Technician tripped on trucks outrigger, twisted right knee and it swelled up. MRI determined nothing torn or broken, but still requires roughly 3 weeks before return to normal job description because of light duty restriction.

Employee strained back while moving blocks to get rid of a portable stacker. On 5/23/2024, employee alleges while moving metal scraps, EE's old injury was reaggravated.
A rock dislodged from the wall while scaling landing on miner's right hand and scale bar. Miner was wearing CUT 5 level gloves. Injury did not require sutures, but x-ray showed minor fractures of the thumb proximal phalangeal shaft.
Employee was helping load a shot and while walking in the area the employee tripped over an emulsion hose. Employee landed on knees and sustained a scrape to one of them.

While retrieving a Rock Hog 4.5"" DTH Hammer from the pickup truck, EE realized the hammer wasn't operating properly. Co-worker told EE to slam it onto the ground to remove the hammer internal parts. This is when EE's hand went between the bit and the chuck. This movement is what caused the injury to EE's hand.
While in the process of hand scaling a highwall, a rock chipped off the wall, striking the employee in the face.

Employee was taking oil samples from the #10 loader. EE checked for pressure by loosening the plug slowly. While doing so, the plug abruptly shot out, spraying oil at the EE's face and upper body.

At approximately 9:00 AM an employee opened a butterfly valve to fill the water truck. When the truck was filled with water, the employee closed the butterfly valve and alleges that EE hurt back while doing so. Employee left site and visited their personal chiropractor who took EE off of work.
Two employees were trying top move a metal table that weighed roughly 300 lbs. into the back of a pickup truck. They used the tailgate as a pivot point to flip the table over and when they were lowering it one employee felt a sharp pain in the left shoulder. The employee is currently at work under restrictive duty and being treated by a physician.
Employee was moving screen into storage. As employee pushed the screen a finger became caught between the screen and handrail, crushing left ring finger resulting in a fracture.
Employee was traveling in a man lift and hit a pothole in the road causing EE to get jarred in the basket and claims EE hyperextended their knee.
Employee was conducting an OPs-9 procedure and as they were placing the grounds, there was an arc flash resulting in burns to their head.
EE was walking towards the building from EE's vehicle. EE slipped and fell landing on rear.

EE was cleaning out material that spilled onto a take up pulley. EE was positioned inside the belt standing on the pulley. As a rock on one side of the pulley was freed, the pulley began to roll and EE's foot was caught between the belt and the pulley. EE was taken to the ER. X-rays showed no broken bones but was taken off of work to follow-up with an orthopedic surgeon.
At approximately 1:20 pm, an employee was removing scrap steel from a chute when a piece of metal struck the top side of EE's arm above the wrist. EE was taken to the ER and received 19 stitches and returned to work with no restrictions.

EE was walking around the manlift towards the basket of the machine. As EE was walking the EE tripped on own feet and went to the ground. It appears that EE struck left shin on the corner of a metal pump sled that was lying on the ground next to the manlift basket. EE was taken to ER and received 9 stitches, released to return to work on light duty.
This claim has been denied by W/C for inability to provide specific event or reliable date. Employee alleges back soreness / aggravation over time possibly from seat in 988H Loader #54. Employee claims back has not improved over time. Sought medical treatment on 6/10/19, prescribed physical therapy on 6/14/19.
EE was lifting and moving the supply line for a pump, upon repositioning EE's body, while holding the hose EE's right knee popped causing EE to collapse to ground. On 7/15/19, requested to see an orthopedic and on 7/19, employee was seen by the doctor where EE was diagnosed with a meniscus sprain with possible tear, an MRI ordered, and restrictions given.

