Two maintenance employees were adding a return roller and brackets to BC-124. While putting the roller into the brackets one side went in with ease. That employee then went to help EE's partner by lifting the belt. The struggling employee hit the roller with EE's had causing it to fall out of the bracket and land on the side of EE's boot fracturing EE's pinky toe

Employee was going to help clean at the top of the penthouse. While walking to the area one of the plugs for a clean out hole was laying on the catwalk and the employee tripped over the plug and hit their knee on the expanded metal flooring.

While shutting down to change a roller the torch for BC-61 was not shut down properly causing the belt to catch on fire and the fire department was called and had the fire was out at 4:50AM.
Employee was using a grinder and slipped and lost balance while grinding. Employee received an injury to right index finger which required 3 stitches.

Employee was driving a club car in the quarry when they reached for their radio releasing the steering wheel, which caused the wheel to spin hard to the left. When the employee attempted to correct the steering wheel, they also pressed the brakes. This caused the mobile equipment to ascend the berm and tip over on the passenger side.
The Dryer operator was using a mini skid to perform general cleanup of the Dryer Building. While traveling in reverse, they pinched their little finger between the mini skid and an I-beam cross member.

The miner lost footing while entering DR-02. When they went to catch themselves their left elbow came in contact with a sharp edge inside of the dryer drum. This caused a laceration and they received 5 stiches.
On 6/22/22 at 9:00am a maintenance employee was tightening a bolt on the jaw crusher and the wind blew something into EE's eye. EE tried washing it out but was unsuccessful. The miner went to the local doctor office and they were able to remove a foreign object from EE's eye.
The miner injured EE's knee while stepping across the wet screen to perform maintenance. The miner slipped when stepping from the top of the eccentric housing to a round support member.
Employee was using a bar to try to unplug HO-18. It became plugged due to extreme cold weather and frozen chunks of sand. While trying to free the hopper of frozen sand employee strained left shoulder.

At approx. 3:15am the belt tender lit a torch on BC-09 then started walking to truck. While EE was walking to truck, EE lost footing and twisted ankle. EE called supervisor and reported the incident. EE stated that the soft spot on the bottom of the foot was hurting. The miner has a fractured bone in foot.
On 11/4/21 at 6:45am employee was leaving the trailer to drop off paperwork in the main office. While going down the stairs they missed the last step and fell to the ground. EE stated it was their first week in the new office and was use to having 3 steps not the 4 we have now. EE missed the last step thinking they were on the ground level.
Employee was tasked with cleaning frozen material from quarry hoppers. While using an ice scraper to free the material employee felt pain in shoulder and neck.

Employee was attempting to clean out the sump hole at the west end of the reclaim building. When EE tried to lift the sump pump from the hole EE felt a pop in EE's right forearm. Employee reported the incident to EE's supervisor and finished EE's shift. The following Monday (9/14/20) the employee visited a doctor who placed EE on modified duty and suggested an MRI.
The employee works in a position where frequent shoveling is required. EE's shoulder has been sore for a while and aggravated by shoveling. Is experiencing tingling in EE's arm when trying to sleep. Went to Dr. who says scar tissue has built up and prescribed 6 weeks of physical therapy.

FLW was removing an idler pulley from a piece of tracked equipment. While striking the pulley with a 3# hammer a shard of metal came off of the hammer and struck the worker in upper thigh.
Employee was replacing a troughing roller on BC54 at the head end of the belt. While turning to pick up a bolt to fasten down the new assembly EE hit knee on the torque arm support for the reducer. This caused EE's knee to flex sideways. EE was experiencing some swelling and soreness. Update: Went to doctor on 9-20-19 and scheduled an MRI for 9-25-19.
Employee was thawing out bridged material above BC-107 and a large piece broke off. Employee pulled the torch back that EE was running and in doing so twisted shoulder. Required no medical attention at the time. 8 months after the incident EE was having issues with the shoulder and went to the doctor for an examination.

