The IE was descending stairs from the second floor in the Prep Plant. When the IE reached the landing between the 2nd and 1st floors, EE turned right with EE's left leg to start descending the next flight of stairs. As EE stepped down with EE's left leg, EE twisted the left knee.

On 01/02/23 a prep plant employee was descending a (3) step walkway over the Magnetite Chute. The IE slipped on the second step twisting the left knee. The injured employee continued to work until 08/28/23 where IE underwent surgery to repair a tear in the knee.

Employee was preparing a piece of plate for installation; Plate was clamped down on saw horses and when clamps were released the plate slide off the saw horses which came down on employee's lower right leg and made a laceration.

The raw coal pile at the Preparation Plant had 3 separate locations where there was smoke and warming coming from the coal.
IE alleged carpal tunnel in both hands due to working at the prep plant. IE first reported pain in left hand on 01/07/2019 and right hand on 03/01/2019. IE had surgery on right hand/wrist on 06/12/2019.

Ice had built up on a bottom roller of the clean coal collecting belt. IE (temporary employee)struck the bottom roller with a hammer to dislodge the ice buildup. IE's left hand was caught between the bottom roller and the belt. IE received a fractured left humerus.

IE was standing checking rail car doors to make sure they were latched. IE alleges that right knee twisted/hyperextended as IE was standing there. IE did not report this incident until 06/26/2017. IE continued to work up until 08/10/2017 when IE had right knee surgery. Surgery was arthroscopic partial medical meniscectomy and debridement of plica.

IE was using a hand held side grinder with a cut brush. Apparently a piece of wire came off the cut brush and embedded in his upper abdomen. He sought medical attention on 9/2/2014 and a piece of wire was removed from his abdomen.

Employee was taking a shower at the end of his shift and felt a slight bulge in his left lower groin. He did not feel any pain and had no specific event that caused the bulge. On 4/23/2012 he notified his supervisor, but could not recall any event. He went to the Doctor and was diagnosed with an left inguinal hernia. He continued to work until he had surgery on July 5, 2012.

Employee was washing a refuse feed box in the prep plant. He stepped from a concrete floor down to a metal floor approximately 3 inches lower. He slipped and fell striking his left elbow on the floor. He continued to work up until September 1 when he was taken off for surgery to repair a tendon in his left elbow.

Employee pulled his Gob Truck up to a berm. As he dismounted from his truck he slipped on the berm and twisted his lower back. He was seen by a physician on June 1 and did not return to work.

Contractor employee was assisting in the changing out of a clean coal screen in the prep plant. He suffered a massive heart attack and apparently died on the job. No outside influences affected the incident. A coroner has ruled the death natural causes.

Employee was stepping up on a clean coal shaker to help change out a screen. When he stepped up on the side of the shaker he felt a pop in his right knee. He could not straighten out his leg and left the mine for medical attention. He had continued to work, but his knee did not improve and he was seen by a doctor and taken off work on 5/9/2006.

EE was assisting in removal of bolts from bearing cover on scalping screen at rotary breaker. He had an allen wrench in head of bolt & 4' cheater bar on wrench for added leverage. Co-worker struck wrench w/3lb sledge hammer trying to knock bolt loose. Wrench slipped out of bolt head. EE was applying downward pressure with bar & this caused him to shift forward suddenly.

THE INJURED EE WAS WORKING IN THE PREPARATION PLANT CLEANING UNDER A SCALPING SCREEN. AS HE REACHED UNDER THE SCALPING SCREEN HE FELT PAIN IN HIS LEFT SHOULDER. THE EE WILL BE INSTRUCTED TO ALWAYS USE PROPER BODY POSITION WHEN PERFORM-ING A TASK.
EE A SUPERVISOR WAS ASSISTING A FELLOW EE IN CHANGING A WATERLINE IN THE PREP -PLANT. HE WAS STANDING ON A LADDER AND AS THE WATERLINE BROKE LOOSE HE LOST BALLANCE AND FELL TO THE FLOOR LANDIN G ON HIS RIGHT FOOT. THE INCIDENT CAUSED A FRACTURE TO THE RIGHT HEEL. THE EE STARTED MISSING TIME ON 8-21-03 AND IS STILL UNDER THE DR. CARE AT THIS TIME.
EE WAS WALKING BACKWARDS MOVING A SECITON OF WATER PIPE/WHEN HE STEPPED IN A DRAIN AND STRAINED HIS BACK EE STARTED MISSING WORK 6-26-03 AND IS UNDER DR. CARE AT THIS TIME INCIDENT HAS BEEN UN DER INVESTIGATION PENDING DR DIAGNOSIS.

