A employee sustained a bursting laceration to the middle finger of their left hand when it became pinched between two pieces of 3/4-inch steel.

Employee was working in a man lift basket and they were extending the boom. They caught themselves between the basket and the conveyor frame.

Employee was disconnecting flexible water hose with cam lock fittings. While trying to pry the fittings apart with a tool, one of the hose ends struck the employee on the nose.

Third Party consultant was at site and felt his knee buckle as he was entering a pick-up truck to leave the plant area.

While lifting railroad ties, , a tie rolled over and smashed the employee left middle finger. He received a crushing type injury.

A welder received flash burns to the eyes from reflection inside a 10 inch bore. Employee received prescription meds and return to work.

While ascending onto D9N dozer, lost grip on hand hold, lost balance and fell.

Employee was grinding some weld off a clamp, grinder caught and cut right ring finger

4/27/06 am: 2 Employee changed dragline 3p.m.: Injured complained to the other that his back was sore. 4:28/06:EE reported a sore back-Did not feel he needed to go to the doctor. Said that he must have turned wrong and that this happens every so often due to pre-existing conditions. Degenerative Disc and Austioarthritis. 5/8/06: Reported back sore. 5/9/06:Went to doctor.

Employee was coming off the machine down a ladder, his foot slipped off the ladder and he twisted his right knee.

Employee was pulling on a com-a-long, bolt broke and employee fell down.

While cleaning the conveyor FP28, the employee slipped and fell backwards. He pulled a muscle in his inner right thigh, bruised his left shoulder and sand entered his right eye, causing a slight abrasion to the cornea.

WHILE TORCH CUTTING BOLTS ON THE TAIL SKIRT, HOT SLAG BURNED THROUGH HIS PANT LEG, ENTERED THE TOP OF HIS LEFT BOOT AND CONTINUED DOWN HIS ANKLE TO THE TOP OF HIS FOOT. WHEN HE REMOVED HIS BOOT, HIS SOCK BURST INTO FLAMES.

While removing a troughing idler from fp10 conveyor, he pulled the idler off the frame from the end and the other end swung down and hit him in the left shin.

WHILE ASCENDING A LADDER TO ACCESS THE HYDRAULIC UNIT ON THE PEP SCREEN PLANT, EE GRABBED HOLD OF THE CONVEYOR SUPPORT TUBE. THE SUPPORT WAS NOT FASTENED ON THE BOTTOM AND SWUNG OUT, CAUSING EE TO LOSE HIS BALANCE AND THE LADDER SHIFTED SIDEWAYS ABOUT ONE FOOT, CAUSING EE TO LOSE HIS BALANCE FURTHER. EE FELL FIVE FEET TO THE GROUND.

EE DID NOT REPORT INJURY UNTIL 9 AM ON 03/10/05. WHILE ADJUSTING THE TURNBUCKLE ON THE GEARBOX OF THE F2 CONVEYOR, THE JAM NUT LOCKED UP AND WHEN EE PUSHED TO BREAK IT FREE, THE CHEATER PIPE ON THE END OF THE WRENCH SLIPPED AND STRUCK EE IN THE CHEST

Miner initially indicated that he injured his shoulder on or about 3-26-04 but no medical treatment. On or about 10-1-04 miner indicated that his shoulder was bothering him again while reaching or twisting; miner went to clinic on 11/18/04 and was diagnosed with a rt. shoulder sprain.

Employee attempted to remove a rock that was wedged between the return side of the conveyor belt and the truss frame. The rock pinched his finger between the rock and cross angle, breaking the bone and cutting the skin.

Removing bucket pin when grease, dirt, & spark fell on right side of face and rt. ear. On 8-27-04 employee went to MD and had the ear looked at. Employee was told to return because swelling prevented the doctor from seeing inside the ear. Employee return to MD on October 8, 04 and was seen and told by doctor that ear drum was perforated and surgery is set for 12/20/04.

THE EMPLOYEE WAS USING A POCKET KNIFE TO REMOVE A WIRE TIE FROM AN ELECTRICAL CABLE. THE TIE BROKE AND THE KNIFE SLIPPED, WHICH CUT THE LEFT THUMB.

EE was preparing to ascend ladder and stepped on motor of cone crusher, lost his footing due to mud on his boots, slipped and struck his left leg against the mid rail of the handrail around the crusher platform.

EE HAD PULLED PIN OUT OF BOOM TO EXTEND IT. ANOTHER EE SWUNG THE BOOM INSTEAD OF RAISING IT. EE JUMPED BACKWARDS TO AVOID BEING HIT BY THE BOOM. EE FELL BACKWARDS AND LANDED ON BOTH ARMS AND B UTTOCK. BOOM DID NOT HIT EE.

