hose attached to the tank came off as well. Employee was struck by debris from the regulator. Laceration to the left cheek and left orbital fracture.
Employee was setting timber when they bent over to pick up wedges a timber fell striking them on the head above the left ear.
Alleged repetitive trauma to both arms and hands, carpal tunnel.
Carried coats to plant, set them on a belt crossover. Turned to leave area when right foot caught a small pile of coal fines. Employee fell to the ground (same level) onto their right hip and elbow.
Employee approached and entered equipment doors at the slope bottom to exit the mine. Employee closed one door and turned to find the other door near them closing. Employee reached out to catch the door. Another employee was already inside the air-lock opened the out-by set of doors to exit the mine.
EE was unrolling felt liner and picked one end up to go over the dirt pile. EE felt a twinge in left shoulder. EE returned to work but did seek treatment on 01/07/2023. The x-ray showed no fractures or significant findings. Noted and EE returned without restrictions.

Individual was cleaning and raking along MN4 beltline XC96 when they tripped on wire rope falling to the ground. Put their arm out to catch themselves causing a fracture to the Greater Tuberosity.
Helping change ram car batteries. The safety chains were tight, so the car pushed the battery into the rib to take tension off the chains. I.E. reached in to loosen the chains when they became tight again pinching their right index finger between the battery and the safety chain. Caused a fracture and laceration that required stitches to the I.E's right index finger.
Employee was helping another start the face scoop. Employee sat down in the cab and leaned forward to pull on the breaker handle when employee felt a pain in lower back. Through company investigation, information received January 6th, 2023 it was determined this incident need be reported.
A roof fall occurred at South Submain #1, Entry #4 in the intersection of crosscut XC #20. There was no injury or property damage with this occurrence.
2LMN Unit Intersection 10+50 1R XC Actively bolting a brow on a blow through that was 4 to 5 feet in length. The brow broke up causing multiple pieces of shale to fall. Employee was inserting pin into previously drilled hole when employee was struck on the back and legs knocking employee to the ground.
A roof fall occurred at NSM XC44 Entry 5. There was no injury, a ram car that was out of service was partially covered with material from the fall.

A roof fall occurred 1RMS XC 31-1/2 Entry 1. Entry had a significant clay vein running with the entry. There was no injury or property damage with this occurrence.
A roof fall occurred along 2L #2 beltline XC16 Entry #4. Entry had clay veins running with the entry. Additional clay vein in the crosscut joined the first in the intersection. There was no injury or property damage with this occurrence.
A roof fall occurred along 1R #2 beltline XC #9 Entry #2. Entry had a clay vein running along right side rib line through the intersection. A second clay vein entered the intersection from the left rib joining the first forming a cantilever in the intersection. Rubble from the fall compromised a Kennedy stopping. There was no injury or property damage with this occurrence.
While removing top structure from overland belt the employee dropped the structure pinching right thumb between the structure and structure railing. This resulted in a fracture of the right hand.
An established intersection with no limestone present and bolted with fully grouted 8' bolts, had a clay vein measuring from 5' to 10' thick through the middle of the intersection. The roof failed along the clay vein. Initially discovered at 2:03 pm and verified by management at 2:37 pm.
While helping build a solid block wall bumped EE's left elbow. There was no visible wound, swelling or discoloration at the time. Reported swelling and inflammation 4/29/2021 and sought medical treatment for cellulitis. Began missing work after Diagnosis of and treatment for infection due to Septic Bursitis 4/30/2021.
While reaching over a ram car to tighten a battery plug, felt a pop below EE's left calf muscle and EE's leg began cramping. Went to Midwest Occupational Health Associates the following morning and was released to unrestricted duty. Became Lost Time after hospitalization 3/17/2021 with pulmonary edema due to a blood clot from the strained area.

While picking a piece of 1/4"" steel plate measuring 17"" X 25"", and taking a couple steps, felt intense pain in EE's right upper arm. X-rays revealed that EE's right shoulder was dislocated. The shoulder was put back in place and EE's arm placed in a sling. EE began missing work Thursday, July 23, 2020.
While walking near a co-worker setting 6' props, was struck on top the right foot when a prop fell unexpectedly. The impact caused fractures to 2 Tarsal bones in EE's right foot. Treated and released to restricted duty.
While getting out of a tractor, hit head against the top in an area of low height. EE stumbled in the cab and fell, twisting left leg and felt a pop in left knee. EE continued working. A follow-up MRI revealed damage to the left knee that required surgery. EE began missing work when the procedure was performed 5/11/2020.

