Caught in, under or between a moving and a stationary object

Employee was tasked with installing an idler on a telestacker. As employee guided the idler into position with right hand, EE lost control of the idler and it fell unexpectedly causing it to pinch EE's left middle finger tip between the idler and the conveyor frame resulting in a laceration. EE was taken to the ER and received 6 stitches and returned to full duty.

Accident type, without injuries

At approx. 11:20am on 11/30/17, a fire was discovered at the primary MCC room. Fire department was dispatched and arrived at 11:30am. Due to fire involving switchgear & located next to 4160v transformer, firefighting activities were delayed until power could be de-energized. The fire was extinguished at 12:38pm, minutes after electrical services could arrive and disconnect power.

EE alleged on 8/28/14 a repetitive task induced illness caused by hand shaking material samples. EE received test results on 10/14/14 revealing moderate carpal tunnel syndrome in both wrists. EE was released to work with no restrictions given at this time.

EE was assisting contractor with removing brake assembly on CAT 990 Loader. While in compartment, the prop rod for the compartment door became unlocked & a gust of wind blew the door down, striking the EE on top of his hardhat. The EE experienced neck pain from this incident. IME report on 10/16/13 changed diagnosis to cervical radiculopathy requiring physical therapy treatments.

Dewatering a hole. EE had a boom out to the front of the truck, put the pump up on the bed & hurried up & went around the dewatering truck, forgot to duck for the boom, hit chin.

EMPLOYEE BENT OVER TO UNHOOK CHAIN LOCATED APPROX. 2 FT. OFF THE GROUND. EMPLOYEE DID NOT USE ANY TWISTING MOTION WHEN HE FELT PAIN IN HIS LOWER RIGHT SIDE OF BACK AND HIP AREA. WITNESSES REPORTED THAT THE EMPLOYEE LEANED TO THE RIGHT WITHOUT TWISTING HIS UPPER BODY & DIDN'T EVEN TOUCH THE CHAIN. HIS RANGE OF MOTION SEEMED NORMAL.

EE WAS MANUALLY FLIPPING A SCREEN BEARING HOUSING OVER THAT WAS ON THE ELEVATED FORKS OF THE FORK TRUCK, WHEN HE LOST CONTROL OF THE HOUSING. THE PART FELL & STRUCK HIS LOWER LEG & ANKLE. HE S OUGHT MEDICAL TREATMENT WHERE HE WAS DIAGNOSED WITH A BRUISE TO THE LEFT INNER CALF & A SPRAINEDANKLE. HE WAS RETURNED TO WORK WITH LIGHT DUTY RESTRICTIONS.

EE WAS CUTTING A CONDUIT DOWN OFF OF PUGMILL WHEN THE CONDUIT SLID DOWN UNEXPECTEDLY AND PINCHEDHIS RIGHT THUMB BETWEEN THE MANLIFT BASKET AND PIPE. HE RECEIVED A LACERATION TO HIS RIGHT THUMB WHICH REQUIRED STITICHES.

EMPLOYEE STRAINED HIS RIGHT CALF MUSCLE WHEN HE WAS PRESSING A RETURN IDLER INTO PLACE ON THE C-1 CONVEYER. THE INCIDENT WAS NOT REPORTED TO HIS SUPERVISOR UNTIL AUG 24, AFTER HIS CONDITION HA D DEGENERATED. EMPLOYEE WAS ASSIGNED TO RESTRICTED DUTY.
