Employee was operating a diesel personnel carrier on the Main South Travelway. It appears that his vehicle struck a coal pillar at xcut #193 and he was ejected from the vehicle. He sustained fatal crushing injuries to his chest area.

IE was struck by a piece of 20ft. aluminum water pipe as it was being unloaded from a pipe sled. The pipe struck him on the side of his right foot causing a contusion.
IE was walking Unit 1 belt line. He stumbled and lost his balance then fell to the ground catching himself with his right hand. He continued to work up until he had right shoulder surgery on October 27, 2015 to repair a SLAP tear.
Employee reported to his supervisor pain in right wrist on 07/28/2015. Employee thinks he may have injured his right wrist on 07/15/2015 as he was building hollow block stoppings. He had surgery on 08/07/2015 for a right wrist TFC tear and volar ulnar ganglion cyst excision. Mine received doctors report on 08/17/2015.
THE MINE DISCOVERED AN UNPLANNED ROOF FALL ON UNIT 2, MMU 003-0. THE FALL OCCURRED IN ENTRY #2 FROM 139+30 TO 139+90. THE AREA WAS MINED ON 6/30/2015, NO INJURIES OCCURRED WITH THIS EVENT. IT HAS BEEN DETERMINED TO ABANDON THIS AREA OF THE MINE AND MMU 003-0.
Employee alleges that he injured his back while walking the main East belt during his on-shift examination. Operator has been waiting on MRI results. MRI was conducted on 8/06/2015 and results were received on 08/12/2015. MRI indicates a small disc protrusion. Employee remains off work.
Employee reported increased pain in his lower back on 5/20/15. Mine has been waiting on medical reports which were received on 7/6/15. MRI report revealed disc desiccation, degeneration, and extrusions due to wear and tear associated with twisting, bending, and lifting. Employee is currently off work with surgery pending at a later date.
(IE) was roof bolting on Unit 2. He had just pushed an 8ft. roof bolt up into its hole when he felt pain and a pop in his left forearm. He was transported out of the mine for medical attention. He was examined at 4 different medical facilities before having surgery on May 28, 2015 to repair a partial thickness tear to his left distal bicep.
Injured employee was in the parking lot after his shift working on his personal truck. IE's personal knife slipped while cutting plastic off his truck bumper and struck him on the right wrist. He received a laceration which required medical attention.
Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low-dust area of the mine.
IE was roof bolting on Unit 1 when EE was struck on left shoulder by a piece of roof rock. The rock was approximately 3ft.x3ft.x1-3in. It fell out between bolts. EE's injury resulted in a displaced left distal clavicle fracture.

IE was operating a ram car when he ran over a piece of coal. Apparently part of it broke off and came into the cab and struck him on the right side of his nose. It took 6 sutures to close the laceration.
Employee alleged Carpal Tunnel syndrome due to working in the coal mine. Reported to Safety Department on 11/25/2014.
IE was roof bolting when a small piece of roof rock fell and struck him on top of his upper lip. The rock was 3in. x 3in. x 7in. He received 3 sutures to close the laceration.
Employee alleged Carpal Tunnel syndrome due to working in the coal mine on 08/28/2014. He had Carpal Tunnel surgery on his right hand on 11/18/2014.
IE was loading tools into the tailgate area of his maintenance ride. A rock measuring approximately 3'X 1.5'X 1.5"" fell and struck him on top of his right hand. He received a laceration on top of his right hand requiring 5 staples to close.
The EE was operating a coal hauler. He stated that he was walking on the unit and felt pain in his left knee on 8/1/14. He continued to work up until Aug 8, 2014. He was taken off work on 8/11/14. He had left knee arthroscopy on 8/20/14. A postoperative report received on 8/25/14 along with the attached information indicated wear and tear and degeneration in his left knee.
IE was helping hang ventilation curtain when a rock fell out between roof bolts and struck her on the lower left leg. She felt pain in her lower left leg and knee. She has a fracture to the lower left tibia.
IE was spotting in an additional roof bolt when a rock fell out between bolts and struck him on his right leg. He was originally treated for a sprained right ankle. We have been waiting on the results of his MRI and it has revealed a lisfranc fracture to his right foot.
Employee was cleaning gob out of a conveyor deck using a screwdriver. He felt pain and a pop in his right elbow. He received medical care and was placed on restricted duty.
Employee was roof bolting and had positioned a roof bolt in front of him resting on the bolter wrench holder. As he lowered the bolter boom it came in contact with the roof bolt knocking off the expander shell and it struck him in the mouth. He received a laceration to his lip requiring 2 sutures and chipped 2 teeth.
Employee was hanging miner cable and stepped on a small piece of roof rock and felt pain in his left foot. There is no mechanism of injury due to the sole of the work boots being extremely thick. He has had no prior foot pain due to his pre-existing condition of having a cavovarus foot. He was taken off work on April 11, 2014.
Employee was assisting in roof bolting when he was struck on top of his left hand by top coal that fell from the mine roof. He received a contusion to his hand and prescribed a brace to wear.
Employee was roof bolting when a piece of top coal fell and struck him on top of his head and right shoulder. He finished his shift and did not report the incident until 04/14/2014. His first shift missed due to the injury was 04/28/2014. An MRI revealed prominent disc extrusions in his upper neck.
Employee alleged Carpal Tunnel Syndrome due to working in the coal mine on 02/27/2014. He had Carpal Tunnel surgery on his left hand on 12/8/2015 and is scheduled to have Carpal Tunnel surgery on his right hand on 12/22/2015.
Employee was operating a Continuous Miner and while standing stationary he had to turn and twist his body to see the approaching Ram Cars. He felt pain in his right knee. He had right knee arthroscopy and partial medial meniscectomy on May 13, 2014.
The emergency hoist became inoperable due to ice build up in the shaft. The ice was removed at 7:10 AM on 02/06/2014 and the hoist returned to operation at 7:15 AM on 02/06/2014.
Employee was struck on the heel of his right foot by a hollow concrete block. The block fell from a wall that he was building. He received a contusion to his right heel and taken off work for a period of (2) days.