On 5/11 an employee was working in the welding shop and felt something fall on the back of neck from a light fixture above. EE thought the light was going to fall so ee quickly jumped out of the way and right foot was caught causing to trip and land on the inside of right foot. EE felt right knee become sore. On 6/5 ee saw a doctor and put on restrictions.
An employee was tasked with changing a screen and EE was hammering a bolt into place EE felt a pain in EE's right shoulder. While exiting the screen deck EE grabbed onto the spray bar to pull self out of the screen and felt a pop in the same shoulder. EE was brought to the clinic and diagnosed with a sprained shoulder. EE was put on restrictions and physical therapy.
On 2/14 A loader operator was climbing into loader and felt a pain/spasm in lower back. EE immediately began icing and taking OTC medication. On 2/28 ee requested to go to the clinic and was diagnosed with a sprained lumbar ligament and returned full duty. EE had a follow-up appt. On 3/10 and was prescribed physical therapy making it a reportable injury.
Contractor working at Vulcan Materials facility was working on a garage door and one of tension rods fell and struck self in the hard hat knocking it off and cutting forehead. EE also cut hand. EE was taken to the ER by ambulance where ee received staples in head and stitches in hand.

A maintenance employee noticed that left eye was bothering them and told the foreman that, while working on the Eljay screen, EE was using an impact wrench and upon engagement the structure shook causing dust to fall. The next day EE stated that the eye was swollen and irritated. EE went to the clinic and was diagnosed with a minor abrasion and given prescription eye drops.
Maintenance employee was walking to the crusher building to perform general maintenance activity. Upon entering the building EE slipped on some wet material and fell on back. EE was taken to the clinic and diagnosed with a lower back bruise and a bulging disc.
A maintenance employee was using a torch to cut metal from a piece of equipment. Debris fell into the top of safety glasses and got into eyes. Employee used an eye wash bottle to wash eye out. On 5/17 the employee contacted supervisor and said that eye was still bothering and was watering a lot. The employee was brought to the clinic and prescribed eye drops.
On 4/6 an employee working in the welding shop was transporting a piece of angle iron to cutting table. While carrying the piece of iron the employee stated that right knee felt like it buckled and gave away while walking. On 5/4 employee requested to see a doctor and was put on restrictions.
On 1/27 an employee was closing the gate on a hopper when a piece of frozen material fell on the inside of the tank and jammed the door open. The piece of material prevented the wheel from turning freely causing the employee to twist shoulder. This resulted in a sprained shoulder.
On 1/26 a maintenance employee was transporting an oxygen bottle using a dolly. The dolly shifted away from the employee and EE grabbed a handrail with one hand and the dolly with the other to try and stabilize the dolly. This caused EE to feel discomfort in lower abdomen. EE was diagnosed with a hernia.

On 12/1/15 an employee was walking across the yard to leave for the day and stepped on some loose material and twisted his knee. Based on the MRI results the employee was scheduled for a surgery on 2/9/16. At that time this injury became MSHA reportable.
On 11/10 Haul Truck Driver complained of back pain at the end of his shift. The employee reported his seat bottoming out while driving. The mechanic inspected the seat and determined the seat was fine. On 11/23 he requested medical treatment and returned to full duty. On 12/18 he saw his personal doctor and taken off work for 2 weeks.
On 11/2 the EE was assigned to clean out the Scalping Tower due to a backed up screen. He felt tightening in his back while shoveling and after lunch the pain became more severe. He reported it to his supervisor and attempted to work through the soreness for remainder of the week. He was taken to the clinic on 11/9 and was diagnosed with a back sprain and placed on restrictions.
Maintenance employee slipped/tripped after removing the upper cover of the chain drive from C-70 conveyor head pulley striking his forehead on the lower flange of the chain box resulting in lacerations to his forehead and face requiring stitches. The employee was released to return to work with restrictions.
Haul Truck Driver backed CAT 786C to primary crusher to dump shot rock. The bed had fully extended when the left lift cylinder hydraulic hose ruptured causing the bed to fall back down onto the frame. EE was jolted in his seat, and felt pain in his rt. hip & foot. EE was diagnosed with sciatica radiculopathy, prescribed medications, PT, MRI, & taken off work until next appointment.
Mobile equipment mechanic experienced pain on side of right knee while kneeling/crawling underneath 100-ton haul truck changing steering filter during a PM. Employee was wearing knee pads at the time of the event. On 6/17/15, MRI revealed a tear to his medial meniscus and a mild sprain of the MCL. Restrictions were given and surgery recommended.