Miner put ladder into a hopper to unplug frozen sand. Miner had fall protection on. Before standing on frozen sand the area was 'tested' with sledge hammer. The area gave way & miner fell 3 ft. Miner's rt knee hit the hopper causing bruising. The miner grabbed ladder to help arrest fall causing a strained rt shoulder. Miner completed shift & was examined the next day.
Miner was walking to the parking lot at the end of EE's shift. EE removed EE's half face respirator while enroute and took EE's eyes off the ground EE was walking on. As a consequence, EE tripped on the pavement, and fell landing on EE's right shoulder.
Miner was on a step ladder, approximately 5' High. Miner had 3 points of contact while attaching a hose. Miner twisted while installing the hose shifting EE's body weight causing the ladder to kick out from under EE. The miner fell striking EE's rib area. Miner was assessed and had no signs of injury but was taken to emergency room to be checked out.

The employee was covering the end loader during the operators lunch and jarred their back. Employee went to the Chiropractor and was given light duty.
Employee was attempting to clean out auger when EE's left index finger came in contact with the auger blade lacerating tip. First aid and medical treatment was provided. The index finger was partially amputated. Employee returned to work on 4/29/16 with restrictions.

Original incident happened on 11/13/2014, has now become an MSHA reportable as of 07/09/2015. Employee was working in a warehouse storage building used for spare parts. He went to open a large spring loaded overhead door, he reached up to pull the chain to open the door, the door did not move and employee felt a pop in his left shoulder.
Employee was helping to remove head pulley lagging on a conveyor, as he was turning the pulley by use of drive belts, his finger became caught in the motor sheave, since conveyor had a back/stop sheave would not reverse, so two drive belts were cut off to free employees index finger from between drive belts and sheave of motor. Tip of employees index finger was broken.
EMPLOYEE WAS WORKING IN MOBILE MAINTENANCE SHOP AS AN ASSISTANT. A MAINTENANCE TRUCK WAS ON THE HOIST AND EMPLOYEE WAS SERVICING FRONT DIFFERENTIAL WITH/80/90 GREASE. FILLING WAS COMPLETE AND HE DUCKED TO GET PAST REAR DIFFERENTIAL AND THEN WALKED INTO THE SHOCK ABSORBER MOUNT, WHICH IS LOCATED UNDER THE REAR AXLE. CUTTING THE TOP OF HIS HEAD.

Injured employee was performing his normal belt tending job. Some of the conveyor belts stopped running and employee had to turn the LP gas off which was heating the conveyor belts. He got out of the truck and started towards the shutoff valve and then slipped on the sand, snow, and ice with his right foot. His left shoulder struck the vertical beam under the BC03 stairway.
Employee electrician was splitting telephone wire with a utility knife, he was splitting wire by pulling the utility knife towards him, in doing so the knife slipped and cut employee in the abdomen requiring (4) stitches.
ORIGINAL INJURY WAS REPORTED TO MSHA ON 10/3/2011 ON FORM 7000-1. RETURN TO FULL DUTY REPORT WAS SENT IN ON 09/04/2012. EMPLOYEE REPORTED HE HAS INJURED HIS SHOULDER AGAIN AND HAS BEEN PUT ON RESTRICTED DUTY AS OF 01/18/2013. THIS FORM IS BEING RESUBMITTED AS A NEW INCIDENT.
EMPLOYEES WERE ATTEMPTING TO CLEAN OUT A SAND SAMPLER 2"" PIPE OF WET SAND, ONE EMPLOYEE WAS UP ONE LEVEL AND OTHER EMPLOYEE WAS AT BOTTOM OF THE PIPE TO BE CLEANED OUT. AS UPPER EMPLOYEE WAS RODDING OUT WET MATERIAL BAR SLIPPED FROM HIS HANDS AND IN DOING SO LOWER PERSON WAS HIT WITH BAR ON HIS LEFT INDEX FINGER.