IE was riding in a mantrip traveling to the bottom at the end of his shift. An unknown object struck him in the right shin. He received a laceration to his right shin requiring 8 sutures to close the wound.
Employee reported left shoulder pain on 10/16/13 with no know event. he denied any injury. Pre-operative and post-operative notes attached indicated AC joint degenerative disease and a clean up of the left rotator cuff.
EE reported lower back pain on 10/09/13. Medical evaluation indicated chronic advanced degenerative disc disease associated with spinal stenosis.
Employee pinched his left index finger between a clevis and scoop bucket. He sought medical attention and given (2) days restricted duty.
Employee was struck on the left leg by a battery hauler tire as he was removing it. He sustained a contusion to his left leg and knee. Placed on restricted duty.
An unintentional roof fall occurred on Unit #3 in Entry 1 at the 52+80 survey station. The fall occurred due to the roof strata broke at the anchor point. The fall area had been supported with 4ft. and 5ft. conventional bolts. The fall is approximately 40 ft. long, 20-25ft. wide and 0-5ft. high. The fall will be cleaned up and re-supported.
The employee was standing in a crosscut waiting for the continuous miner to move from entry 7 to entry 8 on unit 3 and a rock fell between bolts and struck him on the head and right ankle causing a contusion and strain.
Employee was getting out of trash dumpster when he missed a ledge on the dumpster and twisted his right ankle. Restricted duty and wear a brace on right ankle.
Injured employee was exiting the cab of a diesel scoop when he twisted his right knee on the mine floor. He continued to work up until September 13th when he was taken off work for a torn meniscus in his right knee.
Employee was walking to his coal hauler. The roof popped and a rock measuring 10ft.4in. x 4ft. 5in. x 8-10in. fell. The rock struck the employee on his lower right side and back. The rock dislodged one roof bolt from the roof.
Employee was assisting in the recovery of high voltage cable and got his right foot caught between the bumper of a ram car and the mine floor. He received a contusion to the top of his right foot.

Employee was walking thru air lock staging area on the bottom and struck his head on low roof. Receiving chiropractic treatment.
The employee has experienced gradual pain due to 23 years working underground lifting, pulling, twisting, and turning. He has been diagnosed with degeneration of the abdominal muscles.
The employee is a mine examiner who walks approximately 4 miles each day. He started feeling pain in his right knee on July 1, 2013. He continued to work up until July 30, 2013. A doctors report was received on August 16, 2013 with information on his knee. He is scheduled for surgery on August 28, 2013. The attached information describes his present condition.
Employee states that he has carpal tunnel in both wrists due to working in the mine. Employee has had previous carpal tunnel surgery on both hands. Started missing work on 08/22/2013.
EE was roof bolting when he was hit on his hard hat by roof rock. He was brought out of the mine for medical attention. he had an MRI which revealed Spondylosis at C5-6 and congenial fusion at C3-4. The MRI report is attached. he has not returned to work.
Employee alleged Carpal Tunnel Syndrome due to working in the coal mine on 01/31/2013. He had Carpal Tunnel surgery on his left hand on 02/23/2015.
Injured employee was roof bolting and in the process of pushing a two piece drill steel into the roof when it sprung back and struck him on the right hand. He received a comminuted fracture to the distal 2nd metacarpal head.

EE is a mechanic who has also been a roof bolter has had years of repeated pulling and pushing on pry bars and wenches. On 11/03/12 he felt pain in his right shoulder. On 12/14/12 was diag. with rotator cuff tendonitis, hypertropic degen. changes of the joint, and bursitis. He worked up until 2/16/13 and was taken off work for surgery. See attached MRI results.
Employee came out of the mine complaining of pain in his left shoulder and upper back and neck. He went to the hospital where xrays were negative. The mine has been waiting for doctor's results of an MRI. The MRI revealed degeneration of the Glenoid Labrum with moderate Osteoarthritis. Moderate degenerative changes in C3-4, C4-5, C6-7, C7-T1 and T2-3.
Employee was standing along the operator side of his ram car when a rock approximately 3ft. x 3ft. x 2in. fell out from between the rib and rib bolt and struck him on the left side of his head and then fell on top of his left foot. He received a fracture to his left big toe.
An unintentional roof fall occurred on Unit #1 in Entry 5 @ 37+30. The fall occurred due to the strata broke in the center of the intersection above the anchor point. Several roof bolts were left hanging in the roof. The fall is approximately 0 - 5.5 ft. high, 18 ft. wide, 18 ft. - 20 ft. long. The fall will not be cleaned up.
An unintentional roof fall occurred on Unit #1 in Entry 8 @ 38+50.The fall was found by a mine examiner. The fall occurred due to roof slips coming together in the intersection. The fall is approximately 30 ft. long, 18 ft. wide, 6-7 ft. high. The fall will not be cleaned up.
Employee was in the process of putting a cover on the side of a Continuous Miner. The cover slipped and caught his right thumb between the cover and a pry bar. He received a laceration to his right thumb requiring 9 sutures to close.
Employee was struck by a roof bolter cable that knocked him into a coal rib. He received abrasions to his left shoulder and a laceration to the left side of his face. He received 5 sutures to close the laceration on his face.
Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low-dust area of the mine.
Employee was on top of a de-energized continuous miner. He stepped up on a flare board on the miner tail and slipped off striking his left leg on the top of the flare board. He received a laceration to his left shin that required 6 sutures to close.
Employee was struck on the right arm & shoulder by a piece of roof rock on May 6, 2012. The attached reports describe that event did not involve his neck or have an impact on his illness. Employee worked up until January 15, 2014 & had surgery on January 16, 2014. The orthopaedic physician diagnosed him with advanced disc degeneration. See attached MRI & post surgery report.
Employee was diagnosed with carpal tunnel syndrome in her right hand and had surgery on 5/18/12.
Employee felt pain in his lower back with no specific incident. He worked until June 22, 2012 when he was taken off work by a doctor. The mine has been waiting on a doctors report which revealed right L5-S1 recurrent disc herniation from a 1995 microdiskectomy and disc degeneration at L5-S1.
Employee alleged Carpal Tunnel syndrome due to working in the coal mine on 4/4/2012. He had Carpal Tunnel surgery on his left hand on 4/15/2013.
An unintentional roof fall occurred on Unit 3 in Entry #4 at 66+00. The fall is approximately 8-9 ft. high, 17-18 ft. wide, 50 ft. long. The fall will not be cleaned up. The fall area has a slick sided slip running through the intersection.
Employee was diagnosed with carpal tunnel syndrome in both hands and had first surgery on 11/2/2012.
Employee was roof bolting on Unit #3 when a rock fell out and stuck him on the front of his hard hat knocking his hat to the mine floor. As he bent down to retrieve his hard hat a rock 1ft. x 1ft. x 1-2in. fell and struck his on the top left side of his head causing a laceration. He received 5 staples to close the laceration.
An unintentional roof fall occurred on Unit #3 in Entry #3 at the 59+40 survey station. The fall occurred due to the strata broke at and above the anchor point. There were two slips on each side of the roof fall. The area had been supported with 6' double lock bolts. The fall is approximately 30 ft. long, 19-20 ft. wide, 0-7 ft. high. The fall will be cleaned up and re-supported.
Employee was beginning to start roof bolting. He had inserted a drill steel into the drill pot. He was holding the steel with his right hand when he thought he reached for the lever to raise the pot. Instead he activated the lever to close the auger clamp and it caught his right hand. He received fractures to his right ring and middle finger.
Employee was in the process of jacking up a belt tail using a track jack. He felt pain in his right shoulder. He continued to work until 01/20/2012 when he was seen by a physician. An MRI revealed a torn right rotator cuff and a partial tear to his right bicep tendon. Surgery has not yet been scheduled.

