A roof fall approximately 20'x 16' by 6' high was found by mine examiner in the North SubMain in Entry 10 between crosscuts 4 and 5. No harm to personnel or equipment was experienced.
Near the end of his shift on 5/24/12, Employee stepped in uneven mine floor in an outby area and hurt his left ankle and foot, and his right knee. Employee did not fully report incident until 5/29/12. Employee did not begin missing work due to the injury until 06/06/2012.
Miner was preparing to move tracking system reader. When he cut the wire to the reader, the reader swung down and struck him in the face/mouth - striking his teeth. Employee did not miss work until June 8 thru 13, 2012.

While hanging feeder power cable from roof, employee heard a ""pop"" and felt pain in his right shoulder. The employee subsequently had surgery on February 3, 2011, which was deemed ""worked-related"" by the attending physician. Because the employee was able to work for three months, this case was disputed. The employee returned to work on full duty status on April 18, 2011.

Employee, complaining of back pain, was transported out of mine to receive medical treatment. He made no claim or assertion that he was injured at mine at time of incident. Upon investigation, management determined that incident was not work-related. MSHA issued 1046 citation. This 7000-1 submission is to abate citation #6675934.
Employee was walking alongside the roofbolting maching, stepped on a chunk of coal and twisted his right ankle.

Employee was installing boards on props to protect SCSR storage area when a rib rolled over and fell, striking him on his back knocking him to the mine floor. The coal rib also struck his feet. He worked the next two scheduled shifts but started missing on April 14. His first Dr. visit was April 15 when a left foot fracture was found after xrays were taken.
Installing a curtain board on the mine roof with roof bolting machine, when on side operator pressured ATRS against roof, the floor jack pressed employees right foot into the mine floor. His right foot suffered a contusion from the middle to the distal end.
Employee was unpluging a battery from a ram car when he slipped and fell while standing on loose coal. His left thumb was cut when he fell. Three stitches were required to close the wound.
While washing a Roof Bolting Machine he stepped in a hole and twisted his left knee. He did not begin missing until April 2, 2009 after a surgical procedure was preformed on his left knee.

Employee was assigned to work on the conveyor belt. As he was traveling (walking) in a x-cut that contained muddy gob on the floor he slipped on a 3"" water pipe and fell onto his buttock. He was taken by ambulance to a hospital and treated for a contusion.

Installing an 8 ft roof bolt, while straighting the bolt to install he slipped and strained his back.
Employee set lunch on skid on ground, raised up and bumped into roof bolt. Loose debris and one 6"" flat coal piece rolled down back.

Employee was backing a personnel carrier into a crosscut when the tongue of a wet duster struck and lacerated his left leg just below the outside of his knee. 12 stitches were used to close the wound.
Employee was operating the continuous miner loading a ram when a rib fell striking the employee on the back of his right leg and knocking him into the ram car. He continued to work until Nov 18, 08 until which time his physician began treating him for an infection in the affected area of his right lower leg.

While roof bolting the drill steel plugged up; once cleared we began to raise the drill steel to the roof. When the steel struck the roof a clod fell and the steel bent and snapped back striking his left hand.

Twisted left wrist. 2"" water pipe fell of sled, while trying to reload, he twisted his left wrist. Did not begin missing until Jan 28, 2009 due to surgical procedure to repair the damaged wrist.

IE was working demolition of a steel structure at the bottom of the slope. He and another employee were torch cutting 8"" channel iron into 36"" long pieces. IE cut one piece off and set it down on the ground, leaning it up against the belt frame. Soon after, the piece tipped and fell against IE's lower rt leg, causing an approx 3"" long laceration to his lower rt leg.