EMP felt discomfort in both his hands on December 15th 2014. EMP tried to remedy the pain until he decided to see his own DR. on April 7th 2015. His DR. prescribed an EMG test, the EMP had the test done on April 21st 2015. EMP was notified on April 28th, 2015 he had Mononeuropathty (carpal tunnel) in both hands. The company was notified of the diagnosis on April 30th, 2015.
On 6/4/14 at 3:00pm employee was removing the top shell from the bottom shell of a cone crusher. While backing out push bolt his left hand was under the top flange. When he hit the impact gun trigger his left hand was pinched between the impact handle and flange. He was diagnosed with a hand contusion and placed on restriction as of 6/5/2014.
Friday, January 24, 2014, while spotting a customer truck, in the Clean-Fill Area, the employee slipped on a dirt rut and experienced pain in his left knee. Monday, January 27, 2014, he saw a physician and was diagnosed with a knee sprain. He was given Ibuprofen and released to work. February 15, an MRI showed torn meniscus. March 4, 2014, the employee had microscopic surgery.

On December 5, 2013, the employee walked out of the Mobile Shop when the wind picked up and dust blew into his left eye. The employee washed his eye with eyewash and clean it through the night. On December 6, 2013 the employee's eye was sore and sensitive to light and received medical attention. He was diagnosed with a cornea abrasion and dust particles were removed from the eye.
On October 10,2013 the Electrical Foreman was using Channel Locks to loosen a seal-tight connector on a 2-inch conduit when he felt pain in his left shoulder. On October 15, 2013, the employee went for a diagnostic exam. The doctor sent him for an MRI. On November 13, 2013, the employee saw a specialist who read the MRI and recommended therapy and a lifting restriction.

On 8/9/12, employee team lifted a shackle and cables from the ground to a loader tow pin. He reported that he experienced pain in the abdomen. After taking a pain reliever the pain went away. The pain seemed to come and go. On 9/24/12, the employee visited a physician who sent him to see a specialist. On 10/4/2012, the Specialist confirmed a hernia. Surgery is scheduled.
On 3/29/2012, employee was hauling waste in a Euclid R85 Haul truck. While lowering the truck bed the bed slammed down bouncing him in the cab. It was reported as a first aid injury (neck sprain). On 5/25/2012, he saw a physician for diagnostic purposes (no medical treatment or restrictions). On June 14, 2012, he began physical therapy for his lower back (no restrictions).
On 8/25/11 employee reported back pain. He said he had back pain over the years but it worsened on 6/21/11 when team-lifting a 80 lb pump cover. On 8/25/12 diagnostic test revealed a back strain and preventative treatment was performed. On 1/17/12 additional diagnostic test revealed degenerative disc disease and fusion surgery was performed on 3/12/12 (first day off)

On Friday, November 19, 2010 at about 11:00 a.m. the haul truck driver lifted a 35 pound wheel chock to replace it on the front of a Cat 785C haul truck when he felt pain in his lower back. On Tuesday, December 01, 2010 the physician sent the employee out for an MRI and took him off work for a low back strain.
Wednesday, November 17, 2010, at 7:30 p.m. the employee was driving a Ford F250 when it overturned in the flux pit. A second employee was a passenger. Both employees were taken to LaGrange Memorial Hospital for treatment / observation. This employee was diagnosed with Scapular strain (right), lumbar strain (left), and a 10 cm abrasion on the right leg.
Wednesday, November 17, 2010, at 7:30 p.m. the employee was passenger of a Ford F250 when it overturned in the flux pit. Another employee was driving. Both employees were taken to LaGrange Memorial Hospital for treatment / observation. This employee was diagnosed with a scapular strain and a lumbar strain.
On September 30, 2010, the employee stepped on loose ground and his right foot slipped into a hole causing him to fall. On October 6, 2010 the employee received first aid care. On December 10, 2010, the employee complained about continued pain and the physician diagnosed him with a back strain and a groin injury and placed him on restrictions.