A roof fall occurred at XC#75 MS#13 belt line. The belt had been removed and was under planned rehabilitation. Personnel had withdrawn from the work site. There was no interruption to production. There was no injury or property damage with this occurrence. The area has been cleaned and bolted according to plan.
A roof fall occurred along MW#1 belt, XC #31/station 35+45, entry #0. Limestone had 2 slips that contained quartz, 1 in the middle and 1 larger slip in the heavy side of the cantilever. There was no injury or property damage with this occurrence. The fall was approximately length 18' X height 11'.
While roof bolting, EE's drill steel became clogged. As EE reached to remove the steel, shale fell from the roof hitting EE's hand and driving EE's thumb into the drill pot. The impact caused a contusion to EE's right thumbnail. After shift EE went to an urgent care facility where X-rays revealed a fracture to the tip of EE's thumb.
While using a pipe wrench and attempting to remove a clean out plug on a continuous miner de-mister, knocked a rub rail off striking left shin. EE reported the incident as minor and insignificant 7 days later. EE sought medical attention 11/05/2019 where EE was placed 'off work' till further notice. Additional information has not been provided.
A roof fall was found at 08:00 AM in a 3 way intersection of entry #5 MS10 XC14 station #22+84.95 intake travelway. No injury or property damage occurred, travelway is being re-routed and access was propped off. Fall was approximately 30'x20'x10' area was mined in Oct. 2002.
While moving miner cable, felt a pinch in lower back. They went to pick up cable again and felt additional pain in back and into legs. They called for help to get out of the mine. They returned to work without restriction. They contacted a physician after allegedly aggravating the injury while sweeping with a broom and began missing work 10/25/2019.
While making a belt splice and preparing to loosen tension on a chain come-along, EE had left index finger pulled into the come-along when the free end of the chain moved unexpectedly. The pinch caused a laceration to the left index finger that required sutures to close.
While making a belt splice, the splice board was sliding off the platform. As EE reached to push it back onto the platform, EE's partner inadvertently struck EE on the left ring finger tip with a splicing hammer. The impact caused a fracture and lacerations that required sutures to close. Released to return to work and reported for next scheduled shift.

While walking ran EE's head into a low brow. EE experienced pain in EE's neck, right shoulder and right arm. EE sought medical treatment 12/6/2018 and was diagnosed with cervical disc disorder and released to perform all job functions and work with caution. Began missing work 7/22/2020 after C4-5 cervical disc replacement surgery.
While installing a 6' bolt and holding the bolt as the pot was raising, the bolt bent, pinching the right thumb at the base of the nail. The pinch caused a laceration of the nailbed and fractures to the finger. Employee was treated and released to restricted duty, no use of right hand.
While roof bolting, reached to remove a plugged drill steel from the roof. As they reached, shale and rock fell around the steel, knocking the injured to the ground. The rock and drill steel struck their right calf and left a laceration that required sutures to close.
At the completion of a job, EE's left arm got caught by a belt roller on the plant feed belt causing abrasions. EE attempted to knock belt skirting down against the belt with a hammer and missed, allowing coat sleeve to be caught between the top roller and the belt. Transported to Springfield Memorial Medical ER for treatment where EE was admitted for observation.
Our employee is a truck driver. Had just finished dumping coal, went to walk back to push in tailgate valve to relock the tailgate, when employee slipped in mud and fell on left side. Driver did not wish to seek medical attention immediately. Thought had just bruised side.
While roof bolting and removing a steel from the roof, employee was lining the steel up with the drill pot when a piece of shale struck employee's hand mashing it against the pot. The impact caused a fracture and laceration to the right index finger.