EE WAS INSIDE THE SCREEN HOLDING THE HOUSING BOLTS FROM TURNING THE DEISTER SERVICEMAN STARTED DRAWING THE BOLT OUT, THE BOLT WAS DRIVEN OUT AND FLEW AT EE AND STRUCK HIM IN THE MOUTH, BREAKIN G HIS RIGHT UPPER INCISSOR.

WHEN TIGHTEN UP A BOLT ON HOSE ON DOZER WRENCH SLIPPED AND STRUCK EMPLOYEE IN MOUTH.

TO HELP CENTER PRIMARY CRUSHER ON TOP OF CONCRETE BLOCKS RAILROAD TIES ARE USED. THE RAILROAD TIE SLIPPED AGAINST LEFT HAND AND SMASHED LEFT RING FINGER.

EMPLOYEE SLIPPED AND STRUCK RIGHT ELBOW ON I BEAM STEEL WAS SLIPPERY FROM RAIN.

EE WAS WALKING TO TRUCK AND SLIPPED ON SNOW COVERED ICE. HE LANDED ON HIS BUTTOCKS, CAUSING BRUISING TO TAILBONE AREA.

EE STATES HE WAS LIFTING A SMALL CHAIN APPROX. 5 LBS. LATER HE BEGAN TO HAVE LOW PAIN.

THE CRANE WAS SITTING UP ON A HILL AND I WAS PICKING UP CRIBBING. WHEN I PICKED UP A MAT THAT WE USED FOR CRIBBING. I HAD TO PICK IT UP OVER MY HEAD AND THROW IT UP ON THE CRANE. AS I DID THAT TWISTED TO THE LEFT, RESULTING IN THE INJURY.

EE LOST BALANCE GRABBED A STEEL PLATE TO CATCH HIS BALANCE, HURTING HIS LEFT ELBOW. ELBOW STRAIN. FIRST TEATED ON OR ABOUT 2-5-02, CONTINUED TO DO REGULAR WORK UNTIL 4-8-02, HE WENT TO CLINIC FOR ADDITIONAL FOLLOW UP STRAIN CONTINUED TO LEFT ELBOW, EE WAS PLACED ON RESTRICTED DUTY, BUT HE WAS ABLE TO CONTINUE HIS NORMAL DUTIES, EE RETURNED TO

IMPROPER LIFTING PROCEDURES BY EMPLOYEE.

EE WAS DRIVING PINS OUT OF A BRAKE ASSEMBLY WHEN A CHIP BORKE OFF THE AIR CHISEL TOOL AND STRUCK HIS LEFT FOREARM.

DROPPED BLOW BAR APPROX. 1' AND STRUCK ANOTHER BLOW BAR. A PIECE CHIPPED OFF AND STRUCK LEFT LEG.

EMPLOYEE WAS WELDING ON THE GRAY FEEDER WHILE WEARING A WELDING HOOD. HE HAD SET HIS HARD HAT ON THE CONVEYER NEXT TO HIM. ANOTHER EMPLOYEE ACCIDENTLY IGNITED THE HAT. HE INHALED FUMES FROM TH E BURNING HARD HAT. THE INCIDENT OCCURRED ON JAN18, 2001, BUT WAS NOT REPORTED TO MANAGEMENT UNTIL 1-21-01 WHEN THE EMPLOYEE DECIDED TO SEEK MEDICAL TREATMENT.

EE WAS REMOVING A PLUG FROM THE 54"" SAND SCREW WHILE DRAINING DOWN THE PLANT-WATER SPLASHED IN HIS FACE CAUSING HIM TO JUMP BCK, HE TRIPPED OVER A CONCRETE PIER AND FELL BACKWARDS ONTO A CONCR ETE PAD. HE REPORTED THIS TO SUPERVISOR LATER BECAUSE HE DID NOT WANT TO GO TO DR.

EMPLOYEE SLIPPED AND FELL AGAINST AN I-BEAM AND STRUCK THE I-BEAM WITH HIS RIGHT SIDE OF BODY. HE WAS USING A LINING BAR TO UNPLUG CHUTE IN THEPRIMARY CRUSHER.

MINOR INJURY STRAINED LOWER BACK REMOVING ANVILS FROM CRUSHER. EMPLOYEE WORKED ALL DAY WEDNESDAY1-12-2000 AND DID NOT NOTICE ANY PAIN. THURSDAY MORNING 1-13 HIS BACK WAS SORE AND HAD TROUBLE GETTING OUT OF BED.