The emergency escape hoist became inoperable during an inspection. The PLC program malfunctioned and the hoist would not operate. The PLC was reprogrammed and the hoist returned to operation at 5:15 PM.
Employee was struck on the upper right arm, right leg, and right foot and ankle by a rock that fell out between roof bolts. The rock measured 80in. x 42in. x 0-10in. He received fractures to h is upper right arm and upper right leg. His right foot and ankle had to be surgically amputated 6 in. above the ankle due to the crushing injuries they received.
Employee was operating a battery ram car. He stated at the end of his shift that he bumped his head on the canopy. He continued to work until 1/16/12 when he went in for surgery to repair L5 b ulged disc. Doctors report indicate that he had inter vertebral disc disorder in 2009 and mild degenerative changes, mild disc desiccation at L4-5 at the time he bumped his head.
Employee was using a utility razor knife to cut off the edge of a conveyor belt. The knife slipped and he struck himself of top of the right leg with the blade of the knife. He received a laceration approximately 1.5 inches long that took 5 sutures to close.
Employee was walking underground. As he walked up to a ventilation curtain to remove it he felt pain in his right ankle & stumbled & fell to the mine floor. He ruptured his right Achilles tendon. He is scheduled to have surgery to repair the tear. Employee has a history of problems due to repeated trauma of walking.
An unintentional roof fall occurred on Unit #2 in Entry #5 at the 4+20 survey station. The fall occurred due to the strata broke at and above the anchor point. The area had been supported with 4 ft. conventional roof bolts. The fall is approximately 39 ft. long, 19-20 ft. wide, 0-5 ft. high. The fall will be cleaned up and resupported.
The emergency hoist became inoperable during an inspection by MSHA. The braking system would not release. A minor adjustment was made to the brake system and the hoist returned to normal operation at 10:55 am.
Employee was walking between a coal rib and his battery ram car. He slipped on a wind row of loose fire clay and twisted his left knee and ankle. He fell to the floor and rolled his left ankle resulting in a fracture to his left ankle.
Employee was walking next to the Unit #3 travel road. A rock fell out between roof bolts and struck him on the back of his lower right leg. The rock was approximately 24in. x 24in. x 3in.
The emergency escape hoist became inoperable during an inspection by MSHA. The braking system on the hoist became engaged during normal operations. The lilly overspeed was adjusted and the hoist returned to normal operations at 6:00 PM.

Employee was operating a Continuous Miner in Unit #3. He was turning a xcut to the left standing directly under a roof bolt. A rock approximately 2ft. x 4ft. x 1in. to 1ft. fell and struck him on the left side of his head, neck, shoulder and leg. He received a puncture wound to the left side of his head behind his left ear that required 5 staples to close.
An unintentional roof fall occured in the intake entry outby the #3 working section. The fall occured due to the fact that the roof strata broke at and above the anchor point, exposing a slip in the fall area. The fall will not be cleaned up, the fall area will be timbered off and all approaches will be dangered off.
EE was walking underground when he felt a pop in the back of his left foot. There was no trauma or event that caused this to occur. He had been experiencing additional foot problems and was being treated by a doctor. He has had surgery to repair the achilles tear. The doctor reports are attached as well as a history on the employee.
An unintentional roof fall occurred due to the strata broke at and above the anchor point. The fall occurred at Unit #1 alternate escapeway travelway in entry #4 @ survey station 3+75 and 4+40. Dimensions 18 ft. wide, 30 ft. long, 4 to 5 ft. high. Fall will be cleaned up and resupported.
Injured employee was helping hang roof bolter cable using tie wire. The bolter trammed foward and the cable became tight and pulled loose from the tie wire. The cable struck the employee in the back of his head and neck. He is experiencing pain in his neck.
The man/material hoist became inoperable due to a malfunction with the counter weight. A guide had worn out on the counter weight and the counterweight came out of the rails that contain it in place. The guide was replaced and the hoist returned to operation at 9:20 AM.
An unintentional roof fall occurred due to the strata broke at and above the anchor point. A smooth slip ran through the entry. The fall occurred in a neutral entry off Unit 2 travelway @ entry 3-4 @ 2+25 east. The fall was approximately 25 ft. long, 18 ft. wide and 1-6 ft. high. Found by mine examiner.
Injured employee was pulling on a hydraulic hose that went thru the frame of a continuous miner. As he was pulling on the hose his feet slipped out from under him and he felt a burning sensation in his groin area. He continued to work until 12/04/2010, and then had surgery to repair a inguinal hernia on 12/06/2010.
Injured employee was using a screwdriver to break the metal bands on a new roll of conveyor belt. When he broke the last band the metal band struck him on the right forearm. He received a laceration that required 5 sutures to close. The employee returned to work on full duty.
Injured employee was in the process of removing the bottom section tub from a belt drive gear. The tub fell and pinched off the end of his left thumb. He had surgery to repair his thumb.
The man/material hoist became inoperable due to a malfunction in the braking system. A rod that applies tension to the brake itself broke. Repairs were made and the hoist was returned to normal operation at 12:35 PM.
Employee was struck by a piece of roof slate on the right forearm while roofbolting. The piece of slate roof was approximately 3' x 2' x 1"". He received 10 sutures to close the laceration.
Employee was helping hang miner cable on a rib. A piece of roof rock approximately 1/4in. - 3 in. x 9in. - 12in. x 30in. fell from the roof and struck her above her right eye. She received 7 sutures.
Employee stepped on a piece of coal lying on the mine floor and twisted his right ankle. He continued to work until the ankle began to swell and was transported out of the mine to a medical facility. X-rays revealed a lateral malleolus fracture to his right ankle.