While preparing to start the roof bolting machine, was struck on the head and left cheek by falling shale. The shale, approximately the size of a dinner platter, struck the edge of the canopy as well, allowing the sharp material to bounce into the operator's left cheek and leaving a laceration that required sutures to close. EE was treated and released to unrestricted duty.
While standing on the roof bolter and reaching to connect curtain to an S hook, had a sharp pain in lower back and down both legs causing body to jerk. Had experienced similar pain for 2 weeks and sought chiropractic treatment along with the use of an inversion table. Diagnosed as sciatic inflammation, work related illness.
While roof bolting, EE helped move a piece of shale that had struck EE's partner. The shale then flipped over while moving and struck EE on the right ankle. Initially reported as a first aid only 10/09/2017, EE began missing work after an MRI detected an osteochondral defect ankle sprain and corrective surgery was performed 01/29/2018.
While roof bolting in old works, a piece of shale fell and struck employee on the head. Employee felt a sharp pain described as a ""Stinger"" in the neck and shoulder. Employee did not seek medical attention until the following day. Initially reported as a first aid only on 10/2/2017, became a lost time after surgery to repair herniated disks 2/12/2018.
While inspecting a work area and trying to pull a rib down, was struck on top of the right foot by the rib when EE couldn't move away fast enough. The material landed on top of EE's tarsal guard resulting in a closed fracture to a tarsal bone. Restricted to sitting work only until EE's next evaluation.
While cleaning a belt drive with a rake, turned and struck left elbow against a steel I-beam. EE continued working and finished the shift. Diagnosed as a bone chip type fracture along with a ruptured bursa sack in left elbow. Limited to a ""No use of left upper extremity"" restriction.
A non injury roof fall was found at 7:00 AM on 8/8/17 at XC 4 entry 3 2rMS #1 belt. The fall was in a four way intersection that had been mined December 2014 and supported with 8' resin bolts. The fall will be cleaned up upon approval of clean-up plan.
The 20 person elevator at Williamsville intake shaft became inoperable due to wiring issue, all underground personnel were brought out of mine until car was back in service at 2:50 PM
A roof fall was found at 10:20 PM in a four way intersection of entry #4 SS 61+20 of the 2RMS (MMU 006). No injury or property damage occurred, travel-way was re-routed and access was propped off. Fall was aprox. 20'X10'X20' area had been mined June 27th 2017 and bolted with 8' full resin pins.
The 20 person elevator was not operational due to brake issues, repairs were made and mine resumed work at 10:15AM
While exiting a tow tractor, slipped in an area of wet conditions. EE grabbed the tractor to keep from falling and felt a sharp pain in EE's left arm and shoulder. EE completed EE's shift before reporting the incident and continued working. Initially reported as first aid only, began missing work due to cervical fusion surgery on 08/15/2018.
While roof bolting drilled the first four feet and grabbed a four foot extension. While they were lowering the drill pot they had the extension in their right hand when it caught beneath the pot and on top of their right foot. The steel poked through their boot and into their fourth and fifth toes. X-rays revealed a fracture to each of the toes.
The injured was making an exam as another mantrip was trying to squeeze between the examiner's ride and the rib, catching the ride and bumping it forward. The bumper struck EE's left knee. EE continued working, finished EE's shift and did not seek medical treatment. Became a lost time after receiving a ""no work"" restriction 8/17/2017 pending surgery to repair a meniscus tear.
The 20 man elevator was out of service from 11PM on 4/30/17 until 2:30 AM on May 1st. A shorted wire in junction box in sump pit plus an open wire in trailing cable was found and repaired to put car back in service.
A roof fall was found on 3/26/17 at XC 13 entry 5 8L #6 travel-way. Fall was in a four way inter section that had been supported with 8' resign bolts. Fall was approx. 20' high X 20' long X 10' no pro or injury resulted. There are no plans to clean up the fall.

While riding in a mantrip was bounced off the seat when the vehicle ran over an unknown object. The injured experienced pain prompting a first aid only report 8/25/2016. Employee continued working without restriction until they began missing work due to surgery 3/23/2017.
While walking down the stairs at the refuge bin slipped on a step and caught self with both hands on the handrails. As ee did so ee felt a pop in left bicep that caused a burning pain. Diagnosed as a left bicep tendon tear and restricted to no use of left arm pending additional treatment.
Operator was standing between the second and third row of pins when shale fell and bounced off the miner and slid, striking the operator in the right shin and foot. EE did not seek medical treatment. Swelling and pain prompted EE to seek treatment June 22, 2016 when X-rays revealed fractures to right foot. EE returned to work with a sedentary work only restriction.
While exiting the plant due to an impending thunderstorm the employee was struck on the left cheek by an airborne piece of broken fiberglass siding. The impact left a laceration that required sutures to close. This was a weather related incident and act of God. EE was released to unrestricted duty.
A non injury roof fall was found during exam at XC 21 entry 4 along 8L #6 travel-way. Fall was in a 4 way intersection that had been mined January 2013 and supported with 8' full resin pins. There are no plans to clean up this fall travel-way has been rerouted and rehab work is being completed according to approved plan from MSHA
While roof bolting shale fell from the roof and struck EE on top the head driving hard hat into EE's glasses and causing lacerations to nose and forehead. 6/13/2016 Orthopedic Specialist diagnosed 2 herniated discs allegedly associated with this occurrence that were previously determined to be degenerative in nature as related to this incident.
While exiting the water truck and closing the door was holding the handrail with left hand when the mounting bracket for the rear view mirror attached to the door struck left thumb. Treatment included removal of the nail, suturing of the nail bed and re-attaching the nail. EE returned to unrestricted duty on next scheduled shift.
While recovering belt material and removing belt sail channels from the roof had the belt sail channel move unexpectedly. This movement pinched left index finger between a steel plate inside the channel and the Dewey Dag nut holding the sail up. The pinch resulted in a laceration that required sutures to close. EE returned to work the following shift to unrestricted duty.
While working and loading beams onto a belt caught ee left thumb between the beam and a standing prop. EE suffered a laceration at the base of ee left thumb that required sutures to close. EE was treated and returned to unrestricted duty finishing ee regular shift.
While roof bolting and drilling to install outside pin the operator's steel plugged. After clearing the steel and resuming drilling ee was struck in the face by a piece of falling shale leaving a laceration between lip and nose that required sutures to close. Treated and released to unrestricted duty.

