Over-exertion in pulling or pushing objects

A miner suffered a strain to right biceps/arm while dragging a water hose. The miner was walking down #2 conveyor towards the tail pulley dragging the water hose the miner was using to clean up material. The hose got caught on the leg of the conveyor pulling the miner backwards causing the injury.

Fall to the walkway or working surface

A miner sustained a fracture to right ankle when EE tripped and fell. The fall resulted from the six lanyard that was attached to the full body harness getting hooked on EE's left leg as EE was moving into the work area.

Caught in, under or between a moving and a stationary object

A repairman sustained a nondisplaced fracture and laceration to the tip of left ring finger. The incident occurred while the injured was attempting to adjust the forks on the telehandler. While sliding a fork the worker lost control of it and caught finger between the fork and the fork carriage resulting in the injury to left ring finger.

A repairman sustained lacerations to two fingers on his left hand requiring sutures to close the wounds. The injured miner was attempting to move a 1/2 steel plate away from a beam. His hand was caught between the plate and beam causing the injury.

Over-exertion in lifting objects

A miner strained his left shoulder while removing a guard for access to conduct maintenance. Physical Therapy and additional medical now make this incident MSHA reportable.

A truck driver suffered a strain to his left shoulder while descending the ladder on his haul truck. The miner was holding onto the ladder with both hands and states he felt a pull in his shoulder when he missed the last step on the ladder. Follow-up visits to the clinic with an MRI being taken shows a small tear of the rotator cuff.

Over-exertion in lifting objects

A laborer injured his left shoulder while moving conveyor idlers from the back of a truck to the manlift basket. Additional diagnostics (MRI) positive for rotator cuff tear now make this reportable.

Over-exertion in pulling or pushing objects

A Loader Operator sustained an injury to his right biceps tendon while servicing his loader. He pulled a hose from a reel to add fluid to his machine then less than one minute in filling the machine the hose began to retract pulling his arm with the hose resulting in the injury. A tear of the biceps tendon was confirmed by results of an MRI.

Accident type, without injuries

A mechanical problem with the SC4 conveyor head pulley resulted in the conveyor belt catching fire. A plant supervisor shut down the conveyor and local Fire Department was called to extinguish the burning conveyor belt. A limited amout of damage (36 feet of conveyor belt) and no injuries as a result of this incident.

A laborer suffered a contusion and laceration to his left hand while installing a section of conveyor. The injured miner was cutting a bolt that was in the way and when the cut was complete the conveyor moved causing his left hand (off hand) to be caught between the conveyor and another object.

Over-exertion (Not Elsewhere Classified)

A laborer suffered an acute lumbar strain while using a 90lb jackhammer to break concrete. While breaking the concrete a chunk broke free causing him to lose control of the jackhammer. When he attempted to regain control of the jackhammer he strained his back.

The injured miner was rigging scrap steel to an extended reach fork lift when he was struck on the top of his hard hat by one of the forks as it was lowered. He is now experiencing pain and stiffness in his neck. He did not seek medical treatment until May 28, 2010

The injured miner was leveling a conveyor using railroad ties. While stacking the ties he pinched his finger between two ties. He received first aid treatment and has continued working his regular job. He has since developed a conditon with this finger that needed surgery to correct. He had the surgery on 11/20/09.

Over-exertion in lifting objects

The injured miner was cleaning up spillage using a shovel. During the process of shoveling the spilt material he felt pain in his left forearm.

Bodily reaction, (Not Elsewhere Classified)

The injured miner states while descending stairs on 04/28/08 he twisted his right knee. He was treated and worked for 30 days on restriction until full medical release on 6/10/08. He recently asked to have his knee looked at again and it was determined surgery was required to correct his problem.

A miner suffered a sprain to his right knee while inspecting the 3"" stacker conveyor. The injured states while walking around the conveyor he slipped on wet material and injured his knee. The miner received initial first aid treatment however, he has now started physical therapy making the incident reportable.

Fall from machine

The injured employee was removing old conveyor rollers from a p/u truck when one of the rollers caught the striker he was wearing causing him to lose his balance and fall from the truck injuring his right shoulder.

The injured employee was traveling by foot when he stepped on eneven ground and twisted his left knee resulting in the injury.

Employee states while descending stairs in the #2 Feeder Room he twisted his right knee.

The employee was using a hammer to strike the side of the Primary Shaker when his left little finger was caught between the hammer handle and side of the shaker.

Struck by falling object

The employee was filling the hydraulic oil for the hammer when he dropped the oil can on his left foot injuring his second toe.

While installing wear plates on the crusher, the employee moved the plates and caught his finger in between the plates.

The injured attempted to break an old tamping pole by stepping on it. The brass end came back at the employee; struck his left cheek resulting in a laceration that required sutures.

The employee did not properly secure whip check in place and air hose struck him in left cheek when pressurized. This injury required sutures.

THE MINER WAS IN THE PROCESS OF ADJUSTING A DIVIDER GATE WHEN THE CHAIN HOIST SLIPPED ALLOWING THE GATE TO FALL BACK ONTO HIS ARM. THIS INJURY REQUIRED SUTURES MAKING THIS A RECORDABLE INJURY .

EMPLOYEE WAS CUTTING A CONVEYOR BELT WHEN HIS RAZOR KNIFE SLIPPED AND CUT HIS RIGHT WRIST. THIS INJURY REQUIRED SUTURES MAKING THIS A RECORDABLE INJURY.

THE EE WAS USING A RATCHET WRENCH TO REMOVE THE OIL PLUG ON A R-35 TRUCK. THE WRENCH SLIPPED & STRUCK THE EE ON THE LEFT HAND. THE INJURY CONSISTED OF A LACERATION REQUIRING FIVE STITCHES.

WHILE DRIVING A HAUL TRUCK THE EE STRUCK HIS HAND IN THE CAB AND INJURED HIS LEFT INDEX FINGER.

Contact with hot objects or substances

THE INJURED WAS ASSIGNED TO STEAM CLEAN A FRONT END LOADER PRIOR TO REPAIRS. IT BEGAN TO RAIN SO THE EE CHOSE TO RETURN TO THE GARAGE. WHILE DESCENDING FROM THE LOADER, HE STATES THAT THE WAND FROM THE STEAM CLEANER WENT ACROSS HIS BOOT AND BURNT HIS RIGHT FOOT.