Man material hoist located in the primary escapeway was inoperable in excess of thirty minutes. MSHA was notified at 7:45 PM and the repairs were completed and the hoist returned to full operation at 3:15 AM 11/4/09.
An unintentional roof fall occurred due to the roof strata broke at and above the anchor point. Once the fall occurred it exposed a slip running parallel with the entry. All approaches will be reinforced and the fall area will be cleaned up and supported. The area was supported with 5 foot conventional roof bolts and 6 foot double lock bolts. The fall measured 30'x25'x6'.
An unintentional roof fall occurred on Unit 2 at Entry #7 at 1+80. The fall occurred due to roof failure there was a slip running diagonally through the intersection. The fall was originally supported with 5 ft.conventional roof bolts. The fall is approximately 16 ft. wide, 25 ft. long, and 0-5 ft. high. The fall will be cleaned up.
An unintentional roof fall occurred on Unit #2 at Entry #4 at 46+50. The fall occurred due to roof failure, cracks in the immediate strata. The fall was originally supported with 4 ft. conventional roof bolts. The fall is approximately 16 ft. wide and 16 ft. in length and 0 in. to 4 ft. high. The fall was cleaned up and resupported with longer roof bolts than originally installed.
Injured employee was putting up mining sights in Unit 1 at entry 8 @184+35. A piece of roof rock fell out between roof bolts and struck him on the left forearm and above his left wrist. He received a laceration that required 7 sutures to close. He returned to work his next shift.
An unintentional roof fall occurred on Unit #3 at Entry #7 at 18+60. The fall occurred due to a slip running through the intersection. The fall was originally supported with 5 and 6 ft. conventional bolts. The fall is approximately 30 ft. long, 20 ft. wide and 5-7 ft. high. The fall will be cleaned up and resupported.
Employee was assisting (2) other ram car operators move miner cable and water hose over to the rib line. As he threw the cable over to the rib he felt a sharp pain to the upper mid part of his back. He has been diagnosed with a severe back strain and taken off work.
Injured employee set a roofbolter drill boom pot down on top of his left foot. He received a fracture to the little toe on his left foot and soft tissue damage to the top of his left foot.
Man material hoist located in the primary escapeway was inoperable in excess of thirty minutes. MSHA notified at 7:30 PM. Trailing cable was damaged and repairs made to return hoist to operating condition at 9:15 PM.

The fall area was originally supported with 5 foot conventional roof bolts. The fall is aproximately 15 feet long, 25-30 feet wide, 5-6 feet high. The fall will be cleaned up and resuppported with 6 foot double lock roof bolts.

Employee is a Supervisor who had years of repeated overhead curtain hanging & sight markings. On 11/10/08 he felt pain in his left shoulder. He was diagnosed with a lateral impingement and moderate AC joint arthritis. He worked up until 3/27/09 and was taken off work for surgery on his left shoulder. He had left shoulder arthroscopy on 3/31/09.
Employee was struck on top of his left hand by a rock that was approximately 0-1/2 inches thick by 6 inches by 6 inches. He received a laceration to the top of his left index finger and his middle finger. 6 sutures were required to close the lacerations and he returned to full duty.
Employee had just finished installing a roof bolt and walked back to the control levers to drop the canopy, when a rock fell out between roof bolts and struck him on top of his left wrist, causing an approximate 2 inch laceration. The rock that struck him was approximately 0 to 1/2 inches thick and 18 inches x 18 inches. Five sutures were required to close the laceration.
Employee stepped on a piece of coal on the mine floor. He twisted his left knee causing a strain. He continued to work the rest of his shift and sought medical attention the next day. He has been diagnosed with a torn ligament in his left knee.
An unintentional roof fall occurred in the Main South Belt Entry at the 122+15 foot mark. The fall occurred due to the fact that the roof strata broke just above the anchor point. The fall area was originally supported with 4,5, and 6 foot conventional type roof bolts. Approaches to the fall area will be reinforced, fall area cleaned up and the fall area supported.
Roof fall found by mine repairman at 7:30 PM. Fall is on Unit 2, Entry 7 at +1820. The fall is 18 ft. wide, 4-6 ft. high, 50 ft. long. Area was supported with 5 ft. conventional bolts, and 6 ft. double lock bolts. A visible slip ran through the intersection and limestone was absent in the anchorage zone. The fall will not be cleaned up.
EE & 2 others were taking down Continuous Miner cable from cable hooks. EE's rt glove caught on the latch of a cable hook. As the miner cable fell it pushed the latch of the cable hook into his rt hand. He rec'd a laceration between his rt thumb & index finger. He finished working his shift & sought medical attention after he got home. He rec'd 6 sutures & was released to work
Injured employee was standing on the back of a golf cart flagging some bad top conditions and fell off the cart to the mine floor. He received a fracture to his right wrist.
EE was assisting in the installation of a new foot shaft on a CM.He was using a pry bar to hold the shaft in place while another EE was triyng to drive the shaft in using a hammer.A piece of metal broke off from the hammer or the shaft and struck the injured in the left forearm. (After seeing a surgeon the decision was made to leave the metal in his arm-Misc Mail 7/1/08 #3)
Employee was struck in the left eye by a piece of metal either from a brass hammer or from a roof bolt plate. He was wearing safety glasses but the metal entered in behind his glasses. He continued to work up until 6/18/08 when he sought medical attention to have the metal removed.
EE was in the process of repairing or replacing a grease hose to a pillow block bearing. The bearing was on a hold down roller on the overland ""C"" belt. The belt was running at the time. His left hand and wrist came in contact with the belt and roller, causing a laceration to his left wrist and a tear to the palm of his left hand. He was sent to local medical facility.
The Main East primary escapeway in Entry 2 between 7 and 8 crosscut was blocked due to roof slough age. Material fell out around roof bolts pulling the plates and heads off of the roof bolts. The affected area is approximately 20ftx16ftx3.5ft. The area will be cleaned up and resupported.
Employee was loading a roof bolter on Unit 1. He was reaching over onto a scoop bucket to pick up a bundle of roof bolts when his left forearm came in contact with a metal band used to hold the bundle of roof bolts together. He received a laceration to the top of his left forearm just above his wrist. 9 sutures to close the wound.
Employee was roof bolting on unit 1. As he was drilling a hole a fitting broke off a trunion jack and sprayed out oil. As he tried to get out of the way he stumbled and struck his mouth on the drill pot. He suffered injuries to three fron teeth. He had to have one tooth extracted.
An unintentional roof fall outby on Unit 1 in Entry 7 between 5070 south and 5135 south. The fall was found by a mine examiner making his weekly inspection. The fall is approximately 25ft long, 18ft. wide, and 0-6ft. high. The fall will not be cleaned up. Fall area was supported with 5ft. conventional roof bolts.
An unintentional roof fall occurred in Unit 1 Entry 6 at +6045. The area was supported with 5ft. conventional bolts. The fall is 18ft. wide, 30ft. long, and 5-6ft. high. The fall will not be cleaned up.
Employee was roof bolting in Unit 2 on the right side in Rm 2. A rock fell out between the rib pin and center pin and struck him on the right side of his head and right shoulder. He received an abrasion to the top of his right shoulder and a laceration to the right side of his head requiring 4 sutures. He was released and returned to work the same shift.
Employee medical examination indicates a sufficient degree of pneumoconiosis. He has exercised his option to work in a low dust of the mine.
An unintentional roof fall occurred in the Main South at Entry #4 at +2625 south. The area was supported with 4' conventional bolts. The fall area is approximately 21'W x 20""L x 4'- 5' H. The strata broke just at the anchor point. An evaluation of the area was conducted.