Employee claimed that over the last year and a half employee's arms and hands were getting worse, burning, going numb, locking up and hurting to a point employee couldn't go to sleep at night affecting employee's ability to do work safely. This was the first report of any symptoms.
Roof fall was discovered in a four way intersection of entry #4, xc 41 on the main west #2 travelway. The area had been mined in August, 2001. The roof was supported by fully grouted resin bolts ranging from 4 to 8 feet. No injuries resulted from this occurrence. The fall measured 20'X20'X10' high and did not reveal any obvious condition that may have contributed to failure.
Employee reported a pop in EE's right shoulder while lifting CM cable. Claim was initially denied, then ultimately determined to be work related illness due to repetitive trauma. Began missing work 12/7/2015 and has not returned.
A roof fall was found on 8/16/15 at 6:30 AM in a four way intersection at XC 65 entry 4 main west. Area had been mined July 21st 2009 and supported with 4-6' full resin bolts. No injury resulted. A slip above bolt anchorage was reveled and likely contributed to failure. Fall measured 54' longX30' wide X15' high.
A roof fall was found in entry 6 XC 70-71 west intakes. Area was mined in August of 2009 and had been supported with 4-6 ' resin pins. No injury or property loss occurred. There are no plans to clean up fall. Travelway is being rehabbed in entry 5 and will serve as the new primary escapeway. Fall was approximately 35' long X 20' wide X 8' high.
While rushing to get into a ramcar caught his foot and fell forward into the opening. As he tried to catch himself struck his left arm against the plate behind the seat bruising his shoulder. X-rays were negative but he was placed on a no use of left arm restriction due to pain.
While working on the water pump stood up and leaned against a tool box. The box's lid fell closed and pinched his right thumb causing a laceration that required sutures to close. He was released to unrestricted duty and returned to work his next scheduled shift.
While attempting to loosen a bolt on a ram car speed reducer using a ratchet and a cheater bar had his left middle and ring finger tips caught between the ratchet and frame when the wrench slipped. The pinch caused a laceration that required sutures and a fracture to the tuft of the middle finger. He was treated and returned to work to his assigned duty.
Treated for Cubital Tunnel Syndrome by surgery 1/5/2016. Accepted as compensable by workers compensatation 1/9/2018.
Upon examination it was found that the 20 man elevator at the Main Streets portal was inoperable. Maintenance was notified to make necessary repairs consisting of replacing the lower limit switch and adjusting the linkage on the governor rope. Elevator was back in service at 6:35 AM
A roof fall was found on 3/14/2015 at entry 7 at survey station 35+70 in 8L intake, MMU 010. Area had been mined 2/20/2015 and was supported with 8' full resin pins, 4 additional 10' cable bolts had been installed in intersection. Two significant slips likely contributed to failure. No injury or property damage resulted, no plan to clean up fall.
Our driver slipped and fell on ice in the parking lot by the stoker scale at Viper Mine. His right leg bent under his body when he fell and he landed on his right foot. Driver thought he was just sore and continued working, but went to the hospital the next morning and found out he had broken his ankle. Driver is off work but expected to return within a few weeks.
While loading 8 ft bolts onto a bolter felt pain in his left shoulder. He continued working, finished his shift and used vacation days not reporting the incident until 1/23/2015. X-rays were inconclusive and he was referred for an MRI. Became lost time when an MRI report was made available 2/4/2015 revealing a superior and posterior labral tear of the left shoulder.
While going to her car in the parking lot slipped on ice and twisted her left ankle. EMTs applied ice to the ankle. She then went on her own to Taylorville Memorial Hospital where she was diagnosed with a fracture. She has not returned to work.

