Employee was using hand grinder to grind down weld. After shutting off and preparing to set the grinder on the bench, the wheel became tangled in the string of his hooded sweatshirt, which then wrapped up and contacted his jaw causing a laceration.

Two employees were in the process of opening a rail car hopper gate. Both employees were using bars to rotate the gate mechanism when the injured employee felt a pop in his elbow.

Injured stepped up to the 10 belt ramp in the north plant hummer house when his foot slipped and he began to fall. Injured reached out with his left hand to catch himself when his middle finger contacted the conveyor roller.

Security reported an explosion and fire in the south plant at approximately 11:05 am. CFD was contacted and extinguished the fire. Investigation determined that a ComEd transformer had experienced an explosion and fire. No miners were on the property at the time of incident, and no damage occurred to mine equipment as a result. MSHA was contacted at approximately 11:08 am.

Injured was loading trucks from a stockpile with a 980H loader when a large piece of material rolled down and struck the loader arms jarring the machine. Injured initially requested medical attention, but was released with no restrictions. Injured subsequently was diagnosed with a hernia, underwent surgery on 5/21, and was unable to return for scheduled work on 5/23.

The employee was walking from a work location carrying a shovel and broom. The employee reportedly slipped, fell, and struck his head while walking from the pay loader to the main office. The employee was disoriented and did not clearly remember the exact location he fell, or what events led up the fall.

The employee was cleaning out a lime elevator, he went up the stairs to get a sledge hammer and while walking down the stairs he stepped on a piece of lime and twisted his right ankle.
Employee was performing clean up of a maintenance job and came into contact with lime dust causing irritation to his forearms and thighs.
Employee operating Hi-Vac for routine housekeeping, struck a structural cross beam with the hose, causing settled lime dust to fall down and make contact with employees eye which caused irritation.

Injured was carrying a piece of angle iron through a doorway. The door closed and pinched his finger between the angle iron and the door, causing a laceration.
EE was moving a pallet of brick, 3,900 lbs, via manuel pallet jack. He felt pain in his neck (right side). Electric pallet jack was not working.
Injured was walking down the stairway in the south loadout, when she stepped awkwardly and twisted her left ankle.
Injured was angry over a discussion that he had with his supervisor. While exiting the plant, he punched the side of the building by #4 coal hopper. Injured reported to work the next day with a cast and informed management that he had broken his right hand.

A fire ignited on the L-500 conveyor at approximately 3:10 am on October 4. The fire was the result of hot lime contacting the tail pulley area of the conveyor. The hot lime fell through a small floor opening next to a dust collection pipe. The fire was extinguished in approximately 40 minutes. No injuries occurred as a result of this fire.
Injured was attempting to feed coal throught the Kiln 4 coal grates when he stepped into an opening twisting his knee. It occurred on 9/7 but medical attention was not requested until 9/20 when the condition did not improve.
Kiln 4 was in the process of being shut down when the oil in the bull gear housing caught fire. The fire was fully contained within the gear housing and was extinguished in approximately 30 minutes.
DRIVER WAS COMING DOWN LADDER FROM TOP OF TRAILER, WEATHER WAS NOT AN ISSUE, MISSTEPPED AND FELL. WAS NOT WEARING FALL PROTECTION HARNESS.

Injured was attempting to remove a drive guard from a crusher to change the V-belts. While positioning the guard, the employee's pinky finger was pinched between the guard and the mounting structure causing a laceration/contusion requiring medical treatment.
Injured was cleaning in grinding mill area when lime dust entered his left eye.
Injured was climbing down a ladder in the #5 baghouse when he felt a pop in his right shoulder.
Injured was throwing a rail switch near the main office. When he flipped the switch handle into the alternate position with his left hand, his pinky finger was pinched between the handle and a concrete barricade next to it.
Injured was attempting to unplug stone chute with a bar when he pinched his finger between the bar and the chute.
Injured slipped while walking down a ramp and struck his right foot on the structure causing a contusion to the foot. Incident was reported on 1/19, but medical treatment was not requested by the employee until 1/27.

EE was changing a lightbulb outside of the lunchroom. He placed a 6' ladder on the landing and climbed the ladder. One of the ladder legs was not on solid footing and the ladder tipped, causing EE to fall approximately 3' to the ground (fractured right wrist).