On November 24, 2009 worker was moving a 5 gallon bucket of used oil from the ground to a pickup truck when he felt pain. On November 25, 2009 the employee received first aid and worked without restriction until his winter lay-off on December 18, 2009. Upon return from lay-off, February 19, 2010, his physician indicated restricted duty.
Two emps. attempted to remove a steering cylinder from a CAT 992G loader using an overhead crane to bump the cylinder horizontally. Emp. A and B were standing between the right side tires. Emp A's right hand was on the cylinder while Emp B operated the crane. After 3-4 bumps, the cylinder swung free pinching Emp A's hand between the hydraulic tank and cylinder.
At approximately 5:30 a.m. the employee was dismounting from a haul truck. The ladder was wet from snow. He lost his grip and fell approximately 15 feet to the ground. He was transported to the hospital. His diagnosis was a dislocated finger and compression fracture of the L1 and L3 vertebrae.

On 12/01/2008, employee was ending his shift. After dismounting the haul truck, he lifted a wheel chock to block the tires. While swinging it into place he felt a pop in the left shoulder. On 12/02/2008, he had a diagnostic visit with a physician for a left shoulder strain. During the follow-up visit on 12/12/2008, the physician schedueled him for physical therapy.

Employee was descending the ladder from his CAT 992G Loader (at the end of his shift). As he was stepping down from the last rung to the ground (using his right leg) he heard a pop in his left knee that was still on the last rung of the ladder (the swinging rung).
Employee exited from his pickup tuck and made an attempt to close the door when he tripped and fell over a depression in the dirt. His hand was caught between the door resulting in two stiches.
During the removal/repair work of the primary crusher, the concave was being hammered on using the rock breaker. The employee remained on the crusher floor to view the operation from about 12 feet away. He was struck by a piece of sharp packing material in his right inner thigh. He received three stiches and returned to work.
Employee and co-worker were replacing the element of an Omega coupling. While aligning bolt holes (by spinning the motor), the injured employee's left middle finger slipped into a bolt hole. The finger was caught between the Omega flange and the bolt hole in the crusher coupling. This resulted in a finger cut and fracture.
While adjusting/changing a chain on the finger stacker, the chain slipped and struck the employee on the hand.

Ice partially covered catwalk due to dripping dust control hose. Expanded steel catwalk had raised profile parallel with travel/incline. Employee slipped forward and caught himself with left arm resulting in a dislocated left shoulder.

Employee was climbing 4-foot step ladder to gain access to D10 dozer in order to fuel it. When the employee reached the 3rd step, the ladder started to wobble and the employee lost his balance and fell. This resulted in a compound fracture of the right lower leg.
Wet, muddy conditions on boots and around machine caused buildup and a subsequent slip resulting in dislocation of 4th digit on right hand.
Poor lifting behavior/method, use of a single wheeled wheelbarrow, load not evenly distributed. These causes led to employee to lose balance and fall (same level) and partially dislocate left shoulder.
EE operating defective equipment (steering), operating track mounted machine on solid stone, and lifting heavy solid outrigger pad, all contributed to lower back strain
Employee was drilling through the wire trough on a C-Container when the drill bit caught and caused the employee's hand to strike the top of the trough. Employee suffered lacerations to his right 5th digit and required 5 stitches.
EE WAS PULLING ON HYDRAULIC LINE WHEN HE TRIPPED OVER A WHEEL CHOCK ON THE SHOP FLOOR. EE FELL ON HIS LEFT THUMB CAUSING IT TO BECOME DISLOCATED.

Employee experienced painful back spasms while operating an articulating haul truck while moving stripped material. Employee had experienced back spasms before and the case was put under investigation. As of 1/18/2005, the case was accepted as an injury that was reportable.
Employee was working on #6 loader inside the cab when he stood up and jammed his head against the inside of the cab, straining his neck.
Employee was pulling on fuel nozzle on fuel truck and strained his shoulder. Employee refused medical treatment at time of actual accident, claiming it was nothing. He later reported pain (on June 17, 2004) claiming it was from the original April accident. The investigation of this claim did not begin until 6/17/04.

