A employee sustained a bursting laceration to the middle finger of their left hand when it became pinched between two pieces of 3/4-inch steel.

Employee was working in a man lift basket and they were extending the boom. They caught themselves between the basket and the conveyor frame.

Struck by flying object

Employee was disconnecting flexible water hose with cam lock fittings. While trying to pry the fittings apart with a tool, one of the hose ends struck the employee on the nose.

4/27/06 am: 2 Employee changed dragline 3p.m.: Injured complained to the other that his back was sore. 4:28/06:EE reported a sore back-Did not feel he needed to go to the doctor. Said that he must have turned wrong and that this happens every so often due to pre-existing conditions. Degenerative Disc and Austioarthritis. 5/8/06: Reported back sore. 5/9/06:Went to doctor.

While cleaning the conveyor FP28, the employee slipped and fell backwards. He pulled a muscle in his inner right thigh, bruised his left shoulder and sand entered his right eye, causing a slight abrasion to the cornea.

WHILE TORCH CUTTING BOLTS ON THE TAIL SKIRT, HOT SLAG BURNED THROUGH HIS PANT LEG, ENTERED THE TOP OF HIS LEFT BOOT AND CONTINUED DOWN HIS ANKLE TO THE TOP OF HIS FOOT. WHEN HE REMOVED HIS BOOT, HIS SOCK BURST INTO FLAMES.

While removing a troughing idler from fp10 conveyor, he pulled the idler off the frame from the end and the other end swung down and hit him in the left shin.

Fall from ladders

WHILE ASCENDING A LADDER TO ACCESS THE HYDRAULIC UNIT ON THE PEP SCREEN PLANT, EE GRABBED HOLD OF THE CONVEYOR SUPPORT TUBE. THE SUPPORT WAS NOT FASTENED ON THE BOTTOM AND SWUNG OUT, CAUSING EE TO LOSE HIS BALANCE AND THE LADDER SHIFTED SIDEWAYS ABOUT ONE FOOT, CAUSING EE TO LOSE HIS BALANCE FURTHER. EE FELL FIVE FEET TO THE GROUND.

Miner initially indicated that he injured his shoulder on or about 3-26-04 but no medical treatment. On or about 10-1-04 miner indicated that his shoulder was bothering him again while reaching or twisting; miner went to clinic on 11/18/04 and was diagnosed with a rt. shoulder sprain.

Employee attempted to remove a rock that was wedged between the return side of the conveyor belt and the truss frame. The rock pinched his finger between the rock and cross angle, breaking the bone and cutting the skin.

Struck by falling object

Removing bucket pin when grease, dirt, & spark fell on right side of face and rt. ear. On 8-27-04 employee went to MD and had the ear looked at. Employee was told to return because swelling prevented the doctor from seeing inside the ear. Employee return to MD on October 8, 04 and was seen and told by doctor that ear drum was perforated and surgery is set for 12/20/04.

THE EMPLOYEE WAS USING A POCKET KNIFE TO REMOVE A WIRE TIE FROM AN ELECTRICAL CABLE. THE TIE BROKE AND THE KNIFE SLIPPED, WHICH CUT THE LEFT THUMB.

EE WAS INSIDE THE SCREEN HOLDING THE HOUSING BOLTS FROM TURNING THE DEISTER SERVICEMAN STARTED DRAWING THE BOLT OUT, THE BOLT WAS DRIVEN OUT AND FLEW AT EE AND STRUCK HIM IN THE MOUTH, BREAKIN G HIS RIGHT UPPER INCISSOR.

THE CRANE WAS SITTING UP ON A HILL AND I WAS PICKING UP CRIBBING. WHEN I PICKED UP A MAT THAT WE USED FOR CRIBBING. I HAD TO PICK IT UP OVER MY HEAD AND THROW IT UP ON THE CRANE. AS I DID THAT TWISTED TO THE LEFT, RESULTING IN THE INJURY.