Unintentional fall occurred in face area of #2 working section MMU-003. Fall occurred due to fact that rock broke above anchor point. A cont. mining machine & load end of ram car were covered up. No bodily injury or damage to property. Entrances to area will be reinforced, rock cleaned off machinery, & machinery recovered. All adjoining areas will be inspected prior to recovery.
EE was helping move continuous miner cable over to rib. He & another employee were lifting the cable to their mid sections & walking with it to rib. There was a small wind row of coal on mine floor & EE stumbled over it & fell to his knees. He felt a strain to lower back. He continued to work rest of shift & sought medical attn later in day & was taken off work by dr.
An unintentional roof fall occurred on the shaft bottom area between the cage area and the man way. The roof strata broke at and above the anchor point.
EE felt pain in lower back while underground. Had no mechanism of injury for back pain. Under chiropractor care for 15 yrs., went in for MRI 9/19/07. Results of MRI 10/3/07 clearly shows degenerative disc disease. MRI report attached.
Employee had surgery to his right shoulder on August 3, 2007. Surgery revealed repetitive and degenerative joint disease. Attached is a full post surgery report from his surgeon.
EE was operating a continuous miner on Unit #1. He was loading coal on the right side of the unit. He was standing directly under a roof bolt, when a rock fell out between roof bolts and struck him on top of his left foot. The rock fell such that it struck him behind his metatrasal support on his boot. The rock was apprx 36""X30""X0-6"". He received a fracture to top of left foot.
EE was moving a roof bolter using the boom controls. A large rock fell out between roof bolts and struck him on top of the head, between his shoulders and down his right leg. The rock pinned him to the ground. He had abrasions to the top of his head, soreness to the shoulders and right knee. July 26 he was taken off work by a Dr. to repair a torn meniscus on his right knee.
Employee was walking beside a sitting still ram car on a 6"" high ""wind row"" caused by the ram cars traveling in soft bottoms, when his right foot slipped off of the side of the ""wind row"" twisting his knee.
Employee was bolting a straight in entry #1 44 + 20 in Unit #1. He was installing the middle bolt when a rock fell out and struck him on the back of the neck and between the shoulders. The rock was 3 ft long, 26 in wide, and 3 1/2 in thick. The rock broke up when it struck the employee.
An uninentional roof fall occurred in the main North seal area parallel entry to #3 main no. seal. The fall was noticed in adjacent entry by mine examiner. Area will be propped off & exam of all adjoining areas will be made for adverse condtions. Fall did not affect travel or ventilation in this area.
An unintentional roof fall occured in the #8 return entry outby unit 2. The fall occured do to the strata breaking at and below the anchor point. The area will be propped off. An inspection of all adjoining areas for adverse conditions will be conducted. The fall measured 20' x 20' x 4'. The fall area was located at 9+10 survey station.
EE was assisting another ee in loading a disconnect box from the Unit 2 bench into the bed of a diesel powered duv. They were skiding the disconnect box from the bench into the bed of a diesel powered duv in order to transport the box to the continuous miner. EE reached over the side of the duv. took hold of the disconnect box with his fingers inside the hole for the conduit
An unintentional roof fall occurred on Unit 1 MMU (001) working section in the last open crosscut between #3 and #4 entries at the +1885 foot mark. The fall occurred due to the failure at just above the anchor point, slip broke out from the roof at approx. 5-6'. The fall measured approx. 18' w x 40'. 1x5-6'. h. The roof was orginally supported with 5' conventional type roof bolts.

An unintentional roof fall occured in the #2 entry of the Main South Entries at the +2730 foot mark. The fall occured due to the fact that the roof strata broke just above the anchor point. The fall measured 20'x30'x6'. An investigation will be conducted in all adjoining areas for adverse conditions. The fall area will be cleaned up and resupported.
EE was operating a continuous miner when a piece of roof rock fell out between the rib and rib bolts. The rock was approx. 7'L,2'W, 0-5"" thick. The rock struck the back of his head and upper back. It drove him down onto the mine floor and he was partially pinned down by the rock being hung on his mine bolt. He receved abrasions and contusion to his back and leg.
EE was roof bolting on Unit #1. He was tightening up a roof bolt when he realized he forgot to put a steel plate under the wooden roof block. The wood roof block split when he applies torque pressure. He tried to back the roof bolt out of the roof, the wood block flew off and struck him on top of his right eye and cheekbone. Received a laceration above right eyelid. 4 sutures.
Employee was taking off the material hitch from the getman tractor. The pins were pulled the hitch slipped and pinched his right index finger between the hitch and the frame of the getman tractor. He was wearing gloves. 12 sitichers were required to close the laceration. He return to work the next day.
The Hoist for the Man/Material Shaft for the Gateway Mine was inoperable for 45 minutes due to a mechanical failure involving the braking system. Repairs were made and the Hoist became operational.
Employee had just finished helping unload belt structure. He was struck on the head by a rock that knocked his hard hat off. Before he could put his hat back on, a rock approximately 12 inches x 16 inches x 0-2 inches fell and struck him on the back of his head. He received a laceration to the back right side of his head. Four sutures were put in to close the laceration.
Employee was roofbolting in Unit #2 in Entry #2. He walked to the back of the roofbolter to make up a roofbolt when a rock fell out between the rib pin and the center pin. The rock was approx. 2'x2'x2"". The rock struck him on the left side of his neck and then slid down his left arm. He received a laceration to the left side of his neck requiring sutures.
Employee was descending the ladder on the 966 end loader. When his left leg hit the ground, he turned his body to the left & his left knee popped. He had both hands on the hand rails and one foot on the ladder. He continued to work up until 1/19/07 when he went in for arthroscopic surgery on his left knee. He returned to work on his next shift on 1/22/07 on restricted duty.