THIS IS AN ALLEGED REINJURY TO AN ORIGINAL INJURY OCCURING IN 2000 (WHICH WAS AGAIN REINJURED IN 2001 AND RECENTLY IN JULY OF 2003). THIS ALLEGED INJURY/ACCIDENT IS CURRENTLY UNDER INVESTIGATION.
EMPLOYEE WAS DISCARDING A USED AIR FILTER INTO THE DUMPSTER AND SUFFERED A CUT ON HIS PALM RESULTING IN STITCHES.
EMPLOYEE (A TRUCK DRIVER) BACKED HIS TRUCK INTO A WALL CAUSING THE OPERATOR TO JAR HIS BACK IN HIS SEAT.
EE WAS RUNNING LOADER AND COMPLAINED OF SORENESS IN LOWER BACK FROM MOVING IN THE SEAT OF THE LOADER. EE SUFFERED ACUTE LUMBAR STRAIN FROM REINJURY OF PREVIOUS 2000 INJURY.
EE WAS INSTALLING A ROLLER. ROLLER FRAME SLIPPED & PINCHED THUMB BETWEEN ROLLER FRAME & CONVEYOR FRAME RESULTING IN LOSS OF NAIL ON RIGHT THUMB.
EE REINJURED/ REAGGRAVATED AN OLD BACK INJURY WHILE DRIVING THE WATER TRUCK. EE CLAIMS IT WAS THE BOUNCING OF THE SEAT THAT CAUSED THE INJURY.
EE WAS STANDING ON DECK, STEPPED BACK TO ENTER CAB OF DOZER WHEN HIS LEFT FOOT SLID BETWEEN THE TRACK AND FRAME OF EQUIPMENT RESULTING IN THE EETWISTING HIS BACK AND RECEIVING ABRASIONS TO HIS LEFT LEG.
EE WAS WALKING DOWN THE STAIRS BEHIND #13 & #14 CONVEYOR AND FELT SOMETHING IN HIS KNEE ""GIVE WAY"".
EE WAS ASSISTING ANOTHER EE WITH A BEARING HOUSING. THE 7 1/2"" BEARING HOUSING WAS BEING MANUALLY LIFTED INTO POSITION WHEN EE FELT PAIN IN LOWER BACK.
TRUCK WAS BEING LOADED WITH 6-8' DIAMETER CULVERTS. A CULVERT SLIPPED DURING LOADING AND THE IMPACT SHOOK THE TRUCK BED WHICH IN TURN CAUSED THE OPERATOR TO BE BOUNCED/SHOOK FORWARD IN THE SEA T CAUSING LOWER BACK STRAIN AND SPASM.
EE STEPPED ON UNSUPPORTED PORTION OF FLOOR AND FELL INTO CRAWL SPACE BELOW BUILDING. EE STRUCK HIS FACE ON REMAINING PORTION OF EXISTING FLOOR DURING HIS FALL.

