Employee was working underground in a drilling area with dusty conditions. Employee's N95 face mask was not properly fitted to employee causing dust to leak around the seal to nose and mouth. Employee stated that it was hard to breathe and needed to go to the doctor.

Employee was in the process of filling chute to fill ore cars with a cable drag. Employee lost their balance and accidentally got their hand in a snatch block resulting in the amputation of the end of the left pinky finger.
Accident type, without injuries
Secondary escapeway is checked once per week, upon inspection noticed secondary escapeway middle of R4 raise back-lagging came loose and hanging wall came in. Plugging Raise at this time. Not sure of exact date or time of incident. No employees were present when this happened.
Over-exertion in pulling or pushing objects
Employee was pulling muck using a fin hoe, shoveling muck into ore cart. Right shoulder received strain.

Moving a Diaphram water pump in the shaft of the mine, standing on uneven ground, when lifting pump, right knee give out.
