Man was taking bolt off of Roof Bolter to install. Bolt hung up. He pulled, pin broke loose pinching his right hand against rib. Mashed right little finger. Stitches. Accident was under investigation.

Man was walking main north belt linem slipped in mud, fell striking right knee cap on mine floor. Off for surgery 8-3-07.

Checking feeder under raw coal silo, wheel used to open gate on feeder fell off shaft and hit employee on left leg. Stitches received on left lower leg.

Man was moving rock from the top of the miner. Rock pinched his right ring finger against the frame, causing a laceration to end of finger, received 4 stitches.

Struck against stationary object

Stepped on walk board 3rd south submain intake overcast. Walk board broke. Employee broke through board and sprained his right ankle.

Struck against a moving object

Man was riding in mantrip; mantrip rubbed against rib and caught his lamp pouch, pulled man around straining his right shoulder. He started missing 3-30-07.

EE was straightening on 8' TR bolt in bolt hole and hyperextended his left thumb. Started missing on 1/4/07.

Over-exertion in lifting objects

Picking up parts in warehouse, lifting caused strain in stomach - had hernia surgery. Started missing on 1/22/07. Reason for delay in reporting - waiting on medical from doctors.

EE SLIPPED ON SLATE AND COAL WHILE FILLING WATER TANK ON THE MAINTENANCE VEHICLE FOR FILLING BATTERIES. EE FELL DOWN UNDER THE VEHICLE AND HURT HIS RIGHT HIP AND RIGHT SHOULDER - LOWER BACK PAIN. INITIAL X-RAYS NEG. 1ST DAY MISSED WAS 1/6/06 (MINE WAS SHUT DOWN 1 WEEK AT CHRISTMAS). X-RAY & MRI BY ORTHO. SPECIALIST. DIAG. VERTEBRA PROBLEM, SURGICAL REPAIR REQUIRED.