Employee was roofbolting, drilling a hole, when a rock fell from the roof and struck the canopy, struck him on the top of his head and right forearm. He received sutures to close the laceration.
Employee was unloading 4"" water pipe out of supply shed with another employee. The other employee had lifted one end of the pipe while he reached down to pick up the pipe with his right hand. As he did so he felt a tear just above the elbow of his right upper arm (bicep). Employee went in for out-patient repair of his tendon on 7/13/06.
The Hoist for the Man/Material Shaft for the Gateway Mine was unable to be operated due to a mechanical condition. The employees were called out of the mine and the mine idled until repairs were completed. The hoist was inoperable from 2:55PM until 4:25PM.
Injured was using a utility knife to cut off a piece of electrical tape from its roll. As the knife cut through the tape it slipped and stuck him on top of the upper right leg. 4 staples were installed to close the laceration.

THE HOIST FOR THE MAN MATERIAL SHAFT FOR THE GATEWAY MINE WAS UNABLE TO BE OPERATED DUE TO A MECHANICAL CONDITION. THE MAN MATERIAL SHAFT IS THE PRIMARY ESCAPEWAY FOR THE GATEWAY MINE.THE EE'S WERE CALLED OUT OF THE MINE AND MINE WAS IDLED UNTIL MECHINICAL CONDITION WAS CORRECTED. THE HOIST WAS UNOPERABLE FROM 7:10AM UNTIL 8:30PM
EE was loading overcast rails onto mine track, rail slipped, caught finger causing a fracture to left little finger.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE UNIT 3 WORKING SECTION IN THE NO.3 ENTRY AT THE +9620 FOOT MARK. THE FALL OCCURRED BECAUSE THE ROCK BROKE AT & ABOVE THE ANCHOR POINT. THE AREA HAD BEEN SUPPORTED WITH 4', 5', & 6' CONVENTIONAL ROOF BOLTS. FALL MEASURED 30'X16'X4-6'. FALL AREA WILL BE CLEANED UP & SUPPORTED. SURROUNDING AREAS WILL BE CHECKED FOR ADVERSE CONDITIONS.

An unintentional roof fall occurred in the main east travelway adjacent to mine shaft bottom. The area was originally mined in 1977. Fall occurred due to a hidden slip just below the anchor point. The brows will be reinforced, fall area will be cleaned up and the area supported. The fall area measured 20' x 20' x 5'.
INJURED EMPLOYEE WAS IN THE BATHHOUSE CHANGING CLOTHES PRIOR TO THE START OF HIS SHIFT. A BASKET ADJACENT TO HIS FELL DOWN & STRUCK HIM ON THE TOP OF THE HEAD. THE BASKET WAS EMPTY & IT SLIPPED OFF OF THE HOOK TO HOLD IT SECURE. HE RECEIVED 6 STITCHES TO CLOSE THE LACERATION ON THE TOP OF HIS HEAD.
An unintentional roof fall occured on the Unit 3 working section in the last open crosscut of the No. 5 entry at the 8060 foot mark. The fall occured due to the fact of roof failure at and just above the anchor point. The fall measured 20' by 17' by 5'. The brows will be reinforced with longer roof bolts, fall area cleaned up, and fall area supported.

The injured employee was repairing on the unit 2 roof bolting machine. he had removed the rear main control panel and as he was possitiong the cover during reinstallation he felt a sharp pain in his low back. This is incident has been under investigation awaiting information from the dr. ee should always use proper body position and ask for help when handling heavy objects.
Ee was operating diesel personnel carrier traveling outby to the ""A""""B"" power center located outby the working section. As he reached location of ""A""""B"" power box he stepped out of personnel carrier & proceeded to power center. As he was walking he stepped on a loose piece of coal causing him to twist his left ankle & left knee.
Ee was roof bolting in last open crosscut between entry 1 & entry 2 on unit 2. As he was inserting roof bolt a piece of roof rock fell from between the roof bolts, striking him on rt forearm. The incident caused a laceration to his rt forearm that required sutures. He started missing time on Aug 13 & is still under dr's care at this time.
EE was roof bolting on Unit 3. While on the process of drilling a hole a bolt came out of the pod jack allowing the jack to slide to the side of the pod. The ee reached in to move the jack free from the pod and the pod slid back and caught the ee's left index finger. Incident caused a fracture to the ee's left index finger.
EMPLOYEE WAS OPERATING AN OPEN DIESEL PERSONNEL CARRIER. HE WAS TRAVELING ALONG THE UNIT 1 TRAVEL WAY. THE BELT CREW WAS PULLING THE UNIT 1 BELT TOGETHER. THE BELT WAS BEING PULLED W/A DIESEL SCOOP ATTACHED BY A WIRE ROPE. THE DIESEL SCOOP PULLED ACROSS THE TRAVEL WAY AND EMPLOYEE RAN INTO THE CABLE. THE INCIDENT CAUSED A LACERATION TO HIS FOREHEAD REQUIRING STITCHES.

WRIST. THIS INCIDENT IS UNDER INVESTIGATION PENDING THE DR.'S REPORT. EE MUST KEEP HANDS CLEAR FROM PINCH POINTS.
THE INJURED EE WAS ROOF BOLTING ON UNIT 2. HE WAS WALKING OUT OF ROOM #1 AND A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS, SCRAPING DOWN BOTH LOWER LEGS AND LANDING ON HIS RIGHT FOOT. THE INCIDENT CAUSED CONTUSION/ABRASION TO BOTH LOWER LEGS AND CONTUSION TO THE RIGHT FOOT. THE EE WILL BE INSTRUCTED TO ALWAYS CHECK ROOF AND RIB IN HIS WORK AREA AND AREA OF TRAVEL.
EE was roof bolting on unit 3. He was in the process of installing an additional roof bolt for the purpose of hanging the trailing cable across the haul road. As he was reaching for the tightner wrench a piece of roof rock fell from between the roof bolts, striking his left hand.
EE was operating the cont miner on unit 1. He was rolling up the ventilation curtain in order to start a knotch, a piece of roof rock fell from between the roof bolts, striking him on the left knee. The incident caused a contusion to the left knee. EE started missing time 5-17-04.
EE was in the process of unhooking a material wagon from the diesel scoop. the material wagon tongue pin was stuck. The ee was using a pry bar to work the pin loose and as he applied pressure he felt pain in his right shoulder.