Employee was carrying 24""x24"" sheets of roof siding. While carrying pieces of this sheeting a piece fell onto the catwalk he was walking on. Employee did not notice he had dropped a piece and stepped on the piece lying on the catwalk causing him to slip and fall hitting his knee on the expanded metal catwalk.
Employee was picking up (2) 50lb. bags of sand from a pallet, as employee was lifting bags he felt a popping in his stomach area. He continued to carry bags to bagging building loading door to be loaded out into a vehicle.
EMPLOYEE WAS HELPING TO RE-RAIL A RAIL ROAD CAR. RE-RAILING EQUIPMENT WAS BEING PUT INTO PLACE AND RAIL ROAD SPIKES WERE DRIVEN INTO RAILROAD TIES. AS EMPLOYEE WAS DRIVING A SPIKE THE SPIKE CAME OUT OF TIE AND STRUCK EMPLOYEE IN LEFT INDEX FINGER
Maint. needed to be performed in quarry, so production supervisor fueled welder which was mounted in back of pick-up truck. When finished he stepped down from back of pick-up with his right leg. Once his right foot was one foot from ground he stepped down and felt pain in his right knee.

EE was attempting to move a Drisco Pipe 6"" Dia X 22'8"" in Length with steel flanges on both ends. Steel flange on pipe caught on floor, from a steel block measuring 6""x6"" square and sticking up 1/4"" that had been welded over a bin hole in screen house. EE jerked on pipe to free up, pipe came loose and rolled into a steel support beam, striking L hand between beam and pipe.
Employee was bagging 100lb bags from a bag conveyor to pallets, while stacking bags he felt a pain in his right shoulder area.

EE was working on uneven quarry floor. He is not sure what caused injury in his words, more of a cummlative injury by being jostled in the endloader cab.
Employee was exiting F-250 pickup truck-while stepping down he slipped on ice striking his back and ribs on running board. This was reported but did not become reportable until 1/20/10
Due to the heart attack, the operator was not in controll of the truck and ran into the cut wall causing minor damage to the front of the truck. This occured while performing overburden work at the mine

EE had just put a ladder in back of a pick up truck as he was walking to the drivers door he slipped on the ice and fell. A light dusting of snow had fallen and covered the ice.
Employee was working from South Quarry stockpile with endloader. While backing from pile tire entered uneven area swaying endloader. At this time employee felt pain in his right side lower back.
Injury was working on dryer line 3 with other empoyee. After lunch. He went back up the stairs and started across the elevated walkway. As he crossed the walkway, the grating shifted and he fell approx. 10' to the concrete floor below.
EE was performing clean up work with a shovel and wheel barrow, EE was walking behind line #3 cooler when he slipped and fell, EE slipped on the thin layer of ice. EE struck his back when falling on the old cooler.

Employee was cutting weeds on hill beside belt conveyor #11 with hand held gas weed eater. He stepped into a hole twisting and straining his right knee. (Note knee surgery May 1, 2009)
Employee had completed tying up 4"" rubber hose with 1/4"" nylon rope. He turned to leave area and stepped onto 4' high toe board or toe guard. He fell when his right foot became partially stuck in small hole in floor grating along side hose going thru floor grating.
Employee was welding the edges of a roll crusher, while adjusting his welding hood he exposed himself to welding flash from surrounding equipment.

OPERATORS WERE WORKING IN BAGGING DEPARTMENT; AN AIR-OPERATED ACTIVATOR AND DIVERTER WERE NOT WORKING. WHILE WORKING ON EQUIPMENT, INJURED STEPPED OFF OF FLOOR GRATING AND TRIPPED, LANDING ON HIS RIGHT SIDE.
INJURED CLAIMS TO HAVE BEEN USING AN OVERHEAD HOIST TO RAISE A CRATE OF SCREEN PANELS UP TWO FLOORS IN THE SCREEN HOUSE. WHEN THE LOAD WAS BEING LOWERED IT BECAME CAUGHT ON A HAND RAILING. WHE N THE CRATE WAS MOVED OFF THE RAILING IT DROPPED SLIGHTLY, WHICH STARTLED THE EMPLOYEE. HE STATED THAT HE MOVED BACK AND OFF TO ONE SIDE AND HE FELT SOMETHING POP IN HIS BACK.
EMPLOYEE WAS HOOKING UP RAIL CARS FOR LOADING WHILE WALKING ALONG THE TRACKS HE STATED HE STEPPED IN A HOLE OR ON A ROCK TWISTING HIS LEFT KNEE.