Injured was changing oil in coal mill gearbox. When he removed drain plug, hot oil from the gearbox splashed and contacted his right forearm. The contact with the hot oil caused a second degree burn.
Injured slipped while going through door, caught left hand in door when it shut, causing closed fracture to left third finger.
EE WAS WORKING ON #4 BRIQUETTER WHEN A FLANGE HE WAS WORKING ON CAME DOWN AND SMASHED HIS RIGHT HAND CATCHING HIS FINGERS, HE WAS TAKEN TO THE COMPANY CLINIC.

EE WAS WORKING WITH A CO-WORKER TO CHANGE A PLOW PLATE AND ONE OF THE PLATES SHIFTED CATCHING EMPLOYEE IN RIGHT HAND.
EE STARTED WORK @0600. NORMAL DUTIES DRIVING PAYLOADER UNTIL 0800. AT 0800 EE WENT ON BREAK. HE WAS EATING A SANDWICH & STARTED TO SEIZE UP. WITNESSES CLEARED MOUTH, CALLED 911. E.R. RESPONSE SHOWED UP SHORTLY THEREAFTER. EE PASSED AWAY.
THE EE WAS DECENDING A LADDER FROM AN ELEVATED WORK PLATFORM AND SLIPPED OFF OF THE NEXT TO LAST LADDER RUNG RESULTING A STRAIN OF THE EE S LOWER BACK. THE EE WAS PLACED ON RESTRICTED DUTY FOR 10 DAYS.

EE WAS WALKING DOWN A FLIGHT OF 5 STAIRS. THE BOTTOM STAIR HAD A SMALL CHIP ON THE CORNER. EE SLIPPED ON BOTTOM STAIR & FELL BACKWARD. HE ATTEMPTED TO BREAK HIS FALL WITH HIS LEFT WRIST. HE SU STAINED A SMALL CONTUSION ON HIS WRIST & A SPRAIN OF THE SAME WRIST.
EE WENT TO SOUTH CHICAGO MARBLEHEAD LIME TO RETRIEVE A WELDER FROM THE JOB SITE. AS THE EE WAS HOOKING THE WELDER UP TO THE TRUCK, HE DROPPED THE TRAILER TONGUE ON HIS ANKLE.
EE WAS WALKING DOWN STAIRWAY AND MISSED LAST STAIR. TWISTED KNEE.
HOT LIME LEFT THE LIME COOLER ONTO THE TAKE-AWAY BELT AND THE HEAT FROM THE LIME STARTED A FIRE ON THE BELT AT THE HEAD PULLEY AREA. THE FIRE DEPT RESPONDED IN APPROX 20 MINUTES AND CONTROLLED THE FIRE. DAMAGE LIMITED TO THE COOLER BELT.
EE WAS UNCLOGGING AN OZ PIPE, ROTARY KILN LIQUID IN PIPE EJECTED OUT OF PIPE CAUSING BURNS ON EE'S LT. ARM AND SIDE OF CHEST. BURNS CAUSED BY HOT WATER.

EE WAS ATTEMPTING TO PLACE A HIGH PRESSURE HOSE INTO A SURFACE DRAIN IN ORDER TO FILL WITH WATER A SURFACE PIT USED TO WASH LIME FROM SEMI TRUCK. THE EE PLACED THE HOSE INTO THE DRAIN, WHILE W ATER WAS ON, AND WHEN HE LET GO OF THE HOSE HANDLE THE NOSE DUE TO PRESSURE, CAME BACK TO OF THE DRAIN STRIKING EE LEFT LEG. UPON INTINUED OBSERVATION AT DINIE INCIDENT WAS CLASSIFIED AS FIRS
EE WAS LOADING FLUE DUST INTO A TRUCK. A ROPE USED IN THE LOADING PROCESS BECAME ENTANGLED AROUND HIS LEG. THE EE ATTEMPTED TO BREAK HIS FALL BY GRABBING A HANDRAIL. THE EE'S LEFT SHOULDER WAS IN THE PROCESS OF CONTROLLING HIS FALL.
WHILE DUMPING A HOPPER WORKER WAS RESETTING REEASE HANDLE. WORKER WAS DUMPING HOPPER. RELEASE HANDLE IS LOOSE NOT PER PIN TO THE FRAME OF THE HOPPER WORKER WAS PULLING HOPPER BACK INTO PLACE. WITH HIS OTHER HAND. HOPPER CAME DOWN HOLDING BAR HIT THE HOOK ON THE HOPPER AND PUSHED REEASE HANDLE PINCHING WORKERS MIDDLE FINGER ON LEFT HAND.