Employee was hanging a pull-through curtain on the working section, using a brass hammer to drive a spad. He struck his left thumb with the hammer.
EE WAS SUPERVISING REHABILITATION WORK IN THE MAIN EAST RETURNS. HE WAS OBSERVING THE ROOF BOLTER OPERATOR INSTALL A ROOF BOLT WHEN A LOOSE ROCK ROLLED OFF THE RIB AREA, STRIKING INJURED EE ON THE LEFT LEG & ANKLE. EE WAS TREATED IN THE HOSPITAL FOR A POSSIBLE BLOOD CLOT. HE STARTED MISSING TIME ON 02/06/04 & IS STILL UNDER THE DR.'S CARE AT THIS TIME. EE SHOULD ALWAYS CHECK AREA.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER ON UNIT 1. HE WAS STANDING JUST OUTBY THE CONTINUOUS MINER TAIL AND A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS, STRIKING HIS BACK. THE INCI DENT CAUSED A CONTUSION TO THE BACK AND A FRACTURE TO A RIB. THE EMPLOYEE WILL BE INSTRUCTED TO ALWAYS CHECK THE ROOF AND RIB IN HIS WORK AREA AND TO SCALE DOWN LOOSE MATERIAL.
THE EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE REMOVE A BELT SAIL. INJURED EE WAS HOLDING THE SAIL AND THE OTHER EMPOYEE WAS APPLYING PRESSURE TO THE WRENCH. THE WRENCH SLIPPED, MASHING EE'S RIGH T INDEX FINGER. THE INCIDENT CAUSED AN ABRASION / LACERATION TO HIS FINGER. THE EMPLOYEE WILL BE INSTRUCTED TO ALWAYS AVOID PINCH POINTS AND TO ALWAYS COMMUNICATE WITH FELLOW EMPLOYEES.

HE WILL BE INSTRUCTED TO ALWAYS STORE THE ROOF BOLT STEEL IN AN AREA CLEAR OF THE ATRS AND CANOPY. HE STARTED MISSING TIME ON 12/30/03.
EMPLOYEE WAS IN THE PROCESS OF KNOCKING OUT A PROP. AS HE WAS STRIKING THE PROP HE STRUCK THE TIP OF THE LEFT MIDDLE FINGER, FRACTURING IT. AS THE INJURED EE WAS HOLDING THE CURTAIN BOARD IN PLACE, A PIECE OF ROOF ROCK FELL FROM BETWEEN ROOF BOLTS STRIKING HIM ON T HE BACK. THE INCIDENT CAUSED A CONTUSION & STRAIN TO THE EE'S BACK. HE WILL BE INSTRUCTED TO ALWAYS CHECK THE ROOF & RIB CONDITIONS IN HIS WORK AREA PRIOR TO PERFORMING A TASK. THIS INCIDENT
EE WAS HELPING PUT THE BUMPER BACK ON THE CONTINUOUS MINER. HE STATED LATER THAT HE FELT PAIN IN HIS LEFT SHOULDER. THE INCIDENT CAUSED A STRAIN TO THE SHOULDER AND THE EE STARTED MISSING TIME ON DECEMBER 9. HE WILL BE INSTRUCTED TO ALWAYS USE PROPER LIFTING PROCEDURES. THIS INCIDENT HAS BEEN UNDER INVESTIGATION PENDING MEDICAL REPORTS.