HIS HEAD WAS COCKED TO THE SIDE WHEN WELDING SPLATTER ENTERED THE EAR CANAL AND DAMAGED THE EAR DRUM. INJURED WENT TO SEE A DOCTOR ON 11-11-02, RETURNED TO WORK 11-11-02.
EE WAS PREPARING TO SERVICE A PIONEER JAW CRUSHER, HE EXITED CONTROL BOOTH WITH A 5 GAL. GREASE PUMPER WHICH IS USED FOR SERVICING. WHILE WALKING ALONG SIDE OF CONTROL BOOTH HE TRIPPED OVER A WIRE ALONG SIDE OF BUILDING.
EE WAS OPENING BOTTOM DOOR ON RAIL CAR TO CLEAN COMPARTMENT. DOING SO HE STTATED HE STRAINED HIS LOWER BACK.
EE WAS DECENDING DOWN AN EXTENSION LADDER. EE WAS HOLDING ONTO A 5 GALLON BUCKET WITH TOOLS, WHEN HIS FOOT SLIPPED ON THE RUNG OF THE LADDER.
THE BOTTOM DECK BEAMS ON VS-07 WERE BEING TORCHED OUT FOR REPLACEMENT. EE WAS TORCHING THE STEEL BEAMS ON VIBRATING SCREEN-07 WHEN A SPARK LANDED ON HIS LEFT GLOVE (RING FINGER). EE DID NOT NO TICE IT UNTIL THE SPARK HAD BURNT THE END OF THE GLOVE, CAUSING SECOND DEGREE BURN TO THE LEFT RING FINGER.
A RAILCAR WAS PULLED OVER DUMP AREA AFTER LOADING WHEN IT WAS DISCOVERED A CENTER BOTTOM DOOR WAS LEAKING, WHILE EE WAS ATTEMPTING TO OPEN DOOR,HE FELT A PAIN IN HIS UPPER RT. BACK & SHOULDER.

WHILE DIGGING, CRAWL ER FORCED A ROCK UP INTO SHOVEL AND EE HAD HIS RIGHT HAND ON TOP OF SHOVEL, FORCING SHOVEL INTO FRAME, CATCHING HISRIGHT HAND BETWEEN FRAM OF CRUSHER AND TOP OF SHOVEL HANDLE.
EMPLOYEE WAS GOING TO HOOK UP AIR LINES ON RAIL CARS WHEN LOADOUT OPERATOR PUSHED TOGETHER. WHEN CONNECTING MARK, HE FELT HIS ARM BEING PINCHED BETWEEN KNUCKLE OF NEXT RAILCAR. HIS FORARM WAS CAUGHT.
EMPLOYEES WERE CHANGING PUMP LINERS IN A DENVER SLURRY PUMP. WHEN PUTTING PUMP BACK TOGETHER EE CAUGHT TIP OF RINGER ON BACK SIDE OF PUMP LINER FRACTURING/LACERATING TIP OF FINGER.
EE WAS REMOVING LADDER FROM BACK OF MAINTENANCE TRUCK. WHILE TURNING ON UNEVEN SURFACE HE FELT HIS BACK PULL. EE WENT TO DR ON 2-7-01.

EE WAS SETTING ON DOZER TRACK, ATTEMPTING TO LIFT STARTER MOTOR INTO ENGINE AREA. THE AREA WAS LOWER THAN DOZER TRACK HE HAD TO WORK FROM AND WHILE WORKING IN THE AWKWARD POSITION HE FELT SOME THING PULL IN HIS LOWER BACK.