A roof fall was found at 2:31PM on 11/3/14 at xc 58 entry 3 main west #2 belt. Area had been mined June of 2006 and supported with 8' full resin pins. Fall was in a 4-way intersection and measured 20'X20' X8 high. No injury resulted fall will be cleaned up and re-supported according to approved plan.
Employee provided no description of incident. Claimed cramping of hands and forearm and electric feeling from hands to shoulders. Determined by medical examination that it could have been work related. Work assignment was a mine examiner with use of hands limited to driving and writing. Sought treatment 12/30/2014 with Carpal Tunnel release surgery. Accepted as work comp 4/15/2015.

While roof bolting and having already installed the first two bolts of a row, had drilled the inside hole, inserted the glue and turned to get a bolt when the operator was struck by shale on the back of his left leg. His leg was splinted and he was transported to the surface. Diagnosis revealed fractures to the left Fibula and Tibia that will require surgery to correct.
A lightning strike damaged the Williamsville sub station knocking power to the mine fan and elevator for primary escape way. All personnel were evacuated to secondary slope escape way. Repairs were made and elevator was operational at 7 PM on 8/29/14.
While roof bolting and adding a 27"" extension was struck on the left hand by falling shale leaving a fracture to the left little finger and a laceration to the left index finger that required sutures to close. He was treated and returned to work with a ""no use of left hand"" restriction.
After filling the surface dust tank, he was struck in the left abdomen when a 2"" air hose came loose at a fitting. Diagnosed with a bleed from the spleen, corrected by surgery.

Removing 1 of 2 covers above pump motor compartment, 1 hand on the side of cover, the other hand on 1 of the handles on top of cover. He pulled cover to him, lost his grip with his left hand, right hand was still in handle and twisted his arm. Released to go back to work next day. Primary doctor's opinion was he needed therapy and could return back to work 3/28/14 or sooner.

employee was working on scoop when rock fell from between roof bolts and struck him on the back of head/neck causing small laceration. 3 stitches were required
While bolting was struck by a roof bolt. The bolt was assembled and laid across his drill mast. While inserting his wrench a piece of shale struck the end of the bolt causing it to flip and strike his safety glasses. Two parallel lacerations in and below the left eyebrow required sutures to close. He was released to unrestricted duty reporting to work his next scheduled shift.
While helping to pull a loop of miner cable felt his stomach tighten. He continued working and didn't think anything of it, took his belt off later and did not feel anything. After he went home and was using the bathroom observed a bulge above his left groin. He completed this incident report on his next scheduled shift. After being seen by a specialist began missing work 9/9/2013.
A roof fall occurred at 10:05 AM at 10+50 entry 6 in 8LMS MMU 010. Fall was in a four way intersection that had been mined May 8, 2013. Fall was 18' wide X 18' long X 8' high. Fall was caused by a transition zone that went from 4' pins t o8"". No injury resulted.
While moving a piece of a sulfur ball from on top of a 4"" waterline the rock, 24 X 16 X 4, moved much faster than expected pinching his left little fingertip against the roof. The pinch resulted in a laceration to the pad of the fingertip that required sutures to close. He was released to unrestricted duty.
While picking up bolts from the bumper of a truck, the tailgate fell open unexpectedly and struck the back of his right hand. He did not seek immediate attention and worked the remainder of that shift and the next two. Persistent pain and swelling prompted the completion of this report. X-rays subsequently revealed a fracture to the right index finger.
While setting 6' X 6' X 6' cribbing on top of steel sets was struck on the tip of his right thumb when the crib he tossed rolled back unexpectedly. The thumbnail was broken loose and X-rays revealed a fracture beneath the nail. He was treated and released to unrestricted duty reporting to work his next scheduled shift.

