Employee was on a Vermeer walk behind machine performing clean up tasks in the wet plant area of Utica 3. EE was pulling material out near the surge bin when a finger became wedged between a pipe and the toggle on the machine controls. EE sustained a fracture on the left hand ring finger tip.

Employee was cutting weeds and brush at the intersection of the Utica 1 and 2 roads to make better visibility with traffic and they got poison ivy rash on their body. Employee received medical treatment to relieve the itching on 8/23/19. Employee did not miss work or have modified duty as a result of this incident.
EE was doing a routine area inspection in the Utica I compressor room & tripped on a drain hose. EE was initially taken to a hospital to be checked by a doctor & the X-ray was negative & determined to be a mild strain & continued EE's normal duties. On 9/20/2019 EE had a medical procedure that requires this incident to be reportable to MSHA.

Employee was removing a blow bar from the Hazemag secondary crusher on 12/21/18 and a tiny sliver hit their finger. They initially felt no pain however it started to hurt on 12/23/18 when they reported the incident. They had a medical procedure on 12/ 28/2018 that required it to be reportable. They did not miss work.
EE was helping another employee place a submersible pump into the bed of a pick up truck and as a result EE set the pump on EE's finger causing an injury.
Employee was in the process of aligning a knife valve and lost balance and twisted left knee causing pain.
The employee was lifting a piece of ductwork improperly. EE was on knees and extending right arm away from body to push up on the piece of ductwork being assembled. In doing this function the employee reports that ee strained a muscle in lower back
Employee was using a shovel to clean frozen sand near the Utica 2 Lippmann and felt a pull in groin area.
EE was attempting to remove a fan blade from FA-1301 and was struck in the face with the blade. EE had the fan locked and tagged out. The fan slid off the shaft as EE was removing it and was struck in the face.

EE was removing an oil filter from a track mobile engine and later that day ee felt pain in left elbow. This was considered a first aid case however on 08/02/2017 ee received a medical procedure that made this incident MSHA reportable.
Employee was walking on the walkway near the cyclones in the Utica I dryer building and tripped on a step. Employee braced themselves with the left hand as they were falling and fractured the left wrist. The front of the step was painted yellow and a caution step sign was directly overhead.

Employee was shoveling sand at the Utica II crusher to improve housekeeping and reported pain in lower back at the end of shift. This was originally classified as a first aid case only, however on 1/13/2017 employee received therapy that made this case MSHA reportable. Employee did not miss work and continued to do normal assigned job duties.
Employee pinched finger on a jack stand while attempting to reposition a diesel powered portable pump so that it could be moved by a fork lift.

Employee was welding and grinding and felt irritation in his right eye. He was wearing all necessary PPE but conditions were windy and work was occurring on the floor above. A small piece of metal was removed by the eye doctor and a prescription antibiotic was given.
EE was involved in re-attaching a dust collector boot on VS-307 and thought it was secured to the machine, however it slipped off and struck his head knocked his hard hat off causing a cut on his hair line.

Employee was holding a piece of sheet poly in place as another person was drilling a hole in it to bolt it in place. The bit nicked his arm causing a cut.
Employee was shoveling spilled sand at the BC-01 tail pulley located in the Utica I basement. He reported to his supervisor that his left shoulder was sore. He had an X-ray that was negative. He received physical therapy on 04/14/2014 that made this incident reportable.

Employee was dusting the supervisor's desk and she bent over and hit her forehead on a computer printer shelf.
On 04/17/2012 at approximately 8:15 am. EE was allegedly lifting up on the diffuser bar on the VS302 and he felt pain and a pop in his right shoulder. He was taken to the hospital for medical evaluation and was given an X-Ray that was negative for breaks and other signs of trauma.He had a medical procedure on 05/30/2012 that made this incident MSHA reportable.

We are unsure when this started EE went to her doctor for painful back she had been having for a while and the doctor required her to be off work until 2/21/12 so she could go to a specialist for further testing of what is happening with her back.
EE was opening a rail car door with a metal bar and his hand slipped causing his middle knuckle on his right hand to strike the car. At the time of the incident he felt no pain. On 10-3-11 he was taken to the hospital and the X-ray was negative and was considered first aid. He had a medical procedure on 10-25-11 that made this reportable. No lost time, No modified duty.
EE was working on the number one dryer replacing the thrust idler. He was repositioning the old idler on the base for removal when he felt pain in his left shoulder. He was not using excessive force in the move because graphite was on the base acting as a lubricant allowing it to slide easily. The dryer was locked/tagged out and was jacked up hill and blocked against motion.
EE was climbing down a ladder when he lost his footing on the ladder. He caught himself on the ladder rung, but felt pain in his right shoulder. He reported the injury, but thought the pain would go away with time. He sought medical care on 7-15-2011 and had an X-ray that was negative. He was on full duty during the whole time. He did have a medical procedure on 10-07-2011.

EE was pouring concrete at pond spillway and later that day when he relaxed taking his break he felt pain and stiffness in his back. He continued to work his normal duties. He later went to a chiropractor on his own. He made it known to the safety dept. that he went to a chiropractor on 8-7-10 which made this incident reportable. Medical only no lost time or restrictions.

EE was shoveling sand near the BC13 head pulley for one hour to clean up the area and allegedly felt pain in his left shoulder. The X-ray was negative and he continued to do his normal job. He had a medical procedure on 02-2-2010 that made it reportable to MSHA. He continued to work. He had surgery on 3/8/10 that changed it to lost time status.

Employee claimed he slipped off a bolt with a wrench and felt a twinge in his rt. shoulder. He continued working. He was seen by medical doctor on 9/21/07. Pain has persisted for this employee, and he is now diagnosed (11-08-07) with AC joint DJD and is being treated with injections of Cortizone. He has a lifting restriction until released from this treatment.

HE WAS TAKEN TO LOCAL MEDICAL FACILITY WHERE INITIAL DIAGNOSIS WAS A SPRAIN. SUBSEQUENT MEDICAL EVALUATION INDICATES A TORN LIGAMENT THAT WILL REQUIRE ARTHROSCOPIC SURGERY.
EMPLOYEE STEPPED ON SMALL ROCK WHILE PERFORMING CLEANUP. HE TWISTED HIS LEFT KNEE. IT HAS NOW BEEN DETERMINED THAT HIS INJURY WILL REQUIRE A SURGICAL PROCEDURE. IT IS SCHEDULED FOR 1/29/07.

THE EMPLOYEE HAD ARTHROSCOPIC SURGERY FOR THIS CONDITION ON 4/30/03. HE WILL BE OFF WORK FOR AT LEAST 2 WEEKS.
EE WAS PERFORMING PRE-SHIFT INSPECTION ON HAUL TRUCK. AS HE WAS DISMOUNTING MACHINE, HE FAILED TO MAINTAIN A 3 PT. CONTACT WITH ACCESS LADDER. EE FELL, STRIKING HIS LEFT WRIST AGAINST THE ACC ESS LADDER.