EMPLOYEE WAS HIT BY #1 BAY DOOR AS IT WAS LOWERED UNDER POWER. REPORTS INDICATE HE WAS STANDING IN THE DOORWAY WHEN ANOTHER EMPLOYEE PUSHED THE DOWN BUTTON. HE THOUGHT SOMEONE WAS TAPPING HIM ON THE SHOULDER.
EE WAS EXITING CAT 16G GRADER. WHEN HE REACHED THE GROUND LEVEL WITH HIS RIGHT LEG, HIS LEFT LEG- WHICH WAS STILL ON THE STEP - SWUNG UNDER THE GRADER AND HE STRAINED HIS LEFT KNEE. INJURY REP ORTED 9/13/2002 EVALUATED AND RELEASED. RESTRICTIONS AFTER 9/19/02. AMEND AT REQUEST OF MSHA FIELD OFFICE.
EE APPARENTLY LEFT THE SERVICE AREA WITH #10 EUCLID PARKING BRAKE SET. ON ROUTE TO PRIMARY CRUSHER AREA HE NOTICED SMOKE & PULLED OVER AND ASSISTED ANOTHER DRIVER USING A FIRE EXTINGUISHER ON SMOKING AREA.
EMPLOYEE LEANED AGAINST WALL TO WELD INSIDE A HOPPER. EMPLOYEE TACKED PLATE TO WASS AND WELDING ROD BECAME STUCK. EMPLOYEE PULLED ON ROD TO LOOSEN AND PLATE CAME FREE AND HIT HIM IN THE LEG.
EE WAS DIRECTING THE PLACEMENT OF A STEEL STAND TO BLOCK A CONVEYOR COUNTERWEIGHT. THE STAND TIPPED OVER STRIKING A STEEL BARRIER. THE WELDED ATTACHMENT OF THE BARRIER BROKE AND STRUCK THE EE'S ANKLE AFTER HE SLIPPED TO THE GROUND ATTEMPTING TO RETREAT. EE WAS AWAY FROM THE LINE OF FIRE IN HIS ORIGINAL POSITION.
WHILE PULLING BUNDLE OF WIRES, FREE END STRUCK EE IN FACE KNOCKING OFF GLASSES AND HARD HAT. UNDOCUMENTED DATE ON MAY 2002. ON 10-28-02 EE WOKE UP AT HOME WITH PAIN AND SOUGHT MEDICAL ATTENTIO N, CONNECTING MAY INJURY WITH 10-28-02 PAIN.
EE REPORTED NUMBNESS IN ARMS AND LEGS DURING DRIVE HOME FROM WORK. MEDICAL INVESTIGATION REPORTS INDICATE SOME LIKELYHOOD OF BEING RELATED TO OPERATING PIT LOADER. VULCAN ACCEPTS AS COMPENSABL E INJURY: DECEMBER 2002.
EE WAS INSTALLING A DRISCO PIPE USING A PRY BAR AND COME-A-LONG WHEN THE PRY BAR SLIPPED. THE HANDLE STRUCK THE EE BETWEEN THE LIP AND NOSE. THE RESULTING LACERATION REQUIRED STITCHES.

EMPLOYEE WAS TRANSPORTING A LOAD OF STONE UP A STOCKPILE RAMP. THE ENGINE QUIT AND THE OPERATOR LOST PRE-ESTING STEERING AND BREAKING CONTROL. HE ROLLED DOWN THE RAMP BACKWARDS AND STRUCK ANOT HER LOADER. LATE MEDICAL REPORTS QUILIFY THIS INCIDENT AS REPORTABLE.

AT THAT TIME NO MED ATTENTION WAS SOUGHT. ON 10/23 HE DIDGO TO THE DR & WAS RELEASED TO WORK WITH RESTRICTIONS FOR ACUTE LUMBAR STRAIN
EE WAS CLEANING SCRAP STEEL UNDER CONVEYOR AND AT THE END OF HIS SHIFT HE REPORTED TIGHTNESS IN HIS BACK. ON 3/13 HE SOUGHT MEDICAL TREATMENT ON HIS OWN. THIS INCIDENT HAS BEEN UNDER INVESTIGA TION AND IT HAS BEEN DETERMINED TO BE COMPENSABLE.
EE WAS USING A WHITNEE PUNCH TO MAKE HOLES IN THE I-BEAMS. THE NEXT DAY HIS LOWER BACK WAS SORE. ON 4/19 HE NOTIFIED HIS SUPERVISOR THAT HE HAD SOUGHT MEDICAL ATTNETION AND WAS WORKING UNDER R ESTRICTIONS.
EE WAS MOVING A BOULDER WITH A DOZER WHEN THE BOULDER MOVED. THE DOZER JUMPED JARRING THE EE IN HIS SEAT CAUSING HIM TO TWIST HIS BACK. THE EE DID NOT SEEK MEDICAL ATTENTION UNTIL 2/28/00 AND WAS PUT ON A NO-WORK STATUS.