An unplanned outage of the Williamsville elevator occurred when the governor rope switch was found to be malfunctioning while doing the scheduled two month hoist inspection. men were withdrawn from the face, and repairers were made. Hoist was back in service by 4:10
While bolting and removing a drill steel while it was rotating looked up to see a piece of shale slightly bigger than a softball slide down the steel and fly toward his face. He was struck on the nose leaving a laceration that required sutures to close and X-rays identified a nasal fracture. He was treated and released to unrestricted duty.
A roof fall was found at xc 58 entry 2 8LMS at 9:30 am, fall was in 4 way intersection had been mined in November 2008. No injuries resulted but a return stopping was knocked out.
While roof bolting and after cleaning a plugged drill steel was re-inserting the steel in the roof when a piece of shale fell striking him on the head. The material knocked his hard hat off and caused a laceration that required staples to close. He was treated and released to unrestricted duty.
While roof bolting and inserting a bolt into the roof was struck by a sharp edge on the pizza pan or plate on the roof bolt after the plate was hit by a piece of falling shale. The contact caused a laceration to his left forearm that required sutures to close. Treated and released to unrestricted duty returning to work on his next schedule shift.
While swinging out the boom on his roof bolter had his hand on the canopy and pinched his left little finger against the coal rib causing a partial amputation. Transported to Memorial Medical Center Springfield where the finger was re-attached.
A roof fall was found by examiner doing weekly exam at c.c. 52 entry 00 in main west. Fall was in a 4 way intersection and was supported with 8' full resin pins. Travelway was re-routed and adjacent area was timbered off. There are no plans to clean up this fall.

The metal was removed by a physician and the wound closed with sutures. He was initially restricted from returning to full duty to keep the area clean and prevent infection.

While bolting a piece of shale fell striking him on top the hard hat. He felt dizzy and nauseated, was treated with a C-collar, back boarded and transported to the surface. No fractures or internal injuries were identified during treatment and he was released from St. John's Springfield where he was instructed not to work and to return for evaluation in two weeks.
While working slipped in mud and fell landing on a large rock and striking the inside of his left forearm against a sharp edge. The impact resulted in a laceration that required sutures to close. He was treated and released to unrestricted duty reporting for his next scheduled shift.
After working in an awkward position carried several 5 gallon buckets of oil to fill the tank and felt pain in his lower back. The pain worsened overnight and he completed a report of incident and sought medical attention the following morning. Placed on a lifting, pushing, pulling and bending restriction until seen again by his physician 11/23/2011.
A roof fall was found on 11/1/11 at xc 58 entry 4 in 8 LMS. Fall was in a four way intersection that had been mined in 10/2008. Fall was 20' W. X 20' L. X 10' H. Roof had been supported with 8' full resin bolts. No injury or property damage resulted. Fall will not be cleaned up.
A roof fall was found 10/14/11 at xc 86 entry 4 in 6R. This was adjacent to travel way in a four way intersection that had been mined 8/2009. Area was bolted with 8' full resin pins. No injury or property damage resulted. There are no plans to clean up this fall. Fall was approx. 20'W.X 30'L. and 10' H.
A roof fall was found by pre-shift examiner at 7:00am on Oct 8, 2011. The fall was in a four way intersection at xc 29 main south, blocking access to #10 main east seal. The fall is approx 20'wide X 100'long X 10'high. Area was mined Sept of 1983, and had been supported with 8' full resin pins. A roof fall occurred at 4:30PM on 9/26/11 at xc 15 main south 10 entry 2. Fall was in a four way intersection that was mined in March of 2002 and was supported with 8' full resin bolts. Additional support consisted of truss bolts. No injury resulted water line and power cable was lost. This fall will be cleaned up in future pending approval from MSHA.
While attempting to change a tire on a bulk dust car and reaching behind the tire to pull it off was pinched through the fingernail of his right middle finger causing a fracture to the fingertip and leaving a laceration that required sutures to close.

While hooking a chain between a scoop and a disabled ram car had his right foot pinched when the ram car moved. He was transported by ambulance where X-rays revealed no fractures yet the removal of damaged tissue and the suturing of lacerations caused this to be a lost time injury.

Area had been supported with 8' full resin bolts. Upon approval from MSHA the fall will be cleaned up and resupported. No injury resulted, but aprox. 50' of belt structure was lost.
The injured grabbed a rope attached to high line being dropped down a borehole. He was struck nearly simultaneously by a piece of wood that had been used to align the cable. The impact caused a puncture and laceration to the back of his left hand. X-rays revealed a fracture and he is awaiting consultation with an orthopedic surgeon.
A roof fall was found at 11:10p.m. on 7/26/11 at c.c. 118 entry 5 in 6R unit. Area had been mined in 3/10 and top was supported with 8' full resin bolts, 4 cable bolts had also been installed in addition to pattern pins. Clay slip present had weakened roof. No injury or property damage resulted. Access to area has been cribbed off. There are no plans to clean up this fall.

