Fall down stairs

Employee was descending steps between two shaker screens. They missed a step and fell, injuring an ankle. Employee was taken to a clinic, X-ray revealed an ankle avulsion fracture. Seen by ortho the next day, fitted w/ a boot. RTW their next sched day with restricted duty.

Contact with heat

Employee was working on conveyor pulley & started to feel dizzy & nauseated. Temps were 90+ with Heat index 100+ Went to control room to cool off. After drinking fluids, EE vomited. As a precautionary measure, ambulance was called, EMT's checked vitals, all were ok. by that time employee had improved. Heat Exhaustion. Employee returned to work with no complications

Struck against stationary object

Employee was replacing screen panels. Employee was standing on top deck when EE lost footing and slipped to screen deck and then rolled to the opening where screen had been removed. Resulting in employee hitting right lower back into I beam. Employee had harness and lanyard on. Employee went to hospital from home on 3/6/2024 and had been at work until that point.

Contact with hot objects or substances

Miner was performing maintenance using a propane torch. Miner attempted to get slack in the hoses, hose got caught on a beam which caused the torch to turn towards the miner and crossed their right forearm.

Fall from machine

An operator suffered an injury to EE's right knee while making repairs to the C-15 chain drive. The injured had positioned self to pull on a come a long to remove the guard. When EE pulled on it the come a long came free striking EE in the arm causing EE to jump down to the walkway 30 inches landing on EE's feet. Recent MRI was positive for a tear to the meniscus.

Caught in, under or between a moving and a stationary object

A laborer suffered a sprain to the left knee while stepping down from a conveyor frame. The injured miner slipped while stepping down and got left leg caught in the conveyor frame resulting in the sprain.

Fall onto or against objects

A miner sustained a laceration to the back of head requiring sutures. The miner was attempting to move a pallet by hand to a position where the telehandler could remove it from the truck when the board the miner was pulling on came free causing miner to fall backwards hitting head on the backrest of the telehandler. Miner's hard hat came off when miner fell.

A laborer sustained a strain to EE's right arm when EE slipped but did not fall. The injured miner was walking in the area of the Thor Stacker tail pulley when EE slipped. To avoid a fall EE reached first with EE's left hand and then caught self with EE's right hand by grabbing the conveyor frame. EE states at that time EE felt pain in right bicep area.

While repairing a hole in the chute using a cutting torch to enlarge a bolt hole, we suspect hot slag dropped into the hopper below the east shaker igniting the poly liner and screen media.

Over-exertion in pulling or pushing objects

A truck driver states EE has pain in EE's left shoulder. EE was attempting to free a frozen clutch pedal cable by pulling on the pedal and felt something pop in EE's left shoulder area. A positive MRI and work restrictions now make this incident reportable.

A laborer sustained an injury to his left knee while preforming housekeeping duties in the area of #9 conveyor tail pulley. The injured was using a water hose to clean and while moving the hose he stepped back onto uneven ground that was water covered causing him to slip and injure his left knee. An MRI has now been taken showing a tear to the meniscus.

Caught in, under or between a moving and a stationary object

A working foreman sustained a laceration and fracture to the tip of his left index finger while positioning a motor base. The miner was attempting to move the motor base with a coworker into position when his finger got caught between the motor base and frame.

Fall from machine

A driller sustained a closed fracture to the 4th finger on the right hand when he fell exiting the Sanvik Track Drill. A compressor hose broke causing synthetic compressor oil to be sprayed over the drill. While descending the access ladder to address the problem he lost his grip on the oil covered ladder and fell. He injured his finger when he put his hand out to break his fall.

A repairman sustained an injury to the top of his left hand while using a chipping hammer. A steel chip came off the tool and went through the cloth gloves being worn. He did not seek medical treatment until 2/27/2015, the chip was removed by a hand surgeon.

A Truck Driver suffered a sprain to his right shoulder while climbing up into the cab of the Water Truck. The miner states his right foot slipped off the bottom step and to prevent from falling he held on with his right arm causing the sprain to his shoulder.

Flash burns (electric)

A miner sustained a burn to his right arm while pulling the 15 conveyor electrical disconnect switch.

While positioning a framework for repairs by hand it slipped off one of the horses it had been set on. The handrail attached to the framework struck the miner on the hard hat when it rolled off the horse. The hard hat was pushed down on his face causing an abrasion and laceration which required one suture to close the wound.

Fall from ladders

A repair worker sustained a fractured rib and contusion when he fell to the floor while ascending an extension ladder. The injured employee was attempting to gain access to a shaker when he states the ladder slipped causing his fall. The fall was approximately 3-5 feet to the floor.

Accident type, without injuries

A fall of material from the roof of the K2 tunnel resulted in damage to the K2 conveyor. No miners were injured in the incident. The incident was called into MSHA via the 800 number as an immediately reportable accident.