THE INJURED EE WAS ROOF BOLTING ON UNIT 3. HE POSITIONED THE ROOF BOLTING MACHINE FOR THE PURPOSE OF INSTALLING A ROOF BOLT. AS HE SET THE BOOM OF THE ROOF BOLTER MACHINE DOWN ONTO THE MINE FLOOR HE CAUGHT HIS LEFT FOOT BETWEEN THE MINE FLOOR & THE BOOM. THE INCIDENT CAUSED A FRACTURE TO THE LEFT BIG TOE. EE WILL BE INSTRUCTED TO ALWAYS BE AWARE OF PINCH POINTS.
THE INJURED EE WAS DELIVERING MATERIAL TO UNIT 2. AS HE WAS UNLOADING CONCRETE BLOCKS FROM THE MATERIAL WAGON, A CONCRETE BLOCK ROLLED OFF THE MATERIAL WAGON & ONTO HIS RT.FOOT CAUSING A CONT USION. THE EE WILL BE INSTRUCTED TO ALWAYS USE PROPER BODY POSITION & KEEP CLEAR OF SHIFTING LOADS.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE UNIT 2 WORKING SECTION AT THE +1750 FOOT MARK OF THE NO.4 ENTRY. THE FALL OCCURRED DUE TO FAILURE AT AND BELOW THE ANCHOR POINT. A SLIP WAS PRESENT I N THE AREA AND THE AREA HAD BEEN SUPPORTED BY 4', 5', AND 6' CONVENTIONAL TYPE ROOF BOLTS. THE FALL AREA WILL BE REINFORCED, THE FALL CLEANED UP AND SUPPORTED. THE FALL MEASURED 30' X 40' X 6
EE WAS ROOF BOLTING ON UNIT 2. AS HE WAS SETTING UP TO BOLT A NOTCH, HE SET THE ATRS AND A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS, STRIKING HIM ON THE LEFT LOWER LEG. THE INCIDENT CAUSED A FRACTURE TO THE LEFT LOWER LEG. THIS INCIDENT HAS BEEN UNDER INVESTIGATION WAITING FOR MEDICAL INFORMATION FROM THE DR'S OFFICE.
THE INJURED EMPLOYEE WAS CHECKING SCSR'S ON THE OLD UNIT 3 WORKED OUT, RECOVERY AREA. AS HE TURNED TO WALK AWAY FROM THE SCSR'S A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS AND STRUCK HIM ON THE BACK LOWER RIGHT LEG AND ANKLE. THE EMPLOYEE STARTED MISSING TIME ON 9/16/03 AND HE IS STILL UNDER THE DOCTOR'S CARE AT THIS TIME.
EE WAS WORKING IN THE OLD UNIT 3 RECOVERY AREA. HE WAS IN THE PROCESS OF SECURING A ROLL-UP CURTAIN WITH THE ATTACHED CHAIN. AS HE WAS SECURING THE CURTAIN HE CAUGHT HIS LT.HAND BETWEEN THE CU RTAIN & THE CHAIN. THE INCIDENT CAUSED A CONTUSION & STRAIN TO HIS LT. HAND. HE MISSED A TOTAL OF 13 DAYS, HE IS STILL UNDER THE DR.'S CARE AT THIS TIME. THIS INCIDENT IS UNDER INVESTIGATION.
THE INJURED EMPLOYEE WAS ROOF BOLTING & A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS STRIKING HIM ON THE LEFT SIDE & LEFT HAND. THE INCIDENT CAUSED A FRACTURE TO THE LEFT HAND. HE STA RTED MISSING TIME ON 9/1/03 & IS STILL UNDER THE DOCTOR'S CARE AT THIS TIME.
THE INJURED EMPLOYEE WAS HELPING SET BITS ON THE CONTINUOUS MINER. AS THE OTHER EMPLOYEE WAS STRIKING A BIT WITH A HAMMER A PIECE OF STEEL STRUCK EMPLOYEE IN THE UPPER RIGHT CHEST AREA. HE WAS TAKEN TO THE LOCAL HOSPITAL AND THE PIECE OF STEEL WAS REMOVED. THE INCIDENT REQUIRED SUTURES. HE DID NOT MISS ANY TIME AWAY FROM WORK. HE IS WORKING WITH SUTURES.
AN UNINTENTIONAL ROOF FALL OCCURRED ON MAIN EAST TRAVELWAY BETWEEN 189-190 CROSSCUT IN NO. 4 ENTRY. THE FALL WILL BE CLEANED UP AND SUPPORTED.
THE INJURED EMPLOYEE WAS ROOF BOLTING ON UNIT 1. A PIECE OR ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS AS HE WAS DRILLING A HOLE. THIS INCIDENT HAD BEEN UNDER INVESTIGATION PENDING INFORMATION FROM THE ATTENDING DOCTOR. INFORMATION WAS RECEIEVED FROM THE DOCTOR'S OFFICE ON SEPT. 7, 2003.
EE WAS ASSISTING A REPAIRMAN IN REMOVING A HINGE PIN BUSHING. HE CAUGHT HIS LEFT THUMB WITH THE SLIDE HAMMER WHILE TRYING TO KEEP THE SLIDE HAMMER CENTERED FOR THE REPAIRMAN. THE INCIDENT CAUS ED A LACERATION TO THE LEFT THUMB THAT REQUIRED SUTURES. HE DID NOT MISS ANY TIME AWAY FROM WORK (HE IS WORKING WITH SUTURES).
EMPLOYEE WAS WORKING WITH A BELT CREW INSTALLING BELT ON UNIT 4. WHILE HE WAS IN THE PROCESS OF INSTALLING BELT HARDWARE, HE STEPPED ON A PIECE OF LOOSE COAL CAUSING HIS RIGHT FOOT TO SLIP, TW ISTING HIS RIGHT KNEE. HE DID NOT START MISSING TIME UNTIL AUGUST 1, 2003 AND HE IS STILL UNDER THE DOCTOR'S CARE AT THIS TIME.
EE WAS OPERATING THE UNIT 1 CONTINUOS MINER. WHILE CUTTING COAL IN THE NO. 2 ENTRY A PIECE OF ROOF ROCK FELL FROM BETWEEN THE ROOF BOLTS, STRIKING THE BACK OF HIS NECK. THE INCIDENT CAUSED A L ACERATION THAT REQUIRED SUTURES. THE INJURED EE IS WORKING WITH SUTURES AND DID NOT MISS TIME AWAY FROM WORK.
AN UNINTENTIONAL ROOF FALL OCCURED IN THE 1ST SOUTH TRAVELWAY OFF THE 2ND MAIN EAST TRAVELWAY ATTHE #75 CROSSCUT. THE FALL OCCURED DUE TO THE FACT THAT THE ROOF STRATA BROKE LOOSE BELOW THE ANCHOR POINT. AN INVESTIGATION OF ALL ADJOINING AREAS WILL BE MADE FOR THE PURPOSE OF CHECKING FOR ADVERSE ROOF CONDITIONS. THE FALL AREA WILL BE CLEANED UP & AREA SUPPORTED. THE FALL MEASURED
EE WAS INSTALLING A BUCKET LIFT JACK ON THE UNIT 1 CONTINUOUS MINER AND THE JACK DROPPED AND STRUCK THE TOP OF HIS RIGHT HAND CAUSING A LACERATION. THE INCIDENT CAUSED A LACERATION THAT REQUIR ED SUTURES. THE INJURED EE DID NOT MISS ANY TIMEAND IS WORKING WITH SUTURES.
EE WAS ROOF BOLTING ON UNIT 1. AS HE WAS INSTALLING ROOF BOLT A PIECE OF LOOSE ROCK FELL, STRIKING HIM ON HARD HAT, RT SHOULER, RT ARM. EE STARTED MISSING TIME JUNE 4, 03 & IS UNDER THE DR'S C ARE AT THIS TIME. THIS INCIDENT HAS BEEN UNDER INVESTIGATION PENDING THE DR'S DIAGNOSIS & REPORTS. THE REPORTS WERE RECEIVED JUNE 10, 2003 CONCERNING THE EE'S CONDITION.
AN UNINTENTIONAL ROOF FALL OCCURED IN THE 2ND MAIN EAST RETURN ENTRY AT THE 450 FOOT MARK OF 11TH ENTRY. THE ROOF FALL WAS FOUND BY THE MSHA INSPECTOR DURING ROUTINE INSPECTION. THE FALL ACCES S AREAS WILL BE TIMBERED OFF TO PREVENT ACCESS. THE AREA WILL BE CLEANED UP. ALL ADJOINING AREAS WILL BE CHECKED FOR ADVERSE CONDITIONS AND CORRECTED IF NEEDED.
AN UNINTENTIONAL ROOF FALL OCCURED ON THE UNIT 3WORKING SECTION ON THE RIGHT SIDE OF THE UNIT INROOM 1 AT THE +165 FT MARK. THE AREA HAD A SLIP PRESENT & THE AREA HAD BEEN SUPPORTED BY 5 & 6 FT CONVENTIONAL TYPE ROOF BOLTS. THE FALL WILL NOT BE CLEANED UP & ALL APPROACHES HAVE BEEN TIMBERED OFF & DANGER SIGNS POSTED. THE FALL MEASURED 5' HIGH, 16' WIDE & 20' LONG
EE WAS OPERATING THE DIESEL MATERIAL SCOOP. AS HE TRAVELED ALONG THE UNIT 2 TRAVELWAY THE SCOOP STRUCK A BUMP CAUSING THE SCOOP TO BOUNCE AND EE STRUCK HIS HEAD AGAINST THE SCOOP CANOPY. THE INCIDENT HAS BEEN UNDER INVESTIGATION WAITING FOR MEDICAL REPORTS FROM THE DR.
EE WAS USING A COME-ALONG CHAIN HOIST TO RAISE A BELT STRUCTURE. AS HE WAS APPLYING PRESSURE THE CHAIN HOIST OR THE CHAIN HANGER BROKE LOOSE AND STRUCK HIS RIGHT EYELID. THE INCIDENT CAUSED A LACERATION TO THE RIGHT EYELID THAT REQUIRED SUTURES. HE RETURNED TO WORK THE NEXT SCHEDULED SHIFT.