At the end of his shift and as he finished showering and was walking to his locker he slipped in water on the floor. His feet went out from beneath him and he struck the lockers with the little toe of his left foot and big toe of his right foot causing lacerations that required sutures to close. While grinding and welding beneath a ramcar tailgate and lying on his back he raised his welding hood and foreign material fell into his left eye. He finished his shift and worked through the next day before continued irritation prompted him to seek medical attention. A piece of material was removed from the eye; he was prescribed medication and then released to unrestricted duty.
While roof bolting along a clay slip a piece of shale fell and struck him on the left foot causing a contusion to the fifth toe. He continued working but after pain persisted he sought treatment at St. Johns ER Springfield where he was diagnosed with a fracture to the toe. After follow up he was released to unrestricted duty.
While roof bolting a drill steel fell against the side of his neck. The steel was hot from use and caused a burn extending behind his right ear. He continued working and finished his shift before seeking medical attention, the wound was cleaned and he was issued a full release. The application of prescription medication made this a reportable injury.
A roof fall was found at 5:40am on 4/4/11 at cross cut 100 entry 4 main south. Fall was 20'wide, 55'long and 12'high. Area had been mined 10/14/1991 and was supported with 5' full resin pins. No injury resulted however 100' of hi-line was lost. There are currently no plans to clean up this fall.
While changing ram batteries, a cylinder lock would not open. He tried hitting the hinge with a hammer. As soon as the hammer struck, the lock sprang open, striking his right thumb. X-rays revealed a non displaced fracture, he was splinted and returned to work without restriction.
While roof bolting and adding a drill extension the extension bowed, sprung free and struck his right hand near the base of the index finger. X-rays revealed a fracture and he was placed on a 'no use of right hand' restriction.

After work experienced discomfort prompting him to seek medical attention. Diagnosed with a right Lingual Hernia. He returned to duty with a 10 pound lifting restriction and desk work only with surgery expected to correct the condition.

After bending down to place a magnet stepped through a hole in floor grating puncturing his left shin. Transported to St. John's ER Springfield for treatment including sutures and staples to close the wound. He returned to his next scheduled shift and has not missed any work.
While setting props and looking up got something in his left eye. At the end of his shift he attempted to flush the eye. EMTs assisted in rinsing the eye but irritation persisted prompting him to seek medical attention after he went home. He was diagnosed with a Corneal Abrasion and prescribed an antibiotic. He returned to work the following shift.
While shoveling felt a pain in his abdomen that descended into his groin area. Transported to St. John's ER and released to follow up with his own physician. Additional medical services were not available until Monday, 10/25/2010 when he was seen by MOHA and diagnosed with traumatic epydidimitis. He was placed on a ""sitting work only"" restriction 10/27/2010.
While unloading hi-line from a car 10/11/2010 caught the two small toes of his right foot under a wheel. He continued working not seeking medical attention until 10/13/2010 after swelling and pain persisted. X-rays did not reveal any fractures and he was released to unrestricted duty. Became a Restricted Duty after the site of initial injury became infected 10/27/2010.
While loading a bundle of chain hangers on a mantrip they slipped and as he attempted to catch them pinched his right index finger against the ride blowing out the side of the finger. Transported to Springfield Memorial ER where X-rays revealed no fractures and sutures were used to close the wound. He was released to unrestricted duty and returned to the mine finishing his shift.

He was transported to Springfield Memorial ER where the fragment was removed. After a follow up visit to his physician 8/23/2010 he returned to unrestricted duty.

No property damage or injury occurred. Fall was in a secondary escapeway, an alternate route was marked for travel. There are no plans to clean this fall.
A roof fall was found at cross cut 113 entry 6 in 6RMS. Fall was in 4-way intersection that was mined in March of 2010. Area had bee supproted with 8' full resin bolts. No injury or property damage resulted. However, it did loosen an intake stopping. There are no plans to clean up this fall.
A roof fall was found at 1:15pm at entry 1 xc 238 main south. Fall was in a four way intersection that had been supported with 4 & 5' full resin bolts. In addition, truss bolts had been installed in this intersection. No injury or property damage resulted. At present time there is no plan to clean up this fall.
While roof bolting and installing an 8' bolt with the plate on the bend hit a 4' brow with the plate and chipped off a small rock shooting it towards him and striking him above his upper lip leaving a laceration that required sutures to close. He was released after treatment to unrestricted duty.

Initially reported 6/3/2010 as a Minor First Aid became a Lost Time Accident 7/13/2010 after being diagnosed by his physician as a left calcaneal fracture although X-rays were inconclusive and directed to perform ""no work until further notice"".
While opening an acetylene bottle to clear the valve was burned on the back of his left hand when the gas ignited. Transported by private vehicle to Springfield Memorial ER and treated for a second degree burn and released to restricted duty.