Contact with hot objects or substances

A repairman sustained a 2nd degree burn to his left wrist while using a torch to remove troughing rollers. His torch hose snagged on the conveyor frame and when he attempted to free the hose it twisted and the lite torch was redirected over his left wrist.

The injured miner was changing a hose on a WA800 Loader. To change the hose, the injured was working from a kneeling position. When he stood up, he states he twisted his right knee, causing the injury.

The employee and co-worker were in the process of replacing a grease barrel on the Hitachi shovel. While moving the full barrel with the co-worker, the injured employee felt a sharp pain in the abdominal area.

The employee was assigned the task of greasing rollers in Building 7. As the employee was walking he twisted his knee.

Struck against a moving object

The employee was operating the locomotive and when he was given the signal to stop he struck his shoulder on the window frame.

Over-exertion in wielding or throwing objects

The employee was assisting the railroad crew and attempted to throw a switch and felt a pain in his back. The employee refused treatment until July 2, 2007.

Struck by rolling or sliding object

The employee and co-workers were attempting to replace a motor on a stacking conveyor. As the motor was on its base it slid off and struck the employee on the left wrist.

Employee stated that on 8/19/06 he was changing the roller chain on a conveyor and strained himself. Employee did not report any injury at that time. Employee performed his normal daily duties until 10/31/2006 when a hernia was confirmed.

The injured was attempting to pick up a slip ring when it sprung open and lacerated two fingers.

Struck by flying object

The injured employee had attached a battery pac booster to the battery and starter of a drill. He realized the pack was not turned on. As he reached to turn the unit on, the unit exploded, fracturing his right thumb.

Employee was standing on front end loader's counter weight ladder when he lost his balance and fell 4' landing on his right foot. this injury consists of a fractured right foot.

THE EE WAS PICKING UP A PIECE OF STEEL WHEN HE SUFFERED A SEVERE STRAIN, TEARING HIS LEFT BICEP MUSCLE. THIS INJURY WILL REQUIRE SURGERY TO REPAIR.

THE EE WAS STANDING BELOW THE SCREEN DECK UPON RESTRTING THE MILL. A ROCK WAS THROWN FROM A SCREEN AND STRUCK THE EE'S HARD HAT LACERATING HIS RIGHT EAR. EIGHT SUTURES WERE NEEDED TO CLOSE THE WOUND, THUS MAKING THIS A RECORDABLE.

THE INJURED MINER INADVERTENTLY PLACED HIS RIGHT HAND ON THE RAIL OF A TELESCOPING SHUTTLE CONVEYOR. THE DRIVE WHEEL ROLLED OVER HIS HAND DELIVERING A CRUSHING INJURY. SUTURES WERE REQUIRED MA KING THIS A RECORDABLE INJURY. T

ON JULY 28TH, 2003, AFTER A FOUR INCH RAINFALL & DISRUPTION OF POWER BY THE LOCAL UTILITY, THE QUARRY BECAME FLOODED. AT THIS TIME AN ESTIMATE OF CONTINUED PRODUCTION IS UNDETERMINED.

WHILE ON BREAK, EE ATTEMPTED TO LIGHT AN ILLEGAL FIREWORK. WITNESSES STATE AS THE FIREWORK EXITED THE PIPE LAUNCHER, IT EXPLODED AND STRUCK HIM IN THE CHEST. THE DUPAGE COUNTY BOMB SQUAD AND R OMEOVILLE POLICE DEPARTMENT INVESTIGATED THIS INCIDENT IMMEDIATELY AND CONFISCATED ALL THE ILLEGAL DEVICES BEFORE ALLOWING THE COMPANY TO PERFORM AN INVESTIGATION.

EE WAS USING A HAMMER TO BLOCK CABLE AND HAMMER SLID AND STRUCK HIS RIGHT FINGER AGAINST TRUCK BED FRAME.

THE EMPLOYEE WAS IN THE PROCESS OF DRIVING THE PLANT CHERRY PICKER TO THE QUARRY. AS HE DESCENDED DOWN THE FIRST RAMP THE VEHICLE DIED ON THE INCLINE, THE EMPLOYEE JUMPED DOWN AND INJURED HIS LEFT LEG. THE INVESTIGATION REVEALED THE VEHICLERAN OUT OF FUEL.

THE EE WAS INJURED WHILE ASSISTING ANOTHER EE WITH INSTALLING A FEEL CHUTE IN THE MILL BUILDING. AS THE CHUTE WAS BEING POSITIONED INTO PLACE, THE EE JUMPED BACK AND STRUCK HIS SIDE WITH A WAL KWAY RAIL (HANDRAIL).

WHIEL INSTALLING REBAR FOR THE NEW PRIMARY CRUSHER AT ROMEOVILLE, A PIECE OF REBAR STRUCK THE EMPLOYEE IN THE ARM, LACERATING IT. THE ALCERATION REQUIRED THREE SUTURES TO CLOSE THE WOUND.