EE WAS SWINGING A SLEDGEHAMMER & HIT A CHAINFALL CHAIN THAT WAS PULLED TIGHT, WHEN THE HAMMER ""BOUNCED"" OFF THE CHAIN, EE LOST HIS BALANCE & LANDED ON HIS TAILBONE CAUSING A BRUISE.
THE INJURED EE WAS PERFORMING MANUAL MATERIAL HANDLING, UTILIZING A SHOVEL AND WHEELBARROW. WHILE FILLING THE WHEELBARROW, HE HIT HIS HAND ON THE WALL CAUSING THE WHEELBARROW TO TILT. IN TRYIN G TO PREVENT THE WHEELBARROW FROM TIPPING OVER, HE PULLED A GROIN MUSCLE RESULTING IN AN INGUINAL HERNIA ON THE RIGHT SIDE.
EE WAS TRYING TO DISLODGE PRODUCT FROM HIS HOSE.THERE WAS STANDING WATER ON THE PROPERTY THAT APPARENTLY HAD HYDRATED LIME IN IT. HE SUSTAINED CHEMICAL BURNS TO HIS LEGS & FEET AS A RESULT.
THE CONTRACTOR WAS PERFORMING SCHEDULED HOUSEKEEPING ACTIVITIES BY REMOVING SPILLAGE AROUND THE #6 CONVEYOR BELT HLE. THE CAB OF THE VACUUM TRUCK WAS FOUND TO BE SMOKING AND SOON WAS ON FIRE. THE CHICAGO CITY FIRE DEPT RESPONDED AND THE FIRE WAS EXTINGUISHED BY 10:15 PM. THE CAUSE OF THE FIRE IS UNKNOWN AND IS BEING INVESTIGATED.
A FIRE OCCURRED IN THE 480 VOLT ELECTRICAL SWITCH GEAR SERVICING THE NORTH 1/2 OF THE PLANT. CHICAGO CITY FIRE DEPT AND COMMONWEALTH EDISON (ELECTRICAL PROVIDER) RESPONDED. THE FIREFIGHTING R ESPONSE WAS DELAYED UNTIL THE SWITCHBOARD WAS DE-ENERGIZED. DAMAGE WAS LIMITED TO THE SWITCHGEAR PROPER. NO STRUCTURE DAMAGE TO FACILITY. ALL PLANT ELECTRICAL EQUIPMENT IS PCB FREE. NONE PRESE

FIRE STARTED IN STORAGE BUILDING. ATTEMPTS MADE TO COMBAT FIRE. FIRE DEPT. CALLED AT 2:10 AM. ONCE UNDER CONTROL WASTE ASSOCIATED WITH FIRE WAS REMOVED. BUILDING WAS INSPECTED BY INDEPENDENT ENGINEER WHO ASSESSED IT AS SOUND.

EMPLOYEE WAS ENTERING PLANT AND DIDN'T HEAR THE TRAIN AS HE WAS APPROACHING THE ENTRANCE. THE TRAIN HI THE LEFT SIDE OF THE EMPLOYEE'S VAN. THE TRAIN WAS ON THE MAIN LINE, NOT ON PLANT PROPERTY.
WHILE EE WAS CLEANING RAW STONE USING A SHOVEL AS HE TWISTED TO DUMP HIS SHOVEL HE FELT A PAIN IN HIS LOWER BACK.
EMPLOYEE WAS CLEANING OUT THE EAST DUST CHAMBER OF #5 KILN WITH HIGH PRESSURE WATER. WHILE CLEANING THE CHAMBER, MUD AND WATER SPLASHED BACK IN HIS FACE, KNOCKING OFF HIS SAFETY GLASSES AND SPRAYING LIME BACK INTO HIS EYES.