A roof fall was found at 3pm on 6/13/10 at xc 3 travel-way along 1B belt, fall was 20X20X10 high in a 3 way intersection. Area had been supported with 8' full resin bolts. No injury resulted, discharge line and communication cable was lost. Fall will not be cleaned up. A new travel way has been re-routed around fall after additional support was added to access to fall.
While attempting to pull down a rib that MSHA had deemed a hazard and using a slate bar a small piece of the rib broke off allowing him to fall backwards striking the back of his head on an unknown object. The impact caused an open laceration approximately 1.5"" long that required sutures to close. He returned to work after treatment without restrictions.
While operating a continuous miner and cutting the slab side of a straight cut a rock slid off the miner and struck his left foot leaving a laceration. Transported by ambulance to Memorial Medical ER Springfield where X-rays also revealed a fracture to the foot.

While bolting, allowed his left hand to rest on the drill pot where it was struck by a drill steel that had been left in the roof, causing a crushing injury to the fingertip and nail. X-rays revealed a fracture to the fingertip and treatment included removing the nail and suturing to close the wound.
While installing an 8' bolt the corner of the plate rubbed against his right arm allowing a burr on the plate to slice his forearm leaving a laceration. He was transported to Midwest Occupational Health Associates where treatment included sutures to close the wound. He returned to work completing his regular shift.
While lifting and moving cable and water line against the rib and out of the travelway pulled a lower right back muscle. He finished his shift but sought medical attention the following day and was placed on a weight and bending restriction.

Transported by ambulance to Springfield Memorial ER where the index finger of his right hand was removed due to the extent of the injury. He was admitted for observation.
While cutting a section of rail on a cutting table and attempting to roll a six foot piece off the table it landed on top of his left foot causing pain and swelling. Transported by personal vehicle to Springfield Memorial ER where X-rays revealed several fractures in the foot. Treated and released to see an orthopedic surgeon for surgery to re-align the fractures.
While operating a mantrip struck a battery ramcar on the travelway. The impact caused him to strike his head leaving a laceration. EMTs placed him on a backboard and transported by ambulance to Springfield Memorial ER. Treatment included staples to close the wound. A CT Scan revealed transverse process fractures. He was admitted for observation and discharged 2/24/2010.
While building a Kennedy stopping, the rail came out of the rib, striking him in the mouth and chipping a tooth. He finished working his shift; the chip was placed in a bottle of milk and then re-attached by his dentist. He has been released to unrestricted duty and did not miss any work.
While operating the scraper, hit a hole or bump in the road, causing him to bounce in the seat and causing a strong pain in his lower back. X-rays and a CT scan revealed a Vertebral Compression Fracture and he was directed to return to work and placed on desk work only restriction.

A fall was found by an MSHA inspector on 11/30/09 at 79 + 10 entry 1 in 8 LMS. Fall was in a four way intersection that had been mined 10/29/09 and supported with 4' and 8' full resin bolts. There are no plans to clean up this fall. No injuries resulted. It was reported due to the fact that an intake stopping was damaged.
While operating a grinder felt right eye irritation. Followed up next day with a visit to his personal physician, the eye was irrigated and a piece of dark material was removed. The irritation remained and a subsequent visit removed an additional piece of imbedded material. Released to unrestricted duty and continued to report to work for all his scheduled shifts.
While bolting and installing an 8' pin, a piece of shale fell and hit the Spider plate forcing it into his left forearm and slicing through his Kevlar armguard, resulting in a laceration that required staples to close. Transported by private vehicle to St. John's ER Springfield for treatment and released to unrestricted duty.

Area had been supported with 6' full resin pins and had been truss bolted. No injury resulted. No plans to clean up the fall. Pending approval of a seal plan from MSHA, new seals will be built to replace those which are inaccessable.

A roof fall was found on 9/05/2009 at 4+90 room 5 in 6R MS MMU 007. The fall was in a four way intersection that was mined August 31,2009 and had been supported with 6' full resin bolts. No injury resulted and fall will not be cleaned up. An intake stopping and approx. 100' of highline was lost,
Employee was adjusting the supply forks on the #38 scoop and had the sliding arm raised. He could not move it. After pulling several times and the arm not moving, he put it down. After he put it down his hand slid. When it slid his little finger on his right hand was sticking out. At this point, it was pinched between the fork prong and the support frame.

While cutting open a damaged repair on a jumper cable, his knife slipped and cut into the top of his right hand between the knuckles of the index and middle finger.