EMPLOYEE WAS WALKING ALONG THE PLANT FEED BELT AND HE TRIPPED ON AN ""I"" BEAM CAUSING HIM TO FALL. AS HE WAS FALLING HE REACHED OUT HIS RIGHT HAND TO BREAK HIS FALL CAUSING A SPRAIN TO HIS RIGH T HAND.
EMPLOYEE WAS REMOVING A PIECE OF STEEL FROM THE TUNNEL AREA. HE CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE STEEL AND THE BELT STRUCTURE. THE INCIDENT CAUSED A LACERATION AND A FRACTURE TO THE E ND OF THE FINGER. THIS INCIDENT HAS BEEN UNDER INVESTIGATION PENDING DOCTOR'S REPORTS AND ALSO THE PLANT WAS SHUT DOWN FROM 7/20 TO 8/3/02.
EE WAS MOVING A CLEAN COAL SCREEN, THE SCREEN SLIPPED AND AS IT FELL, THE CORNER STRUCK THE EE IN THE CHEST AND RIB AREA. HE STATED THAT HE THOUGH HE WAS ALRIGHT AND WORKED UNTIL 5-6-02 AND FE LT HE NEEDED TO BE CHECKED OUT. HE WAS TRANSPORTED TO THE HOSPITAL AND DIAGNOSED WITH A FRACTURE RIB. HE IS UNDER THE DOCTOR'S CAR AT THIS TIME.

EMPLOYEE WAS IN THE PROCESS OF CHANGING A 12"" IN THE PREP PLANT. WHILE POSITIONING THE PIPE THE EYE BOLT USED TO HOLD THE PIPE CAME LOOSE ALLOWING THE PIPE TO MOVE STRIKING THE EMPLOYEE'S RIGH T LEG, JUST ABOVE THE KNEE.
EE WAS WORKING ON #2 VC 48 DRYER HELPING PUT A FEED CHUTE JACK ON AND HE RAISED HIS LEFT HAND AND HIT IT ON THE CHUTE ABOVE ON A SHARP PIECE OF METAL. ACCIDENT HAS BEEN UNDER INVESTGATION AWAN TING MEDICAL REPORTS. STARTED MISSING TIME 3-19-00.
EE WAS WASHING IN THE SAMPLE HOUSE ON THE SURFACE AT THE PREPARATION PLANT. HE WAS PULLING THE HOSE FROM THE 2ND FLOOR TO THE 3RD FLOOR, HIS FOOT SLIPPED ON ONE OF THE STEP TREADS AND CAUSED H IM TO LOSE HIS BALANCE AND SLIP. HE PUT HIS HAND OUT IN FRONT OF HIM AND INJURED HIS HAND. DID NOT RECEIVE ACCIDENT REPORT UNTIL 3-20-01.

EMPLOYEE WAS DRIVING A GOB TRUCK ON THE GOB HILL AND HIT A DIP IN THE ROAD. HE SAID THE TRUCK BOTTOMED OUT AND THAT HE FELT A PAIN IN HIS BACK.
WHILE COMING DOWN FROM THE END LOADER, WITH THE FUEL NOZZLE IN HIS RIGHT HAND, HE WENT TO STEP ON THE GROUND. HE WAS COMING DOWN ON THE LADDER MOUNTED TO THE MACHINE. HE WENT TO STEP BACK WITH THE WEIGHT ON HIS RIGHT ARM AND FELT A PULL IN THE ARM.
EMPLOYEE WAS OUT OF THE GOB TRUCK PREPARING TO FUEL IT. HE WAS STANDING ON A HOME MADE LADDER LEANING AGAINST THE WEST SIDE OF THE 1000 GALLON FUEL TANK. WHILE STANDING THE SECOND WRUNG OF T HE LADDER GAVE WAY CAUSING HIM TO FALL HITTING HIS HEAD ON THE FUEL TANK AND TWISTING HIS BACK.
EE WAS WORKING ON THE 4TH FLOOR OF THE PREP PLANT CHANGING OUT 14"" RETURN LINE OFF THE HYDRO CYCLONES. THEY WERE USING AN 8' FIBERGLASS LADDER TO REACH CERTAIN PARTS OF THE JOB. EE WAS ON THE LADDER AND STARTED DOWN WHEN HE FELT HIS RIGHT KNEE TWIST.
THE ROCK BIN AT THE CENTRAL CLEANING PLANT FOR OLD BEN COAL COMPANY BROKE LOOSE FROM THE STRUCTURE AT THE BOLTS, CAUSING THE BIN TO FALL INTO THE BED OF THE REFUSE TRUCK. THE EE WAS IN A GOB R TRUCK PARKED UNDER THE REFUSE BIN WHEN IT FELL INTO THE BED OF THE REFUSE TRUCK. THIS ACCIDENT IS UNDER INVESTIGATION.
